1.Nosocomial Candidal Infection among Tuberculosis Patients and Its Drug Sensitivity

OBJECTIVE To understand nosocomial candidal infection among tuberculosis patients and its drug(sensitivity).(METHODS) Totally 6280 clinical samples from Jan 2002 to Dec 2004 were cultured;and part of(positive) ones were tested their drug sensitivity to FC,FLC,KET and ITC.RESULTS From all cultured samples 2767 Candida strains(positive rate was 44.06%) were isolated.From them C.albicans,C.glabrata,C.tropicalis,C.krusei and other Candida were 2006, 344,206,95 and 116 strains,their positive rate was 31.94%,(5.48%),3.28%, 1.51% and 1.85%,respectively.Their drug sensitivity rate to four drugs were as follows: to FC was 93.73%,to TTC 62.76%,to FLC 51.03% and to KET was 47.59%. CONCLUSIONS The condition of(tuberculosis) patient would become serious due to higher dual infection rate by Candida after too long anti-(tuberculosis) treatment.Among the four kinds of drugs the FC has the best anti-candidal activity.
2.Effects of FXR gene deletion on neurobehaviors and neurotransmission systems in female mice

Aim To investigate how Farnesoid X re-ceptor (FXR),a nuclear hormone receptor,acts on neurological behaviors such as emotion,social behav-ior,memory and so on.Methods FXR’s function in central nervous system was evaluated by conducting a battery of behavioral tests including elevated plus maze test (EPMT),forced-swimming test (FST),social in-teraction test (SIT ), and passive avoidance test (PAT),and the contents of neurotransmitters were de-termined by the LC-MS /MS method in FXR knockout (KO)female mice and their wild-type controls.Re-sults FXR KO mice showed significantly increased immobility time in FST (P <0.01 ),and it showed in-creased tendency to enter the open arms in EMPT (P<0.01 ).The number of probing the open arms by FXR KO mice was more than that of the controls.Mo-reover,in SIT,FXR KO mice had remarkably in-creased sniffing interactions with the stranger mouse in the same cage (P <0.01 ).But in PAT,the latency for FXR KO mice to enter the dark chamber on the test day and the number of FXR KO mice to enter the dark chamber didn’t differ from those wild-type mice.In hippocampus,the contents of GABA,Glu,and NE were decreased prominently in FXR KO mice (P <0.05,P <0.05 and P <0.01 ,respectively)as well as the ratio of GABA to Glu (P <0.05).But in pre-frontal cortex,none of the neurotransmitters examined showed any difference between FXR KO mice and their controls.Conclusion FXR may be involved in main-tenance of the homeostasis of neural transmission in the central nervous system,thereby influences the emotion-al and social behavior in animals.
3.Association of intestinal microecology with hepatic encephalopathy

Hepatic encephalopathy (HE) is a common and complex neuropsychiatric complication in patients with liver cirrhosis and has the features of high incidence, recurrence, and mortality rates, and it is also an important factor for the increased risk of hospitalization and rehospitalization. At present, there are still no effective drugs for HE. Recent studies have found that intestinal microecological changes are associated with the development, progression, and prognosis of HE, and the regulation of intestinal flora has a certain preventive effect on HE. This article summarizes the research on the association between intestinal microecology and HE in recent years and hopes to provide new ideas for clinical prevention and treatment of HE.

5.Recurrence and survival analysis of postoperative patients aged 25 to 59 years with differentiated thyroid carcinoma

Objective To investigate the recurrence and survival of postoperative patients with differentiated thyroid carcinoma (DTC) aged from 25 to 59 years.Methods We retrospectively analyzed the clinical data of 36 patients with DTC treated in our hospital from 1996 to 2011,and the recurrence and survival status of the patients were recorded.Kaplan-Meier analysis was carried out to analyze factors that affect the patient's survival.Results Nine patients died of recurrence or metastasis,and the interval between the initial surgery and recurrence ranged from 22 to 46 months.The survival time of the 36 patients ranged from 34 to 135 months with a 10-year survival rate of 75.0%.Kaplan-Meier analysis showed that male patients had a significantly shorter mean survival time than female patients (χ2=3.164,P=0.041);the median survival time of patients aged 45-59 years was obviously shorter than that of patients aged 25-44 years (χ2=4.622,P=0.032);the postoperative survival in patients with 131I therapy was significantly longer than those who did not receive the therapy (χ2=4.527,P=0.033),and was not affected by total excision of the thyroid gland (χ2=0.988,P=0.320).No significant difference was found in the median survival of patients in different clinical stages (χ2=2.2132,P=0.167).Conclusion In young and middle-aged patients with DTC,postoperative recurrence is the most likely in 2 to 4 years after the surgery.Male patients at 45-59 years of age who do not receive 131I treatment are at high risks of tumor recurrence.
6.Application of the continuing care in dysphagia patients with indwelling gastric tube

