1.Function analysis of outer membrane protein VirB4 in Brucella infecting embryo trophoblastic cells

Potential target proteins binding to VirB4 of type Ⅳ secretion system were screened during Brucella infected bovine embryonic trophoblast cells .Brucella VirB4 genes were amplified by PCR with species-specific primers .Expression vector pGBKT7-virB4 was constructed and analysed by sequencing and restriction enzymes ,transforming to the yeast strain Y187 and testing self-activation and toxicity .The cells model and cDNA library of bovine embryonic trophoblast cells infected with Brucella abortus strain were constructed respectively .Utilizing yeast two-hybrid system was employed to screen the target proteins of bovine embryo trophoblastic cells which was conjunctive with virB4 .These proteins were detected by real-time fluo-rescence quantitative PCR .The results suggested that bait plasmid pGBKT7-virB4 was successfully transformed into the Y187 and there was no toxicity and self-activation;the cDNA library of bovine embryonic trophoblast cells infected with Brucella abortus strain was constructed .There screened 13 positive plasmids in which Q10 and SLC3A2 were up-regulated at the mRNA level .In this paper ,we reported the interactions between the VirB4 protein of Brucella and the bovine embryo trophoblastic cells ,which provide an upstream work for further elucidating the pathogenesis of Brucella infection of the host cell .
2.Effect of α-glycan isolated from Isatis indigotica on humoral and cellular immunities in mice immunized with H1N1 influenza vaccine

OBJECTlVE To investigate the effect of α-glycan isolated from Isatis indigotica on humoral immunity and cellular immunity functions in mice immunized with H1N1 influenza vaccine. METHODS BALB/c mice were immunized intramuscularly once with H1N1 influenza vaccine ( 3 μg) plusα-glycan ( 100μg) each mouse. The serum total antibody titer and its isotype antibody titer of immu-nized mice were analyzed by ELlSA at 5, 8, 10, 12 and 14 d after injection at vaccine. The proliferation activities of spleen T and B lymphocytes were determined with MTT method. The levels of cytokines interferon-γ( lFN-γ) , tumor necrosis factorα( TNF-α) , interleukin-4( lL-4) and lL-12 were measured by ELlSA kits. The populations of CD4+, CD8+, CD3+ and CD19+ lymphocytes were determined by flow cytometry. Furthermore, the proliferation rate of macrophages was studied with MTT method in vitro. RESULTS The α-glycan from I.indigotica could gradually induce high specific-antibody production 5-14 d after immunization with H1N1 influenza antigen plus theα-glycan in mice compared to immunization with antigen alone ( P<0.01) . After injection of antigen withα-glycan for 5 d, the main lgG isotype was lgM, and the titer levels of total lgG, lgG1 , lgG2a and lgG2b were also significantly raised following 5-14 d after immunization. The α-glycan significantly promoted the spleen T and B lymphocytes proliferation ( growth rate 44.2%and 37.8%) , stimulated the secretion of lFN-γand lL-12 of splenocytes ( P<0.01, P<0.05) , and also promoted lL-4 secretion of thymocytes (P<0.01). The polysaccharide significantly raised the percent age of CD3+T cells ( P<0.01) , CD3+/CD19+ T lymphocytes ( P<0.01) , and CD8+ T cells ( P<0.01) but decreased the percentage of CD4+/CD8+ T lymphocytes compared with antigen alone group ( P<0.01) . Furthermore, the α-glycan exhibited significant effects on the proliferation and TNF-α secre-tion of MH-S macrophages. CONCLUSlON Theα-glycan isolated from I.indigotica can improve humoral and cellular immunity response in mice immunized with H1N1 influenza vaccine.
3.Reasonsfor nursing postgraduates to become clinical practice nurses: qualitative research

Objective To understand the various causes of nursing postgraduates to engage in clinical nursing work, so as to provide the reference basis for hospital managers to the cultivation of the postgraduate nurses. Methods Using the convenience sampling method, semi-structured interviews were conducted among 12 postgraduate nurses from August to October in 2016. The interview data were collected and analyzed using phenomenological research method. Results Three themes were extracted, including salary and stability factors, self-development factors and career development factors. Conclusions Clinical managers should pay attention to the basic requirements, self-development and career development of postgraduate nurses, and provide more career development opportunities and platforms for them.
4.Effect comparison of different flow rate of oxygen inhalation on relieving scapulodynia after laparoscopic cholecystectomy with carbon dioxide pneumoperitoneum

