1.Effective treatment and optimal prognosis of peripheral artery disease

OBJECTIVE: Newer medicine therapies have been proved efficacy in patients with intermittent claudication. With rapid advances of intervention devices and techniques for peripheral vessels in recent years, which has improved the therapy of peripheral artery disease (PAD). To review the clinical developments in treatment of PAD so as to provide references for optimal integrated therapy of medicine with proper interventional treatment, which enable PAD patients to achieve the greatest long-term prognosis. DATA SOURCES: A computer-based search for literatures related to PAD from 1991 to April 2006 was conducted in Medline databasewith the of "peripheral arterial disease, catheterization, peripheral, genetics", and the language was limited to English. Meanwhile, Chinese relevant literatures between 1994 and April 2006 were searched in Journal Full-text Database and Wanfang Database by computer with the keywords of "peripheral arterial disease, intermittent claudication, management". STUDY SELECTION: Primary selection was performed by following inclusive criteria: PDA and drug treatment, stent, angioplasty, genetic therapy, surgery, and articles related to PDA were selected. Scientific criteria of treatment for PDA were looked for in basic researches, clinical studies and those published in formal organizations. Exclusive criteria: Repetitive studies, reviews and Meta analytical literatures were excluded. DATA EXTRACTION: A total of 60 literatures about the treatment for PDA were collected, 30 enrolled articles in which were reviewed. Thirty repetitive studies, reviews and Meta analytical literatures were excluded. DATA SYNTHESIS: ①The prevalence of PAD depends on the diagnosis. Given the inaccuracy of physical examination, use of pulse examination as the sole criterion would grossly overestimate the prevalence. In contrast, a historical query for intermittent claudication would underestimate the prevalence of PAD. The ankle-brachial index (ABI) has been validated by angiography with the sensitivity of 95% and the specificity of 100%.②The risk factors of PAD were similar to those in patients with cardiovascular disease and cerebrovascular disease. The goals of therapy for patients with PAD are to prevent systemic atherosclerotic disease progression and clinical cardiovascular events as well as prevent limb loss and improve the functional status of patients with intermittent caludication.③Medical therapy of PAD should include manipulations of risk factors for atherosclerotic heart disease, including smoke-stopping, body mass reducing, blood glucose controlling, blood pressure controlling and hypercholestermia reducing etc. Instructive exercise therapy is the most effective way in non-interventional therapy, while medicine therapy mainly included antiplatelet therapy, angiotenic therapy and anticoagulation therapy. If the patients has obvious clinical ischemia symptoms and cannot be relieved by medical treatment, then interventional therapy or surgery is necessary. CONCLUSION: Patients with PAD can achieve the best long-term prognosis by an instructive exercise treatment integrated with optimal medicine therapy and proper interventional therapy.
2.Personality and psychological characteristics in perinatal women

Objective To investigate the psychological characteristics and personality of perinatal women, and to provide evidence for effective psychological intervention. Methods Five hundred and seventy pregnant women were randomly selected from the out-patient department from October, 2002 to March, 2003. Eysenck Personality Questionnaire(EPQ) , Self-rating Anxiety Scale(SAS) and Self-rating Depression Scale(SDS) were applied for their personality, psychological characteristics and the influencing factors. Psychological surveys were performed in 216 cases in labor, and 300 cases were followed up during 1-5 days and 42-60 days postpartum. Results During pregnant period, the incidence of anxiety and depression was 10. 5% and 10. 2%, respectively, while 4. 0% and 5. 3% during early stage of puerperium. The rate of depression was 40. 7% and 10. 0% during late stage of puerperium and after puerperium. Pregnant women with personality of stability were less likely to have psychological disorder. 3.Protective effect of reduced glutathione on renal mitochondria in rats with obstructive jaundice

Objective: To investigate the role of oxygen free radicals in renal dysfunction of rats with obstructive jaundice and the protective effect of reduced glutathione on renal mitochondria. Methods: Fifty-four Wistar rats were randomly divided into three groups: sham-operation control group (group A), obstructive jaundice group (group B), reduced glutathione treated group (group C). The model of obstructive jaundice in rats was established by common bile duct ligation (CBDL). In group C, reduced glutathione (150mg/kg.d) was given through abdominal cavity for 21 days. The rats were executed at 7th, 14th, and 21st day after operation respectively. The contents of serum BIL, Cr, BUN and the contents of renal mitochondria MDA and cholesterol were detected respectively, and the changes of renal histopathology were observed. 4.Analysis and Deployment of Network Security in Hospital Information System Construction

