1.Expression of DNA methyltransferase 1 in transitional cell carcinoma of bladder by real-time PCR

Objective To investigate the expression of DNA methyltransferase 1 (DNMT1)mRNA in tissue of human transitional cell carcinoma of bladder(TCCB)and its clinical significance.Methods DNMT1 mRNA expression in 35 tumor tissues and 10 normal bladder tissues was measured by real-time PCR.Results Higher expression of DNMT1 transcripts was detected in tumor(3.25±0.74)than in normal tissues(1.53±0.44,P<0.001).The expression level of DNMT1 mRNA in stage Tis-T1,T2-T4 was 3.14±0.67,3.31±0.84;in grade Ⅰ,Ⅱ,Ⅲ was 3.13±0.59,3.25±0.64,3.43±0.55;in patients older tham 40 years and younger than 40 years was 3.34±0.50.3.01±0.27;in male and female group was 3.31±0.42,3.01±0.20;in primary and recurrence group was 3.21±0.63,3.45±0.33.mRNA levels did not correlate with grade.stage and recurrence (P>0.05).Higher levels of expression were associated with advanced age(P<0.05).Male group had significant higher levels of expression than female group(P<0.05).Conclusions Over-expres-sion of DNMT1 may play an important role in TCCB.DNMT1 can be an important therapeutic target in TCCB.

The aim of this paper is to study the methods of 3D visualization and the 3D interactive clipping of CT/MRI image sequence in arbitrary orientation based on the Visualization Toolkit (VTK). A new method for 3D CT/MRI reconstructed image clipping is presented, which can clip 3D object and 3D space of medical image sequence to observe the inner structure using 3D widget for manipulating an infinite plane. Experiment results show that the proposed method can implement 3D interactive clipping of medical image effectively and get satisfied results with good quality in short time.

4.Cultivation of prescription-auditing pharmacists’ability in prescription pre-audit and case analysis

OBJECTIVE To summariz e the expe rience of the ability training of prescription-auditing pharmacists in prescription pre-audit,and introduce the typical cases of the prescription-auditing pharmacists participating in the drug intervention. METHODS From October ,2020 to October ,2021,under the audit mode of “prescription pre-audit system+prescription-auditing pharmacists ” adopted by Yuxi People ’s Hospital (hereinafter referred to as “the hospital ”),the abilities of prescription-auditing pharmacists were cultivated from the aspects of training in pharmaceutical related professional knowledge ,training in the use of Chinese and English medical retrieval tools ,databases and websites ,and clinical thinking and communication ability ;through the construction of ability evaluation form of prescription-auditing pharmacists ,their abilities were assessed. RESULTS & CONCLUSIONS After one year ’s ability training ,the rational rate of prescription (doctor’s order ),the proportion of doctors ’active revision of problem prescription (doctor’s order )and the doctor ’s acceptance rate of intervention by prescription-auditing pharmacists showed an upward trend ,the average time of irrational prescription (doctor’s order )by prescription-auditing pharmacists showed a shortening trend ,and the intervention rate of prescription (doctor’s order )showed a downward trend. In addition to the publication of papers (belonging to the bonus item ),the average score of the ability evaluation form of prescription-auditing pharmacists had significantly increased , from 45.2 in October 2020 to 97.6 in October 2021.
5.Effects of hyperbaric oxygen on the activation of β-catenin in hypoxic ischemic neonate rats

