Objective To detect the initial results of total hip arthroplasty (THA) after failure of internal fixation of femoral neck fractures and investigate the key points in surgical procedures and prosthesis selection.Methods A retrospective analysis was performed on 36 cases who had THA for femoral neck fractures due to the initially failed internal fixation between January 2007 and December 2012.There were 21 males and 15 females,with age ranging from 46 to 72 years (mean,58.6 years).After internal fixation,13 cases were complicated by femoral head necrosis,eight fracture nonunion,and 15 fracture nonunion combined with femoral head necrosis.All the 36 cases had cementless THA,including proximally fixed stem in 25 cases and distally fixed stem in 11 cases.Harris hip score was assessed before operation and at follow-up.Results All the cases received a follow-up of 26-52 months (mean,32.5 months).Mean Harris hip score improved from 26.3 to 83.6,including excellent results in 12 cases,good in 22,and fair in two,with excellence-good rate of 94％.Conclusion With respect to the failed internal fixation of femoral neck fractures,THA is an effective remedial method to improve hip function and quality of life of the patients.

Patients with gastrointestinal diseases are more likely to exhibit malnutrition. Nutritional support is needed for those who are with malnutrition or at risk. Nutritional support can be categorized into enteral nutrition and parenteral nutrition. With the widely application of nutritional support,more and more attention has been paid on its role in gastrointestinal diseases. It not only can improve the nutritional status,but also can alleviate the clinical symptoms to some extent and improve prognosis,which makes it a kind of extremely important treatment modality for gastrointestinal diseases. In this article,the advances in study on nutritional support of gastrointestinal diseases were reviewed.

Objective:This study was conducted to investigate Epstein-Barr virus (EBV) infection in gastric cancer in Wuwei ar-ea of Gansu province (China) and the roles of miR-101, EZH2, and COX-2 in EBV-associated gastric carcinomas (EBVaGC). Meth-ods:Tissue microarray technique, immunohistochemistry, and in situ hybridization were performed to detect the expression of EBV-en-coded small RNAs (EBERs), miR-101, EZH2, and COX-2, in gastric cancer tissues (n=120) and the corresponding adjacent tissues (n=120). Results:The positive rate of EBV was 10.0% in 120 cases of gastric cancer tissues. EBVaGC was not significantly associated with lymph node metastasis and developed most often in the cardia and body (P<0.05). The differences in the positive rates of miR-101, EZH2, and COX-2 in 120 cases of gastric cancer tissues and corresponding adjacent tissues were significant (P<0.05). The differences in the positive rates of miR-101, EZH2, and COX-2 in 12 cases of EBVaGC and in 108 cases of EBV-negative gastric can-cer (EBVnGC) tissues were significant (P<0.05). The infection of EBV in gastric cancer tissues was positively related to miR-101 ex-pression. By contrast, miR-101 expression was negatively related to lymph node metastasis and expression of COX-2 and EZH2 in EB-VaGC tissues (P<0.05). Conclusion:EBV infection was related to gastric cancer in Wuwei area of Gansu province. EBVaGC and EB-VnGC have significant differences in lymph node metastasis and in the location of cancer. MiR-101, EZH2, and COX-2 were related to the development of EBVaGC.

Total cholesterol(TC),low-density lipoprotein-cholesterol (LDL-C),high-density lipoprotein-cholesterol (HDL-C),triglyceride(TG),endothelin(ET) and nitrous oxide(NO) were measured before and after pravastatin therapy. The endothelium-dependent relaxing function was measured before and post pravastatin therapy by high-resolution ultrasound. Thirty people with normal blood cholesterol accepted same examination as control. Results The level of ET in renal transplantation group was significantly higher than that of control group,and the level of NO in renal transplantation group was significantly lower than that of control group. After 8 week′s therapy,the level of NO rose significantly,and the level of ET,TC,LDL-C,TG decreased significantly. The level of HDL-C increased but there was no significant difference between two groups. Flow-mediated vasodilations were improved after pravastatin therapy,while the level in transplantation group was lower than that of control group. Conclusion Pravastatin is effective in treatment of dyslipidemia after renal transplantation,which can improve the endothelium-dependent vasodilation.

