The freshmen column in 39 985 Projectacademic libraries was investigated , which showed that 15 out of the 39 985 Projectacademic libraries have established freshmen column on their library website .Its posi-tion, contents and forms were analyzed.The typical freshmen column in libraries of Qinghua University, Shanghai Jiaotong University , and Sun Yat-sen University was described with suggestions put forward for improvement of its position, contents and forms.
2.Stady on the expression of miRNA-21 in renal tubular epithelial cells

To describe the relationship between miRNA-21 and renal fibrosis.Methods NRK-52E cells cultured in vitro,then constructed fibrosis model induced by transforming growth factorβ1 ( TGF-β1 ) , then bone morphogenetic protein-7 ( BMP-7 )-containing lentivirus vector was used to transfect the NRK-52E cells to anti-fibrosis, SMAD1, SMAD6, FN1, rno-miR-21-5p gene expression were detected.Results After TGF-β1 treatment, the BMP-7 mRNA and protein expressions were significantly decreased, FN1 mRNA and protein expressions were significantly increased ( P<0.05 ) . After transfected by BMP-7-containing lentivirus vector, BMP-7 mRNA expression significantly increased(P<0.05), FN1, SMAD6 and rno-miRNA-21-5p mRNA decreased, SMAD1 mRNA increased(P<0.05).Conclusion Epithelial-to-mesenchymal transition (EMT) is positively correlated with the miRNA21 and TGF-beta 1, its maybe related to the changes of BMP-7, SMAD1 and SMAD6.

Nutrition Risk Screening 2002 (NRS 2002) was developed on the basis of 128 randomized controlled clinical studies by a group of experts led by Kondrup from the European Society for Parenteral and Enteral Nutrition (ESPEN).As the first evidence-based nutritional screening tool worldwide,NRS 2002 has been recommended for nutritional risk assessment of hospitalized patients in Europe for the addition of disease metabolism and its simplicity.In this article,we reviewed the increasing applications of NRS 2002 in China,pointed out the existing problems and made several suggestions on improvement for popularization and standardization of its clinical use.
4.The involvement of IL-1β/p38MAPK/NF-κB signal transduction pathway in regulating the expression of glucocorticoid receptor isoformsin human nasal epithelia in vitro

Objective To explore the possible upstream signal transduction mechanisms responsible for the decrease of glucocorticoid receptor (GR)α/GRβ in chronic rhinosinusitis (CRS) in vitro. Methods The GRα/GRβ decrease cell model was established by IL-1β-induced human nasal epithelia (HNE) in vitro. Changes in the protein and mRNA expression of GRα, GRβ and the key enzymes in the p38MAPK and NF-κB pathways were measured respectively, before and after being induced with different doses of IL-1β and specific inhibitors of p38MAPK and NF-κB. Analysis of variance (ANOVA) was used to analyze the data. Results With IL-1β-induction, the GRα/GRβ ratio declined in both a time-dependent manner and a concentration-dependent manner in HNE, which demonstrated the successful establishment of a GRα/GRβ decrease model in vitro. After cultured HNE were induced with the same set of IL-1β, the p38MAPK and NF-κB signal pathways were also activated. Either a specific inhibitor (SB203580) of the p38MAPK pathway or a specific inhibitor (PDTC) of the NF-κB pathway increased the GRα/GRβ ratio at the meantime of inhibiting their pathways. The expressions of phospho-NF-κBp50 and phospho-NF-κBp65 were significantly decreased by SB203580. PDTC had no influence on the expression of the key enzymes in the p38MAPK. Conclusion The decrease of GRα/GRβ ratio in HNE induced by IL-1β in vitro is induced through the p38MAPK/NF-κB signal pathways.
5.The use of covered stent for the treatment of spontaneous peripheral artery dissection: preliminary results in 9 cases

