Objective To explore the changes of deceleration capacity of rate (DC) and analyze its correlation with heart rate variability (HRV) and other factors in patients with coronary heart disease. Methods One hundred and twenty-nine patients with coronary heart disease (coronary heart disease group) and 109 healthy people (control group) were enrolled in this study. DC and HRV parameters were measured by using digitized 24 h Holter. The correlation between DC and HRV parameters, other factors were analyzed. Results The levels of DC, SDNN, SDANN, SDNNi, PNN50, TP, LF, HF, AC in coronary heart disease group were significantly lower than those in control group:(5.64±1.67) ms vs. (6.71±1.47) ms, (106.60±20.53) ms vs. (138.82±31.22) ms, (96.94±20.06) ms vs. (127.47±31.87) ms,(28.53±14.75) ms vs. (52.24±14.65) ms, 87.72%vs. 103.86%,(1 967.10±966.16) ms2/Hz vs. (2 846.70±1 443.41) ms2/Hz,(326.43±195.35) ms2/Hz vs.(457.64±254.30) ms2/Hz, 85.88 vs. 106.39, (-6.18±2.15) ms vs. (-7.00±2.51) ms, P<0.05 or<0.01. DC was correlated with SDNNi, PNN50,TP,LF, HF, AC both in total population or in coronary heart disease group and control group by using multiple linear correlation analysis ( r=0.586, 0.356, 0.531, 0.563, 0.435,-0.433, P<0.01). After removing confounders, DC was correlated with age, SDNNi, rMSSD, PNN50 and AC (P<0.01). Conclusions DC decreases in patients with coronary heart disease and is strong correlativity with HRV parameters. DC could be used for quantitative detection of autonomic nervous function.

Background Scarring of filter bleb is a main cause of failure after trabeculectomy.Administration of anti-proliferation dugs during filtering surgery can maintain the opening of filtering pathway, but some serious complications occure after the drug use.Researches showed that polylactic acid caprolactone (PLCA) drug-loaded electrospinning film can release drug slowly, but its application in glaucoma is seldom. Objective The aim of this study was to evaluate the anti-scarring effects of non-penetrating trabeculectomy combined with sclera interlayer implantation of triamcinolone acetonide (TA)/PLCA drug-loaded electrospinning film in rabbit. Methods TA/PLCA drug-loaded electospinning film was prepared by electrospinning and its surface ultrastructure was observed under the scanning electron microscope. The drug release properties were detected by high performance liquid chromatograph.Ocular hypertensive models were established in New Zealand white rabbits by injecting carbomer into the anterior chamber of the right eyes, and the 40 models were randomized into 5 groups,8 eyes for each group.TA/PLCA drugloaded electrospun membrane,PLCA electrospun membrane or amniotic membrane was implanted beneath the scleral flap during trabeculectomy respectively in the TA/PLCA group,PLCA group or amniotic group, and 40 mg/ml TA was subconjunctivally injected in the TA group.Only trabeculectomy was performed in the simple trabeculectomy group.Intraocular pressure (IOP) was measured, and the shape of filtering bleb was examined under the slit lamp 1 week,2,4,8 and 12 weeks after surgery. The section of filtering bleb was prepared 12 weeks after surgery for the histopathological examination. The use and management of experimental animals was in line with animal ethics. Results The similar three-dimensional reticular structure was seen between TA/PLCA and electrospun membrane with the fiber diameter 0.5-1.5 μm.TA was released stably for 14 days. All the filtering blebs were disappeared in the simple trabeculectomy group during 8-week duration after operation. At 12 weeks after operation, the functional bleb was found in all the 8 eyes of the TA/PLCA group,5 eyes of PLCA group,4 eyes of the amniotic group and 4 eyes of the TA group. The IOP was significantly different among the groups, with the lowest IOP in the TA/PLCA group and the highest IOP in the simple trabeculectomy group (all at P =0.000).Histopathological examination showed that the filtering pathway remained opening in all 8 eyes with the epithelization of bleb surface in the TA/PLCA group, and interspaces of the filtering pathway was explayed in the PLCA group, amniotic group and TA group, while fibrosis of filtering pathway was seen in the simple trabeculectomy group at 12 weeks after surgery. Conclusions TA/PLCA drugloading electrospun membranes presents with nanoscale microstructure and good drug-release properties in vitro.Implantaion of TA/PLCA beneath the scleral flap during trabeculectomy can inhibit the fibrosis of filtering pathway.

