1.Present situation and prospect of Da Vinci robot-assisted pancreatic surgery

With the continuous improvement of the diagnosis rate of benign pancreatic disease and the younger age of disease onset,the demand for minimally invasive pancreatic surgery is increasing,meanwhile,how to keep the normal pancreatic tissue as much as possible to reduce the impact on the patients' life has gained surgeons' thinking.The Da Vinci robotic surgical system,providing a clearer 3D vision and more accurate operation,makes some difficult minimally invasive pancreatic surgery such as pancreaticoduodenectomy can be carried out extensively.Based on clinical experiences and related literatures,this paper will analyze the present situation of Da Vinci robot-assisted pancreatic surgery and give prospects.

3.Saddle prostheses in the treatment of resecting pelvis sarcoma around the acetabulum to salvage limbs of 12 cases

Twelve cases of pelvis sarcoma around acetabulum were selected from Department of Orthopaedics, Wuxi Second Hospital, Nanjing Medical University between January 2001 and December 2007 including 5 males and 7 females, aged 51-72 years. According to pathological diagnosis, there were 3 cases of sarcoenchondroma, 2 cases of osteosarcoma, 3 cases of giant cell tumor of bone, 2 cases of adenocarcinoma, and 2 cases of osteoblastoma. According to tumor position by Enneking dividatur sectors, there were 5 cases of Ⅰsector, 4 cases of Ⅱsector, and 3 cases of Ⅲ sector. The Harris score of pre-replacement was 88.5?8.8. All cases were treated by saddle prosthetic replacement successfully, and no accidents happened. The operation time was 2.5-3.5 hours; blood volume was (1 250?407) mL. Twelve cases were followed up for 18 months (range 5-36 months). One case died of tumor metastasis and body exhaustion 5 months postoperation. Infection of incision took place in 1 case, and cured by operation with antibiotic combined with bone cement chain pearls. The Harris scores 4 months post-replacement: 6 cases were between 40-49, 3 cases between 50-59, and 3 cases between 60-69. Twelve cases could walk with double orthosis 3 months postoperation, and 3 cases could walk with single orthosis 6 months postoperation; 1 case walked with near normal walking, but major walking was bad. No artificial joint loosening or dislocation occurred. Although the joint function of major cases treated by saddle prosthetic replacement was not fairly satisfactory, it is a better treating method for malignant tumor whose limbs can be reserved and live their daily life by orthosis.
4.Investigation of Sanitary Status of Waterworks for Centralized Water Supply in Anhui Province

Objective To know the sanitary status of waterworks for centralized water supply in Anhui province,and try to enhance the implementation Standards for Drinking Water Quality (GB 5749-2006).Methods From April to November in 2008,the hygienic status and water quality self-test capacity of the waterworks at city,county,enterprise and town levels were investigated.Results As for the source water,over 70% of waterworks were up to the Grade Ⅲ standard.In the hygienic management and water treatment process,the city level water workswere better.The rates of complete water treatment were 80.63%,73.78%,58.12% and 60.46%,respectively for different level water works.The rates of the water quality test equipments fulfilled were 77.77%,42.99%,38.18% and 13.77% respectively.For water quality test,the 119 waterworks only focused on the aesthetic,general chemical and microbiological indexes.The performance rates of toxicological indicators,disinfectants indicators,radioactive indicators and other indicators were 17.65%,26.68%,15.97% and 3.49%,respectively.Conclusion Hygienic management of waterworks of levels for centralized water supply in Anhui province needs to be strengthened,and the water treatment process needs to be improved,the capacity building for self-test of water quality in the waterworks needs to be enhanced.
5.Clinical analysis of diagnosis and surgical treatment of pulmonary sequestration

Objective To summarize the experience in the diagnosis and surgical treatment of pulmonary sequestration. Method The clinical data from 15 patients with pulmonary sequestration underwent surgical operation from July 2000 to January 2009 were reviewed and analyzed retrospectively. Results Pulmonary sequestration was diagnosed in 14 patients before the operation with the diagnosis rate of 93.33% (14 /15).Preoperation plane and contrast chest CT scan were performed in all cases,and 9 patients performed angiography reconstruction. Twelve patients with intralobar sequestration underwent lobectomy and 3 patients with extralobular sequestration underwent local lesion resection. Abnormal supply arteries were intraoperatively found in 14 patients originating from the thoracic main artery, 1 patient from the eeliac artery. Smooth recovery was achieved in all patients. No perioperarive death occurred. Conclusions Pulmonary sequestration is a rare disease with high misdiagnosis rate in some patients. Contrast enhanced CT scan and angiography may improve the diagnosis of pulmonary sequestration. Operation is a safe and effective method for the treatment of pulmonary sequestration.
6.Minimally invasive Nuss procedure for pectus excavatum repair in 7 patients

