A 36-year-old female with a high-grade fever and epigastric abdominal pain was prescribed antibiotics, but developed hypoxia and dyspnea. An echocardiography revealed diffuse hypokinesis and massive pericardial effusion, after which diagnostic cardiac catheterization and an endomyocardial biopsy (EMB) were peformed to reveal fibrosis and infiltration of inflammation cells composed primarily of neutrophils. Clinical manifestation of a spiking fever, leukocytosis, elevated ferritin levels, skin rash and EMB findings led to a diagnosis of adult-onset Still's disease (AOSD) with acute myocarditis. Pulse therapy of intravenous methylprednisolone was performed for three days, followed by a daily dose of prednisone (60 mg). After a course of steroid therapy for fever and pericardial effusion, and conducting a left ventricular ejection fraction, the patient showed improvement and was discharged asymptomatic within 32 days of admission. This study is the first to report on a case of myocarditis in AOSD diagnosed by neutrophil infiltration in the myocardium.

PURPOSE: The purpose of this study was to evaluate the efficacy of two different metal conditioners for non-precious metal alloys for the bonding of porcelain to a cobalt-chromium (Co-Cr) alloy. MATERIALS AND METHODS: Disk-shaped specimens (2.5x10.0 mm) were cast with Co-Cr alloy and used as adherend materials. The bonding surfaces were polished with a 600-grid silicon carbide paper and airborne-particle abraded using 110 microm alumina particles. Bonding specimens were fabricated by applying and firing either of the metal conditioners on the airborne-particle abraded surface, followed by firing porcelain into 5 mm in diameter and 3 mm in height. Specimens without metal conditioner were also fabricated. Shear bond strength for each group (n=8) were measured and compared (alpha=.05). Sectional view of bonding interface was observed by SEM. EDS analysis was performed to determine the chemical elements of metal conditioners and to determine the failure modes after shear test. RESULTS: There were significant differences among three groups, and two metal conditioner-applied groups showed significantly higher values compared to the non-metal conditioner group. The SEM observation of the sectional view at bonding interface revealed loose contact at porcelain-alloy surface for non-metal conditioner group, however, close contact at both alloy-metal conditioner and metal conditioner-porcelain interfaces for both metal conditioner-applied groups. All the specimens showed mixed failures. EDS analysis showed that one metal conditioner was Si-based material, and another was Ti-based material. Si-based metal conditioner showed higher bond strengths compared to the Ti-based metal conditioner, but exhibited more porous failure surface failure. CONCLUSION: Based on the results of this study, it can be stated that the application of metal conditioner is recommended for the bonding of porcelain to cobalt-chromium alloys.

High-flow nasal cannula oxygen therapy (HFNC) in palliative care is mentioned in several guidelines, however, the indication for this procedure has not been established yet. At our department, HFNC has increasingly been adopted for end-stage cancer patients when their dyspnea needs to be alleviated. This is a case report on three patients treated with this procedure at our department. Although they had severe dyspnea with respiratory failure, their daily activities, such as enjoying meals and conversation with their families, were improved with HFNC. It can be a treatment of choice to maintain and improve patients’ quality of life (QOL) in palliative setting, where benefits and risks should be considered for each patient.

The purpose of this study was to examine epidemiology of injury in a male collegiate volleyball team in Japan during 2019 season. Thirty-six collegiate male volleyball players were followed during 2019 season. Injuries that required more than 24 hours to recover were classified as Time Loss (TL) injury, and those that required less than 24 hours were classified as Non-Time Loss (NTL) injury. Exposure was calculated by total number of players and hours of participation. Then, we computed injury rate per 1000 Player Hour (PH) and 1000 Athlete Exposure (AE) as outcome measures. A chi-square test was utilized to statistically compare. Eighteen injuries occurred and the overall injury rate was 1.28/1000PH and 2.05/1000AE, respectively. Of which, 11 cases were NTL injuries (0.78/1000PH, 1.26/1000AE) and 7 cases were TL injuries (0.50/1000PH, 0.80/1000AE). Within the 7 TL injuries, 5 (0.36/1000PH, 0.57/1000AE) were of minor severity. Most TL injury were classified as chronic in nature (n=5, 0.36/1000PH, 0.57/1000AE). There was no statistically significant difference between injury rate of match (n=2, 1.33/1000PH, 2.88/1000AE) and training (n=16, 1.27/1000PH, 1.98/1000AE). The most frequently injured body part in training was lower back (n=7, 0.56/1000PH, 0.87/1000AE). In match, highest injury rate was observed in ankle (n=2, 1.33/1000PH, 2.88/1000AE). In conclusion, our results showed relatively low injury rate, and majority of reported injuries were categorized as minor severity. Our study also demonstrated injury characteristics of Japanese male collegiate players that lower back injuries were prevalent in training, and acute ankle injury rate was highest in match.

A 62-year-old man presented with hemoptysis and hoarseness. He was diagnosed with an aortopulmonary fistula due to a thoracic aortic aneurysm rupture and was referred to our department. Emergency in-situ reconstruction of the aorta and pulmonary lobectomy were performed. Nine days postoperative, he developed empyema. Intrapleural urokinase and antibiotic therapy were selected as management instead of a video-assisted thoracoscopic debridement and decortication due to his worsening condition. The treatment was successful, and he was discharged from the hospital without any further complications. This study highlights the benefit of intrapleural administration of urokinase and antibiotics in patients with acute empyema, when surgical treatment is inappropriate.
