1.Study of emergency temporary cardiac pacing

Objective To assess the feasibility and the reliability of emergency temporary cardiac pacing under fluoroscopy and nofluoroscopy,and compare to the superiority and inferiority between two groups.Methods Fifty-seven patients were temporarily paced under fluoroscopy and nofluoroscopy by the way which the common bi-pole temporary endocardium pacing electrode was introduced into the right ventricle by the bi-subclavian vein and right internal jugular.Result Twenty-six patients were successfully paced under fluoroscopy,and 27 patients of 31 were successfully paced without fluoroscopy,the other 4 patients were not paced successfully.Besides,there was no any complication in all patients.Conclusion Emergency temporary cardiac pacing without fluoroscopy is utility,saving time and safety,Compared to other method,the effect is proximity and even superior than the way under fluoroscopy.The way of the emergency temporary cardiac pacing without fluoroscopy is applied widespreadly in the work of clinical first aid.
2.Mental health and the related factors of Zangzu undergraduates in inland

Objective To explore the state of the mental health and related factors of Zangzu undergraduates in inland.Methods Two inland typical universities were chosen,according to random whole-class sampling principle from different grades,and 370 Zangzu undergraduates in inland were tested with Symptom Checklist 90(SCL-90),Self-Esteem Scale(SES),Simplified Coping Style Questionnaire (SCSQ) and social support scales,and compared with norm and Hanzu undergraduates.Results The scores of the factors of compulsion,interpersonal relation sensitiveness,depression,anxiety,hostility of Zangzu undergraduates in inland in SCL-90 were higher than 2.The rate of their mental problems(the total average score was higher than 2) was 45.3%,among them the rate of mild mental problems was 42.0%,the rate of secondary mental problems was 3.3%,without undergraduates of serious mental problems.Scores of the 9 factors were higher than that of the norm,and differences were significant (P< 0.01).Scores of the 9 factors were higher than that of Hanzu undergraduates,and scores of somatization,compulsion,anxiety,hostility,terror were significantly higher than that of Hanzu undergraduates (P <0.01).Rate of their mild mental problems was significantly higher than that of Hanzu undergraduates(P<0.01).Scores of their self-esteem and positive coping styles were lower than that of Hanzu undergraduates,and differences were significant (P =0.000).The negative coping styles and self-esteem could predict the state of mental health of Zangzu undergraduates in a great extent.Conclusion Zangzu undergraduates in inland have more mild mental problems,which are correlated with their self-esteem and coping styles.

Objective PB840 ventilator to explore non-invasive ventilation (NIV) in the ICU of the application,efficacy and safety.Methods Application PB840 Ventilator-Non-invasive mechanical ventilation in the treatment of the seven cases of different causes of respiratory failure patients,non-invasive ventilation in the treatment of 4 h,the blood gas (PaO2,PaCO2 and pH value) analysis and analysis of clinical symptoms to improve the situation.Results Patient before and after treatment of PaO2,PaCO2 and pH compared to the value there is significant difference.Target to improve the blood after treatment significantly (P<0.05),reduce or alleviate the symptoms of sick;cyanosis improved.However,leakage is still a major disadvantage,some patients can not tolerate when there is need to immediately replace invasive mechanical ventilation.Conclusion PB840 ventilator mode non-invasive ventilation can improve the patient′s respiratory failure and clinical symptoms of blood-gas target,but most critically ill patients should be carefully applied.
4.Effect of hydrogen sulfide donor on oxidative stress of myocardium in adriamycin-induced dilated cardiomyopathy rats

