Chinese Journal of Nosocomiology 2009;0(16):-
OBJECTIVE To investigate drug-resistance status of Gram-positive cocci in the local region and offer scientific basis for reasonable usage of antibiotics.METHODS 336 strains of Gram-positive cocci were isolated from all kinds of specimens from Jan 2003 to Dec 2007 in the local hospital.Organisms were cultivated and identified complying with National Operation Procedure of the Clinical Laboratory.Drug sensitivity test was performed with K-B methods.RESULTS Among the 336 strains,Staphylococcus aureus(SAU) ranked the top, accounting for 45.8%;then the coagulase-negative Staphylococcus(CNS) accounting for 23.2%;Enterococcus faecalis ranked the third, accounting for 18.2%;the detection rate of meticillin-resistant Staphylococcus aureus(MRSA) was 31.8%;that of meticillin-resistant coagulase-negative Staphylococcus(MRCNS) was 35.9%.The results of drug sensitivity test showed the Gram-positive cocci were resistant to the many antibiotics in different degrees except for 100% sensitive to vancomycin.CONCLUSIONS Hospital should attach importance to supervise the drug-resistance of Gram-positive cocci and prevent the dissemination and epidemic of drug resistant strains.

Objective To study the relationship between malnutrition and respiratory muscle strengthas well as the effect of diet supplement on respiratory muscle strength in elderlypatients with chronic cor pulmonale in high altitude areas(HAECCP). Methods 46 patients with HAECCP not in exacerbation were divided into two groups, namely, diet supplement group (23 cases) and control group (23 cases). The caloric intake was increased by 45%-50% in the diet supplement group. Ten parameters with regard to body weight, muscle strength and respiratory function were measured in all the patients. These parameters included percentage of ideal body weight (%IBW), triceps skinfold thickness(TSF), midarm muscle circumference (MAMC), maximal inspiratory pressure(PImax), maximal expiratory pressure (PEmax),maximal transdiaphragmatic pressure(Pdimax),forced expiratory volume in 1 second (FEV_1) ,expressed as percentage predicted (FEV_1%), ratio of FEV_1 to forced vital capacity (FEV_1/FVC), partial pressure of arterial O_2 (PaO_2) and partial pressure of alveolar CO_2(PaCO_2). All the parameters were measured again after 4 weeks. Results There was no significant differece between the parameters in the two groups before the treatment. After 4 weeks of treatment, there was significant improvement in %IBW, serum albumin (Alb), TSF, MAMC, PImax,PEmax,Pdimax in the diet supplement group, indicating the effect of increased nutrition on body weight and respiratory muscle strength. However, no significant difference was found in the parameters of pulmonary function and blood gas analysis after diet supplement. NO change was observed in the parameters of the controls after 4 weeks. Conclusions In stable patients with HAECCP not in exacerbation, the strength of respiratory muscle decreased as a result of malnutrition. After diet supplement, the strength of respiratory muscle increased with accompanying improvement in nutritional status.

Objective To build an architecture for medical information integration based on SOA(Service-Oriented Architecture)by analyzing the characters of medical information integration and the integration technology of SOA. Methods Virtual database was built by XML Schema mapping for conform share of medical data based on XML technology. The method and principle of Web Services for medical information were researched and built. Results Medical information systems could uniformly exchange information based on standards of Web Services, and the conform medical information share mode was realized. Conclusion The integration of medical information systems based on SOA not only has the characteristics of loose-coupling and customization agility but also makes information share possible because of conform mode.

Objective To measure the organ weights , physiological and biochemical parameters and immune cells of NOD/SCID mice. Methods Mice at five and ten weeks of age were selected .The organ weights, blood physiological and biochemical parameters were observed .The percentages of CD 3+, CD4+, CD8+, CD19+, B220+, NK1.1+, and CD11b+were checked by FCM in NOD/SCID mice at six week of age in terms of its T , B lymphocyte function and NK cell activity. Results Among the same sex group of NOD/SCID mice, the weights of kidney, liver, heart and lung, and the blood physiological indexes of RBC , HGB, HCT, MCV, MCH, RDW, and the blood biochemical indexes of TP , ALB, ALP, CHO, TBIL show significant difference between 5 and 10 weeks. At the same age, HCT, GLOB, A/G, CHO, TG, TBIL and the UN are significantly different between male and female NOD /SCID mice.NOD/SCID mice lack T cells (0.37 ±0.26)%、CD4+T cells (0.35 ±0.13)%、CD8 +T cells (0.47 ±0.10)%、CD19+B cells (0.13 ± 0.05)%、B220+B cells (1.20 ±0.44)%.The percentage of NK cells is (6.90 ±0.82)%, and the percentage of granulocytes is (47.88 ±15.54)%.Conclusions The study indicates that NOD/SCID mice show the deficiency of T , B and NK cells function .The organ weights , blood physiological and biochemical parameters are affected by age and gender . The studied parameters of NOD/SCID mice are similar with the same strain in other countries .

Academic Journal of Second Military Medical University 2005;26(6):675-680
Objective: To evaluate the efficacy of the third-generation instrumentation including TSRH, CD and ISOLA in the treatment of adult scoliosis. Methods:Thirty-five adult patients with idiopathic or degenerative scoliosis who received treatment with third-generation instrumentation (TSRH,CD and ISOLA) between July 1999 to January 2003 were retrospectively reviewed. The mean preoperative cobb angle of major curves of the frontal plane was 58.1°(42°-95°). The patients received a combined anteroposterior approach or a single posterior procedure. The mean follow-up time was 20 months(10-48 months). Preoperative and postoperative Cobb angles of the frontal plane and sagittal plane and the distance between C7 and CVLS were measured. The subjective assessment was judged by questionnaire. Results: Postoperative clinical appearance of all patients improved significantly. Mean correction of major curves of the coronal plane was 53.2%. Mean loss of correction of the coronal plane in the last follow-up was 4.3°. The distance between the midline of C7 and CVSL was corrected from 2.6 cm to 0.24 cm. The results of follow-up showed that 89.3% patients were satisfied with the outcome. Pneumatothorax and haematothorax occurred in 2 patients. Three patients still complained of low back pain one year after operation because of adjacent degeneration in 2 patients and pseudoarthrosis in the remaining 1 patient. Conclusion: Imageologic findings and subjective assessment of the patients showed that the third-generation instrumentation can achieve good correction and trunk balance in the treatment of adult scoliosis with fewer complications.