Objective To compare the effect of different flow rate of oxygen inhalation on relieving scapuledynia after laparoscopic cholecystectomy(LC) with carbon dioxide pneumoperitoneum. Methods 100 patients adopted LC were divided into the 2 L/min group (n=50), the 4 Ldmin group (n=50), both re-ceived oxygen inhalation 6 hours after LC. The incidence and degree of scapulodynia and the blood- gas analysis were analyzed. Results The incidence of scapulodynia after LC in the 4 L/min group was signifi-cantly lower than that of 2 L/min group. PaCO2,PaO2 and pH in the 4 L/min group were different from those of the 2 L/min group. Conclusions Increasing flow rate of oxygen inhalation can relieve incidence and degree of scapulodynia after laparoscopic cholecystectomy.
5.Survey of living conditions of hypertensive patients in Karamay Bai-jiantan

Objective To investigate the living conditions of patients with hypertension and their medication compli-ance in the Karamay area to provide evidence for the clinical management of hypertension. Methods Living conditions, medication compliance and related conditions of 239 cases of hypertensive patients were surveyed and analyzed by questionnaire. Results Knowledge of self blood pressure level rate within the jurisdiction was 61.51%, control rate was 53.56%and 156 cases (65.27%) of the 239 cases surveyed followed doctors'instructions to take medicine. Blood pres-sure was related to age, smoking, obesity, family history, alcohol consumption and other factors. Conclusion Although many patients'blood pressures are under control in the jurisdiction, they are still not satisfied with their physical con-dition. As grassroots health workers, we need to further strengthen health education, actively implement the personal-ized management of patients and help them to improve their quality of life.
6.Current status of knowledge, attitude and practice of ICU nurses on enteral nutrition treatment of severe acute pancreatitis and its influencing factors

Objective:To explore the knowledge, attitude and practice (KAP) status of ICU nurses on enteral nutrition treatment of severe acute pancreatitis and to analyze its influencing factors.Methods:A total of 520 ICU nurses from 7 ClassⅢ Grade A hospitals in Henan Province from February to April 2022 were selected as research objects by the convenient sampling method. General Data Questionnaire and Clinical Nurses' Knowledge, Attitude and Practice Questionnaire on Enteral Nutrition Treatment of Acute Pancreatitis were used to investigate.Results:A total of 520 questionnaires were sent out, and 504 were effectively collected, with an effective recovery rate of 96.92%. The total score of ICU nurses' knowledge, attitude and practice in enteral nutrition for severe acute pancreatitis was (113.68±12.48), including knowledge dimension (10.67±2.58), attitude dimension (61.60±6.38), and practice dimension (41.42±7.44). There were positive correlation among ICU nurses' scores of knowledge, attitude and practice on enteral nutrition in patients with severe acute pancreatitis ( P<0.01). The results of multiple linear regression analysis showed that professional title, length of service in ICU nursing, personnel relations, whether to participate in specialized training, and whether to care for patients with severe acute pancreatitis and enteral nutrition were the influencing factors of knowledge dimension ( P<0.05). Post, whether to participate in specialized training, and whether to care for patients with severe acute pancreatitis undergoing enteral nutrition were influencing factors of attitude dimension ( P<0.05). Gender, professional title, post, whether to participate in specialized training, and whether to care for patients with severe acute pancreatitis undergoing enteral nutrition were the influencing factors of behavioral dimensions ( P<0.05) . Conclusions:ICU nurses have a positive attitude towards enteral nutrition treatment for severe acute pancreatitis, and their knowledge attitude and practice level need to be improved. Nursing managers should establish and improve a training system, strengthen the knowledge and behavior of ICU nurses on enteral nutrition for patients with severe acute pancreatitis and improve their management level of enteral nutrition for patients with severe acute pancreatitis, thereby improving the quality of nursing.
7.Application of three-dimensional quality assessment model of structure-process-outcome in continuous nursing of elderly patients with lung cancer after operation

Objective? To explore the application of three-dimensional quality assessment model of structure-process-outcome in continued nursing care for elderly patients with lung cancer after operation. Methods? By purposive sampling, 48 elderly patients discharged after radical resection of lung cancer from January to December 2016 were taken as control group and received routine continuous nursing care. Another 51 elderly patients discharged after radical resection of lung cancer from June 2017 to June 2018 were taken as the observation group. Continuous nursing program based on structure-process-outcome quality assessment model was applied for the observation group. The effect of intervention was assessed by using Strategies Used by People to Promote Health (SUPPH) and Quality of Life Questionnaire-30(QLQ-30). Results? At 6 months after discharge, the scores of each dimension of SUPPH scale in the observation group were statistically higher than those in the control group (P< 0.05). In the observation group the scores of "physical function", "role function", "social function" and "overall health" of QLQ-30 scale were higher than the control group, the scores of "fatigue", "pain", "insomnia", "lack of appetite", "constipation", and "diarrhea" were all lower than the control group with statistical significance (P<0.05). Conclusions? The continuous nursing care based on the three-dimensional quality assessment model of structure-process-outcome for elderly patients with lung cancer can help to improve their sense of self-efficacy and quality of life.
8.Preliminary study on the effect of multiple disciplinary team model nursing team on the treatment of patients with multiple trauma in emergency department