Objective To study the influence of survivin targetedly inhibited with antisense oligonucleotide (ASODN)technique on the apoptosis of hepatoma carcinoma cells SMMC-7221,and to clarify the mechanism of promotion effect of survivin-ASODN on the apoptosis of SMMC-7721 cells. Methods The ASODN sequence of survivin marked by FAM fluorescein was designed and synthized. The SMMC-7721 cells were transfected by different concentrations (100,200,300,400,and 600 nmol· L-1 )of survivin-ASODN (ASODN transfection groups),at the same time blank control group and blank liposome control group and sense oligonucleotide (SODN) control group were set up.The apoptotic rates and the changes of cell cycle of the SMMC-7721 cells 24,48,and 72 h after transfected with different concentrations of survivin-ASODN were detected by FCM. The expression levels of survivin were measured by Western blotting method. Results Compared with each control group,24 h after transfection, the apoptotic rates of survivin-ASODN transfected SMMC-7221 cells were increased, the growth of cells was inhibited (P<0.05),and the effects had time-dose dependent tendency.48 h after transfection, the hypodiploid apoptotic peak appeared in ASODN transfection groups before G1 phase, the number of the cells at G0/G1 phase was decreased (P<0.05)and the number of the cells at G2/M phase wsa increased (P<0.05). Compared with each control group, the survivin expression levels in the SMMC-7721 cells in ASODN transfection groups were decreased (P<0.05 ), and the effects of survivin-ASODN was time-dose dependent (P<0.05 ). Conclusion Survivin-ASODN can block the expression of survivin in SMMC-7721 cells and inhibit the proliferation of SMMC-7721 cells by changing the cell cycle and increasing apoptosis in a time-dose dependent manner.

Objective:To investigate the prognosis and outcome of patients with chronic hepatitis C (CHC) related cirrhosis after achieved sustained virologic response (SVR) treated with direct-acting antiviral agent (DAA).Methods:Ninety-five patients diagnosed with CHC related cirrhosis who had complete data in Tianjin Second People′s Hospital from January 2014 to June 2017 were retrospectively followed up. Among them, 72 patients were treated with DAA and all of them achieved SVR, and the other 23 patients did not receive any antiviral therapy. The differences of mortality and incidence of hepatocellular carcinoma (HCC) between DAA treatment group and non-antiviral treatment group were compared. Statistical analysis was performed by independent sample t test, Mann-Whitney U test and chi-square test. Results:At the end of follow-up for three to 71 months, patients in DAA treatment group had a significant improvements in alanine aminotransferase, aspartate aminotransferase, albumin and liver stiffness measurement compared with those before treatment (42(23, 61) U/L vs 18(13, 28) U/L, 54(37, 75) U/L vs 23(18, 28) U/L, 39(33, 42) g/L vs 45(41, 48) g/L, 26(18, 37) kPa vs 15(11, 26) kPa, respectively, Z=-6.005, -7.008, -6.057 and -3.162, respectively, all P<0.01). However, there were no significant differences in incidence of HCC (12%(9/72) vs 17%(4/23)) and mortality (3%(2/72) vs 13%(3/23)) between the DAA treatment group and non-antiviral treatment group (both P>0.05). There was no significant difference of cumulative incidence of HCC in DAA treatment group compared with non-antiviral treatment group ( P=0.609). The age of patients progressed to HCC was older than those without HCC ((60.3±3.6) years vs (54.4±9.9) years, t=-3.948, P<0.01). In subgroup analysis, among the six patients with HCC, four had diabetes, the prevalence of diabetes in the patients without HCC was 17%(7/42); the level of fasting blood glucose (FBG) ((7.3±1.9) mmol/L vs (5.9±1.1) mmol/L) were higher in patients progressed to HCC than those without HCC in DAA treatment group with compensated cirrhosis ( χ2=7.430 and t=-2.442, respectively, both P=0.019). Conclusions:DAA treatment could notably improve liver function and alleviate liver fibrosis, but could not reduce the mortality and incidence of HCC in patients with CHC related cirrhosis significantly. Diabetes and high level FBG may be the risk factors for occurrence of HCC in patients with CHC related compensated cirrhosis.

