1.The role of bronchial epithelial-mesenchymal transformation and the effect of glucocortiod on it in air-way remodling of asthma

Objective To investigate wether bronchial epithelial-mesenchymal transformation (EMT)take part in airway remodling in asthmatic mice,and the effect of inhaled glucocorticoid to it. Methods Thirty BALB /c mice were randomly divided into control group(n =10),asthma group(n =10) and budesonide group(n =10).The qualitity of serum OVA-sIgE was measured to vertify asthma modle. Bronchial airway thickness collagen deposition area was analyzed by HE and Masson staining to test the level of airway remodling.ELISA was used to test the quantity of transforming growth factor(TGF)-βin bron-choalveolar lavage fluid(BALF).Immunohistochemical assay was used to observe fibroblast specific protein-1 (FSP-1 )expressing area.Western blot and realtime PCR were performed to analyze the protein and mRNA expression of E-caderin,Vimentin and FSP-1 .Results The expression of OVA-sIgE,bronchial airway thick-ness,the collagen deposition area were significantly higher in asthma group than those in control group,while the above index in BUD group were allivated compared to those in asthma group(P <0.01 ,respectively). The expression of TGF-βin BALF increased and the expression of FSP-1 located in bronchial epithlium,the quantity of E-caderin significantly decreased,and the quantity of Vimentin and FSP-1 increased by Western blot and realtime PCR in asthma group,compared to those in control group(P <0.01 ).While the level of E-caderin increased(P <0.01 ),TGF-βand Vimentin reduced partly in BUD group(P <0.05 ),and there was no differece in the level of FSP-1 between BUD group and asthma group(P >0.05).Conclusion Lung tissue EMT take part in airway remodling of asthma,which is mainly focus on bronchial epithlium.The effect of traditional inhaled budesonide can not heal the bronchial EMT in asthma.
2.Study on lobular involution of glands around breast cancer

Objective To study lobular involution of tissues around breast tumor,and to assess the consistency of lobular involution of tissues from different parts of the breast.Methods 22 patients receiving breast conservation surgery in Breast Surgery Department of Peking Union Medical College Hospital from Dec.1 st,2010to Dec.1 st,2011 were collected.88 pieces of HE staining were measured in terms of lobular area and number of acini per lobular,and lobular involution was evaluated.Bivatiate correlation analysis was applied to explore correlation between lobular area and acini per lobular.ANOVA,crosstabs and reliability analysis were applied to explore involution consistency of different parts of breast (P < 0.05).Results The average area of lobuli was (90 248.5 +56 909.4) μm2 and the number of acini was 25.68 ± 18.86 per lobular.The lobular area and number of acini were correlated with each other significantly(Pearson r =0.78,P < 0.01).Involution status of different parts of breast had good consistency(for lobular area 19 cases showed no difference in ANOVA analysis and for lobular area/number of acini 18 cases showed no difference; Kappa coefficient =0.65; ICC =0.73).Conelusions Lobuli around breast tumor have comparatively poor involution,with big lobuli and large number of acini,which are correlated with each other.Involution status of different parts of a breast has good consistency.Biopsy from one site to evaluate involution extent of the whole breast is practicable.
3.Correlated research between the expressions of C-met and COX-2 in papillary thyroid cancer

Objective To study the expressions of C -met and COX -2 in thyroid papillary cancer ( TPC) and to analyze the relationship between their expressions and clinicopathological features .Methods S-P immunocytochemical technique was used to examine the expressions of C -met and COX-2 in 96 cases of TPC and 46 cases of benign tissues ,and their relationship with clinicopathological features were analyzed .Results The positive rates of C -met and COX -2 in 96 cases of TPC were 92.71% and 71.88%,showing obvious differences between TPC and benign tissues (P<0.001).In TPC there is a statistically significant difference of the expression of C-met in older than45 years and less than 45 years of age(P<0.05).Difference of COX-2 expression in TPC was statistically significant in patients with and without lymph node metastasis ( P<0.05 ) . The positive expression of COX -2 was closely related to the expression of C -met(r=0.451,P<0.05).Con-clusion The abnormal expressions of C -met and COX-2 could be important significance in the growth and in-vasion of TPC,and COX-2 might have some relationship with lymph node metastasis .
4.Constructing Prediction Models for Small for Gestational Age Based on Multimodal Clinical and Ultrasonographic Data

