1.Platelet-rich plasma with sodium hyaluronate in repair of rabbit knee osteoarthritis

BACKGROUND: Studies have shown that sodium hyaluronate inhibits cartilage damage in osteoarthritis and accelerates regeneration of cartilage cels, to stabilize and repair the articular cartilage.
OBJECTIVE:To investigate the therapeutic effect of sodium-rich plasma combined with platelet-rich plasma (PRP) on rabbit knee osteoarthritis.
METHODS:Forty New Zealand white rabbits were randomly divided into five groups, control group, combined group, sodium hyaluronate group, PRP group and model group, and then an osteoarthritis model of the right knee was made in each rabbit. After modeling, sodium hyaluronate+PRP, sodium hyaluronate, autologous PRP and normal saline were givenviathe knee joint cavity in the latter four groups, respectively, once a week for 5 weeks. The control group received no treatment, as normal controls. At 1 week after treatment, ELISA assay was used to detect serum interleukin-1, interleukin-6, tumor necrosis factor-α levels, and changes of the articular cartilage were observed under a light microscope.
RESULTS AND CONCLUSION:Compared with the control group, the levels of interleukin-1, interleukin-6 and tumor necrosis factor-α were al increased in the other four groups (P < 0.01). Compared with the model group, the levels of interleukin-1, interleukin-6 and tumor necrosis factor-α were lowered significantly in the combined, sodium hyaluronate and PRP groups (P< 0.01 orP< 0.05), and the most significant decline was in the combined group. Articular cartilage damage was severest in the model group and mildest in the combined group. Experimental findings indicate that intra-articular injection of sodium hyaluronate+PRP can reduce inflammation and protect the articular cartilage in knee osteoarthritis, which is better than a single drug injection.
2.The mediating effect of personality between schizophrenia and prospective memory

Objective To explore the differences between schizophrenic patients and normal at prospective memory,and investigate the effect of personality intermediary between schizophrenia and prospective memory.Methods A case-control study was utilized in this research.It used the color matching task to test prospective memory and MMPI-2 scale to test personality differences between the two groups.Results Schizophrenia prospective memory score(19.29±2.30),lower than the normal group(44.74±2.06),the difference was significant(P<0.01).In mediating effect on the personality inspection,the subscales within MMPI-2 of Social Scale(Si) and Anxiety and tension Scale(ANX) regression coefficients were tested(P<0.05),which showed significant effect.Further analysis with sobel statistics showed that only one factor tested was significant.The test coefficients of depression scale (D),mental weakness scale (Pt) and schizophrenia scale (Sc) were significantly different (P< 0.05),which showed that these subscales also had significant mediating effect.Conclusion The prospective memory performance is low in schizophrenia.The personality characteristics that depression,mental weakness,schizo phrenia,social anxiety and tension play an intermediary role in schizophrenia and prospective memory.
3.A controlled study on identification for depression and effectiveness of its treatment in psychiatric clinics of general hospitals

Objective The study is designed to evaluate the difference in identification for depression and clinical effectiveness of its treatment between psychiatrists and non-psychiatric Dhysieians at out-patient departments of general hospitals and to analyze its related factors.Methods Totally,680 patients who visited psychiatric clinics in nine general hospitals of Shanghai at first time were screened by psychiatrists using Composite International Diagnostic Interview(CIDI)for depression and the screening resuhs were compared to the diagnosis made by non-psychiatric physicians as goal-keepers there.Altogether 297 patients with depression were recruited and assessed using Hamilton Depression Rating Scale(HAMD)and Hamilton Anxiety Rating Scale(HAMA).A self-made questionnaire for basic information was used to assess the ability for identification of depression in non-psychiatric physicians of general hospitals.Results Psychiatrists identified depression correctly in 337 of 680 patients who visited psychiatric clinics in general hospitals at their first visit,but non-psychiatric physicians only identified 216 patients,with statistically significant difference(χ2=30.73,P=0.000).A consistent agreement on depression diagnosis between the findings by psychiatrists and by CIDI was reached with Kappa of 0.774,but Kappa for that between the findings by non-psychiatric physicians and CIDI was 0.439.Effectiveness of treatment for depression by psychiatrists was better than that by non-psychiatric physicians,with higher total score and scores for each item of HAMD four,eight and 12 weeks after treatment,respectively(P<0.05 or P<0.01).Length of professional work of non-psychiatric physicians and annual time for professional training in management of mental disorders associated with their ability of identification for depression(P<0.05).Conclusion Ability of non-psychiatric physicians to identify alQd cope with depression in the psychiatric department of general hospitals was insufficient,suggesting that more importance should be attached to the management of service quality at psychiatric department of general hospitals.
4.Changes of blood flow in posterior cerebral artery in fetus with complete transposition of great arteries by pulsed Doppler ultrasound