This study retrospectively analyzed 7 patients with severe pectus excavatum who underwent thoracoscope-assistant Nuss procedure from July 2006 to June 2008.All the participants were followed up for 1 to 24 months(mean,15.7 months).No intraoperative or postoperative complications were found in any of the patients.The operating time was 40 to 55 minutes.Mean hospital stay was 5 days (range,4 to 7days).Our experience with the minimally invasive repair of pectus excavatum might be encouraging.thoracoscope-assistant Nuss could effectively treat pectus excavatum in children,associated with minimal invasion,lower morbidity,less hospital stay,and considerable improvement in physiologic and psychological function.
8.Dynamic hip screw plates assisted with blocking screws for treatment of intertrochanteric fracture

Objective To investigate effects of dynamic hip screw (DHS) fixation assisted with blocking screws in treatment of intertrochanteric fracture.Methods A total of 150 patients with intertrochanteric fracture were selected as the combined treatment group,of which the patients managed by DHS intemal fixation were followed by the parallel placement of a blocking screw closely beneath the main screw for stability with its end rested on the plate.Another 87 patients managed by DHS fixation alone were determined as the control group.The surgical outcomes of the two groups were analyzed and compared.Results Good fixation without external displacement of femoral neck and cut-out of the main screw in femoral head was achieved in the combined treatment group.Also,fracture nonunion or death did not exist in the combined group.The patients in combined treatment group could take care of themselves after operation in the absence of severe pain and functional disturbance,with higher excellence rate (98.7%)than that in control group (P < 0.01).Whereas,the operation failure rate was significantly lower in combined treatment group than in control group (P < 0.05).Conclusion DHS fixation assisted with blocking screws is an ideal treatment of incomplete intertrochanteric fracture in medial wall of calcar femorale for it can achieve reliable fixation and definite curative effect,favor fracture healing,early functional exercise and low incidence of postoperative complications.
9.Aptamers as molecular probes in tumor molecular imaging

The ideal molecular imaging probe should have characteristics of specificity,sensitivity and safety.Aptamers are sing-strand DNA or RNA oligonucleotides which can bind their targets with high specificity and high affinity.Aptamers are new types of molecular imaging probe because they have many advantages,such as their large sum of possible targets,small molecule weights,easily production and modification,low immuno-genicity,high tissue penetration and so on.Now more and more aptamers research on tumor molecular imaging provides a new technology and tool for tumor diagnosis and therapy.
10.Posterior pedicle screw reduction and fixation, decompressive laminectomy and massive autogenous bone grafting for treatment of lumbar spondylolisthesis

Objective To investigate the effect of posterior pedicle screw reduction and fixation,decompressive laminectomy and massive autogenous bone grafting using bone particles harvested from resected vertebral body in treatment of lumbar spondylolisthesis.Methods Fifty-six cases of lumbar spondylolisthesis treated from June 2005 and June 2006 were reviewed retrospectively.According to Meyerding classification,35 cases were classified as grade Ⅰslippage and 21 cases as grade Ⅱ slippage.There were 19 spondylolisthesis at L4/5 and 37 at L5-S1.After posterior pedicle screw reduction and fixation,decompressive laminectomy and massive autologous bone grafting using particles of resected vertebral body,surgical outcome was analyzed.Results All cases achieved bone fusion without evidence of nail breakage or nerve injury.According to Macrab criteria,the results were excellent in 53 cases and good in three.Moreover,both the full reduction rate and interbody fusion rate were 100%.Conclusion Posterior pedicle screw reduction and fixation,decompressive laminectomy plus massive autogenous bone grafting is one of the ideal treatments for lumbar spondylolisthesis,for it achieves satisfactory fixation,decompression,reduction and fusion.
