1.The clinical observation on Wenxin granule combined with paroxetine in the treatment of the elderly patients with depression after fracture

Objective To observe the effect of Wenxin granule combined with Paroxetine in the treatment of the elderly patients with depression after fracture.Methods 80 patients with depression after fracture were randomly divided into a control group and a study group(n=40 cases,per group),and both groups were treated with the conventional treatment. On this basis, the control group was treated with Paroxetine,while the study group was treated with Wenxin granule combined with Paroxetine. The HAMD score and HAMA score were detected in the patients before the treatment and 4 and 8 weeks after the treatment separately.Results Before the treatment,there was no statistical significance in HAMD and HAMA score(t=0.512, 0.520,P>0.05). After treated for 4 and 8 weeks,compare with the control group,both HAMD score(t=1.828, 2.259,P<0.05) and HAMA score(t=1.819, 2.261,P<0.05) were decreased in the study group. After treatment for 4 and 8 weeks, compare with the control group, the total efficiency showed in the HAMD reduction rate of the study group was better(χ2=3.203, 4.370,P<0.05).Conclusions Wenxin granule combined with Paroxetine can significantly decrease HAMD and HAMA score,obviously improve the depression symptoms in the elderly patients with depression after fracture.

3.Optimization of the Forming Technology of Zuojin Dropping Pill by Orthogonal Experiment

OBJECTIVE:To optimize the forming technology for Zuojin dropping pill.METHODS:The base material and cooling agent were optimized by orthogonal experiment with the type of base material,the ratio of drug to base material,the temperature of physic liquor,the type of the cooling agent as factors taking the mixing state,forming condition and dissolution time as indexes; the dripping condition was optimized with the temperature of the cooling agent,the caliber of the dripping mouth,the dripping distance and dripping speed as factors taking the variation coefficient of the pill weight difference as index. RESULTS:The optimal forming technology was as follows:with polyglycol 4000 as base and paraffin oil as cooling agent; the cooling temperature was 12 ℃;the caliber of the dripping mouth was 3.5/4.9 mm;the dripping distance was 10 cm and the dripping speed was 25 drop?min-1.CONCLUSION:The forming technology is stable,reproducible and the preparation was well-shaped,and the forming technology serves as a reference for the production of Zuojin dropping pill.
4.ENDOMORPHIN-1 IS MORE POTENT THAN ENDOMORPHIN-2 IN INHIBITION OF SYNAPTIC TRANSMISSION IN SUBSTANTIA GELATINOSA OF ADULT RAT SPINAL CORD

Effects of endomorphin-1 (EM-1) and endomorphin-2 (EM-2) on synaptic transmission were investigated on neurons in substantia gelatinosa (SG) of the spinal dorsal horn by whole-cell voltage clamp recording. Both EM-1 (1 μmol/L) and EM-2 (1 μmol/L)remarkably reduced the frequency but not the amplitude of miniature excitatory postsynaptic currents (mEPSCs) and miniature inhibitory postsynaptic currents (mIPSCs). These effects were antagonized by 3-funaltrexamine ( β-FNA, 10 μmol/L), a selective μ-opioid receptor antagonist. Noticeably, EM-1 showed higher potency in decreasing the frequency of mEPSCs and mIPSCs than that of EM-2. These results indicate that EMs suppress both excitatory and inhibitory synaptic transmission by activating presynaptic μ-opioid receptors in the SG and EM-1, compared with EM-2, might be a more potent endogenous analgesic at the spinal cord level.
5.Surgical treatment for acetabular fractures

