1.Late complications of continent ileocolic reservoir with an unaltered in situ appendix conduit

Objective To evaluate the late complications of continent ileocolic reservoir with an unaltered in situ appendix conduit and the management. Methods Thirty patients with bladder cancer who had undergone the surgery of continent ileocolic diversion with an unaltered in situ appendix conduit were followed up for 108 to 126 months (mean 118 months).The long-term post-operative complications were documented. Results The complications occurred as follows:urinary pouch superdistention in 5 cases,calculus formation in the urinary pouch in 6,difficulty with catheterization in 4,uerteral obstruction in 3,hemorrhage of the pouch in 1 and mild hyperchloremic acidosis in 1.These complications were managed accordingly and good results achieved. Conclusions Although continent ileocolic diversion with an unaltered in situ appendix conduit has a long-term efficacy,the complications occur frequently.The procedure needs further improvement.
2.Influence on cellular apoptosis and telomerase activity in bladder cancer treated with BCG-CWS and IL-2

Objective To study the cellular apoptosis and telomerase activity in bladder cancer before and after treatment with cell wall skeleton of bacillus Calmett Guerin(BCG CWS) and interleukin 2(IL 2). Methods The cellular apoptosis and telomerase activity of 33 cases of bladder cancer tissues were detected by terminal deoxynucleotidyl transferase mediated dUTP biotin nick end labeling(TUNEL) and telomeric repeat amplification protocol(TRAP) assay both before and after treated with BCG CWS and IL 2. Objective: To investigate the effect of recombinant BCG secreting IL-2 (rBCG-IL-2) on peritoneal macrophages of mouse and inquire into the effect of rBCG-IL-2 on immunity of host. Methods: Gene engineering technology, electricity transfer- ence and inoculation tumor cell are adopted; MTT assay is used to determine tumoricidal effect of macrophages. Results: Tumo- ricidal effect of macrophages of rBCG-IL-2 groups was higher than those of the control coups and BCG groups, and the tumorici- dal effect of macrophages could reach a high state in a week. Conclusion: rBCG-IL-2 can raise tumoricidal effect of macrophages, improve immunity of host and is a prospect biological preparation.
4.Isolation of Pathogenic Fungi from Patients with Onychomycosis by Multipoint Inoculation Method

Objectives To compare the difference between multipoint inoculation and routine method for isolation of pathogenic fungi from nail samples of onychomycosis,and to analyze the epidemiology of pathogenic fungi in those patients.Methods The nail clipping samples from each patient were inoculated onto the plates with Sabouraud's agar,Sabouraud's agar without cycloheximide and medium containing rapeseed oil,respectively,by an approach of at least seven inoculating points in each plate (multipoint inoculation),and onto medium slope in tubes with the same media as above mentioned (routine method).In the multipoint inoculation method,plates with more than 3 colonies were taken for further identification of pathogenic fungi based on morphological and biochemical properties.Results Based on the data from 150 samples of onychomycosis,significant differences were found between multipoint inoculation method and routine method (P
5.Individualized education for 30 children with learning disability

ObjectiveTo study the method and effect of individualized education on children with learning disability (LD).Methods30 children during 7-10 year old in a local boarding elementary school were chosen and guided individually as the following: sensory integration dysfunction drill, subtle motion drill, life skills drill, cognitive capability drill, behavior therapy, game therapy, music therapy and parents education. One and a half year later, the index of Sensory Integration Rating Scales and study marks were determined and compared with that of the normal children.ResultsThere is not significant difference between the LD children and the normal ones in the sensory integration of behavior and the learning achievement.ConclusionIndividualized education is a valid approach improving the potentiality of the LD children\'s physiology and psychology.
6.Outcomes of autologous osteo-periosteal cylinder graft transplantation for Hepple Ⅴ osteochondral lesions of the talus

Objective To study the outcomes of autologous osteo-periosteal cylinder graft transplantation for Hepple V osteochondral lesions of the talus (OLT) with large subchondral cyst.Methods The data of 27 consecutive patients of OLT with subchondral cyst was retrospectively analyzed who were treated by autologous osteo-periosteal cylinder graft transplantation from October 2007 to September 2011.There were 26 males and 1 female with an average age of 35.8 years (range,22-53 years).Visual analogue score (VAS) for pain during daily activities,the American Orthopaedic Foot and Ankle Society (AOFAS) ankle and hindfoot score,and subjective satisfaction were investigated.The plain radiographs,magnetic resonance imaging (MRI) of the ankle,and second look arthroscopy were analyzed.Results All the 26 patients were followed up for 22.4 months.At the last follow-up,the VAS score decreased from 5.4±1.0 points preoperatively to 0.8±0.8 points postoperatively,and the mean (50%) AOFAS score improved from 73.9±3.1 points preoperatively to 93.0±6.5 points postoperatively.In 26 cases,the radiolucent area of cysts disappeared on plain radiographs.The mean magnetic resonance observation of cartilage repair tissue (MOCART) score was 57.2,though small subchondral bone cyst was still found in 3 cases on postoperative MRI.The mean (50%) ICRS arthroscopic score of cartilage repair was 9.2 points according to second look arthroscopy of 18 cases.There were 16 cases receiving excellent effect,8 good and 2 fair.The excellent and good rate was 92.3% (24/26).There were no major complications.Conclusion Autologous osteo-periosteal cylinder graft transplantation could repair the osteochondral defects.It yields satisfactory results,and is suitable for treating OLT with large subchondral cyst.
7.The resilience characteristics and its influencing factors in the elderly disabled