Objective To study the changes of blood flow in posterior cerebral artery ( PCA ) in complete transposition of great arteries (CTGA) through the application of the pulsed Doppler . Methods Twenty CTGA fetuses ( CTGA group) and 20 healthy control fetuses ( control group) were involved ,the blood flow indexes peak systolic velocity ( Vs) ,end-diastolic velocity ( Vd) ,pulsatility index ( PI) ,resistance index ( RI) ,velocity-time integral ( VTI) of PCA-S1 ,PCA-S2 and MCA of the fetuses in the two groups were detected by pulsed Doppler . The differences in blood flow indexes between CTGA fetuses and healthy controls were analyzed by independent t -test . The rates of abnormal resistance in PCA-S1 and MCA in CTGA fetuses were compared through Chi-square test ( χ2 test) . Results Compared with control group ,the MCA-PI ,MCA-RI ,PCA-S1-PI and PCA-S1-RI of CTGA group decreased significantly( all P < 0 .05) ,MCA-VTI ,PCA-S1-VTI ,PCA-S2-VTI increased significantly ( all P < 0 .05) ,but no significant difference was found in PCA-S2-PI ,PCA-S2-RI ,Vs and Vd of the MCA and the PCA ( all P > 0 .05 ) . The rate of abnormal resistance in the MCA was significantly lower than that in the PCA-S1 in CTGA group ( P <0 .05) . Conclusions The pulsed Doppler can be used to study the changes of blood flow in PCA of CTGA fetuses and the differences of specific hemodynamic alterations may occured in different segments of the PCA in CTGA fetuses ,indicating a tendency to protect the PCA-supplying areas of the brain when ischemia and hypoxia .
5. Z-score analysis of brain fissure depth in normal fetuses

Objective:
To construct the Z-score models for normal fetal brain fissures.
Methods:
Four hundred and eighty-seven pregnant woman who underwent prenatal ultrasonography from January to August 2018 in the Second Xiangya Hospital of Central South University were collected randomly for analysis, and all the fetuses(18-38 weeks) were healthy. The calacarine fissure, pariet-ooccipital fissure, sylvian fissure of the fetuses were measured and they were chosed as the dependent variables, while the independent variable were the gestational age and biparietal diamete, as well as femur length. The regression analyses of the mean(M) and the standard deviation(SD) for each parameter were performed separately. Two observers randomly measured the brain fissures depth two hours apart to evaluate the interobserver repeatability.
Results:
The brain fissure depth in normal fetuses during geatational age between 18 to 38 weeks can be evaluated by Z-score model.Positive correlations were found between the depth of calacarine fissure, pariet-ooccipital fissure, sylvian fissure and gestational age, biparietal diameter and femur length.
Conclusions
Z-score can intuitively reflect the development of brain fissure more accurate, and is valuable, and can be used as a powerful supplement to the traditional method.
6.Application of the fetal adrenal volume evaluated by three-dimensional ultrasound in fetuses with intrauterine growth restriction and its correlation with adverse perinatal outcomes

Objective:To evaluate the fetal adrenal gland volume (AGV) and corrected adrenal gland volume (cAGV) in intrauterine growth restriction (IUGR) fetuses and observe their associations with the adverse perinatal outcomes.Methods:From February 2021 to August 2022, 32 IUGR fetuses who underwent fetal ultrasound examination in the Second Xiangya Hospital of Central South University were prospectively selected as the IUGR group, and 32 normal fetuses matched for gestational age during the same period were selected as the control group. Three-dimensional ultrasound was used to obtain fetal adrenal volume images, and the virtual organ computer-aided analysis (VOCAL) was used to measure AGV, then the cAGV was calculated. The values of AGV and cAGV were appropriately compared between the IUGR and the control groups. The pregnancy outcomes were noted. Multiple logistic regression analysis was employed to evaluate the relationship between the cAGV and adverse perinatal outcomes in IUGR fetus, with maternal age and the CPR included as covariates to control for confounding factors.Results:A total of 32 fetuses with IUGR and 32 controls were involved in this prospective study. There was no significant difference in the AGV between these two groups ( P=0.417). The cAGV of the IUGR fetus was substantially larger than that of the normal fetus ( P=0.034). In the multivariate logistic regression analysis, after adjusting for maternal age and fetal CPR, the fetal cAGV was noticeably associated with the fetal distress (adjusted OR=0.005, 95% CI=0.000-0.587, P=0.029) and the total adverse perinatal outcomes (adjusted OR=0.014, 95% CI=0.000-0.475, P=0.018). Conclusions:The value of cAGV is increased in the IUGR fetuses and associated with adverse perinatal outcomes. The evaluation of fetal AGV could be beneficial to monitoring and managing IUGR fetuses.
7.Renal capsular artery embolization combined with superselective renal artery embolization for the treatment of traumatic renal hemorrhage