Objective To analyze and deploy in the hospital network security for guarantee hospital network and information of safety, and ensure stability operation in the hospital management information system. Methods The network environment was based on the original hospital, which was added to measures about the isolation for network security, the prevention of computer viruses and firewall, data backup and recovery, physical safety management and so on. Results After the comparatively perfect measure of network security was deployed, the security of hospital information and data was effectively protected, and the security threat was solved from the inner and outer network. Conclusion The deployment of network security prevention system can better give fully play to important function on which management information system can efficiently operate in whole hospital.
5.The diagnostic value of neutrophilic alkaline phosphatase activity detection in systemic lupus erythematosus

Objective To explore the diagnostic value of NAP activity detection in SLE.Methods Technique of AS-BI was used to NAP staining of peripheral blood smear in patients with newly-diagnosed SLE(30 cases)and normal healthy women(30 cases),to observe positive rate and positive score of NAP.Results The positive rate was(22.1±8.8)% in group SLE,(56.8±9.4)% in group normal;The positive score was(22.2±8.9)in group SLE,(58.2±9.8)in group normal.The positive rate and positive score were much lower in patients with SLE comparing with those in group normal.Differences were statistically significant(P<0.01).Conclusions The detection of NAP is useful in the patients with newly-diagnosed SLE,but we must go on researching the pathogenesis of NAP activity decreased in SLE.
6.Study on Fingerprint of Amino Acids in Honey by High Performance Liquid Chromatography

Objective To establish a method for fingerprint analysis of amino acids from honey by high performance liquid chromatography ( HPLC). Methods Amino acids of honey were concentrated by 732 cation exchange resin, and then were treated by pre-column derivatization with phenyl isothiocyanate, with praline as control peak. The chromatography was performed on a Waters Symmetry C18 ( 250 mm × 4.6 mm × 5 μm) column, with acetonitrile ∶ water (4∶1) as mobile phase A and 30 mmol/L sodium acetate ∶ acetonitrile (355∶15, acetic acid adjusting pH value to be 6.5) as mobile phase B by gradient elution. The detection wave length was set at 254 nm. The flow rate was 1.0 mL/min. The column temperature was 40℃, and the injection volume was 5μL. Results Sixteen common peaks were shown in the fingerprint of 15 batches of honey samples. The similarity for 15 batches of honey samples was in the range of 0.910 ~ 0.996 . Conclusion The fingerprint detection method is simple, practical, reproducible and specific, and can provide certain reference for quality control of honey.
7.Content Determination of the Polymer in Cefotaxime Sodium and Tazobactam Sodium(6:1)for Injection by HPSEC

OBJECTIVE:To establish a method for the content determination of polymer in Cefotaxime sodium and tazobactam sodium(6∶1)for injection. METHODS:High performance size exclusion chromatography(HPSEC)was performed on the column of TSK-GEL G2000SWXL with mobile phase of 0.01 mol/L phosphate buffer (pH 7.0) at a flow rate of 1.0 ml/min,detection wavelength was 254 nm,injection volume was 10 μl and the column temperature was 25 ℃. RESULTS:Cefotaxime and the poly-mers were well-separated;the linear range of cefotaxime was 2.3-226.4 ng(r=0.999 9);RSDs of precision and reproducibility tests were no more than 0.79%;results of durability test showed the changes of column temperature,flow rate,wavelength,pH and mobile phase salt concentration had little effects on the separation among different polymer peaks and between polymer peaks and main peaks. CONCLUSIONS:The method is simple,specific and sensitive,and can be used for the content determination of poly-mer in Cefotaxime sodium and tazobactam sodium(6∶1)for injuction.
8.Chemical constituents from Hedysarum polybotrys and their antitumor activities