ObjectiveTo explore the predictive value of multimodal clinical and ultrasonographic data in first- and second-trimester for small for gestational age (SGA), so as to build and internally validate SGA prediction models based on multiple machine learning algorithms. MethodsThis retrospective study enrolled 1,307 pregnant women with singleton pregnancies, diagnosed SGA according to INTERGROWTH-21st fetal growth criteria, and collected multimodal clinical data including general clinical information, biochemical test data, and prenatal ultrasound screening data. Extreme gradient boosting (XGBoost) algorithm was used to calculate the importance of variables. Seven machine learning algorithms were used to construct and internally verify the prediction models. The area under the receiver operating characteristic curve (AUC) was used as the main indicator to measure the prediction performance and used to compare predictive performance between models with the sensitivity at a 10% false positive rate. ResultsThe optimal prediction model built based on general clinical information and biochemical test data had an AUC of 0.70, 95%CI (0.609, 0.791) and a sensitivity of 0.38, 95%CI (0.236, 0.519). The optimal prediction model based on prenatal ultrasound screening data was better than the former, with an AUC of 0.77, 95%CI (0.687, 0.858) and a sensitivity of 0.62, 95%CI (0.457, 0.743). The two data sets were combined to form the multimodal clinical dataset, and the performance of the best prediction model was further improved with an AUC of 0.91, 95%CI (0.851, 0.972) and a sensitivity of 0.88, 95%CI (0.745, 0.947), and the model calibration showed good goodness of fit. ConclusionBy using machine learning algorithms to fully explore the predictive value of different types of clinical data for SGA in first- and second-trimester, this study proves the absolute advantages of multimodal clinical data for SGA screening, and provides an accurate and effective reference for personalized management of pregnant women.
5.Clinical Observation on Treatment of 460 Community-acquired Pneumonia Cases in Children with Qing-Fei Tong-Luo Ointment

This study was aimed to evaluate the clinical effect and safety of Qing-Fei Tong-Luo (QFTL) ointment for treating children with pneumonia.Randomized controlled trial (RCT) was conducted among 460 cases of children with pneumonia.The observation group was given QFTL ointment combined with basic treatment.And the control group was only treated by basic treatment.Evaluation was given on the total clinical efficacy,disappeared time of fever,cough,expectoration,shortness of breath,and medication safety.The incidence of respiratory diseases was followed up on the 30th days after drug withdrawal.The results showed that in the aspect of clinical efficacy between two groups,the cure rate of the observation group was 98.26%,and that of the control group was 93.89%,with statistic significance (P < 0.05).The cure rate of the observation group was better than that of the control group.There was statistical difference on expectoration disappeared time (P < 0.05).There was no statistical difference on disappeared time of fever,cough and shortness of breath (P > 0.05).There was statistical difference on the incidence of respiratory diseases on the 30th days followed-up after drug withdrawal (P < 0.05).There was no statistical difference on the incidence of upper respiratory tract infection,pneumonia and asthma (P > 0.05).No adverse reactions occurred in the observation group.It was concluded that QFTL ointment combined with basic therapy on the treatment of pneumonia in children was significantly better than the control group in the aspect of clinical efficacy,expectoration disappeared time and the incidence of bronchitis.It is safe and effective.The prognosis is good and worthy of promotion in the clinical practice.
6.Animal experimental study on the examination of upper digestive tract with medical disposable portable endoscopy

