Objective: To compare the efficacy of paroxetine and amitriptyline in the treatment of primary fibromyalgia syndrome.Methods:46 out- and in-patients with primary fibromyalgia syndrome were randomized to two groups and treated respectively with amitriptyline 25mg~50mg per day and paroxetine 10mg~20mg per day. The Visual analogue scales(VAS) was used to evaluate the efficacy and adverse effects were recorded at baseline, month 1 and month 3.Results:In both groups pain was alleviated with statistical significance after 1 month of treatment. However the efficacy of amitriptyline and paroxetine was not statistically significant at different time points. The occurrence of adverse effects in paroxetine group was less than in amitriptyline group and the difference was statistically significant, especially dry mouth and palpitation.Conclusion:Paroxetine is as effective as amitriptyline in the treatment of primary fibromyalgia syndrome but with a better safety profile.

Purpose To explore the correlation between fetal cardiac function and interventricular septal thickness in fetuses of gestational diabetes mellitus (GDM) mothers. Materials and Methods Thirty-nine GDM mothers were involved in the study and divided into group MH (fetuses of GDM mothers with interventricular septal hypertrophy) and group NMH (fetuses of GDM mothers with normal interventricular septal thickness). Ultrasonic cardiogram was performed for both groups to assess such variables as interventricular septum thickness at end diastole (IVSd), left ventricular ejection fraction (LVEF), left ventricular fraction shortening (LVFS), right ventricular fraction shortening (RVFS), the ratio of E and A of mitral flow (E/AMV), the ratio of E and A of tricuspid flow (E/ATV), peak velocity of aortic valve (VPAO), peak velocity of pulmonary valve (VPPA) and myocardial performance index (MPI). The results were further compared with those of control group. Results The VPAO, VPPA, LVEF and LVFS in group NMH were significantly higher than those in control group (P<0.05). The VPAO, VPPA, LVEF, LVFS and RVFS in group MH were significantly higher than those in control group whilst the E/AMV and E/ATV significantly lower than those in control group (P<0.05). The LVEF, LVFS and RVFS in group MH were significantly higher than those in group NMH whilst the E/AMV and E/ATV significantly lower (P<0.05). MPI in group MH was significantly higher than that in group NMH (P<0.05), whilst both were significantly higher than that in control group (P<0.05). The LVEF, LVFS, RVFS, VPAO and VPPA were positively correlated with IVSd (r=0.48, 0.51, 0.45, 0.30, 0.30;P<0.05). Conclusion Cardiac function in fetuses of GDM mothers may be associated with interventricular septal thickness.

Objective To investigate the effects of different thickness of cell culture dishes on fluorescent image with confocal microscope.Methods The fluorescent staining experiments of live cells and fixed cells were used to determine the differences among three dishes with different thickness coverslips of 0.085～0.13 mm,0.13～0.16 mm and 0.16～0.19 mm,while the cell appearance,fluorescence lightness and mean of fluorescence intensity were studied with confocal microscope.Results Demonstrated by the results of cytoskeleton staining experiments,the dish with 0.13～0.16 mm thickness coverslip was the best choice for confocal microscope,the dish with 0.16～0.19 mm thickness coverslip was the second one,the dish with 0.16～0.19 mmthickness coverslip was the last one.ConclusionThe dish with 0.13～0.16 mm thickness coverslip is the best choice for confocal microscope.On this type of dish,the cytoskeleton is unfolding and clear after staining.The intensity of fluorescence is the strongest,and the imaging effect is the best.

Objective Analyse the causes,diagnosis and treatment for delayed hemorrhage after distal radical gastrectomy.Methods Retrospective study on 45 patients combined with intro-abdominal hemorrhage after distal radical gastrectomy from January 2008 to June 2013.Results Thirteen patients combined with delayed hemorrhage in these 45 patients,all of 13 patients had intro-abdominal hemorrhage in 1week to 4 weeks after operation.And 5 of the 13 patiens were intermittent intro-abdominal hemorrhage 1 week after operation,these patients were demonstrated the blood come from gastroduodenal artery pseudoaneurysm fracture by CT and DSA examine,and they were cured by interventional embolization.Other 8 patients were marginal ulcer hemorrhage diagnosed by gastroscope,and they stoped bleeding with the help of gastroscope.Conclusions The causes of delayed hemorrhage after distal radical gastrectomy were complicated,and CT,DSA and endoscope can use for diagnosis.What was more,interventional embolization and endoscope were helpful for curing the intro-abdominal hemorrhage,avoiding re-operation.

