1.A retrospective study of coblation-assisted treatment in adult with obstructive sleep apnea-hypopnea syndrome.

OBJECTIVE:
To investigate the therapeutic effects of 894 coblation-assisted treatment in adult with obstructive sleep apnea-hypopnea syndrome.
METHOD:
A review of 894 coblation-assisted treatment in adult with obstructive sleep apnea-hypopnea syndrome was presented with respect to the sleep monitoring results, SF-36 health questionnaire survey, therapeutic effects and complications.
RESULT:
After operation for 6 months, the patients' sleep Monitoring results were improved remarkably (P<0. 01) and their symptoms of snore or choke got improved. SF-36 health questionnaire survey showed that social function, energy and mental health dimension scores were significantly higher than the preoperative (P<0. 05). There were 18 patients suffered Secondarily bleeding within 2 weeks and 23 patients recrudesced within 1 year.
CONCLUSION
Radio frequency coblation is an applicable method of therapy for patients with obstructive sleep apnea-hypopnea. The treatments differ with the obstructive location and character of upper airway.

2.The prevalance and risk factors of post-stroke depression after the first ever stroke: an 12-month follow-up study

Objective To investigate the prevalance and risk factors of post-stroke depression(PSD)in patients with the first ever stroke during first year after stroke.Methods A total of 60 eligible stroke patients were followed up for 12 months after the first ever stroke and Hamilton Rating Scale for Depression,Activity of Daily Living Scale,Neurological Functional Deficit Scores,Social Support Scale and Eysenck Personality Questionnaire(EPQ)were used to investigate the incidence and risk factors of PSD in 2 weeks,3,4 and 12 months,respectively.Results The prevalence of PSD after the first ever streke was 18.18%(12/66),29.23%(19/65),38.09%(24/63),31.67%(19/60)in 2 weeks,3,4 and 12 months after the attack of stroke,respectively.Logistic regression analysis indicated that the major risk factors of PSD were female and introvert(b=-1.55,P=0.001;b=1.59,P=0.047)in 2 weeks,poor social support,low grade of neurological functional deficit score and female(b=-0.19,P=0.005;b=0.26,P=0.01;b=2.84,P=0.03)in 3 months.poor social support and low grade of neurological functional deficit score in 6 months and 12 months after stroke re-spectively(6 months:b=-0.24,P<0.001;b=0.35,P=0.004.12 months:b=-0.17,P=0.001;b=0.33,P=0.002).Conclusions The prevalence of PSD is considerable and the risk factors of PSD are different in different stage.
3.Clinical analysis of lavage treatment for inflammatory breast diseases guided by color Doppler sonography

Objective To study clinical effects of lavage treatment for inflammatory breast diseases guided by color Doppler sonography.Methods The lesions of 63 patients with inflammatory breast diseases (30 of breast abscess,20 of plasma cell mastitis,13 of breast chronic sinus)were positioned by color Doppler sonography.Surgical debridement and lavage catheter treatment were performed under general anesthesia.Results 54 patients were healed by first intention,and they were satisfied with their breast shapes.4cases of mammary abscess,3cases of plasma cell mastitis and 2 cases of breast chronic sinus had relapse within half a month.Conclusions Color Doppler sonography guided lavage treatment of inflammatory breast diseases is simple with significant healing effect,fast recovery speed,less recurrence and high cure rate.The operation had little pain to patients and small hidden postoperative scar.The breast shapes are nice-looking.
4.Pilomatricoma behind and below the ear: one case report.

A 6 years old boy came to our department with a complaint of a growing goitre behind and below the right ear that started 4 years ago. Clinical features: there was a 2.0 cm x 1.5 cm nontender goitre behind and below the right ear, protruding from the skin with clear boundary. The temperature and color of the skin covered the goitre were normal. B-mode ultrasound features: there was a mass under the skin with the size of 2.0 cm x 0.8 cm showing strong echo. The borders of the mass were unclear, and no signal of blood flow was detected. Pathology diagnosis: pilomatricoma.

