1.The effect research of specific immunotherapy of allergic rhinitis and allergic rhinitis combined bronchial asthma.

OBJECTIVE:
To study the therapeutic effects of the specific immunotherapy (SIT) on allergic rhinitis and allergic rhinitis combined bronchial asthma. METHED: All patients were classified into allergic rhinitis group (AR group) with 32 patients and allergic rhinitis combined bronchial asthma group (AR+BA group) with 32 patients. Another health control group with 32 cases was designed as well. The allergens,symptom scores and therapeutic effects of the former two-group patients were analysis, and the serums of all three-group cases were extracted to evaluate the specific Immunoglobin E(sIgE), Interleukin-4 (IL-4). The SPSS13. 0 package was applied to conduct t-test and chi-square test, and the difference of P<0. 05 was regarded as statistical significance.
RESULT:
The main allergens of 64 patients were dermatophagoides farinae and dermatophagoides pteronyssinus. The improvement of symptom scores before and after SIT was statistical significant with P<0. 05. Although total effective rate reached 100% , AR group was superior than AR+BA group in term of the efficacy comparison, and P<0. 05 indicated the statistical significance. The serum sIgE, IL-4 values of three groups were brought into comparison, and P<0. 05 indicated the statistical significance of the difference.
CONCLUSION
The SIT on the AR, AR+BA is a safe and effective treatment, but different disease responds diversely. The long-term treatment course is recommended.

2.Therapy experience in NHL patients with diabetics

Objective Discuss therapy problems in NHL patients with diabetics. Methods 40 NHL patients with diabetics are treated with chemotherapy containing prednisone. Compare the blood sugar before using prednisone with the one after that, regulate medicines of reducing glucose and at the same time observe the syndromes. Results Limosis blood sugar of patients just in hospital is (6.1?1.3) mmol/L, and after chemotherapy it becomes (6.5?1.2) mmol/L. There is no significant difference between them. Conclusions On the basis of controlling blood sugar effectively, patients with NHL combining diabetics can use prednisone as normal.
3.THE ANTI-HYPERGLYCEMIC ACTIVITY OF POLYSACCHARIDES FROM TREMELLA AURANTIALBA MYCELIUM

Objective: To study the hypoglycemic activity of polysaccharides from Tremella aurantialba mycelium (TMP). Methods: The anti-hyperglycemic effect of TMP was investigated in normal mice and diabetic rats induced by alloxan in comparison with phenformin and acarbose. Results: TMP not only decreased serum glucose level in normal mice, but also in diabetic rats. After i.g.TMP to hyperglycemic rats for 1w, the glucose and TG in serum were decreased obviously , but serum TC did not change remarkably. After i.g. 100 mg/(kg ?d) TMP to hyperglycemic rats for 23 d, liver glycogen and BLA did not change significantly, but serum glucose was reduced obviously compared with the control . Conclusion: TMP has obvious hypoglycemic activity and can reduce hyperlipemic complications.
4.Effects of tanshinone-Ⅱa sodium injection post-conditioning combined with controlled low central venous pressure on the hepatic ischemia-reperfusion injury

Objective To investigate the protective effects of tanshinone-IIA sodium injection post-conditioning combined with controlled low central venous pressure on hepatic ischemia-reperfusion injury during liver resection.Methods Eighty patients scheduled for liver resection, 46 males and 34 females, aged 30-65 years, BMI 20-26 kg/m2, ASA physical status Ⅰ or Ⅱ, were randomly divided into four groups: tanshinone-IIA sodium post-conditioning (group D), tanshinone-IIAsodium post-conditioning combined with controlled low central venous pressure (CVP 1-5 cm H2O) group (group DL), controlled low central venous pressure (CVP 1-5 cm H2O) group (group L) and control group (group C) that took the static-compound anesthesia and maintained CVP 6-12 cm H2O, 20 cases in each group.The venous blood samples were drawn from internal carotid vein at different time point: pre-occlusion ten minutes (T0), post-occlusion 2 h (T1), 6 h (T2), 12 h (T3), 24 h after operation (T4), and then detected the levels of NF-κB, intercellular cell adhesion molecule-1 (ICAM-1), ALT and AST.The MAP was detected, HR and CVP were recorded.Results Compared with group C and group D, CVP were significantly lower at T0and T1in group L and group DL (P<0.01).Compared with T0, levels of NF-κB, ICAM-1, ALT and AST in four group at T1-T4were significantly increased (P<0.01).Compared with group C, levels of NF-κB, ICAM-1, ALT and AST in group DL, group L and group D at T1-T4 were significantly decreased (P<0.05).Compared with group DL, levels of NF-κB, ICAM-1, ALT and AST in group D and group L at T1-T4 were significantly increased (P<0.01).Conclusion Tanshinone-IIA sodium injection post-conditioning, combined with controlled low central venous pressure in patients with partial hepatectomy, can reduce the degree of ischemia-reperfusion injury.
5.THE ANTIHYPERGLYCEMIC EFFECT OF FLAVONOIDS FROM HIPPOPHAE RHAMNOIDES L.ON DIABETIC RATS INDUCED BY STREPTOZOTOCIN

