1.Laparoscopic Common Bile Duct Exploration and Primary Suture in 669 Cases

Objective To report our clinical experience on laparoscopic common bile duct exploration and primary suture.Methods From March 1992 to December 2006,669 cases of laparoscopic common bile duct exploration and primary suture were carried out in our hospital.Cholangioscopic lithotomy,electrohydraulic lithotripsy,biliary dilation,bile duct drainage,or biliary stenting were performed on the patients.Results The surgeries were completed successfully in 643 of the 669 cases(96.1%).Biliary leakage was observed in 17(2.5%) cases after the operations,and was cured by nasobiliary and intra-abdominal drainage.2(0.3%) patients developed mild stenosis of the bile duct without showing any symptoms.4(0.5%) had residual stones and were cured by a second laparoscopic surgery.The fine duct fell off in 2 cases(0.3%),and was re-fixed by a re-operation.581 of the cases achieved a 6- to 48-month follow-up(mean,25.6 months),during the period,no patient developed biliary leak,and stenosis or hemorrhage of the common bile duct.Conclusion Laparoscopic common bile duct exploration and primary suture is safe and effective.
2.Effects of Leukocyte in the Injured Areas on Brain Edema After Brain Contusion in Rats

Objective To explore the effects of leukocyte in the injured foci on brain edema after brain contusion,in order to provide theoretical basis of clincial treatment of traumatic brain injury.Methods Traumatic brain injured animal model was established with injecton of blood components,forty five rats were randomly divided into three groups i.e,traumatic brain injure (TBI) group,whole blood(WB)group and white blood cell(WBC) group.Each group was further divided into three subgroups.All rats were killed at 24, 72 or 120 hours after injury,the water content, sodium, potassium and calcium contents were measured in brain tissue of areas injured.Results There was a peak of brain water and sodium contents in TBI, WB and WBC groups at 3 days after injury;Brain water content of TBI group was significantly lower than both WB group and WBC group (P
3.Screening and identification of anomously expressed proteins in colon mucosa in diarrhea-predominant irritable bowel syndrome

Objective To screen and identify the anomously expressed proteins at protein level in colon mucosa of diarrhea-predominant irritable bowel syndrome(D-IBS)patients and healthy individuals.Methods Four D-IBS patients and four healthy individuals were included in the study.Biopsies of mucosa at the ileocecal junction and sigmoid colon were performed via colonoscopy.The specimens were cleaned with ice cold saline which contained 0.1% PMSF,and were snap frozen in liquid nitrogen.The two-dimensional gel electrophoresis(2-DE)was used to screen the anomously expressed proteins,and the two proteins which presented the greatest changes in each group were identified by mass spectrometry method.Results Proteomics maps of human colon mucosa were obtained successfully with 2-DE.Average protein spots were 336 in normal control group,and the matching rate among them was 92%,while in D-IBS group,the average protein spots were 426,and the matching rate was 95%.The average matching rate between the test group and the control group was 74%.It was found that the volume value of 24 proteins spots changed more than two folds.The protein expression was found to decrease in 3 spots and increase in 21 spots.Four proteins were chosen for analysis with mass spectrometry analysis.The two up-regulated proteins were identified as Immunoglobulin J chain and heat shock protein 27,while the other two down-regulated proteins were identified as hemoglobin beta subunit and fructose-bisphosphate aldolase A.Conclusion With 2-DE,proteomics maps of human colon mucosa have been obtained successfully.The proteome expressions of colon mucosa in D-IBS patients are remarkably different from that of the healthy individuals.The four identified proteins may play a certain role in the pathogenesis of D-IBS.
4.Level of lipid peroxides and cuprozinc superoxide dismutase in gastric mucosa and serum of gastrectomized patients

The level of lipid peroxides (LPO)and copper-zinc containing superoxide dismutase(SOD-I) in the gastric mucosa and serum was determined in 141 samples from 25 patients after conventional subtotal gastrectomy (11 cases of Billroth I and 14 cases of Billroth Ⅱ and 11 patients after pylorus and antroseromuscular flap preserving gastrectomy (PAFPG).Those of 11 normal subjects were examined likewise to serve as control.It was found that;1.The average LPO level was much higher and the average SOD-I level much lower in the gastric mucosa after conventional subtotal gastrectomy especially the Billroth type Ⅱ than in those after PAFPG.2.In 36 specimens of stump mucosa,the average LPO level was significantly higher in the tissue around the anastomotic ring than in that of the body of the stump;no marked difference of SOD-I level between the 2 was revealed.3.The LPO value in the stump mucosa was positively correlated to and the SOD-I value negatively correlated to the pH value of gastric juice.Thess findings suggest that the reaction of oxygen free radicals in the stump-mucosa may be influenced by the intragastric pH or by the type of digestive continuity reconstruction,and that the reaction of free radicals specially lipidperoxidation may play a role in the pathogenesis of the lesions in the anastomotic stoma.
5.A clinical study of the influence of three types of subtotal gastrectomy on intragastric bacterial growth

0.05).Large amounts of E.coli and intestinal aerobics such as Veillonella spp.,Bacteriodes fragilis,and Clostridium spp.were detected in patients with B-Ⅰ and B-Ⅱ especially the latter but not found in those with PAFPG.These findings suggest that intragastric bacterial overgrowth,can exist in the hypoacidic stumps in the first one to three years afte conventional gastrectomy especially after B-Ⅱ,no such occurrence is found in those after PAFPG since it only moderately reduces the intragastric acidity.