Objective To investigate the safety and effectiveness of using covered stent in endovascular isolation treatment of spontaneous peripheral artery dissection.Methods A total of 9 patients with peripheral artery dissection were collected.The dissectionwas located at iliac artery in 6 patients and at subclavian artery in 3 patients.Preoperative CT angiography was performed in all patients to determine the site and degree of the dissection,based on which the corresponding covered stents were prepared.After puncturing the femoral artery with Seldinger technique,angiography was performed to confirm the lesions in all patients,then,according to the angiographic findings the implantation of covered stent was carried out.Results Among the 9 patients,the dissections were located at the right common iliac artery in 3,at the right external iliac artery in one,at the left common iliac artery in 2 and at the left subclavian artery in 3.The length of the dissection was 0.9-3.3 cm with a mean of 1.7 cm.The distance between the initial part of arterial dissection and the target vessel opening was 0.5-5.6 cm with an average of 2.3 cm.The covered stent was successfully implanted in all patients.In one patient,the implanted stent covered the non-dominant left vertebral artery.During the procedure no obvious complications occurred.The patients were followed up for 3-32 months (mean of 18 months).No procedure-related complications were observed in all patients.Color Doppler ultrasound examination showed that all the implanted stents were unobstructed.Conclusion For peripheral artery dissection,endovascular isolation therapy by using covered stent is safe and effective.
6.Comparative study of DSCT,3.0T MR and DSA in the diagnosis of intracranial aneurysm

Objective To evaluate the characteristics and clinical application value of Dual Source CT(DSCT),3.0T MR and DSA in the diagnosis of intracranial aneurysm.Methods The imaging characteristics of DSCT,3.0T MR and DSA in 102 patients of in-tracranial aneurysm were analyzed retrospectively.Results The cerebral DSA were performed in 102 patients,and the sensitivity and accuracy of DSA were 100%.60 patients were detected by DSCT/CTA,and the sensitivity and accuracy of DSCT/CTA were 93.1% and 97.2% respectively.52 patients were detected by 3.0T MR/MRA,and the sensitivity and accuracy of 3.0T MR/MRA were 89.1% and 92.7% respectively.10 patients were detected simultaneously by CT and MR.The diagnostic sensitivity and accu-racy of these three methods had no significant difference(P >0.05).Conclusion The sensitivity and accuracy of DSCT,3.0T MR and DSA in the diagnosis of intracranial aneurysm are high and had no significant difference,each had its characteristic.Reasonable application of the three methods produces higher clinical value.
7.The significance and role of laparoscopic vertical cutting of the closed distal rectum in dual-anastomosis for patients with low rectal cancer

Background and purpose:Anastomotic leakage and low anterior resection syndrome(LARS) are both common complications in dual-anastomosis for patients with low rectal cancer. The aim of this study was to observe and explore the signiifcance and role of vertical cutting of the closed distal rectum in dual-anastomosis for patients with low rectal cancer.Methods:A total number of 120 patients with mid-low rectal cancer who admitted to and completed laparoscopic rectal cancer resection in the Department of General Surgery, the First Afifliated Hospital of Soochow University from Feb. 2010 to Jun. 2014 were pair-matched into Groups A and B based on their gender,age, tumor size, the distance of lower edge to the dentate line and tumor staging, etc. For the 55 patients in Group A (observation group), the rectum distal end was closed vertically instead of horizontally while disposing “the ifrst intestinal anastomosis”, intestine-intestine anastomosis was conducted in an “end-corner” approach when dealing with “the second intestinal anastomosis”, upper corner (“dog ear”) of the closed line in the distal end of the rectum was removed, the lower corner (“dog ear”) of the closed line in the distal end of the rectum was removed using vascular occlusion clamp method, and the T-shaped interchanges (“dangerous triangle”) of stapled sutures formed after anastomosis were strengthened with absorbable suture. For the 65 patients in Group B (control group), laparoscopic dual anastomosis was conducted using conventional method, and the two “dog-ears” and “dangerous triangles” were kept without any treatment. The clinical outcomes of the two groups of patients were analyzed retrospectively. Results:In group A, It was convenient to complete the operation when the “dog ears” and “dangerous triangle” on the vertical line after cutting the closed distal rectum vertically by “end-corner” anastomosis. The axis of intestine formed a certain angle making the closed distal rectum into “ampulla” sample without “dog ears”. the “dangerous triangle” were strengthened with absorbable suture. In group B, The distal and proximal intestine located on the same axis after intestine-intestine anastomosis leaving two “dog ears” and a “dangerous triangle”. The general clinical data of patients in the two groups were comparable and not signiifcantly different (P>0.05). The two groups of patients showed no signiifcant differences in blood loss, postoperative drainage, postoperative anastomotic bleeding, anal exhaust time, and length of hospital stay (P>0.05). However, the operation time as well as the numbers of anastomotic ifstula occurrence, defecation, tenesmus and post-operation re-ostomy differed significantly (P<0.05).Conclusion:Vertical cutting of the closed distal rectum with dual anastomosis made the “new” intestine closer to the physiological bending and morphology of the rectum, meanwhile, it simpliifed the approach of removing “dog ear” and strengthening “dangerous triangle”, ifnally it signiifcantly reduced the incidence of post-surgical complications.
8.Modification and efficacy observation of laparoscopic dual anastomosis for mid-low rectal cancer.