Objective To investigate the resilience characteristics of the elderly disabled,and the effects of individual,family and social factors on resilience.Methods A total of 138 disabled older adults from four districts of Chengdu City were investigated by 10 items Connor-Davidson resilience scale (CDRISC-10),the automatic thoughts questionnaire (ATQ),self esteem scale (SES),revised eysenck personnality questionnaire short scale for chinese(EPQ-RSC) and self-made population sociology questionnaire.Totally 194 community-dwelling older adults without disabilities,as a comparative population,were included in the analysis.Results Compared with the community-dwelling elderly without disability (35.58 ± 5.75,18.13±3.48,18.43±3.08),the scores of resilience(21.38±7.71),strength(10.50±4.08) and hardiness (10.88±4.07) of the elderly disabled were significantly lower(t=-19.56,-17.81,-18.34,P<0.01).There were no significant differences in the resilience scores of the elderly disabled in terms of gender,urban and rural sources,marital status,education,disability level and family atmosphere.However,there were significant differences in age,disability causes,disability type and family economic conditions (age:thardiess =2.18,P<0.05.disability causes:tresilience =-2.75,tstrength =-2.22,thardiness =-2.68,P<0.05.disability type:Fhardiness =2.60,P< 0.05.family economic conditions:Fresilience =5.45,Fstrength =4.78,Fhardiness =5.21,P<0.01).The resilience,automatic thoughts,self-esteem and personality were significant correlation with each other(r=-0.16-0.65,P<0.05).After controlling age,disability causes,disability type and family economic conditions,self esteem(β=0.291,P<0.01),extraversion (β=0.287,P<0.01) and neuroticism (β=-0.254,P<0.05) were important influential factors of resilience,explained 32.0% variance in resilience.Conclusion It may be an important way to improve resilience level of the elderly disabled by enhancing their selfesteem,promoting their psychological adjustment and strengthening their emotional management.
8. The current status and influencing factors of frailty among community-dwelling old adults

Objective:
To explore the current status and predictive factors of frailty among community-dwelling old adults.
Methods:
A total of 1 012 community-dwelling older adults aged 65 years and above in Sichuan Province were enrolled and investigated by self-made general condition scale, Tilburg Frailty Indicator (TFI), Short Physical Performance Battery(SPPB), 10-item Connor-Davidson Resilience Scale(CD-RISC-10), Mini-Mental State Examination(MMSE) and Geriatric Depression Scale Short-form(GDS-15).
Results:

10.Mediating and moderating effects of resilience on frailty and depression in the community-dwelling old adults

Objective:To investigate the mediating and moderating effects of resilience on frailty and depression in the community-dwelling old adults.Methods:Totally 871 community-dwelling old adults chosen from different communities were investigated by the geriatric depression scale, Tilburg frailty indicator and 10-item Connor-Davidson resilience scale.SPSS 22.0 and AMOS 24.0 were used for data analysis, including descriptive analysis, correlation analysis, structural equation modeling and hierarchical regression analysis.Results:(1)The score of frailty (3.72±2.89)was positively correlated with the score of depression (2.63±2.57, r=0.16-0.58, P<0.01). The score of resilience(28.24±6.80) was negatively correlated with the score of frailty and depression ( r=-0.10~-0.49, both P<0.01), and frailty predicted 35% of the total variation of depression in the elderly.(2)The mediating effect of resilience was significant and the indirect effect was 0.10, accounting for 12.66% of the total variance.(3) Resilience moderated the relationship between frailty and depression ( β=-0.12, t=-4.11, R2=0.41, P<0.001). The frailty of old adults with lower resilience played a stronger predictive role in depression (simple slope=0.50, t=14.73, R2=0.01, P<0.001). Conclusion:There is a close relationship between frailty and depression in the community-dwelling older adults, and resilience plays both mediating and moderating role in the relationship.
