1.Patient preference for Shexiang Baoxin Pill versus extended release Isosorbide 5-Mononitrate in elderly diabetic patients with angina pectoris

AIM: To gain an insight into preference for elderly diabetic patients with angina pectoris between Shexiang Baoxin Pill (SXBXP) (Moschus, Radix Ginseng, Calculus Bovis, etc.) and Extended Release Isosorbide 5-Mononitrate (ERIM). METHODS: Followed a two-week run-in, 56 elderly patients were randomized to take SXBXP 2 pills tid or ERIM 60 mg qd for 4 weeks; then followed a two-week wash-out, patients crossed over to the complementary therapy for another 4 weeks. Daily diary cards were used to record the side effects, the frequency of angina pectoris attacks and use of rescue medication. At the end of the study, participants were asked to complete a 3-item scale questionary to assess their angina pectoris treatment in terms of convenience, side effects and overall preference. RESULTS: No treatment difference in the frequency of angina pectoris attacks and use of rescue medication was found; 71.4% of patients thought use of SXBXP was the same convenient as that of ERIM; patients felt SXBXP had side effects fewer than ERIM and overall 75% of patients preferred SXBXP. CONCLUSION: Elderly diabetic patients with angina pectoris prefer SXBXP to ERIM.
2.Shenmai injection improves insulin resistance in 3T3-L1 cells

AIM:To discuss the effect of Shenmai injection on insulin resistance ( IR) in 3T3-L1 cells and its mechanisms.METHODS:3T3-L1 preadipocytes were induced by chemical reagents to differentiate into fully differentiated adipocytes.Oil red O staining was used to detect the differentiation level of the adipocytes .The insulin-resistant 3T3-L1 cell model was demonstrated using insulin , which was confirmed by glucose concentration in cell supernatant .The IR cell model was given 10 μmol/L rosiglitazone , 25 and 50 g/L Shenmai injection and normal saline for comparison .MTT assay was used to assess the cell activity of 3T3-L1 cells which was treated with drugs for 8, 16, 24 and 36 h.Glucose oxidase method was used to detect the glucose concentration in the cell supernatant at 8, 16 and 24 h.The protein levels of glucose transporter-4 (GLUT4), phosphatidylinositol 3-kinase (PI3K), AKT and p-AKT were determined by Western blot .RE-SULTS:3T3-L1 adipocytes were successfully induced as shown by the positive oil red O staining .The IR cell model was demonstrated , and glucose concentration in the cell supernatant after treatment with Shenmai injection showed that Shenmai injection reduced the IR in 3T3-L1 cell model.The protein levels of GLUT4, PI3K and p-AKT increased compared to con-trol group.CONCLUSION:Shenmai injection reduces the IR in 3T3-L1 cell model, which functions by increasing the protein levels of GLUT4, PI3K and p-AKT.
3.Selection of optimal length and diameter of mini implant in two different forces: a three-dimensional finite element analysis.

OBJECTIVETo investigate the effect of different length and diameters on the stability of mini implant and to select optimal length and diameter using continuous variation of parameters.
METHODSTo perform 3-dimensional finite element analysis, finite element models of a maxilla, and mini implants with length of 6-12 mm and diameters of 1.2-2.0 mm were generated. Load of two different forces were applied to the head of mini implant. One type was horizontal force (HF), the other was composite force (CF). The maximum equivalent stress (Max EQV) in maxilla and the maximum displacement (Max DM) of mini implant were evaluated.
RESULTSThe Max EQV in maxilla and Max DM of mini implant decreased as length and diameter increased. When length was more than 9 mm, the evaluation indexes were small and had a less change. Datas indicated that diameter played a more important role in reducing target, and was a more effective parameter in reducing Max EQV when CF was loaded.
CONCLUSIONFrom biomechanical point of view, the choice of the length should not be more than 9 mm. When CF is loaded using the mini implant, diameter exceeding 1.2 mm are optimal design for mini implant.

4.Correlation between central venous catheter-related staphylococcus epidermidis icaA, icaD, transforming growth factor beta 1 and biofilms in lung cancer patients

BACKGROUND: It is confirmed that pathogenicity of biomaterials-centered infection is positive correlated to bacterial biofilm formation of Staphylococcus epidermidis on the surface of catheter-related materials. OBJECTIVE: To analyze the relations between expressions of central venous catheter-related Staphylococcus epidermidis icaA, icaD, transforming growth factor β1 (TGF-β1) and formation of bacterial biofilm. METHODS: The type of Staphylococcus epidermidis in lung cancer cases with catheter-related bloodstream infections (CRBSI) was indentified, followed by bacterial genomic DNA extraction. The expression of biofilm formation-related genes icaA, icaD mRNA and phenotype of biofilm were detected by PCR. The serum TGF-β1 levels in cases with or without CRBSI were measured by enzyme-linked immunosorbent assay.RESULTS AND CONCLUSION: The expression of Staphylococcus epidermidis operon icaA and icaD gene was positive correlated to biofilm formation in lung cancer cases with CRBSI (P < 0.01); particularly, the TGF-β1 levels in CRBSI cases were greater than that of non-CRBSI cases (P < 0.05). The results demonstrated that, central venous catheter infection causes positive Staphylococcus epidermidis icaA and icaD gene expressions in lung cancer cases and is prone to form biofilm, high level of peripheral TGF-β1 may play a positive role in bacterial biofilm formation.
5.Clinical study of limited internal fixation contrasting micro?external fixator in the treatment of fractures around the hand

Objective To analyze the effect of limited internal fixation contrasting micro -external fixator in the treatment of fractures around the hand.Methods Sixty patients with fractures around the hand treated in the First People′s Hospital of Huizhou from May 2015 to May 2017 were selected and randomly divided into the internal fixation group and the external fixation group,and then were treated with effective internal fixation and mini external fixator respectively.The curative effect,operation condition,postoperative recovery and complications of the two groups were compared.Results The treatment effect(excellent and good rate was 96.67%(29/30)),fracture recovery time((6.37 ± 1.25)weeks),hospitalization time((4.32 ±1.23)d)and postoperative complication rate(10.0%(3/30))in the external fixation group were superior to those in the internal fixation group(76.67%(23/30),(8.87 ± 2.12)weeks,(7.29 ± 2.15)d,33.3%(10/30)),the differences were statistically significant(P=0.032;t=15.459,P=0.005;t=17.788,P =0.001;P=0.012).However,there were no significant differences between the external fixation group and internal fixation group in the operation time and the blood loss during operation((28.41±2.87)min vs.(27.67±1.42)min;(16.87 ± 3.71)ml vs.(16.43 ± 2.89)ml)(t=2.459,P=0.423;t=1.788,P =0.619). Conclusion Compared with limited internal fixation,the mini external fixator is reliable and effective,with less complications,and is more conducive to the early activity and functional recovery of the patients with hand fractures.