OBJECTIVETo explore a new procedure of laparoscopic dual anastomosis for mid-low rectal cancer to reduce postoperative complications.
METHODSClinical data of 56 patients with mid-low rectal cancer undergoing laparoscopic rectal cancer resection(modified double-stapling technique, MDST, modification group) in the Department of General Surgery, the First Affiliated Hospital of Soochow University from February 2010 to June 2014 were compared with the data of 64 patients with mid-low rectal cancer (conventional double-stapling technique, DST, convention group) in the same period based on gender, age, tumor size, the distance from lower edge to the dentate line and tumor staging, etc. Patients in the modification group received operation as follows: (1) the rectum distal end was closed vertically instead of horizontally. (2) the anastomosis was conducted in an "end-corner" approach. (3) upper corner of the closed line in the distal end of rectum was removed. (4) the lower corner of closed line in the distal end of rectum was removed using vascular occlusion clamp method. (5) two T-shaped interchanges ("dangerous triangle") of stapled sutures formed after anastomosis were strengthened with absorbable suture. Patients in the convention group received laparoscopic dual anastomosis using conventional method: two corners and "dangerous triangles" were kept without any treatment. The clinical outcomes of two groups were analyzed retrospectively.
RESULTSThe intraoperational blood loss, postoperative drainage volume, postoperative anastomotic stoma bleeding, bowel function return and hospital stay were not significantly different between the two groups (all P>0.05). As compared to the convention group, the modification group had longer operation time [(211 ± 91) min vs. (174 ± 57) min, P<0.05], lower incidence of postoperative anastomotic leakage [1.8%(1/56) vs. 12.5% (8/64), P=0.030], lower tenesmus rate [3.6% (2/56) vs. 14.1% (9/64), P<0.05], less postoperative stoma re-creation [0 vs. 9.4% (6/64), P<0.05].
CONCLUSIONModified laparoscopic dual anastomosis for mid-low rectal cancer can significantly reduce the incidence of post-surgical complications such as anastomotic leakage.

9.Establishment and initial application of a medicine screening technique based on human promoter of CCR5.

This research ws carried out to construct a medicine screening system targeting at human promoter of CCR5. The gene Human promoter of CCR5 was inserted into the rebuilt vector pGL3-neo. The pGL3-neo-CCR5 plasmids were transfected into Jurkat cells (the cell line of acute T lymphocyte leukemia). The lasting transfected cells were screened by G418. After seven kinds of traditional Chinese medicine had acted separately on the lasting transfected cells for 16h, the expression levels of CCR5 promoter in the cells were detected. The results showed that the level of luciferase activity of Shuanghuanglian-injectio group was remarkably lower than that of control (P < 0.05), and the levels of luciferase activity of Chuanhuning group, Baical skullcap root group, and Milkvetch root group were remarkably higher than that of control (P < 0.01). Shuanghuanglian-injectio depressed the activity of the transfected CCR5 promoter in cells cultivated in vitro; Chuanhuning, Baical skullcap root and Milkvetch root boosted the activity of the transfected CCR5 promoter in cells cultivated in vitro. Thus a medicine screening system based on Human promoter of CCR5 was initially constructed.

10.The establishment and evaluation of the application of assessment system in the ability of physician-patient communication of pediatric residents

Objective To explore the assessment system of humanistic communication in the grad-uation examination of pediatric residents. Methods A humanistic communication assessment program and an evaluation form were designed and put into practice in the graduation examination of pediatric residents in 2016 and 2017 in Beijing Children's Hospital affiliated to Capital Medical University and the scores were collected and analyzed to compare the differences in pediatric residents who have undergone system-atic physician-patient communication scene training and who didn't. With this analysis, the assessment system was further improved. Results Of all the 5 dimensions, communication content, nonverbal skills, and communication effects and the comparison of their respective scores were statistically significant. The scores of the residents who have undergone systematic physician-patient communication scene training are much better than those who didn't. Conclusion This assessment system focuses on the investigation of ability. From the design of examination questions and assessment scenes, to the evaluation of students per- formances, only when candidates have mastered the skills of communication can they do well in examina-tions and achieve good results.