Objective:To evaluate the safety, feasibility and operational performance of self-developed medical disposable portable endoscopy (YunSendo) for upper gastrointestinal endoscopy examination in Ba-Ma mini-pigs.Methods:A total of 10 Guangxi Ba-Ma mini-pigs were used in the experiment, and mucosal injury models were established in advance by biopsy forceps in esophagus, stomach, and duodenum. Each experimental animal underwent medical disposable portable endoscopy and Olympus endoscopy (GIF-Q260J) performed by two endoscopists separately. The time when the endoscope reached the duodenum, the number of detected mucosal injuries and endoscopic pictures of different parts with standard image acquisition were recorded. Endoscopic operational performance and endoscopic image quality were evaluated. Different endoscopists recorded experimental results with blind method. The procedures of the two endoscopic examinations were performed by coin-tossing method. The paired t test was used for statistical analysis. Results:There were no statistically significant differences in the insertion time and total operation time between medical disposable portable endoscopy and Olympus endoscopy ( (171.00±9.96) s vs. (164.00±17.84) s, (285.00±33.94) s vs. (273.40±23.46) s; t=1.289 and 1.281, P=0.230 and 0.232). There were no statistically significant differences in the percentage of time of clear visual field during endoscopy insertion and total operation between medical disposable portable endoscopy and Olympus endoscopy ((91.83±1.85)% vs. (91.52±1.51)%, (93.07±3.10)% vs. (92.06±2.57)%; t=0.401 and 0.689, P=0.698 and 0.508). Moreover, there were no statistically significant differences in the score of comprehensive operation performance, score of clear image number, score of image color recognition, score of image illumination, comprehensive score of image quality and number of detected mucosal injuries ((9.66±0.30) points vs. (9.86±0.15) points, (39.50±0.71) points vs. (39.30±1.06) points, (39.70±0.48) points vs. (39.40±0.70) points, (39.40±0.70) points vs. (39.50±0.71) points, (9.88±0.09) points vs. (9.85±0.20) points, 9.80±0.42 vs. 9.90±0.32; t=2.176, 1.000, 1.152, 0.317, 0.629 and 0.557, all P>0.05). There were no adverse events after operation in medical disposable portable endoscopy group and Olympus endoscopy group. Conclusions:The medical disposable portable endoscopy is safe and feasible for endoscopy examination in live animal models. Different parts of upper gastrointestinal tract and mucosal lesions can be clearly detected. The operational performance and the image quality are excellent, which is similar to Olympus endoscopy (GIF-Q260J).

8.Evaluation of clinical efficacy and safety on phacoemulsification combined with gonioscopy-assisted angle plasty for primary angle-closure glaucoma with cataract

Objective:To evaluate the efficacy and safety of phacoemulsification combined with gonioscopy-assisted angle plasty (Phaco-GAAP) for primary angle closure glaucoma (PACG) with cataract.Methods:A case series study was carried out.Twenty-five eyes of 22 patients with PACG and cataract were enrolled in Beijing Tongren Hospital from April 2022 to August 2022.All of the patients received Phaco-GAAP surgery.During the operation, viscoelastic-assisted goniosynechialysis was performed at first, followed by a secondary angle plasty for residual peripheral anterior synechiae (PAS) based on the quantified assessment by gonioscopy, and the extent of PAS was recorded intraoperatively.The operated eyes were followed at 1 day, 1 week, 1 month and 3 months after surgery to evaluate intraocular pressure (IOP), PAS range, the number of anti-glaucoma drugs application, operation-related complications, and success rate.The qualified success rate was defined as medicine-controlled IOP ≤21 mmHg after surgery, and complete success rate was defined as IOP ≤21 mmHg without any anti-glaucoma medication.This study complied with the Declaration of Helsinki and was approved by the Ethics Committee of Beijing Tongren Hospital (TRECKY2021-136). Written informed consent was obtained from each patient prior to entering the research cohort.Results:The extent of PAS was [270(225, 360)°], [165(110, 215)°] and [100(35, 175)°] at preoperation, first and secondary angle plasty, respectively, showing a significant difference among them ( χ2 =40.742, P<0.001). The PAS range was significantly reduced at first angle plasty in comparison with preoperation and was significantly reduced at secondary angle plasty in comparison with at first angle plasty (both at P<0.001), and the proportion of the angle PAS range ≥180° decreased from 48% to 24% after second angle plasty.In 13 eyes finished gonioscopy, the PAS range was [240(195, 305)°], [60(25, 182.5)°], [170(120, 275)°]and [180(140, 280)°]at preoperation, at the end of operation, postoperative 1 month and 3 months, respectively, with a significant difference ( χ2 =23.631, P<0.001). The PAS range was significantly smaller at postoperative 1 month, 3 months than that at preoperation (both at P=0.004) and larger than that at the end of operation ( P=0.011, P=0.003). The IOP was (40.19±17.23), (15.80±7.98), (13.89±5.09), (12.80±3.79) and(13.24±2.78) mmHg before operation and 1 day, 1 week, 1 month and 3 months after operation, respectively, showing a significant difference ( F=44.031, P<0.001), and the IOP was significantly reduced after operation (all at P<0.001). The PAS range at the end of operation was positively correlated with preoperation ( rs=0.409, P=0.042). The complete and qualified success rates were 95.8%, 95.8% for postoperative 1 month, 95.8% and 100% for postoperative 3 months, respectively.The primary complication was intraoperative anterior chamber angle hemorrhage, with an incidence of 68%. Conclusions:Phaco-GAAP can intraoperatively quantify PAS range and guide secondary angle plasty, therefore, it is an effective and safe surgical intervention for PACG with cataract.
9.Research on real-world study of tafluprost in glaucoma and ocular hypertension

