Chinese Journal of Geriatrics 2023;42(7):789-793
Objective:To examine the relationship between fasting blood glucose(FBG)levels and H. pylori infection, as well as their impact on eradication and recurrence rates. Methods:A total of 1 584 patients with type 2 diabetes, ranging in age from 44 to 91 years old(mean age of 66.6±7.6 years), were included in this study conducted at the Physical Examination Center of the First Hospital of Lanzhou University between March 2010 and December 2019.Of the total participants, 1 063(67.1%)were male and 521(32.9%)were female.Logistic regression analysis was performed to investigate the impact of age, gender, fasting blood glucose, and H. pylori infection on the patients. In the study, 263 patients in the euglycemic control group(HbA1c<7%), 271 patients in the poor glycemic control group(HbA1c≥7%), and 269 patients in the control group underwent H. pylori eradication using bismuth-containing quadruple therapy, and the eradication efficacy, adverse reactions, and recurrence rates were compared after 1 year. Results:In patients with type 2 diabetes, the infection rate of H. pylori was found to be 48.2%.Further regression analysis revealed that poor glycemic control increased the risk of H. pylori infection( OR=1.611, 95% CI: 1.269~2.045, P<0.01). However, the constituent ratio and infection rate of different hypoglycemic drug groups did not show any significant statistical difference. The eradication rate of H. pylori was 90.9%, and there was no significant difference in the eradication rate among different blood glucose groups and control groups. However, after 1 year, the recurrence rate was 3.8%, with a higher rate observed in the poor glycemic control group compared to the euglycemic control group and the control group( P<0.05). There was a statistically significant difference in constipation as the main side effect of eradication therapy between the diabetic group and the control group[25.1%(134/253) vs.16.4%(44/269), P<0.01]. Conclusions:Diabetic patients with fasting hyperglycemia or poor glycemic control are at an increased risk of H. pylori infection.Additionally, these patients have a higher risk of recurrence after eradication. Therefore, it is important for clinicians to closely monitor and manage glycemic control in diabetic patients to reduce the risk of H. pylori infection and improve treatment outcomes.

Objective To discuss the use of the APP to continue nursing effect on the quality of life of children with cerebral palsy. Methods A total of 100 cases of cerebral palsy hospitalized in July to December of 2016 were selected as the observation group, and the continuation nursing based on"Xinjiang cerebral palsy mobile APP"was carried out. A total of 102 cases of cerebral palsy were used as the control group in the hospital in January to June of 2016, and the traditional continuation nursing was carried out, and the self-care ability and quality of life of the children were evaluated by Activity of Daily Living Scale (ADL), Pediatric Quality of Life Inventory TM3.0-Rheumatology (PedsQLTM3.0) on admission hospital, discharge and 6 months after discharge. Results The time, time and group interaction of 2 groups of children with cerebral palsy in ADL score were statistically significant (F=231.819, 33.382, P<0.01). The ADL score of 6 months after discharge in the observation group was (66.100 ± 7.368) points, the control group was (59.230 ± 10.534) points, and there was statistically significant (t=3.802, P<0.01). The time, group and time group interaction of 2 groups of children with cerebral palsy in PedsQLTM3.0 score were statistically significant (F=23.398, 234.374, 149.209, P<0.01). There was a statistically significant difference in the observation group between admission hospital and discharge, 6 months after discharge and discharge (t=-3.384,-4.198, P<0.05 or 0.01). The difference was statistically significant in control group between discharge and admission hospital(t =-17.809, P <0.01), but there was no significant difference between 6 months after discharge and discharge (P> 0.05). Conclusions Based on the continuation of"Xinjiang cerebral palsy mobile APP", nursing can improve the ability to care for children with cerebral palsy life, help children improve quality of life. Worthy of further promotion.

Objective:To evaluate the performance of rural practitioners of endoscopic cleaning and disinfection participating in the Early Diagnosis and Treatment Program for Upper Gastrointestinal Cancers and to analyze the influencing factors. Methods:The questionnaires for skill assessment were designed based on the skill scheme and clinical practice of the Early Diagnosis and Treatment Program for Upper Gastrointestinal Cancers in rural China, and the App Early diagnosis, Early treatment was used as the examination platform. The practitioners in 539 county hospitals from 25 provinces participating in the program in 2019 were assessed for techniques and skills for endoscopic cleaning and disinfection and the excellence rate was calculated. Multivariate logistic regression model was used to analyze the influencing factors for the examination. Results:A total of 1 671 endoscopic cleaning and disinfection practitioners participated in the assessment with the score of 73.41±16.60. The passing rate was 85.82%, and the excellence rate was 44.94%. Among all questions, the correct rate of "opportunistic screening flow chart" was the highest (98.21%), and that of "the evaluation index for mass screening" was the lowest (57.89%). The multivariate logistic regression analysis showed that the excellence rate was high in practitioners who had a bachelor degree or above ( OR=1.627,95% CI:1.319-2.007, P<0.001), the career for 5 to <15 years (5 to <10 years: OR=1.329,95% CI:1.045-1.689, P=0.020; 10 to <15 years: OR=1.384,95% CI:1.026-1.867, P=0.033), working in eastern and central regions (eastern regions: OR=3.476,95% CI:2.368-5.103, P<0.001;central regions: OR=4.028,95% CI:2.679-6.057, P<0.001) and with full understanding of the screening scheme ( OR=1.547,95% CI:1.246-1.921, P<0.001) . Conclusion:Practitioners on the Early Diagnosis and Treatment Program for Upper Gastrointestinal Cancers in rural China have mastered the basic screening scheme and skills for endoscopic cleaning and disinfection. The education background, duration of the career, area and understanding of screening scheme are influencing factors for the excellence rate of endoscopic cleaning and disinfection.

Background: Disrupted circadian rhythms have been associated with the development of irritable bowel syndrome (IBS). In some IBS patients, the symptoms may present with circadian fluctuations. Enterochromaffin cells (EC cells) and tryptophan hydroxylase 1 (TPH1) - 5 - hydroxytryptamine (5 - HT) signaling pathway are currently recognized as the key pathophysiological mechanism of IBS. Aims: To explore whether Bmal1, the core circadian clock gene, is involved in the occurrence of IBS by regulating TPH1-5-HT signaling pathway in EC cells. Methods: Normal Sprague-Dawley (SD) rats and IBS-model SD rats, as well as wild type (WT) and intestine-specific Bmal1 knockout (Bmal1

Biliary atresia is a rare infant disease that predisposes patients to liver transplantation and death if not treated in time. However, early diagnosis is challenging because the clinical manifestations and laboratory tests of biliary atresia overlap with other cholestatic diseases. Therefore, it is very important to develop a simple, safe and reliable method for the early diagnosis of biliary atresia. Herein, a novel NIR-II fluorescence probe, HZL2, with high quantum yield, excellent biocompatibility, low cytotoxicity and rapid excretion through the liver and gallbladder was developed based on the oil/water partition coefficient and permeability. A simple fecal sample after injection of HZL2 can be used to efficiently identify the success of the mouse model of biliary atresia for the first time, allowing for an early diagnosis of the disease. This study not only developed a simple and safe method for the early diagnosis of biliary atresia with great potential in clinical translation but also provides a research tool for the development of pathogenesis and therapeutic medicines for biliary atresia.