Objective To investigate the chemcial constituents in Hedysarum polybotrys and their antitumor activities.Methods The chemical constituents were isolated by various column chromatographic methods,and their structures were elucidated mainly by NMR and MS evidences;Cytotoxicities of the purified compounds against human cancer cell lines HepG2 were evaluated by MTT method.Results Eleven compounds were isolated and identified as: n-tetracosanoic acid(Ⅰ),n-hexacosanic acid(Ⅱ),trioleic glyceride(Ⅲ),glycerol monopalmitate(Ⅳ),cetyl ferulate(Ⅴ),(+)syringaresinol(Ⅵ),7-hydroxy-4′-methoxyisoflavane(Ⅶ),isoliquiritigenin(Ⅷ),3-hydroxy-9-methoxypterocarpan(Ⅸ), ?-sitosterol(Ⅹ),and daucosterol(Ⅺ).Conclusion Compounds Ⅰ—Ⅳ and Ⅵ are isolated from this plant for the first time.Compound Ⅸ shows the inhibitory activity on HepG2 with IC50 values of 10.69 ?mol/L.
9.The protective effects of saxagliptin onβ-cell proliferation by inhibiting the degradation of SDF-1 in type 2 diabetes rats

Objective To investigate the mechanism of a dipeptidyl-peptidase-4 (DPP-4) inhibitor, saxagliptin, pro?moting the regeneration of islet beta cells in diabetic rats. Methods The male SD rats were randomly divided into three groups including control group (NC, n=10), diabetes group (DM, n=10) and diabetes treated with saxagliptin group (DM-S, n=10). DM-S group was treated with saxagliptin 1 mg/(kg·d) for twelve weeks. The pancreaticβcell function was analysed by hyperglycemic clamps. Immunohistochemistry with anti-PCNA was performed to observe the proliferation rate of pancreaticβcells. Immunofluorescence double staining with anti-insulin, anti-glucagon, anti-DPP-4 and anti-SDF-1 were performed to observe the expression of insulin, glucagon, DPP-4 and SDF-1 in pancreatic tissue. Western blot assay was performed to test the expression of Akt, p-Akt,β-catenin and free-β-catenin protein, and RT-PCR was performed to test the expressionlevels of c-myc and cyclinD1 mRNA in pancreatic tissue. Results Compared with NC group, there were significantly in?creased blood glucose, decreased islet function andβcell mass in DM group. Compared with DM rats, saxagliptin treatment significantly inhibited the expression of DPP-4, decreased the degradation of SDF-1, stimulated the proliferation ofβcells, and ultimately improved the islet function and histopathological changes of pancreas. Conclusion DPP-4 inhibitor saxa?gliptin can significantly improve islet function, which involved in the inhibition of the expression of DPP-4, the decreased degradation of SDF-1 and the stimulation of the proliferation ofβcells.
10.Mechanism of glucagon-like peptide 1 receptor agonist induced weight loss of mice

Objective To investigate the possible mechanisms of glucagon-like peptide 1 receptor agonists (GLP-1Ra) induced weight loss. Methods High fat diet induced obese c57BL/6 mice were divided into normal control group (N, n=8), high fat feeding group (HF, n=32) and GLP-1Ra group treated with GLP-1Ra (liraglutide 200μg/(kg·d) or 400μg/(kg·d) for 8 weeks). Changes of body weight, blood glucose and three acyl glycosides (TG) levels were observed in three groups. HE staining was used to observe the morphological changes. Immunofluorescence staining and real-time PCR were used to mea?sure the expression of UCP-1. Furthermore, the expression of PGC-1αin protein level was observed to explore the possible mechanism of GLP-1Ra induced browning in white fat (WAT). Results After 8-week liraglutide (Lira) administration, the body weights were significantly reduced in obese mice (P<0.05). The levels of blood glucose and TG were significantly high?er in HF group than those in N group, which reduced significantly in Lira (200μg·kg-1) and Lira (400μg·kg-1) administra?tion groups (P<0.05). HE staining showed adipocytes in perirenal and inguinal subcutaneous adipose tissue partly acquired brown-like morphological characteristics. The expression levels of UCP-1 protein and mRNA and PGC-1αprotein were ele?vated in adipse tissues, which increased more in Lira (400) than those in Lira (200, P<0.05). Conclusion GLP-1Ra can induce weight loss through white fat browning by activation of UCP-1.
