Medical Journal of Chinese People's Liberation Army 1981;0(04):-
18 out of 779 operated cases for lumbar prolapsed disc at our department were reoperated with excellent or good results obtained in 17 and improvement in 1. 14 out of these were initially treated at our department with satisfactory results for from 2 months to 12 years before symptoms recurred while 4 were first operated upon at other hospitals with good results in 2 and failure for the others after the primary operation. The recurrence rate is 14/779 in our service, i. e. 1.8%, lower than those previously reported.Reoperation for recurrent cases is generally considered extremely difficult and result uncertain because of extensive scar and dense adhesions about the nerve coot and dura. More often than not the results reported in literature were not good enough to warrant reoperation. Consequently those recurrent patients in our series so badly incapacitated would have to suffer or remain crippled for lifetime were they not reoperated. However our patients were salvaged by reoperation with meticulous performance after careful clinical evaluation as to diagnosis, indication and tyfe of operation adopted. Operative procedure for exploration and resolution of coexisting spinal stenosis is discussed in detail.

Surgical treatment of postinjection injury of peripheral nerve in 62 patients are reported, 50 patients were followed-up for 3 years in average. Neurolysis is an effective method in the treatment of such injuries with 64 percent of excellent and good result observed in this series. The results of operation were related to the degree of severity of nerve injury, timing of operation, and extent of adhesion of the injured nerve. The result of early operation is better than late operation.

Medical Journal of Chinese People's Liberation Army 1982;0(01):-
Twenty-six patients with stenosis of the thoracic spinal canal and myelopathy caused by ossification of the ligamentum flavum were treated from 1983 to June 1991. There were 21 patients with spastic paraplegia and 5 with flaccid paraplegia in the series. The findings on radiograms, CT scans, myelograms and MRI were described. Total laminectomy was performed in all of the patients which resulted in an effective rate of 91.6% and exellent and good results in 83.3%. The mechanism of the disease our experience in diagnosis and surgical treatment, causes of misdiagnosis and surgical techniques are discussed.

Objective To explore the effects of electroacupuncture on brain derived neurotrophic factor (BDNF) of rats with sciatic nerve injury (SNI); To discuss its biological mechanism for treatment of SNI. Methods Fifty adult male Wistar rats were chosen, and the sciatic nerves of rats were cut off and pulled on both sides of the cut ends into nerve regeneration chamber. The rats were randomly divided into normal group, sham-operation group, model group, and electroacupuncture group. In the electroacupuncture group, the rats were treated by electroacupuncture for 28 days. After the treatment, the nerve regeneration was observed through HE staining. Immunofluorescence was used to analyze the expression changes of BDNF in the nerve tissue and spinal cord. ELISA was used to observe the changes of expression of serum BDNF. Results The amount of axon regeneration in the electroacupuncture group was obviously more than that in the model group, and the outline of the tissue more clear. Electroacupuncture could promote the expression of BDNF in the nerve, spinal cord and serum of SNI of rats compared with model group (P<0.01). Conclusion Electroacupuncture can promote the repairment and regeneration of SNI in rats by upregulating the expression of BDNF.

BACKGROUND: SureFil SDR as a lining material has better adaptability and unique automatic leveling characteristics as compared with other flowing resins, but its application in preventive resin filling has not been reported. OBJECTIVE: To compare the roughness and bacterial adhesion among pit and fissure sealant, Z350 XT fluid resin and SureFil SDR resin, and to explore the feasibility of preventive resin filling with SureFil SDR resin. METHODS: Pit and fissure sealant, Z350 XT fluid resin and SureFil SDR resin were prepared into 8 mm× 2 mm× 2 mm cuboid specimens respectively. The surface roughness of the three specimens was measured by MarSurf roughness meter. Pit and fissure sealant, Z350 XT fluid resin and SureFil SDR resin were prepared into 8 mm× 8 mm× 2 mm cylinder specimens respectively. The specimens were cultured in Streptococcus mutans suspension for 48 hours to detect the bacterial adhesion on the material surface. RESULTS AND CONCLUSION: The roughness of the three materials was ranked as follows: SureFil SDR resin < Z350 XT fluid resin < pit and fissure sealant, and there were significant differences between groups (P < 0.05). The bacterial adhesion on the surface of the three materials was ranked as follows: SureFil SDR resin < Z350 XT fluid resin < pit and fissure sealant, and there were significant difference between groups (P < 0.05). The correlation between roughness and bacterial adhesion was analyzed, and there was a high positive correlation between roughness and bacterial adhesion, indicating, with the decrease of material roughness, bacterial adhesion decreased (r=0.962, P=0.017 6). The results show that SureFil SDR is an ideal preventive resin filling and sealing material relative to pit and fissure sealant and Z350 XT fluid resin.

Objective To analyze the clinical and histology characteristics of a patient with frontal lobe epilepsy diagnosed with mild malformation of cortical development with oligodendroglial hyperplasia, and to recognize the new neuropathological entity. Methods Clinical history, seizure types, neuroimaging, electroencephalography as well as macroscope, histology and immunohistochemistry characteristics were collected from a frontal lobe epilepsy patient and were compared with cases from literature. Results It was a female patient aged 16 years with 12 years history of epilepsy. The seizures manifested as episodes of conscious loss with automatism including grope and voice lasting for seconds. About 10 episodes a day were found and sometimes with secondary generalized tonic-clonic seizures. MRI showed blurring of grey-white matter interface in left orbital frontal cortex. Video-encephalography revealed left frontal lobe origin of seizures. So left prefrontal lobe was removed. Histology showed almost normal cortex neuropil and neurons. Blurring of grey-white interface in some area with patches of proliferation of oligodendrocytes in the corresponding sub-cortical white matter was found. The density of oligodendrocytes was significantly higher in sub-cortical than in deep white matter both shown in HE and Oligo-2 staining. Obvious oligodendrocytes increase and satellite phenomenon in deep cortical layer as well as increased ectopic neurons in sub-cortical white matter were found in the lesion. In proliferation area, there were some nuclei stained with Ki-67, but not as high as tumor. Subsequent follow up for two years proved the operation efficacy and benign prognosis. Conclusions There are special and undiscovered histopathological entities in epilepsy etiology. Although known as grey matter disease, white matter pathology plays an important role in epilepsy pathophysiology which needs further research.

Chronic migraine is one of neurological disorders with high rate of disability, but sufficient attention has not been paid in this field. A large number of clinical studies have shown traditional Chinese acupuncture is a kind of effective treatment with less side effects. Through the analysis of literature regarding acupuncture and migraine published from 1981 to 2017 in CNKI and PubMed databases, the mechanism of neural plasticity of acupuncture on chronic migraine was explored. It was believed the progress of chronic migraine involved the changes of neural plasticity in neural structure and function, and the neural plasticity related with neural sensitization during the process of chronic migraine was discussed from three aspects of electrophysiology, molecular chemistry and radiography. Acupuncture could treat and prevent chronic migraine via the mechanism of neural plasticity, but there was no related literature, hindering the further spreading and development of acupuncture for chronic migraine.