Objective To investigate the application of continuing care in the indwelling gastric tube feeding of patients with dysphagia in order to improve the patients′ satisfaction and reduce complication as well as the readmission rate. Methods A total of 60 discharged patients undergoing indwelling gastric tube feeding were selected as the study subjects. Out-of-hospital nursing team conducted active care in the observation group consisting of 30 patients while the control group with 30 patients went back home and received passive care from the out-of-hospital nursing team. Indexes of the two groups were collected including complications after two-month indwelling gastric tube feeding, patients′ satisfaction, re-attendance rate, active consultation rate and swallowing rehabilitation and so on. Results The observation period, the observation group abdominal distension / diarrhea, nasopharyngeal mucosa injury, aspiration, lung infection were 1, 2, 2 and 1 cases, control group 6, 9, 8 and 7 cases with significant difference between two groups ( χ2 = 4. 04,5. 45,4. 32,5. 19, respectively; P < 0. 05). In the observation group, the rates of ED visits, readmission rate, active consultation rates were 0% , 6. 7% and 50% while these of the control group were 13. 3% ,26. 7% and 6. 7% . There was significant difference between two groups (χ2 = 4. 28,4. 32,13. 87, respectively; P < 0. 05); the two group continuity nursing satisfaction, the observation group than in the control group ( U = 2. 121, P < 0. 05). Conclusions The continuing nursing care outside the hospital can improve the caring quality for patients experiencing indwelling gastric tube feeding, patients′ satisfaction and active consultation rate, and reduce the re-attendance rate.
7.Effects of biofeedback electrical stimulation on stress urinary incontinence and pelvic floor muscle strength during postpartum rehabilitation

Objective:To analyze the effect of biofeedback electrical stimulation on the prevention of stress urinary incontinence and pelvic floor muscle strength during postpartum rehabilitation.Methods:A total of 200 parturients who gave birth in Ningde Municipal Hospital of Ningde Normal University from October 2021 to April 2022 were included as research objects and divided into the control group and the observation group according to different rehabilitation programs, with 100 cases in each group. The control group was given routine rehabilitation, and the observation group was given biofeedback electrical stimulation on the basis of the control group, the parturients in the two groups were treated for 3 months. The occurrence of stress urinary incontinence in the two groups was compared, and the pelvic floor muscle strength before and after treatment were compared between the two groups. The scores of International Consultation on Incontinence Questionnaire Short Form (ICI-Q-SF) and Pelvic Floor Impact Questionnaire-7 (PFIQ-7) were compared between the two groups.Results:The total incidence of stress urinary incontinence in the observation group was lower than that in the control group: 4.00%(4/100) vs. 15.00%(15/100), there was statistical difference( χ2 = 9.00, P<0.05). After treatment, the muscle strength grade, class Ⅰ muscle fiber, class Ⅱ muscle fiber and average voltage of pelvic floor muscle in the observation group were higher than those in the control group: (3.85 ± 0.27) grades vs. (3.74 ± 0.32) grades, (10.23 ± 1.17) μV vs. (8.84 ± 1.13) μV, (11.56 ± 0.19) μV vs. (10.98 ± 0.24) μV, (18.12 ± 3.24) μV vs. (14.69 ± 3.01) μV, there were statistical differences ( P<0.05). After treatment, the scores of ICI Q-SF and PFIQ-7 in the observation group were lower than those in the control group: (7.02 ± 1.26) scores vs. (8.26 ± 1.15) scores, (18.96 ± 4.31) scores vs. (24.17 ± 5.62) scores, there were statistical differences ( P<0.05). Conclusions:The application of biofeedback electrical stimulation in postpartum rehabilitation can reduce the incidence of stress urinary incontinence, improve postpartum pelvic floor muscle strength, and reduce the impact of stress urinary incontinence and pelvic floor muscle disorder on daily life.

9.Effects of anti-liver fibrosis treatment on the occurrence of liver cancer in patients with hepatitis B-related liver cirrhosis

Objective:To investigate the effect of anti-liver fibrosis treatment on the occurrence of liver cancer in patients with hepatitis B-related liver cirrhosis within three years.Methods:1,049 cases with hepatitis B-related liver cirrhosis who were hospitalized in Beijing Ditan Hospital affiliated to Capital Medical University from October 2008 to August 2016 were enrolled. Clinical data were collected, and COX regression analysis was used to find the independent influencing factors for the occurrence of liver cancer in patients with hepatitis B-related liver cirrhosis within three years. According to whether the patients had received anti-liver fibrosis treatment for ≥ 6 months, they were divided into combination and antiviral group. There were 388 cases in combination group and 661 cases in antiviral group. In addition, the combination group received anti-liver fibrosis therapy with Chinese patent medicine on the basis of antivirus, and the antiviral group received antiviral treatment. The incidence of liver cancer within three years were compared between the two groups, and the incidence of liver cancer in patients with different Child-Pugh grades and mPAGE-B risks was further analyzed. The independent samples t-test, Mann Whitney U test, χ2 test or Fisher's exact probability method were used for data comparison.Results:Anti-liver fibrosis treatment was an independent protective factor to prevent liver cancer in patients with hepatitis B-related liver cirrhosis within 3 years ( P < 0.05). The incidence of liver cancer in the combination group was lower than antiviral group within 3 years (10.3% vs. 15.4%, χ2 = 5.480, P < 0.05). Child-Pugh stratified analysis showed that the risk of liver cancer was significantly reduced in Child-Pugh grade A patients (6.7% vs. 12.6%, χ2 = 2.857, P = 0.040). Among high-risk patients with mPAGE-B, the incidence of liver cancer was significantly lower in combination group than control group (13.7% vs. 19.9%, χ2 = 6.671, P = 0.031). Conclusion:Compared to antiviral therapy alone, combined anti-liver fibrosis and antiviral therapy can reduce the liver cancer occurrence risk in patients with hepatitis B-related liver cirrhosis for 3 years. Patients with Child-Pugh grade A and high-risk group by mPAGE-B scores are the dominant population to receive treatment.