Objective To explore the relationship between the proliferation of neural stem cells (NSCs)and the expression of β-catenin protein in neonate rats with hypoxic ischemic brain damage (HIBD) after hyperbaric oxygen (HBO) therapy. Methods One hundred and eighty Sprague-Dawley rats aged 7 days were randomly divided into a normal control group (CON) , a HIBD model group and a HBO treatment group. The HIBD model was induced using Rice's method. Beginning 3h after the HIBD, HBO was administered to the HBO treatment group at 2 atmospheres for 60 min, once daily for 7 days. The HIBD model group was not given any treatment. The expression of nestin/β-catenin protein in the subventricular zone of the ischemic brain was double-stained for immunofluorescence and analyzed by confocal scanning microscopy dynamically at 3 hours, 21 hours, and then on the 3rd, 5th, 7th and 14th day of HBO therapy. The expression of whole cell β-catenin and nuclear β-catenin protein in the left brain were also examined by Western blotting at these 6 time points. Linear correlation was used to analyze the correlation between β-catenin and nestin protein. Results The expression of β-catenin protein in NSCs increased initially at the 21st hour after HBO therapy in the model group and the HBO group as compared with the normal control group.β-catenin protein in the model group reached a higher level, though there was no significant difference between model group and the HBO group. At the 5th day of HBO therapy β-catenin protein in the HBO group had reached a significantly higher level than in the model group. At the 14th day the average expression of β-catenin in the HBO group began to decrease. The expression of nestin protein began to increase 21 hours after HBO therapy began, and it peaked at the 7th day of HBO therapy and then decreased. In the HBO group the increase in nestin protein was linearly correlated with that of β-catenin protein. The whole cell β-catenin protein and β-catenin nucleic protein readings increased initially by the 21st hour of HBO therapy and by the 5th day were significantly higher than the levels in the model group. Conclusion HBO treatment is capable of stimulating the proliferation of NSCs in HIBD neonate rats.The proliferation of NSCs is correlated with the activation of β-catenin protein.

7.Correlational study between ultrasonic elastography strain ratio and serum thyroid stimulating hormone on patients with Hashimoto's thyroiditis

ObjectiveTo investigate the value of strain ratio (SR) in the diagnosis of Hashimoto's thyroiditis.Methods 116 patients who had been diagnosed clinically as Hashimoto' s thyroiditis were enrolled in the study.Those patients were classified into four groups:the hyperthyroidism group,the euthyroidism group,the subclinical hypothyroidism group,and the clinical hypothyroidism group.50volunteers with normal thyroid functions were enrolled as a control group.SR of the thyroid and sternocleidomastoid muscle of same side was calculated.The correlation coefficient between the elasiticity SR and the serum thyroid stimulating hormone (TSH) was evaluated and their differences in the diagnosis of Hashimoto's thyroiditis were compared.ResultsThe Spearman's correlation coefficient between the SR and the serum TSH was 0.605,which was significant (P =0.000).The SR increased in an ascending order among the group with hyperthyroidism,the control group,the group with euthyroidism,the group with subclinical hypothyroidism,and the group with clinical hypothyroidism.ConclusionsElastography SR is valuable way for evaluating the progression of Hashimoto's thyroiditis.
8.Impact of transrectal real-time tissue elastography guiding biopsy combined with peak strain index for diagnosing prostate cancer

Objective To evaluate the value of transrectal real-time tissue elastography (RTE) targeted prostate biopsy in the peripheral zone combined with peak strain index.Methods One hundred and forty-one patients with suspicious prostate lesions in the peripheral zone were evaluated from February 2011 to February 2014.All the patients underwent RTE with a mean age of 71.6 years,PSA of 30 ng/ml,prostate volume of 50.3 ml and measured peak strain index (PSI).The diagnostic value of PSI was assessed by receiver operating characteristic (ROC) curve.Two-core RTE combined with PSI targeted prostate biopsy was taken and subsequently a 10-core systematic biopsy was taken.The value of RTE was evaluated.The data of targeted biopsy and systematic biopsy in prostate were both reviewed and statistically compared.Results Cancer was detected in 72 of 141 patients (PSI,mean 24.79),and 69 patients had benign prostate disease (PSI,mean 3.02).PSI value of prostate cancer was significantly higher than that of the benign lesions (P < 0.05).Prostate cancer could be predicted with the highest sensitivity (87.5%) and specificity (88.6%) using the cutoff value of PSI ≥ 5.97 with an area under the curve of 0.95.RTE targeted biopsy combined with PSI could detect 95.6% of moderate or high risk prostate cancer.One hundred and fifty-nine suspicious areas detected by RTE in 141 patients were biopsied with 2 cores for each area.The positive incidence of prostate cancer in RTE-targeted biopsy cores was 44% and in systematic biopsy was 30.2% (P < 0.05).Among the 72 prostate cancer patients,63 cases (87.5%) were detected by RTE-targeted biopsy,62 cases (86.1%) by systematic biopsy (P > 0.05).Conclusions RTE combined PSI can improve the detection rate of prostate cancer in the peripheral zone and likewise guide targeted biopsy combined with svstematic biopsy to detect more moderate or high risk prostate cancer.
9.Optimal dose of dexmedetomidine for intravenous analgesia after open radical resection of intestinal neoplasms when mixed with flurbiprofen axetil and butorphanol