Medication treatment is the most common measure for lowering intraocular pressure in glaucoma, and prostaglandin analogues have been unanimously recommended as the first choice for the treatment of glaucoma in multiple guidelines or consensus.Real-world studies (RWS) are observational studies in which diverse data are obtained in clinical environment, community or home settings to evaluate the real impact of some treatment on patients’ health.RWS have shown that tafluprost has a good effect on lowering intraocular pressure in patients with primary open angle glaucoma, ocular hypertension, normal tension glaucoma and other types of glaucoma, and it has mild ocular adverse effects in monotherapy for treatment-naive patients, alternative therapy and combination therapy.However, there are limiting factors for RWS such as the absence of the control group, and further studies are needed to evaluate the drug efficacy.In this article, the RWS of patients with different types of glaucoma and high intraocular pressure treated by tafluprost and the efficacy of tafluprost under different treatment modes as well as the safety of tafluprost and medication compliance in RWS were reviewed, so as to provide certain guidance for the practical clinical application of tafluprost.

Objective To analyze the effects of Hual qi huang granules on children with mycoplasma pneumoniae pneumonia.Methods A randomized,multicenter parallel controlled clinical trial was carried out.A total of 3 000 cases of hospitalized children with mycoplasma pneumoniae pneumonia were selected.All of them were given treatment for mycoplasma pneumoniae pneumonia with macrolide antibiotics and symptomatic treatment.They were randomly divided into 2 groups:research group and control group.The children of research group were give oral Huai qi huang granules for three months.According to the classification of pneumonia,these two groups were divided into:lobar pneumonia research group,lobar pneumonia control group,lobular pneumonia research group,lobular pneumonia control group.The hospitalization duration of fever,length of hospital stay,the absorption area of lung inflammation and pneumonia severity sores were observed.The frequency of upper respiratory infections,bronchitis,pneumonia were observed in 3 months after discharge.Results 2 378 cases were investigated.The hospitalization duration of fever,length of hospital stay of research group were significantly shorter than that of in control group (P < 0.001).The children with lobar pneumonia,2 weeks after treatment,the absorption of consolidation of the lobar pneumonia research group is significantly better than lobar pneumonia control group (P <0.001).After two weeks treatment,the pneumonia scores of lobar pneumonia research group is lower than lobar pneumonia control group (P < 0.05).Followup of 3 months after hospital discharge,frequency of upper respiratory infection and bronchitis of research group,were significantly lower than that of control.In addition,appetite increased significantly in research group than control (P < 0.001).There are 21 cases with drug associated adverse reactions (mild diarrhea),including 12 cases of research group,9 cases of control group,and there was no statistical significance (P >0.05).Conclusion Standard treatment combined with oral Huai qi huang granules in the treatment of mycoplasma pneumoniae pneumonia,can significantly shorten hospitalization duration of fever,length of hospital stay and reduce the severity score of pneumonia.Three months oral Huai qi huang granules can significant reduce the frequency of respiratory infections and bronchitis,also can increase patients appetite,and be safe.