5.Analysis of Irrational Prescriptions and Pharmacists’Intervention in the Pharmacy Intravenous Admixture Service of Our Hospital

OBJECTIVE:To provide some references for rational application of intravenous infusion drugs to ensure the safety of drug use. METHODS:Statistical analysis was made to irrational prescriptions intercepted by the pharmacy intravenous admixture service of our hospital from 2009 to 2013. RESULTS:There were 5,802 groups of irrational prescriptions from 2009 to 2013,accounting for 0.12% of total prescriptions. For the irrationality,the top three reasons were improper solvent dosage, drug overdose and improper solvent selection in order,and the involved types of drugs were mainly antimicrobial agents,anti-neoplastic drugs,common drugs and parenteral nutrition drugs (arranged from more to less). Under the pharmacist’s interven-tion,the percentage of irrational prescriptions of intravenous drugs reduced from 0.169 4% in 2009 to 0.082 8% in 2013. The decrease in the irrational prescriptions involving with improper administration was the most significant,followed by those related to drug overdose,frequency,prescribing errors errors improper solvent dosage,incompatibility and improper solvent. CONCLU-SIONS:Pharmacists’intervention can effectively reduce the irrational prescriptions and significantly promote rational use of in-travenous drugs.
6.The relationship between shear wave velocity and the ratio of typeⅠ/typeⅢ collagen fiber in the carotid plaques

Objective To assess the correlation between the shear wave velocity(SWV) and the ratio of type Ⅰ /type Ⅲ collagen fibers in plaques using virtual touch tissue imaging quantification(VTIQ).Methods Fifty four patients (54 plaques) who scheduled to carotid endarterectomy(CEA) were enrolled in this study.Their carotid plaques were preoperatively examined using VTIQ.The value of SWE of each plaque was obtained.The SWV of the plaque was evaluated by VTIQ.The endarterectomy specimens were transferred to the pathological department immediately after surgery to evaluate the plaque types and the distribution of collagen fibers in plaques.According to HE staining,the 54 plaques were divided into stable and unstable group.Results Among 54 plaques,14 were stable plaque and 40 were unstable plaques.The thickness and stenosis rate had no significant difference between stable group and unstable group (both P > 0.05).The mean value of SWV and the ratio of type Ⅰ/typeⅢ collagen fiber in stable plaques were higher than those in the unstable plaques (both P <0.05).Based on ROC analysis,a cut-off value of SWE of 3.725 m/s (AUC=0.936) showed a sensitivity of 92.9%,a specificity of 80.0% and an accuracy of 83.3% for predicting stable plaques (95% CI 0.818-0.982,P <0.05).The value of SWVs was closely correlated to the ratio of type Ⅰ/typeⅢ collagen fiber in plaques(r =0.882,P <0.05).Conclusions SWV of the carotid atherosclerotic plaque can be used to reflect the ratio of the type Ⅰ/typeⅢ collagen fiber in the plaques,and to evaluate the stability of plaque.
7.Changes of B lymphocyte subsets in peripheral blood of patients with rheumatoid arthritis and their ;significance

Objective To investigate the changes of B lymphocyte subsets ( naive B cells, memory B cells and plasmablasts) in peripheral blood of patients with rheumatoid arthritis ( RA) and their correla-tions with the clinical manifestation and laboratory indexes.Methods Sixty-six patients with RA were di-vided into two groups including the group with active RA and the group with inactive RA according to the dis-ease activity score in 28 Joints (DAS28).A control group with healthy subjects was set up accordingly.The distributions of B lymphocyte subsets in peripheral blood samples were detected with flow cytometry analysis and their correlations with clinical manifestations and laboratory indicators were analyzed.Results ( 1 ) Compared with healthy subjevts, the mean fluorescence intensities ( MFIs) of CD19 and the percentages of memory B cells in peripheral blood of the patients with RA were significantly decreased, while the percenta-ges of naive B cells were increased (P<0.05).The percentages of plasmablasts in the patients with active RA were significantly increased as compared with those of healthy subjects and the patients with inactive RA (P<0.05).(2) The percentages of plasmablasts in peripheral blood of the patients with RA were positively correlated with the joint tenderness count, joint swelling count and joint swelling index (P<0.05).(3) A positive correlation was found between the MFIs of CD19 and the erythrocyte sedimentation rates ( ESRs ) among the patients with RA.The percentages of plasmablasts were positively correlated with C reaction pro-tein (CRP) and anti-cyclic citrullinated peptide (anti-CCP) antibody (P<0.05).(4) The percentages of plasmablasts were also positively correlated with the DAS28 among the patients with RA ( R2=0.343, P<0.01).Conclusion The distributions of B lymphocyte subsets varied among the patients in different stages of RA, which were related to the patient′s clinical symptoms and laboratory indexes.The study suggested that different subsets of the B lymphocytes might play an important role in the pathological process of RA.
8.Detection and phylogenetic tree analysis of human parainfluenza virus type 3 in children with acute respiratory tract infection from a hospital in Shanghai