Objective:To explore the short-term outcomes of a 3D printed trabecular block cage to assist posterior internal fixation for the treatment of patients with basilar invagination and atlantoaxial dislocation. Methods:Between June 2017 and February 2019, 12 patients with basilar invagination and atlantoaxial dislocation underwent atlantoaxial distraction and posterior internal fixation at Department of Orthopedics, The First Affiliated Hospital to Zhengzhou University. They were 5 males and 7 females, aged from 34 to 62 years (average, 45.6 years). 3D printed cages were inserted intraoperatively between the joints of the atlantoaxial lateral mass. The atlanto-dental interval interval (ADI), cervico-medullary angle (CMA) and distance from tip of the odontoid process to Chamberlain's line (DOCL) and the Japanese Orthopedic Association (JOA) scale were compared between preoperation and 12 months postoperation to observe the fusion of the joints of the atlantoaxial lateral mass. Results:Operation went on uneventfully in all the 12 patients. Operation time averaged 116.5 min (from 85 to 190 min), fluoroscopy frequency 9.4 times (from 6 to 21 times), and intraoperative bleeding 82.3 mL (from 50 to 210 mL). No such postoperative complications occurred as cerebrospinal leak, cerebral infarction, or breakage, displacement or loosening of implants. All patients were followed up for 18 to 42 months (mean, 26.3 months). Their preoperative JOA, ADI, CMA and DOCL [8.33±0.98, (8.66±1.64) mm, 119.63°±4.15° and (9.66±2.15) mm] were significantly improved to 14.17±1.03, (2.63±0.59) mm, 153.76°±7.88° and (2.07±0.69) mm ( P<0.05) at 12 months postoperation. Bony fusion was achieved in all the operative segments. Conclusion:In the treatment of patients with basilar invagination and atlantoaxial dislocation, a 3D-printed trabecular block cage can be used to assist posterior internal fixation to achieve satisfactory reduction and maintain the height of joint space, leading to satisfactory short-term outcomes.

Neuroscience Bulletin 2000;16(2):85-89
FOS protein in the lumbosacral spinal cord of the rat was detected to study neuronal activation induced by noxious stimulation of the urethra. In rats receiving infusion of 100 μl of 2% formalin into the urethra, a large number of FOS-positive neurons were seen in the lumbosacral cord segments (L6 and S1), and they were primarily distributed in the medial dorsal horn and dorsal commissural nucleus. Nearly all of FOS expression was blocked by bilateral transection of the pudendal nerve, whereas bilateral transection of the pelvic nerve seemed to have no obvious effect on FOS expression.Behavioral changes were also observed in the nerve-transected rats. Bilateral pelvic nerve-transected rats showed frequently licking of the external urethral orifice after application of the irritant to the urethra, which was quite similar to that of the nerve-intact rats. Such behavior completely disappeared in the rats receiving bilateral pudendal nerve transection. The results suggest that the medial dorsal horn and dorsal commissural nucleus in the L6 and S1 segments are involved in processing nociceptive inputs from the urethra, which is carried by the pudendal nerve.
