In normal rat forebrain, the NR1/NR2A and NR1/NR2B dimmers are the main constitutional forms of NMDA receptors. The present study was carried out to determine the functional properties of the heteromeric NMDA receptor subunits and their inhibition by bis(7)-tacrine (B7T). Rat NR1, NR2A and NR2B cDNAs were transfected into human embryonic kidney 293 cells (HEK-293). The inhibition of NMDA-activated currents by B7T was detected in HEK-293 cell expressing NR1/NR2A or NR1/NR2B receptors by using whole-cell patch-clamp techniques. The results showed that in HEK-293 cells expressing NR1/NR2A receptor, 1 μmol/L B7T inhibited 30 μmol/L NMDA- and 1000 μmol/L NMDA-activated steady-state currents by 46% and 40%, respectively (P>0.05; n=5), suggesting that the inhibition of B7T on NR1/NR2A receptor doesn't depend on NMDA concentration, which is consistent with a non-competitive mechanism of inhibition. But for the NR1/NR2B receptor, 1 μmol/L B7T inhibited 30 μmol/L NMDA- and 1000 μmol/L NMDA-activated steady-state currents by 61% and 13%, respectively (P<0.05; n=6), showing that B7T appears to be competitive with NMDA. In addition, simultaneous application of 1 μmol/L B7T and 1000 μmol/L NMDA produced a moderate inhibition of peak NMDA-activated current, followed by a gradual decline of the current to a steady state. However, the gradual onset of inhibition produced by B7T applied simultaneously with NMDA was eliminated when B7T was given 5 s before NMDA. These results suggested that B7T inhibition of NMDA current mediated by NR1/NR2B receptor was slow onset, and it did not depend on the presence of the agonist. With holding potentials ranging from -50 to +50 mV, the B7T inhibition rate of NMDA currents didn't change significantly, and neither did the reversal potential. We are led to conclude that the NR1/NR2B recombinant receptor can serve as a very useful model for studying the molecular mechanism of NMDA receptor inhibition by B7T.

The resting 12 lead electrocardiograms (ECG) of 45 patients with Wolff-Parkinson-White syndrome and 30 normal persons were assessed by five criterias for locating the accessory pathway. Golden criteria was established by site of accessory pathway confirmed during epicardial mapping and absent accessory pathway from normal person. Compared five criterias of EGG with golden criteria, the idiosyncrasy of every criteria was 100% and the sensitivity of Rosenbaum's cri-terias, Gallagher's, WHO's, Lindsay's, Reddy's were respectively 93. 3% , 71. 1% , 64. 4% , 60% , 53. 3%. The preliminary feature for locating the accessory pathway of ECG was put forward from advantages of five criterias and observation of this report data.

Objective To evaluate the clinical efficacy of percutaneous vertebroplasty (PVP) with flexible bone cement injector through unilateral puncture. Method Unilateral puncture line PVP treatment for thoracolumbar vertebral compression fracture was conducted in 78 patients, which was divided into observation group and control group according to the use of different equipment. The flexible bone cement injector was used in 36 cases of observation group, which injected the bone cement into three places at the vertebral body. The ordinary straight bone cement injector was applied in the 42 cases of the control group. The operation time, the time of radiation exposure, and the amount of bone cement injection were recorded, and the leakage of bone cement was observed. The visual analogue scale (VAS), relative vertebral height and Cobb angle were recorded at 1 week, 1 and 2 years follow-up. The time of operation, the time of radiation exposure, the amount of bone cement injection, the incidence of bone cement leakage, VAS, relative height of vertebral body and Cobb angle were compared between the two groups using independent samples t test orχ2 test. Result All patients in the two groups were successfully treated by surgery. There were no significant differences between the observation group and the control group in the operation time and the time of radiation exposure and the incidence of bone cement leakage (P>0.05). The injection amounts of bone cement in the observation group and the control group were (5.6 ± 0.7) and (3.2 ± 0.6) ml, with statistically significant difference (P<0.05). There were no significant differences in VAS score, relative vertebral height and Cobb angle between the two groups at 1 week after operation (P>0.05). At 1 year and 2 years after operation, the differences of the above indexes between the two groups were significantly different (P<0.05), with the observation group being superior to the control group. Conclusions Application of flexible bone cement injector for PVP operation is safe and feasible. Compared with the traditional straight bone cement injector, the use of flexible bone cement injector will not increase the operation time, radiation exposure time and the incidence of bone cement leakage but will improve the long-term effect.