Objective To understand the epidemiologic feature of human parainfluenza virus type 3 (HPIV-3) in Shanghai,and to provide scientific evidence for formulating prevention and control measures in the future.Methods A total of 164 nasopharyngeal aspirates samples taken from children with acute respiratory infection (ARI) were collected from Xinhua Hospital Affiliated to Shanghai Jiaotong University and sent to Shanghai Public Health Clinical Center from June 2009 to June 2010.Samples were detected for HPIV-3 by reversed transcription-polymerase chain reaction (RT-PCR).Full-length hemagglutininneuraminidase (HN) gene (1 719 bp) of five positive samples were sequenced for phylogenetic analysis.Comparison between two groups was evaluated by the precise chi-square test (two sided).Results Of 164 samples,70 samples were infected with parainfluenza virus,and HPIV-3 was detected positive in 23 samples with the positive rate of 32.86%.HPIV-3 infections were most common in spring and summer,and most of infections were mainly found in 13-36 month-old infants.Five Shanghai isolates and 36 reference sequences from different countries and areas were divided by HN gene-based phylogenetic tree into three clusters (A,B and C).Five Shanghai isolates and five Beijing isolates belonged to C3a group.The homologies of nucleotide and amino acid sequences between five Shanghai isolates and five Beijing isolates were 99.0%-99.5% and 99.7%-100.0%,respectively.Conclusions HPIV-3 accounts for a high proportion in children with ARI in Shanghai.C3a group may be the main lineage of HPIV-3,which suggests that HPIV-3 may be of regionally correlation.
9.Clinical relevance of serum hepatitis C virus core protein level and antiviral therapy response

Objective To evaluate the relationship between serum level of hepatitis C virus (HCV) core protein and antiviral therapy response in patients with hepatitis C. MethodsOne hundred and sixty-two consecutive patients with chronic hepatitis C (CHC) were retrospectively analyzed. Serum level of HCV core protein and HCV RNA were measured. Among them, 35 CHC patients treated with pegylated interferon (PEG-IFN)+ ribavirin (RBV) were followed up before and at week 4, 24, 48 of treatment. The correlations between serum HCV core protein level, HCV RNA and antiviral therapy were evaluated. Comparison between groups was done by t test and comparison of paired data among groups was done by analysis of variance. Results Total 162 patients were divided into six groups according to the HCV virus load: 56 with HCV virus load≤1×103 IU/mL and HCV core protein absorbance (A)=0. 100±0. 029; 9 with 1×103 IU/mL< HCV virus load≤1 × 104 IU/mLand A=0. 246±0. 213; 11 with 1 × 104 IU/mL< HCV virus load≤1 × 105 IU/mL and A=0. 235±0.179; 28 with 1×105 IU/mL< HCVvirusload≤1×106 IU/mL and A=0. 422±0. 319; 51 with 1 × 106 IU/mL< HCV virus load≤ 1 × 107 IU/mL and A = 0. 603 ± 0. 330 ; 7 with 1 × 107 IU/mL<HCV virus load≤ 1 × 108 IU/mL and A = 0. 900± 0. 379. The HCV core protein absorbance was positively correlated with HCV RNA level (r= 0.36, P<0.05). The HCV core protein absorbance values of 35 CHC patients before therapy, at week 4, 24, and 48 of therapy were 0. 564 ±0. 296,0. 144±0. 062, 0. 091 ±0. 035 and 0. 112±0. 103, respectively. The HCV core protein absorbance values at week 4, 24, 48 all decreased significantly compared with that before therapy (t= 8. 563,9. 195, 9. 250; all P<0.05), and there was significant difference between those at week 4 and week 24 (t=4. 301, P<0.05). Conclusion Serum HCV core protein level is closely correlated with HCV viral load during HCV infection process and antiviral treatment.
10.Clinical significance of CK19 and hMAM mRNA detection in peripheral blood of breast cancer patients by real-time fluorescent quantitative RT-PCR

Objective To detect the expression of Cytokeratin 19 (CK19) and human mammaglobin (hMAM) mRNA in the peripheral blood of breast cancer patients dynamically and to evaluate its clinical significance.Methods The expression of CK19 and hMAM mRNA was evaluated by real-time fluorescent quantitative reverse transcriptase pelymerase chain reaction in the peripheral blood of 40 breast cancer patients and 10 healthy volunteers.Results The expression of CK19 and hMAM in the peripheral blood of 40 breast cancer patients were higher than healthy donors.The positive expression rate of CK19 and hMAM was descending tendency during treatment.The different staging expression rate had no differences between before treatment and during treatment,but after treatment the expression rate had remarkably differences (CK19:P=0.044;hMAM:P=0.005).If the expression of CK19 and hMAM was continual positive,the rate of recurrence and metastasis would be higher (P<0.001).Coneluslons Real-time fluorescent quantitative RT-PCR may detect the expression of CK19 and hMAM in the peripheral blood of breast cancer patients,and it is a sensitive and specific technique.The resuits suggest a possible use of this approach for evaluating prognosis,monitoring recurrence.
