1.MAJOR COMPLICATIONS OF CARDIAC CATHETERIZATION AND ANGIOCARDIOGRAPHY IN CHILDREN AND ADULTS (EXPERIENCE FROM 3318 CASES)

4035 consecutive cardiac catheterizations and/or angiocardiography in 3318 patients were studied. The age of the patients ranged from 3.5 to 56 years. 173 major complications were found. Fatal complications were rare in all age groups (0.06%). The incidence of severe arrhythmias was high(2.4% of all cases), however almost all of them responded well to simple therapeutic measures and only two cases resulted in clinical deterioration and death. The incidence of arterial complications was 0.2% of all catheterizations and angiocardiographies. The complications of heart and that due to catheter occurred in 0.8% of catheterization and 0.4% of angiocardiography respectively. Although the total incidence of major complications in this series was much the same as the reports from abroad,severe complications and death were considerably lower. It was believed that the improvements of catheterization technique and patient management contributed to a decreased mortality and morbidity.
2.Contentions on causal relationship between glucose variability and diabetic vascular diseases.

Diabetes is characterized by glycemic disorders which play a key role in the risk for complications of diabetes.Glucose variability is an important parameter of glycometabolism.The study of none evidence-based medicine(EBM) has investigated the relationship among glycemic variability and vascular injury and the subsequent development of diabetes complications.Due to deficiency of large clinical interventional tests,this relationship lack the evidence of EBM.In conclusion,particular attention should be paid to glucose variability in our clinical work and meanwhile the EBM studies should also be conducted.

AIM: To investigate the relationship between the effects of benazepril and plasma neuropeptide Y in renal hypertensive patients with chronic renal failure (CRF). METHODS: The levels of plasma NPY concentration, SBP, DBP and SCr were measured before and after four week treatment with benazepril in thirty renal hypertensive patients. Thirty blood donors from healthy subjects were taken as controls. In the past 2013,American Diabetes Association (ADA),American Association of Clinical Endocrinologists(AACE),Chinese Society of Endocrinology,etc.published some guidelines and consensuses on topics such as clinical diagnnsis,treatment,integrated management,and the relationship between reducing body weight and complications.Characters of blood pressure control in diabetic patients were also announced by the Eighth Joint National Committee (JNC 8).Meanwhile,Food and Drug Administration (FDA) declared the statements on safety of several hypoglycemic agents.These guidelines and consensuses will play key roles in standardizing clinical diagnosis,treatment,and medication.

6.CLINICAL ANALYSIS OF 75 CASES WITH THE DOUBLE-CHAMBERED RIGHT VENTRICLE

Assess retrospectively 75 cases of double chambered right ventricle confirmed by clinical examination and operation. The diagnosis and differential diagnosis were made by physical signs, chest X rays, electrocardiograph, echocardiogram, catheterization and right ventricular cardiograph features. Results showed that there were only 9 cases of simple form among 57 (76 %) cases with double chambered right ventricle before operative confirmation,and in 48 (64 %) cases it was accompanied by other intracardiac malformation. Eighteen (24%) cases were not diagnosed preoperatively. In order to raise the diagnotic rate, it was important to measure the pressure gradient between the pulmonary artery and the inflow tract of right ventricle, and evaluate carefully the right ventricular size and morphology by angiocardiography.

Objective To observe the effect on IL-2,TNF,TGF in uterus and serum,and IL-2RmRNA in spleen of menopausal rats with preventive accupuncture on Guanyuan point,and analysis the mechanism.Methods 112 ten month-aged SD female rats examined by vaginal smear were chosen and divided into seven groups.And another 16 three point five month-aged SD female rats were another group.Radio-immunity and hybridiztion in situ methods were applied to observe the effect on IL-2,TNF,TGF in uterus and serum and IL-2R mRNA in spleen of 12,14 and 16 month-aged menopausal rats with preventive accupuncture on Guanyuan point(twice a week,eight weeks continued) in 10th month.Results Compared with the normal group,IL-2,TGF in uterus and serum and TNF in uterus of menopausal rats reduced,TNF in serum came down in the 12th month and went up in 14th and 16th month,IL-2R mRNA in spleen decreased.Compared with the same-aged model group,IL-2,TGF of serum and uterus and TNF in uterus increased.TGF in serum of 14 and 16 month-aged group,IL-2 in uterus of 12 and 16 month-aged group,TNF in uterus of 12 month-aged group and TGF in uterus of 12 and 16 month-aged group rised significantly(P
8.Accuracy Assess of Five Criterims of Electrocardiogram for Locating Accessory Pathways in Patients With Wolff-Parkinson-White Syndrome