Objective To study the relationship between the cardiac markers in ST-segment elevation myocardial infarction (STEMI) and quality of life (QOL) six months later.Methods N-terminal pro-B-type natriuretic peptide (NT-proBNP),cardiac troponin I (cTnI),creatine kinase isoenzyme-MB (CK-MB)and myoglobin(Myo) was tested in 76 patients with STEMI on admission(0 h) and 24 h after admission.QOL six months later was evaluated by using the SF-36 health survey questionnaire.The relationship between the cardiac markers and QOL was analyzed by multivariate linear regression analysis.Results The total SF-36 scores and the scores of physical function,body pain and general health showed negative linear correlation with NT-proBNP on admission (0 h) and 24 h after admission (P ＜ 0.05).The total SF-36 scores and the scores of physical function and body pain showed negative linear correlation with cTnI on admission (0 h)and 24 h after admission (P ＜ 0.05).The total SF-36 scores and the scores of body pain showed negative linear correlation with CK-MB on 24 h after admission (P ＜ 0.05).The scores of vitality showed negative linear correlation with Myo on admission (0 h) (P ＜ 0.05).Conclusion The higher levels of NT-proBNP,cTnl and CK-MB indicate the lower QOL six months later.

Objective To evaluate the effects of dantrolene pretreatment on diaphragmatic function in septic rats .Methods Thirty adult male Sprague-Dawley rats , weighing 200-220 g , aged 9-10 weeks , were randomized into 3 groups (n=10 each) using a random number table :sham operation group (group S) ,spesis group (group CLP) and dantrolene group (group D) .The animals were anesthetized with pentobarbital sodium . Dantrolene 6 mg/kg was injected intraperitoneally (in dimethyl sulfoxide 500 μl ) .Sepsis was induced by cecal ligation and puncture 1 h later in CLP and D groups .The left and right diaphragm was rapidly excised at 24 h after cecal ligation and puncture . The left diaphragm was used to detect the systolic function including the single stimulation twitch , dmax/dt , dmin/dt , maximal force of tetanic contraction , force-frequency curves , and fatigue index .Results Compared with group S ,the single twitch myopalmus ,dmax/dt ,dmin/dt ,maximum peak tension and fatigue index were significantly decreased in group CLP ,the single stimulation twitch ,dmax/dt ,dmin/dt and maximal force of tetanic contraction were decreased , fatigue index was increased in group D , and the force-frequency curve was shifted downward in CLP and D groups ( P<0.05 or 0.01) .Compared with group CLP ,the single stimulation twitch ,dmax/dt ,dmin/dt ,maximal force of tetanic contraction and fatigue index were significantly increased in group D ( P< 0.05 or 0.01 ) .Conclusion Dantrolene pretreatment can improve diaphragmatic function in septic rats .