Objective To analyze the influence of left atrial dimension (LAD) on the effectiveness of heart valve replacement combined with dipolar radiofrequency ablation in atrial fibrillation (AF). Methods Eighty-one patients with permanent AF and heart valve diseases having undergone surgical treatment were selected. The patients were divided into two groups according to the size of LAD:groupⅠ (45 patients with LAD<60 mm) and group Ⅱ (36 patients with LAD ≥60 mm). All the patients underwent heart valve replacement and douching dipolar radiofrequency ablation, and were given amiodarone therapy after operation. The ultrasonic cardiogram and electrocardiogram after operation were inspected periodically. The changes of cardiac function and electrophysiology activity were observed. The patients were followed up for 12 months. Results The early-stage mortality after operation was 3.7% (3/81), and all of death was caused by postoperative low cardiac output syndrome. Two cases appeared third-degree atrioventricular block after surgery, and they were installed permanent pacemaker. During the follow-up, 1 case died because of sudden cerebrovascular accident. Two cases were loss to follow-up. There was no statistical difference in rate of maintaining sinus rhythm at discharge from hospital between group Ⅰ and group Ⅱ: 75.0% (33/44) vs. 73.5% (25/34), P=0.88. But the rates of maintaining sinus rhythm at 3 and 12 months after operation in groupⅠwere significantly higher than those in groupⅡ:81.4% (35/43) vs. 58.8% (20/34) and 88.1% 37/42) vs. 60.6% (20/33), and there were statistical differences ( P=0.029 and 0.006). Conclusions For the patients in permanent AF and heart valves diseases with LAD <60 mm, the dipolar radiofrequency ablation during heart valve replacement has considerably beneficial effects on rate of maintaining sinus rhythm. But for the patients with LAD≥60 mm, the result is not optimistic and has a lower postoperative sinus rhythm restoration rate.

Purpose To investigate the expression of Periostin and VEGF protein in esophageal squamous cell carcinoma tissue and their significance. Methods Immunohistochemical staining (SP method) was used to detect Periostin and VEGF expression in 130 cases of esophageal squamous cell carcinoma and 130 cases of normal esophageal mucosa tissues. Results There was a significantly difference between esophageal squamous cell carcinoma and normal esophageal mucoma ( P<0. 05 ) . The expression of Periostin was related to the depth of invasion and the metastasis of lymph node (P<0. 05). A dramatic correlation has been found in the expression of VEGF to the degrees of differentiation, the depth of invasion and the metastasis of lymph node (P<0. 05). The expression of Peri-ostin was also positively correlated with the expression of VEGF (P<0. 05). Follow-up data was available in 66 cases with a range of 1 to 48 months. Kaplan-Meier survival curve showed survival rate of Periostin-positive group was significantly lower than that of Perios-tin-negative group (P<0. 05). The survival rate of VEGF-positive group was significantly lower than that of VEGF-negative group (P<0. 05). Conclusion The expression of Periostin has a close relationship with VEGF in the carcinogenesis, combined detection of both Periostin and VEGF can become an objective target to evaluate Potentials of invasion and lymph node metastasis of esophageal squamous cell carcinoma, which has a great significance to the judgment of prognosis in esophageal squamous cell carcinoma.

Objective To observe the clinical efficacy of bamboo-circled salt-partitioned moxibustion in treating arthritis of temporomandibular joint.Method Eighty patients were randomized into two groups. Forty cases in the bamboo-circled salt-partitioned moxibustion group received bamboo-circled salt-partitioned moxibustion at temporomandibular joint; forty cases in the warm needling group were intervened by selecting Xiaguan (ST7), Ashi point, etc. at the affected side. For the two groups, 3-day treatment was taken as a treatment course, and the therapeutic efficacy was analyzed after 2 treatment courses. The improvements in pain and mouth opening were observed before and after the treatment, and the treatment efficacy was evaluated by a 1-month follow-up study.Result The bamboo-circled salt-partitioned moxibustion group was superior to the warm needling group in comparing the real-time analgesic effect (P<0.05) and in the improvement of mouth opening (P<0.05); the comprehensive markedly effective rate was respectively 67.5% and 45.0% in the bamboo-circled salt-partitioned moxibustion group and warm needling group, and the between-group difference was statistically significant (P<0.05), indicating that bamboo-circled salt-partitioned moxibustion is better than warm needling in treating arthritis of temporomandibular joint; the follow-up study revealed satisfactory therapeutic efficacies in both groups: the effective rate was 92.5% in the bamboo-circled salt-partitioned moxibustion group versus 87.5% in the warm needling group, and the difference was statistically insignificant (P>0.05).Conclusion Bamboo-circled salt-partitioned moxibustion can produce a real-time analgesic effect and improve mouth opening; it's especially suitable to treat the patients who are afraid of needling, as it's significantly effective, safe, non-invasive,and easy-to-operate.