AIM:To investigate the effect of hydrogen sulfide(H2S)donor(NAHS)on oxidative stress of adriamycin-induced dilated cardiomyopathy rats.METHODS:Weight-matched adult male Wistar rats were randomly divided into 5 groups as follows:(1)ADR group(n=12),in which 2.5 mg/kg of adriamycin was injected intraperitoneally once a week for 10 weeks(total dose of 25 mg/kg).(2)ADR+small-dose NaHS group(n=12),in which the dosage and the use of adriamycin were as mentioned above,while NaHS solution was injected to rats at a dosage of 2.8 ?mol?kg-1?d-1 at the same time.(3)ADR + large-dose NaHS group(n=12),in which the dosage and the use of adriamycin were as mentioned above,while NaHS solution was injected to rats at a dosage of 14 ?mol?kg-1?d-1 at the same time.(4)Control group(n=9),in which an equivalent volume of physiological saline was administered weekly for a total of 10 weeks.(5)NaHS group(n=9),in which 14 ?mol/kg of NaHS solution was injected to rats intraperitonealy once a week for 10 weeks.Hemodynamic and echocardiographic measurements were obtained 10 weeks after the treatment.Meanwhile,H2S and malondialdehyde(MDA)concentrations,the activities of superoxide dismutase(SOD)and glutathione peroxidase(GSH-Px)in serum and myocardial tissues were evaluated,respectively.RESULTS:The cardiac functions in the group of ADR rats depressed obviously.H2S concentrations,SOD and GSH-Px activities in serum and myocardial tissues of ADR group rats were all significantly decreased as compared with those in the control group(P

Objective To investigate the clinical results of end-to-end invagination pancreaticojejunostomy.end-to-end invagination pancreaticojejunostomy and duct-to-mucosa pancreaticojejunostomy of pancreaticoduodencetomy of patients with pancreatic and periampullary carcinoma. Methods Fifty-two patients underwent different pancreaticoduodenectomy in our hospital from August 1995 to July 2006 were collected and analyzed retrospectively.The incidence of pancreatic leakage,operation time,the blood loss and the postoperative complications were analyzed. Results Twenty-five patients received end-to-end invagination pancreaticojejunostomy.Sixteen patients received end-toside invagination pancreaticojejunostomy.Eleven patients received end-to-side pancreaticojejunal mucosa-mucosa anastomosis.The operation time of the duct-to-mucosa pancreaticojejunostomy was shorter than the two of the others(4 ~7.15 h).The perioperative comphcations occurred in 18 cases and the long-term postoperative complications occurred in 10 cases.The average length of stay in hospital was(19.32 ±5.45)days.Two cases died during perioperative period. Conclusion End-to-eed invagination pancreaticojejunostomy,end-to-side invagination pancreaticojejunostomy and end-to-side pancreaticojejunal mucosa-mucosa anastomosis are all common anastomosis methods.Pancreatic leakage is a serious postoperative complication.The selection of anastomosis method is based on the condition during operation.
6.Effect of hydrogen sulfide on myocardial structure and function of rats with adriamycin-induced dilated cardiomyopathy

Objective To compare the distribution of collagen type Ⅲ and Ⅳ in mucosa of normal and carcinoma cardiac tissues and to analyze the influence of changed collagen on auto-fluorescence after canceration.Methods Using immunohistochemical technique with monoclonal anti-collagen typeⅢ and Ⅳ,the difference of distribution in extracellular matrix of two types collagen between normal and carcinoma tissues from 16 patients were measured with semi-quantitative score methods.Results In normal mucosa,the epithelial basement membrane showed an intense positive reaction for monoclonal anti-collagen Ⅳ and an intensively stained ring band was formed around the grand basement membrane.The collagen type Ⅲ formed ring-like structures apparently representative of fibriform collagen immediately adjacent to the epithelial basement membrane.In carcinomatous mucosa,immunoreaction of the carinomatous cell nest was negative,and only remnants of positively were seen at the site of the former membrane by collagen type Ⅳ.The change of collagen type Ⅲ was not apparent and the positive reaction in carcinomatous stroma was more intensity than in normal stroma.There was significant difference in distribution of collagen type Ⅲ and Ⅳ between the normal and carcinomatous tissues(P