Objective: To study the antihyperglycemic effect of flavonoids from seed (FSH) and fruit(FFH) of Hippophae rhamnoides L.on diabetic rats induced by streptozotocin (STZ). Methods: The STZ-induced diabetic rats were randomly divided into four groups and were given intragastrically (ig) water, metformin, FSH and FFH respectively once a day. After four weeks, the levels of serum glucose, fructosamine, lipid, protein, GSH and lactic acid (LD) were assayed. Results: FSH could reduce the levels of serum glucose, fructosamine and triglyceride significantly, increase the contents of serum total protein and albumin obviously, enhance the ability of antioxidation in STZ-induced diabetic rats. FSH also could decrease food and water intake of diabetic rats evidently. But FSH had little effect on LD, LDH, glycogen and body weight in diabetic rats. FFH was not as good as FSH. Conclusion: FSH is antihyperglycemic , and can improve the metabolic derangements of STZ- induced diabetic rats.

Objective To investigate the effects and mechanism of pre-treatment with an Nrf2 inducer dh404 on renal ischemia reperfusion injury in rats.Methods Thirty SD rats were divided into three groups using completely randomized digital table,dh404 was dissolved in sesame oil and was given orally 1.5 mg/kg the night before procedures and 5 hours before procedures.Rats in group Sham received no treatment of ischemic reperfusion.In group IR and group dh404,the renal ischemia reper-fusion (IR)model was established,24 hours after IR,the levels of serum creatininc (Cr)and urea ni-trogen (BUN),the activities of superoxide dismutase (SOD)and the content of malonaldehyde (MDA)in serum were measured,and hematoxylin eosin(HE)staining observe the changes in renal structure,the levels of γ-glutamate-cysteine ligase catalytic (GCLC)and modifier (GCLM)subunit, the expression of NF-κB,COX-2 and eNOS were measured.Results Compared with group Sham,the values of Cr,BUN in group IR and group dh404 were significantly higher (P <0.05).Compared to the group IR,the group dh404 Cr,BUN values significantly decreased after reperfusion for 24 h(P <0.05 ).Compared to group Sham,group IR SOD activity decreased,while the value of MDA increased(P <0.05 ).Compared to group IR,group dh404 had much higher SOD activity,while the value of MDA significantly decreased.Observed with optical microscopy,compared to group Sham, the renal tubular injury of group IR was obvious.Compared to group IR,group dh404 significantly reduced tubular injury.Compared to group IR,the levels of GCLC and modifier GCLM subunit were higher,while there were no significant differences of levels among NF-κB,COX-2 and eNOS. Conclusion Pre-treatment with an Nrf2 inducer dh404 can protect the kidney from IRI through possi-bly reducing IRI kidney oxidative stress.
8.Qualitative characteristics of contrast-enhanced ultrasound and VueBox quantitative perfusion analysis of the molecular classification of breast cancer

Objective:To investigate the value of qualitative characteristics of contrast-enhanced ultrasound (CEUS) and VueBox quantitative parameters in the evaluation of pathological molecular typing of breast cancer.Methods:A retrospective analysis was performed on 133 patients with pathologically confirmed breast cancer who underwent CEUS examination in the People′s Hospital of Guangxi Zhuang Autonomous Region from January 2020 to July 2022. The patients were divided into Luminal A type, Luminal B type, human epidermal growth factor receptor-2(HER-2) type and triple negative type according to the results of immunohistochemistry. The differences of qualitative characteristics and quantitative parameters of CEUS in different molecular subtypes of breast cancer were analyzed. The ROC curve was used to evaluate the diagnostic efficacy of CEUS in the differentiation of molecular subtypes of breast cancer.Results:There were significant differences in enhancement intensity, post-enhancement boundary, filling defect and peripheral radial convergence among different molecular subtypes of breast cancer(all P<0.05), while there was no significant difference in enhancement uniformity ( P>0.05). Peak enhancement (PE), wash-in and wash-out areas under the curve (WiWoAUC), and wash-in ratio (WiR) of HER-2 type and triple-negative type breast cancer were higher than Luminal A type and Luminal B type (all P<0.05). ROC curve analysis showed that PE and radial convergence had reasonable diagnostic efficiency in Luminal A type, and the areas under the curve were 0.849 and 0.780, sensitivity was 0.711 and 0.889, specificity was 0.909 and 0.671, accuracy was 0.842 and 0.744, respectively. The areas under the curve of PE in diagnosing Luminal B type was 0.852, the sensitivity, specificity, and accuracy were 0.825, 0.763 and 0.782, respectively. The area under the curve of WiWoAUC and filling defect in diagnosing HER-2 type were 0.912 and 0.898, the sensitivity was 0.903 and 0.903, the specificity was 0.853 and 0.892, and the accuracy was 0.865 and 0.895, respectively. The area under the curve of clear boundary after enhancement in diagnosing triple negative type was 0.919, and the sensitivity, specificity and accuracy were 0.941, 0.897 and 0.902, respectively. Conclusions:There are differences in the qualitative characteristics and quantitative parameters of contrast-enhanced ultrasound in different molecular types of breast cancer. CEUS is suggested as a noninvasive modality for preoperative prediction of molecular subtypes of breast cancer.