;Objective To explore the safety and effectiveness of renal capsular artery embolization(RCAE)combined with superselective renal artery embolization(SRAE)in the treatment of traumatic renal hemorrhage(TRH).Methods A total of 50 patients with TRH were retrospectively selected.Among them,25 patients in the observation group were treated with RCAE combined with SRAE;25 patients in the control group were treated with SRAE.The patients were followed up for 3 months,in which the improvement of related indicators and symptoms during the perioperative period and the occurrence of related complications within 3 months after surgery were recorded.Results Both groups of patients successfully underwent interventional surgery to control renal bleeding.There were no statistically significant differences in the location of vascular injury,angiographic manifestations,and embolization materials used for renal artery embolization(RAE)between the two groups(P>0.05).There were no statistically significant differences in preoperative hemoglobin(Hb),visual analogue scale(VAS)score of renal pain,serum creatinine(Scr),systolic blood pressure,postoperative 7 d Scr and postoperative 1 month systolic blood pressure between the two groups(P>0.05).When comparing postoperative 24 h Hb and VAS score of renal pain between the two groups,the differences were statistically significant(P<0.05).The postoperative satisfaction rate of the observation group was higher than that of the control group,and the postoperative hospitalization time was shorter than that of the control group,with statistical significance(P<0.05).Conclusion The treatment of TRH in both groups can increase Hb 24 h after surgery and reduce VAS score of renal pain 24 h after surgery.Compared with the control group,the observation group significantly can improve postoperative 24 h Hb and patient satisfaction rate,reduce postoperative 24 h VAS score of renal pain,and reduce postoperative hospitalization time.RCAE combined with SRAE is a safe,effective,and minimally invasive hemostasis method for the treatment of TRH and is worthy of promotion.
8.In vitro synergistic evaluation of Nisin and NaF on Streptococcus mutans

Objective:To study the effects of the combined application of NaF and Nisin on Streptococcus mutans(S.mutans).Meth-ods:The minimum inhibitory concentration(MIC)of Nisin and NaF against S.mutans was determined by microdilution method respec-tively.The fractional inhibitory concentration(FIC)was calculated by checkerboard method.The inhibition effect of the combination of NaF and Nisin at(NF)the same MIC concentration on acid production and acid resistance of S.mutans was detected.Crystal violet staining was used to detect the effects of NF in the inhibition of the biofilm formation,and the damage and dispersion of the established biofilms.The changes of the biofilms were observed by CLSM.Results:The MIC of NaF and Nisin was 0.6 mg/mL and 20 mg/mL re-spectively.The FIC was 0.75.The 1/8×MIC NF showed significantly higher inhibition on acid production and biofilm formation than 1/4×MIC NaF or Nisin(P<0.05),but it has no obvious dispersion effect on established biofilms.The 1/2×MIC NF showed stronger in-hibition effect on the acid resistance of S.mutans in the membrane than 1×MIC NaF or Nisin(P<0.05).At the concentration of 2x MIC,any component didn't cause obvious damage on the established biofilm structure.Conclusion:Nisin and NaF have synergistic in-hibitory effects on the proliferation,acid production,acid resistance and biofilm formation of S.mutans.

10.Evaluation of input and output efficiency of scientific research in hospital by Bootstrap data envelopment analysis

Objective:To comprehensively evaluate the input and output efficiency of scientific research in hospital by bootstrap data envelopment analysis, to provide useful information for optimization of scientific performance appraisal and hospital discipline development strategy.Methods:37 disciplines were included as decision making unit, input variables include research expenditure and number of research personnel, and output variables include number of science and technology awards, research projects, patent transfer, paper, composition, and academic influence. The bootstrap-DEA method was used to evaluate the efficiency of all DMUs.Results:The main of overall efficiency and pure technical efficiency in basic DEA model are 0.858 and 0.909, but are 0.804 and 0.853 in Bootstrap DEA model, the differences between two models have statistically significant ( P<0.001). There are 11 DMUs with an overall efficiency in 0.9~1.0, 14 DMUs in 0.8~0.9, 7 DMUs in 0.6~0.8, 5 DMUs lower than 0.6. There are 3 DMUs are increasing return to scale, 16 DMUs are constant return to scale, 18 DMUs are decreasing return to scale. No statistically significance was observed between different types of DMUs( P>0.05). There are 4 DMUs reveal input slacks in number of research staffs and 10 DMUs reveal output slacks. Conclusions:The results of Bootstrap-DEA are more accurate than the basic methods for the evaluation of the input-output efficiency of hospital scientific research, so that it is worth popularizing and applying. According to the evaluation results, the hospital management department and disciplines could optimize their discipline development strategies and put forward targeted improvement measurements.