Objective To investigate the method and curative effect for the operative treatment of acetabular fractures.Methods Forty-one patients with acetabular fractures were treated operatively from March 2001 to March 2006.All fractures were classified according to the Letournel-Judet classification,22cases were simple fractures and 19 cases were complicated fractures.Kocher-Langenbeck (K-L) approach were adopted in 31 cases (14 cases through osteotomy of greater trochanter additionally),ilioinguinal approach were adopted in 8 cases and anterior combined posterior approach were adopted in 2 cases.Different fractures were fixed by reconstructive plates and screws.Results All the patients were followed up for 16-36 (24.31 ± 5.31 ) months.According to the reduction criteria of Matta radiography,there were 27 cases with anatomic reduction,8 cases with satisfactory reduction,3 cases with poor reduction and 3 cases with joint contour reduction.According to the Merle D' Aubigne clinical criteria for joint functions,14 cases showed excellent results,18 cases were good,5 cases were fair and 4 cases were poor,and the excellent-good rate was 78%( 32/41 ).Necrosis of the femoral head was found in 3 cases,heterotopic ossification in 4 cases,traumatic arthritis in 3 cases,sciatic nerve injury in 4 cases,femoral nerve injury in 1 case,all the nerve injury recovered in 6 months.Lower extremity deep venous thrombosis in 3 cases,all of them were recovered by given anticoagulation.No death,infection or nonunion were found.Conclusions K-L approach or ilioinguinal approach are adopted in the most of the operative treatment of acetabular fractures.Dual reconstructive plates or single.reconstructive plate combine lag screws can fix solid.It can make hip to do early exercise.
6.Analysis on gene detection in 5 500 cases of thalassemia in Huizhou region

Objective To use the genetic diagnosis technique to perform the gene detection in the patients with thalassemia for understanding the main gene mutation types and their gene frequencies .Methods Totally 5 500 outpatients and individuals un‐dergoing physical examination in the hematology department of the Huizhou Municipal Central People′s Hospital from January 2010 to October 2014 were taken as the research subjects .The GAP‐PCR and membrane reverse hypridition technology were adopted to conduct the gene analysis of α‐andβ‐thalassemia .Results A total of 1 604 cases ofα‐globin gene change were detected ,accounting for 29 .16% of the total detected subjects ;1 096 cases ofβ‐globin gene change were detected ,accounting for 19 .93% of the total de‐tected subjects ;119 cases of αβ‐complex thalassemia were detected .Conclusion The gene screening of α‐ and β‐ thalassemia pro‐vides the valuable basic data for conducting the marital and fertility instructions .
7.Curative observation of Huoxue-Zhitong decoction combined with mannitol in the treatment of swelling and pain after operation of tibiofibular fracture

Objective To observe the clinical curative effect of Huoxue-Zhitong decoction combined with mannitol in the treatment of swelling and pain after operation of tibiofibular fracture. Methods 86 patients with swelling after the operation of fracture of tibia and fibula were randomly divided into two groups, with 43 cases in each group. The observation group was treated with intravenous rapid infusion of mannitol, the control group was treated with Huoxue-Zhitong decoction combined with intravenous rapid infusion of mannitol. Swelling, pain visual analogue scale (VAS) score of limb of patients at 1 D and 10 d after operation, time of detumescence and acesodyne and the total efficiency of the treatment between the two groups were compared. Results The indices of the two groups mentioned above at 7 d after the operation significantly decreased, the value in the observation group was lower than the control group (1.3 ± 0.7 cm vs. 1.9 ± 0.9 cm, t=3.451; 1.5 ± 0.6 points vs. 1.9 ± 0.7 points, t=2.845; all P<0.01); The detumescence and acesodyne time of the observation group was shorter than those of the control group (8.6 ± 1.7 d vs. 9.8 ± 2.3 d, t=2.751; 5.5 ± 1.4 d vs. 6.7 ± 1.7 d, t=3.573; all P<0.01), the total efficiency in the observation group was superior to the control group, there were statistically significant differences (97.7% vs. 83.7%; χ2=4.961, P=0.026). Conclusions Huoxue-Zhitong decoction combined with mannitol is effective and safe in the treatment of swelling and pain after operation of tibiofibular fracture.
8.Radiological and therapeutic study of posterior bicondylar tibial plateau fractures