Objective:
To investigate the incidence and prognosis of tricuspid regurgitation after transcatheter closure of perimembranous ventricular septal defects in pediatric patients as well as the risk factors of regurgitation occurrence or aggravation.
Methods:
Clinical data of 1 108 patients who underwent percutaneous closure in Shanghai Children′s Medical Center, School of Medicine, Shanghai Jiaotong University from January 2011 to January 2017 was analyzed retrospectively, and the prognosis and risk factors of postoperative tricuspid regurgitation and aggravation were also analyzed.
Results:
Mild tricuspid regurgitation occurred in 24 cases after surgery with the incidence of 2.1%.Among 5 cases with mild or above preoperative tricuspid regurgitation, the regurgitation was alleviated after surgery in 4 cases.No severe tricuspid regurgitation requiring surgical intervention occurred in any patient during follow-up.Univariate analysis showed that intervention time (
10.Phillygenin Mitigates LPS/ATP-induced L02 Cell Inflammation by Regulating P2X7R/NF-κB/NLRP3 Inflammasome Signaling Pathway

ObjectiveTo explore the effects of phillygenin (PHI) on the inflammation in L02 cells induced by lipopolysaccharide (LPS) and adenosine triphosphate (ATP) and the expression of purinergic 2X7 receptor (P2X7R), NOD-like receptor family pyrin domain containing 3 (NLRP3), and nuclear factor kappa B (NF-κB) expression. MethodIn this study, the inflammation model was induced in L02 cells by 100 μg·L-1 LPS treatment for 24 h and 5 mmoL·L-1 ATP treatment for 5 h. The cells in the PHI groups were cultured with PHI (100, 50, 25 mg·L-1) for 6 h in the LPS treatment period, followed by LPS treatment for another 18 h. After ATP treatment for 5 h, the mRNA and protein expression of interleukin-1β (IL-1β), interleukin-18 (IL-18), P2X7R, NLRP3, Caspase-1 precursor (pro-Caspase-1), cleaved Caspase-1, NF-κB, and NF-κB inhibitor protein α (IκBα) in L02 cells was detected by real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) and Western blot, respectively. Molecular docking was used to predict whether P2X7R could bind to PHI, and DCFH-DA was employed to detect the accumulation of reactive oxygen species (ROS) in cells. P2X7R was silenced by small interfering ribonucleic acid (siRNA), and then the mRNA expression of IL-1β, IL-18, P2X7R, NLRP3, Caspase-1, NF-κB, and IκBα was detected by Real-time PCR. ResultReal-time PCR and Western blot showed that compared with the normal group, the model group showed increased expression of IL-1β and IL-18 (P<0.05), and compared with the model group, the PHI groups showed down-regulated IL-1β, IL-18 mRNA and protein expression (P<0.05). Molecular docking suggested a good binding effect of PHI to P2X7R. Real-time PCR and Western blot analysis showed that the expression of P2X7R in the model group was significantly up-regulated compared with that in the normal group (P<0.05), and compared with the model group, the PHI groups showed down-regulated mRNA and protein expression of P2X7R (P<0.05). DCFH-DA results showed that compared with the normal group, the model group showed increased content of ROS (P<0.05), and compared with the model group, the PHI groups decreased the accumulation of ROS (P<0.05). As demonstrated by Real-time PCR and Western blot, compared with the normal group, the model group showed increased expression of NLRP3 inflammasome and NF-κB (P<0.05), and compared with the model group, the PHI groups significantly decreased the mRNA and protein expression of NLRP3 and cleaved Caspase-1, and up-regulated the mRNA and protein expression of NF-κB and IκBα (P<0.05). Real-time PCR analysis showed that compared with the results in the model group, after silencing P2X7R by siRNA, the mRNA expression of IL-1β, IL-18, P2X7R, NLRP3, Caspase-1, NF-κB, and IκBα was decreased (P<0.05). PHI exerted the same effect, and the mRNA expression was further reduced after the combination of them. ConclusionPHI is presumed to suppress the expression of the NLRP3/NF-κB signaling pathway by down-regulating upstream P2X7R to alleviate the LPS/ATP-induced inflammation in L02 cells, suggesting that P2X7R may be the target of PHI against inflammation.