Objective The purpose of this study was to investigate the value of serum galactomannan (GM) in the diagnosis of invasive pulmonary aspergillosis (IPA) in patients with chronic obstructive pulmonary disease (COPD).Methods We enrolled 60 COPD patients in the study, including 19 IPA and 41 non-IPA cases.We examined serum GM of the patients by ELISA, evaluate the value of serum GM test for the diagnosis of IPA in patients with COPD, and compared the GM values before and after treatment.Results With 0.5 as the positive cutoff value, the sensitivity, specificity, positive predictive value, and negative predictive value of serum GM were 57.9%, 95.3%, 84.6%, and 83.0%, respectively, with a high specificity and a low sensitivity.The 7 IPA cases showed a significantly decreased GM value after treatment as compared with the baseline (0.30±0.21 vs 1.48±1.37, P=0.004).Conclusion The serum GM test has a limited value in the diagnosis of IPA in patients with COPD, but dynamic monitoring of the changes of the serum GM value may help evaluate the patient's condition.
9.Study on Clinical Functions of Consensus-based Clinical Practice Guidelines in Traditional Chinese Medicine

This study was aimed to explore clinical functions of consensus-based clinical practice guidelines (CB CPGs) in traditional Chinese medicine (TCM). Under the guidance of the State Administration of TCM, 42 TCM hospitals in China had conducted a questionnaire survey for 73 CB CPGs and 2993 questionnaires had been included and analyzed. According to the category of good, general and bad, the proportion of the good in improving safety performance was 69.43%, controlling cost was 58.57%, regulating medical behavior was 55.50%, improving clinical effectiveness was 46.74%. It was concluded that the CB CPGs had played a good role in improving safety performance and controlling cost in TCM clinical practice.
10.Epidemiological survey of in-patients with femoral supracondylar fracture in The Third Hospital of Hebei Medical University from 2009 to 2018

Objective To investigate the epidemiological characteristics of the in-patients with femoral supracondylar fracture in the Third Hospital of Hebei Medical University from 2009 through 2018.Methods The medical imaging computer archiving and transmission system and the medical record inquiry system were used to collect the data of all the in-patients with femoral supracondylar fracture who had been treated from January 2009 through December 2018 in The Third Hospital of Hebei Medical University.Their gender and age distribution,basic sociological data and cause of injury were characterized.The in-patients from January 2009 to December 2013 were assigned into group A while those from January 2004 to December 2018 into group B.The 2 groups were compared to find out the epidemiological characteristics and trends of the in-patients with femoral supracondylar fracture in the 10 years.Results Of the 508 in-patients with femoral supracondylar fracture,279 (54.9%) were male and 229 (45.1%) female.Their age ranged from 1 to 94 years,averaging 44.3 years.The age group from 41 to 50 years accounted for the largest proportion (104 cases,23.9%).In males,the age group from 41 to 50 years accounted for the largest proportion (33.3%,80/279);in females,the age group from 51 to 60 years the largest proportion (30.3 %,59/229).The proportion of females in the in-patients under 50 years of age (37.1%) was significantly lower than that (62.9%) in the in-patients over 50 years of age (P < 0.05).According to the AO classification,there were 193 cases (38%) of type 33-A1,117 ones (23%) of type 33-A2 and 198 ones (39%) of type 33-A3.There were 270 farmers (53.1%),164 workers (32.4%) and 74 students (14.5%).Most injuries were caused by high energy (56.5%).The age of in-patients with low-energy injury (50.5 ±20.7 years) was significantly higher than that of in-patients with high-energy injury (39.5 ± 19.6 years) (P <0.05).The proportions of urban residents (30.0%),patients with low energy injury (37.9%) and type 33-A1 fractures (23.9%) in group A were significantly lower than in group B (43.3%,46.9% and 32.3%,respectively) (P < 0.05);the age of group A (41.9 ± 21.8 years) was significantly younger than that of group B (45.9 ± 19.9 years) (P <0.05);type 33-A3 fractures in group A (46.5%) were significantly more than in group B (36.3%) (P <0.05).Conclusions Femoral supracondylar fractures were more common in farmers.Most of the fractures were type 33-A3.The fractures were common in males from 41 to 50 years old and in females from 51 to 60 years old.There were more males than females in the junior patients but more females than males in senior patients.There were more young patients in complex fractures but there were more old patients in simple fractures and there were more very old patients in low-energy injuries.In the past 10 years,the patients with femoral supracondylar fracture increased year by year.Compared with the first 5 years,the late 5 years witnessed significant increases in age of patients and proportions of low-energy injury and simple fracture.