Objective To compare the influence of washing processed of banked suspended red blood cells (RBCs) by cell saver and the influence by zero-balanced ultrafiltration(Z-BUF) on the inflammatory response after cardiopulmonary bypass(CPB) in infants.Methods Sixty infants with ventricular septal defect (VSD) were randomly divided into a control group (group A,20 cases), group B(20 cases)and group C (20 cases).Banked suspended RBCs were washed by cell saver before priming in group B,in group C the banked suspended RBCs were treated with Z-BUF and in group C the banked suspended RBCs were primed directly without any pre-treatment.Samples of the arterial blood were obtained respectively before the start of the CPB (T1), when the CPB stopped(T2) ,2 h after CPB (T3), 12 h after CPB (T4) and 24 h after CPB (T5).The levels of tumor necrosis factor-α (TNF-α), interleukin(IL)-6, IL-8, IL-10 were detected and analyzed comparatively among 3 groups.Results The levels of TNF-α, IL-6, IL-8 ,IL-10 in 3 groups at T2,T3 ,T4 showed a rising trend markedly,and the above four indicators of A,B and C at T4respectivelywere:(110.3±14.0) ×10-9g/L,(90.6±10.3) ×10-gg/L,(103.3±9.7) ×10-9g/L;(54.1 ± 6.5) ×10-9 g/L,(39.3±4.2) ×10-9 g/L, (46.2±5.7) ×10-9 g/L;(96.8 ±9.2) ×10-9 g/L, (82.5 ±6.5) × 10-9 g/L,(88.4±5.1) ×10-9 g/L;(228.4 ±42.9) ×10 9 g/L,(171.5 ±26.4) ×10-9 g/L,(202.9 ±42.8) × 10-9 g/L.The levels of TNF-α and IL-8 in group B and group C were significantly lower than those in group A at T2, T3 ,T4 and T5(all P ＜0.05) ,but there was no significant differences in the levels of IL-6 and IL-10 among 3 groups at T5;the levels ofTNF-α,IL-6,IL-8 ,IL-10 in group B were significantly lower than those in group C at T2,T3 and T4(all P ＜ 0.05).Conclusions Processing of banked suspended pre-RBCs with cell saver and Z-BUF can relieve systemic inflammatory response, and the effect of washing with cell saver is better compared with that of Z-BUF.

Objective To explore the status and influencing factors of sub-health in inpatient caregivers.Methods A total of 432 caregivers met the inclusion criteria were enrolled.Suboptimal health status questionnaire (SHSQ-25) was used to evaluate the status of sub-health.Sub-health was defined as SHSQ-25 score ≥ 35.100 people undertaking routine physical examinations were enrolled as control group.The incidence and influencing factors of sub-health in caregivers were analyzed.Results (1) The caregivers group valued higher in SHSQ-25score significantly (36.9 ± 8.6 vs.27.5 ± 13.0,P ＜ 0.05).The incidence of sub-health was obviously higher in caregiver group than that in control group (69.9％ vs.39.0％,P ＜ 0.05).(2) A significant difference in the SHSQ-25 scores between female caregivers and male caregivers was observed (37.9 ± 9.0 vs.34.6 ± 7.1,P ＜0.05).The SHSQ-25 score was positively related with age (r =0.578,P ＜ 0.05) and accompanying time (r =0.507,P ＜ 0.05).Parents,children,or spouses of patients got higher scores than other types of caregivers (38.1 ±8.1 vs.31.1 ±8.8,P＜0.05).(3) Among the sub-health caregivers,only 10.9％ of them were aware of their sub-health status.Conclusion The incidence of sub-health may be higher in inpatient caregivers than the general population.Gender,age,accompanying time,and the relationship between patients and caregivers may be influencing factors for sub-health.Effective action should be taken to improve the health status of caregivers.