Objective To determine the optimal dose of dexmedetomidine for intravenous analgesia after open radical resection of intestinal neoplasms when mixed with flurbiprofen axetil and butorphanol.Methods A total of 120 patients of both sexes,of American Society of Anesthesiologists physical status Ⅰ or Ⅱ,aged 20-60 yr,weighing 45-80 kg,undergoing elective open radical resection of intestinal neoplasms,were divided into 4 groups (n =30 each) using a random number table:control group (group C)and different doses of dexmedetomidine groups (group DEX1,group DEX2,group DEX3).Group C received flurbiprofen axetil 2 mg/kg and butorphanol 0.05 mg/kg for intravenous analgesia.In DEX1,DEX2 and DEX3 groups,dexmedetomidine 0.3 μg/kg was intravenously infused starting from 30 min before the end of surgery,and the analgesia solution contained dexmedetomidine 1,2 and 3 μg/kg,respectively,which was mixed with flurbiprofen axetil 2 mg/kg and butorphanol 0.05 mg/kg in 100 ml of 0.9% normal saline,and the mixture was infused at a rate of 2 ml/h.Butorphanol 0.5 mg was intravenously injected as a rescue analgesic,postoperative pain was assessed using the visual analog scale at coughing,and visual analog scale score was maintained <4.The requirement for rescue analgesics was recorded within 48 h after operation.The occurrence of postoperative adverse reactions such as nausea and vomiting,respiratory depression,somnolence,bradycardia,hypotension and over-sedation,patient's satisfaction with analgesia and length of postoperative hospital stay were recorded.Results Compared with group C,the rate of rescue analgesia after operation was significantly decreased,and the degree of satisfaction with analgesia was increased in DEX2 and DEX3 groups,and the incidence of postoperative somnolence was significantly increased in group DEX3 (P<0.05).No other adverse effects were found in DEX1,DEX2 and DEX3 groups.Conclusion When mixed with flurbiprofen axetil and butorphanol,the optimal dose of dexmedetomidine for intravenous analgesia after open radical resection of intestinal neoplasms is 2 μg/kg.
10.Transrectal real-time elastography in normal prostate and benign prostatic hyperplasia

Objective To observe the characteristics of normal prostate gland and benign prostatic hyperplasia (BPH) on transrectal real-time elastography (TRTE). Methods A total of 77 BPH patients and 17 normal subjects underwent TRTE. The elastographic images were analyzed with the five-point subjective elastic scale. The strain ratio of transition zone, peripheral zone, and transition zone to peripheral zone were respectively measured to compare the stiffness of each zone. Results The elasticity scores of both BPH group and normal prostate group were 1 and 2 in scoring system. There was no significant difference of elasticity scores between BPH group and normal prostate group (P=0.2252), as well as in strain ratio of transition zone (P=0.9793), peripheral zone (P=0.8986), and transition zone to peripheral zone (P=0.5233). Conclusion The stiffness of normal prostate gland and BPH could be offered by TRTE.