Objective To explore the application effects of nursing team in patients with acute multiple trauma in multiple disciplinary team (MDT) model. Methods A convenient sampling method was used to select 82 patients with multiple injuries who were treated since the establishment of the trauma center in the First Affiliated Hospital of Zhengzhou University from July 2016 to June 2018. From July 2014 to June 2016, a total of 55 patients with multiple trauma before the establishment of the trauma center were seclected as the control group. The experimental group was used MDT model for rescue, and the control group was used traditional model for rescue. The doctors' arrival time, emergency examination time, emergency stay time, rescue success rate and postoperative complication rate of the specialists in the two groups were compared. Results The doctors' arrival time in the experimental group was (16.80±3.57) min, the time of emergency examination was (22.36±3.49) min, and the time of emergency stay was (38.19±8.18) min, which was shorter than those of the control group (23.27±5.76), (45.69±7.75), (55.49±13.67) min, the differences between the two groups were statistically significant (P<0.01). The successful rescue rate of the treatment group was 96.3% (79/82), which was higher than 87.3% (48/55) of the control group. The difference between the two groups was statistically significant (P< 0.05). The postoperative complication rate was 3.7% (3/82) in the experimental group and 12.7% (7/55) in the control group, the difference between the two groups was statistically significant (P< 0.05). Conclusions Patients with multiple trauma received treatment with MDT model can effectively shorten the time of doctors' arrival, emergency examination time, emergency stay time, improve the success rate of treatment, and reduce the incidence of postoperative complications.
9.Application of evidence-based management intervention in patients with advanced colon cancer

Objective:To explore the effect of evidence-based management intervention on the self-management efficacy, quality of life and psychological status of patients with advanced colon cancer.Methods:From May 2017 to May 2019, convenience sampling was used to select 176 patients with advanced colon cancer admitted to the International Medical Department of Peking Union Medical College Hospital as the research object. According to the order of admission, the first 90 patients were included in the observation group, and the last 86 patients were included in the control group. Observation group carried out evidence-based management intervention, and control group implemented routine intervention. We compared the scores of the Strategies Used by People to Promote Health (SUPPH) , World Health Organization Quality of Life-BREF (WHOQOL-BREF) , Hamilton Anxiety Scale (HAMA) and Hamilton Depression Scale (HAMD) between two groups before and after intervention to evaluate the effect of evidence-based management intervention on self-management efficacy, quality of life and psychological status of patients.Results:Before intervention, there was no statistically significant difference in the scores of each dimension of the SUPPH between two groups ( P>0.05) . After intervention, the dimension scores of positive attitude, self-decompression, and self-decision in the SUPPH scale of observation group were higher than those of control group, and the differences were statistically significant ( t=4.423, 3.196, 3.148; P<0.01) . Before intervention, there was no statistically significant difference in scores of each field in WHOQOL-BREF between two groups ( P>0.05) . After intervention, the scores of physical, psychological, social relations and environmental fields of observation group were higher than those of control group, and the differences were statistically significant ( t=5.450, 6.458, 4.286, 2.104; P<0.05) . Before intervention, there was no statistically significant difference in the scores of HAMA and HAMD between two groups ( P>0.05) . After intervention, the HAMA and HAMD scores of observation group were lower than those of control group, and the differences were statistically significant ( t=10.171, 7.903; P<0.01) . Conclusions:The evidence-based management intervention can improve the self-efficacy of patients with advanced colon cancer, improve the quality of life of patients, and the psychological status of patients.
10.The status and influencing factors of kinesiophobia in patients with diabetic peripheral neuropathic pain

Objective To understand the status and influencing factors of kinesiophobia in patients with diabetic peripheral neuropathic pain(DPNP).Methods From February to June 2023,192 patients with DPNP in a tertiary general hospital in Nanchang City,Jiangxi Province were selected by convenience sampling method.The patients were investigated by the demographic information questionnaire,Tampa Scale for Kinesiophobia,Numerical Rating Scale,Medical Coping Modes Questionnaire,Diabetes Distress Scale,and Summary of Diabetes Self-care Activities.Multiple linear regression were used to analyze the influencing factors of kinesiophobia in patients with DPNP.Results The score of kinesiophobia in patients with DPNP was(36.68±3.88),and the incidence of kinesiophobia was 38.5%.Multiple linear regression analysis showed that marital status,pain level,coping style,diabetic distress and self-management behavior were influential factors for kinesiophobia in patients with DPNP(all P<0.05).Conclusion Patients with DPNP have a moderate level of kinesiophobia.Nursing staff should pay attention to the early assessment of patients'kinesiophobia,promptly identify and take effective measures to reduce the level of patients'kinesiophobia,increase their motivation to exercise,and improve their prognosis and quality of life.