With patch clamp method,the effects of taurine (Tau) on slow inward calcium current (Ica) were investigated in single ventricular cells of guinea pigs. Tau decreased Ica in a dose- and voltage-dependent mode,but had little effect on reversal potential. Tau 0. 1 umol - L-1 decreased Ica by 17% and its maximal effective concentration was near 100 umol -L '. The initial block of Tau had already reached by 47% after a rest out of stimulation. Tau facilitated the reduction of lea through increasing clamp pulse frequency. The obtained results suggested that it decreased Ica in a use-dependent manner.

AIM　To study the characteristic of th e rapidly activating component (Ikr) and the slowly activating compone nt (Iks) of the delayed rectifier K+ current in guinea pig hypertrop hi ed ventricular myocytes induced by thyroxine. METHOD　The whole c ell patch clamp techniques were used. RESULTS　In hypertrophied ve ntricular myocytes, the magnitude of IKr and IKs were great ly augmented with more positive depolarizations, and the degree of increase of IKr tail and IKs were greater than that of IKr an d IKs tail, respectively. By measuring the amplitude of tail currents which reflected the degree of activation, the activation curve of IKs was shifted toward more negative potential, but that of IKr was marked ly unaffected in hypertrophied myocytes. CONCLUSION　Hypertrophied ventricular myocytes induced by thyroxine obviously increased IKr and IKs.

Objective To assess the relationship of methylation status of CpG islands in the promoter region of insulin-like growth factor binding protein 7 (IGFBP7) gene with the expression of IGFBP7 gene in human melanoma cell lines and primary melanocytes.Methods Primary melanocytes from human forcskin tissue as well as 4 human melanoma cell lines,including A375,M14,SK-MEL-1 and MV3,were used in this study.Bisulfite sequencing PCR (BSP) was applied to detect the methylation status of 54 CpG sites in the 5'-flanking promoter region of IGFBP7 gene in all of the melanoma cell lines and primary melanocytes.Results As hierarchical cluster analysis showed,IGFBP7-positive cells (including A375,M14 and SK-MEL-1 ) differed significantly from IGFBP7-negative cells (including MV3 cells and primary melanocytes) in the methylation pattern of IGFBP7 gene promoter region.Conclusion The methylation status of CpG island in the promoter region of IGFBP7 gene may be associated with its expression in melanoma cell lines.

Objective To detect the effective connectivity of resting- state functional magnetic resonance imaging (fMRI) in normal adults. Methods 36 normal adults were performed resting-state fMRI scanning, and 5 brain netwokes were included as regions of interests. Independent component (ICA) was used to evaluate the effective connectivity, and multivariate Granger causality analysis (mGCA) was used to analyze the casuality between the networks. All preprocessing steps were carried out using Statistical Parametric Mapping 5.0 software. Results 5 classic resting brain networks including default mode network (DMN), memory network (MeN), motor network (MoN), auditory network (AN) and executive control network (ECN) were aquired. The mGCA presented significant casuality between DMN and other 4 networks, MeN and ECN, AN and MoN, ECN and AN. Conclusion There are specific brain effective connectivity of resting-state fMRI in normal adults, and there is significant causal link between these networks.

It has been more than 100 years to apply transsphenoidal surgical approach in neurosurgery.It has been a more and more important surgical method as the improvements of surgical technology and medical instrument.We aim to introduce the recent development in neuroendoscopal transsphenoidal surgical method,as well as advantages plus limitations,and postoperative complications when it is compared with traditional microscopy.In addition,we will introduce the most common three surgical methods in transsphenoidal operative approach.It will provide deeper and more experience in further development in transsphenoidal surgery when we analyze the operative route differences between nasal septum and anterior sphenoidal wall.