ObjectiveTo investigate the radiological characteristics,morphological characteristics of fracture fragments and treatments of posterior bicondylar tibial plateau fractures.MethodsA total of 47 patients with posterior bicondylar tibial plateau fractures treated between March 2005 and February 2009 were enrolled in the study.Characteristics of fractures and morphologies of fracture fragments were measured precisely with CT-chip Starpacs system.A retrospective study was carried out on the therapeutic results of the 47 patients undergone lateral condylar plate and medial condylar lag screw fixation via anterolateral combined with anteromedial tibial plateau approaches.Results Posteromedial condylar fracture was split one with small displacement,but the fracture was unstable.In the meantime,the fracture fragments were inverted three prism in shape and remained quite intact.Posterolateral condylar fracture was compression one and was often associated with soft tissue injury of knee joints.According to the Rasmussen radiology score,the results were excellent in 24 patients,good in 18,fair in three and poor in two.According to the Rasmussen function score,the results were excellent in 13 patients,good in 27,fair in two and poor in five.ConclusionsThe morphous of posteromedial condylar fracture is similar with that of Hohl type E fracture.The posterolateral condylar fracture is compression one.Anterolateral combined with anteromedial approaches for posterior tibial plateau fractures allow stage Ⅰbone fusion with internal fixation,simultaneously repair recombined injuries and achieve excellent radiological and clinical results.
9.Axial pressure capacity of injured vertebrae witch in dogs treated with injectable artificial bone composite following thoracolumbar fractures

BACKGROUND: Injectable artificial bone composite has been reported to consider as a moulding filler of vertebrae at the histology level; however, the therapeutic effect needs to be further studied at the level of axial pressure capacity. OBJECTIVE: To explore the axial pressure capacity of dogs with thoracolumbar fractures treated by injectable artificial bone composite. DESIGN, TIME AND SETTING: A randomized controlled animal experiment was performed at the Animal Laboratory of Shenzhen Longgang Central Hospital from September 2008 to January 2009. MATERIALS: Type I and type II vertebral pedicle screw systems were provided by Tianjin Zhengtian Medical Device Company Limited; injectable artificial bone composite, which was made of coral-hydroxylapatite compound, recombinant human bone morphogenetic protein-2 (Academy of Military Medical Sciences), and 2% chitosan solution (the Second Military Medical University of Chinese PLA and Shanghai Qisheng Biomaterials Institute), was manufactured according to the methods provided by Yin et al. METHODS: A total of 20 1-year-old healthy dogs were randomly divided into treatment and control groups with 10 dogs in each group. The model of those dogs with thoracolumbar fractures was made by imitating falling accidents; thereafter, the dogs in the treatment group were treated with vertebral pedicle screw system internal fixation and vertebroplasty by filling the injured vertebrae with injectable artificial bones. The control group was treated with vertebral pedicle screw system internal fixation alone. MAIN OUTCOME MEASURES: The maximum pressure intensity of injured vertebra, upper and lower vertebra at vertebral body center was detected using micro-computer pressure testing system after three months. RESULTS: The maximum pressure intensity at vertebral body center was not significant differences in the treatment group (100% cases) between injured vertebral body and its neighboring vertebral body, and axial loading was recovered. 10.The impact of hypoxia improvement on laryngeal squamous cell carcinoma chemotherapy.

OBJECTIVE:
To evaluate the effect of hypoxia improvement in Hep-2 cell on cisplatin-induced apoptosis.
METHOD:
Human laryngeal squamous cell carcinoma Hep-2 cells and HIF-1α-RNAi-Hep-2 cells were cultured in normoxic, hypoxic and reoxygenation condition. The inhibition of cisplatin on cell proliferation was evaluated by MTT assay. The influence of cisplatin on cell cycle and apoptosis were detected by flow cytometry.
RESULT:
The inhibition of cisplatin on cell proliferation was reduced by hypoxia. After HIF-1α gene was silenced, the inhibition of cisplatin on Hep-2 cell proliferation was increased apparently, but was still interfered partly by hypoxia. Hypoxia could induce cell apoptosis decreased and enhance chemotherapeutic resistance. After reoxygenation, cell apoptosis induced by cisplatin was increased significantly (P < 0.05). HIF-1α-RNAi-Hep-2 cells under hypoxia also showed certain resistance to apoptosis but the sensitivity to cisplatin was higher than that of Hep-2 cells. When cells were returned from hypoxic condition to normoxic condition for some time, the apoptosis induced by cisplatin was increased significantly (P < 0.05).
CONCLUSION
The improvement of hypoxic microenvironment with HIF-1α gene knockout could increase the sensitivity of Hep-2 cells to chemotherapy.