To explore the effects of psychological behavior training on the physical stress level of submariners, 42 submariners were divided into two groups, namely experimental group and control group. The submariners in the former group received psychological behavior training for one week. The level of physical stress was determined after being asked to creep through water-filled torpedo launching tube before and after training. Objective To analyze the clinical and pathological features of lipid storage myopathy (LSM) caused by primary carnitine deficiency (CD).Methods The clinical data of 4 cases of possible LSM caused by primary CD were analyzed retorspectively.Results The clinical features of the 4 patients were subacute or chronic onset, proximal muscle weakness and exercise intolerance. Elevated levels of creatases were measured in serum and myogenic damage was found by EMG examination. Frozen sections of muscle biopsy samples showed many fibers contained numerous vacuoles which was stained by oil red O. No ragged red fiber (RRF) was seen in MGT stain. Type I fibers were more severely affected. In electron microscopy, the prominent abnormality was the presence of excessive amounts of fatty droplets in muscle fibers and subsarcolemmal regions with mild increased mitochondria.Treatment with glucocorticoid and energy supplement had been clinically beneficial. Conclusions Fatigue and muscle weakness are prominent manifestations in LSM caused by primary CD. The main changes are accumulation of lipid droplets in muscle specimen without prominent abnormality in structure in mitochondria. Good clinic effect may be caused by therapy with glucocorticoid.

As the preferred treatment for most patients with hemodynamic instability in the intensive care unit (ICU),the fluid resuscitation can be limited by the cardiac function,that makes the fluid treatment has no responsiveness and may cause the risk of pulmonary edema increase.So,good fluid responsiveness is the basic condition to make fluid therapy successfully.For those patients,in order to lower the risk and correctly evaluate the fluid responsiveness,there are a large number of studies direct at the static and the dynamic pre-load parameters in animals and clinical patients.Those studies show that how to avoid the blindness of fluid expansion,accurately and effectively evaluate and forecast the fluid responsiveness.

Objective To observe the effect on IL-2,TNF,TGF in uterus and serum,and IL-2RmRNA in spleen of menopausal rats with preventive accupuncture on Guanyuan point,and analysis the mechanism.Methods 112 ten month-aged SD female rats examined by vaginal smear were chosen and divided into seven groups.And another 16 three point five month-aged SD female rats were another group.Radio-immunity and hybridiztion in situ methods were applied to observe the effect on IL-2,TNF,TGF in uterus and serum and IL-2R mRNA in spleen of 12,14 and 16 month-aged menopausal rats with preventive accupuncture on Guanyuan point(twice a week,eight weeks continued) in 10th month.Results Compared with the normal group,IL-2,TGF in uterus and serum and TNF in uterus of menopausal rats reduced,TNF in serum came down in the 12th month and went up in 14th and 16th month,IL-2R mRNA in spleen decreased.Compared with the same-aged model group,IL-2,TGF of serum and uterus and TNF in uterus increased.TGF in serum of 14 and 16 month-aged group,IL-2 in uterus of 12 and 16 month-aged group,TNF in uterus of 12 month-aged group and TGF in uterus of 12 and 16 month-aged group rised significantly(P

Objective To evaluate the therapeutic effect of transanal local excision in the treatment of low rectal cancer.Methods The data of 49 cases of low rectal cancer patients treated by transanal local excision (group A,37 cases of T1 patients and 12 cases of T2 patients) were reviewed retrospectively,and compared with the group of 45 cases treated by radical resection( group B,11 cases of T1 patients and 34 cases of T2 patients).The 5-year survival rates,recurrence rates and postoperative complications between the two groups were compared.Results The incidence of postoperative complications in group A was significantly lower than that in group B(4.1％ vs 15.6％,x2 =4.69,P ＜0.05 ).The 5-year survival rate of group A was 97.9％ and the 5-year survival rate of group B was 95.6％.There was no significantly statistical difference on the 5-year survival rate between the two groups( x2 =0.439,P ＞ 0.05 ).But for the T2 patients,the recurrence rates in group A was 33.3％,which was significantly higher than the T2 patients in group B(5.9％ ) ( x2 =5.89,P ＜0.05 ).All patients with low histopathological grade tumor in group A showed postoperative recurrence and metastasis.Conclusion Transanal local excision with a rigid criteria can get good curative effect in the T1 patients,but not suitable for the T2 patients or patients with low histopathological grade tumor.