The resting 12 lead electrocardiograms (ECG) of 45 patients with Wolff-Parkinson-White syndrome and 30 normal persons were assessed by five criterias for locating the accessory pathway. Golden criteria was established by site of accessory pathway confirmed during epicardial mapping and absent accessory pathway from normal person. Compared five criterias of EGG with golden criteria, the idiosyncrasy of every criteria was 100% and the sensitivity of Rosenbaum's cri-terias, Gallagher's, WHO's, Lindsay's, Reddy's were respectively 93. 3% , 71. 1% , 64. 4% , 60% , 53. 3%. The preliminary feature for locating the accessory pathway of ECG was put forward from advantages of five criterias and observation of this report data.
9.Approach to the patients with thyroid nodules

[Summary] Thyroid nodules are the most common endocrine and metabolic diseases,and most of them are accidental nodules.How to distinguish the state between benign and malignant thyroid nodules is the clinical core problems in diagnosis and treatment.In addition to detailed history collecting and physical examination,highresolution ultrasound,serum thyrotrophin levels,and fine needle aspiration biopsy (FNAB) form the basis for the diagnosis of thyroid nodules.Endocrinologists should grasp the indications of FNAB.When the pathological finding is unable to distinguish between benign and malignant nodules,molecular or genetic diagnosis should be adopted for further diagnosis.Appropriate treatment is selected according to the definitive results.In this article,we share the whole process that one case with thyroid nodule was diagnosed as papillary thyroid carcinoma and eventually treated with surgery,so as to make clinicians pay more attention to thyroid nodules with standardized clinical diagnosis and treatment.
10.Clinical assessment of transcatheter closure of patent ductus arteriosus with severe pulmonary hypertention using Amplatzer occluder

Objective To evaluate the application of transcatheter closure of patent ductus arteriosus (PDA) with severe pulmonary hypertention using the Amplatzer occluder device.Methods Fifty-one cases of PDA with severe pulmonary hypertention were treated by transcatheter closure with Amplatzer occluder. Patients mean age was 9.4 years (ranging 3 months to 60 years) and the mean weight was (18.7?13.8) kg (ranging 5.0 to 65.0 kg). The mean PDA diameter at its narrowest segment was (7.0?2.4) (ranging 3.0 to 15.0) mm. The achievement of permanent transcatheter closure was decided according to the change of the pulmonary arterial pressure, aortic pressure and oxygen saturation. Results The devices were successfully placed in all patients except one failure owing to the resistance of pulmonary hypertention. The systolic pulmonary pressure decreased from (84.7?13.5) (range 70 to 137) to mmHg to (46.1?14.9) (24 to 109)mmHg, and the mean pulmonary pressure decreased from (65.0?11.5) (42 to 97) mmHg to (31.3?11.6) (14 to 69) mmHg. Complete angiographic closure was seen 10 minutes after the device deployment in 30 out of 50 patients (60%), while trivial to small leak was present in 20 (40%). Complete echocardiographic closure was demonstrated in 49 out of 50 patients (98%) at 10 min, and 100% at 6-month follow-up in all patients. There were no PDA recanalization and migration of devices after the complete occlusion during following up.Conclusion Transcatheter closure of patent ductus arteriosus with severe pulmonary hypertention by using the Amplatzer occluder is a safe and effective interventional method with excellent short-term and middle-term results.