7.Study on the ability of New Rural Cooperative Medical Scheme to reduce health risk factors in Anhui province

Objective: To describe the degree of poverty caused by diseases among farmers who have joined New Rural Cooperative Medical Scheme( NRCMS) in Anhui province, and analyze the ability of NRCMS to reduce health risk factors in Anhui province, in order to provide suggestions for improving the NRCMS strategy. Methods: This paper randomly selects three counties to conduct comparative analysis on the NRCMS strategic changes from the aspects of registration rates, funding levels and compensation ratios between 2013 and 2014, in order to analyze the ability of NRCMS to reduce health risk factors from the aspects of the rate of poverty caused by diseases, the resolving degree of poverty caused by diseases, the alleviation degree of poverty caused by diseases, etc. Results: The ability of NRCMS to reduce health risk factors in Anhui province has improved between 2013 and 2014 , but the improve-ment is limited. Conclusions:In order to reduce the incidence of poverty caused by diseases among the farmers, gov-ernment should reform the payment methods, control the outflow of patients and improve the multi-level security sys-tem, in order to gradually reduce the poverty caused by diseases among farmers.
8.Tiaohe Ganpi Hexin Decoction in treatment of irritable bowel syndrome with diarrhea: a randomized controlled trial.

Background: The quality of life has been greatly influenced and the cost of medical expenses is very high in patients with irritable bowel syndrome (IBS). The etiology and pathogenesis of IBS are still unclear, and the prevention and treatment of this disease still lack of effective methods. Objective: To explore and analyze the effects of Tiaohe Ganpi Hexin Decoction (TGHD), a compound traditional Chinese herbal medicine for regulating the liver and spleen, on IBS patients with diarrhea. Design, setting, participants and interventions: All 40 IBS patients came from the First Hospital of Jinan University, Guangzhou Red Cross Hospital, and the First Affiliated Hospital of Guangzhou University of Chinese Medicine, and were randomly divided into two groups. Patients in the treatment group (n=20) were given TGHD, while those in the control group (n=20) were prescribed oral pinaverium with a four-week treatment period. Main outcome measures: Traditional Chinese medicine (TCM) syndrome score, total obviously effective rate, disappearance rate of symptoms, and clinical symptom score in the two groups were evaluated before and after four-week treatment. Results: After the treatment, TCM syndrome scores in both groups were decreased (P<0.01), and the TCM syndrome score in the treatment group was significantly lower than that in the control group (P<0.01). There was a significant difference in the total obviously effective rate between the two groups (P<0.01), and the total obviously effective rates in the treatment and control groups were 85%(17/20) and 45%(9/20) respectively. The disappearance rates of abdominal pain, abdominal distention, poor stool output, stool frequency, stool character and mucous stool in the treatment group were higher than those in the control group (P<0.05). The symptom scores of abdominal pain, abdominal distention, poor stool output, stool frequency, stool character and mucous stool in the treatment group were lower than those in the control group (P<0.05). Conclusion: TGHD can significantly improve the clinical symptoms in IBS patients with diarrhea.

10.Effect of Weide'an Tablet on Gastric Function and the Expressions of EGF and EGFR in Gastric Tissue of Model Rats with Stress-induced Gastric Injury

OBJECTIVE:To study the effects Weide'an tablet on gastric function and the expressions of epidermal growth fac-tor(EGF)and its receptor(EGFR)in gastric tissue of model rats with stress-induced gastric injury,and investigate its mechanism in the treatment of gastric ulcer. METHODS:Rats were randomly divided into normal group (normal saline),model group (nor-mal saline),positive group (ranitidine hydrochloride,0.015 mg/kg) and Weide'an tablet high-dose,medium-dose,low-dose groups (1.7,0.87,0.43 g/kg),10 in each group. Except for normal group,rats in other groups were given fasting swimming in cold water to induce gastric ulcer model. After modeling,all rats were intragastrically administrated once a day,for 2 weeks. The body mass of rats in 0,7,14 d of administration was recorded. After 12 h of last administration,gastric juice secretion,gastric juice pH,gastric ulcer area and gastric mucosal damage index of rats were detected,and the expressions of EGF and EGFR in gas-tric tissue were detected. RESULTS:Compared with normal group,body mass and gastric juice pH in 7,14 d in model group were declined;gastric juice secretion and gastric ulcer area were increased,gastric mucosal damage index was increased;and the expressions of EGF and EGFR in gastric tissue were enhanced,with statistical significances(P<0.05 or P<0.01). Compared with model group,except that the body mass,gastric juice secretion,gastric juice pH and gastric ulcer area in Weide'an medium-dose group in 7 d,and body mass,gastric juice secretion,gastric juice pH,gastric ulcer area and the expression of EGF in gastric tis-sue in Weide'an low-dose group in 7,14 d were not significantly improved,above indexes were significantly improved in other ad-ministration groups(P<0.05 or P<0.01);and the effect of Weide'an tablet showed a certain dose-effect relationship. CONCLU-SIONS:Weide'an tablet can significantly improve the gastric function of model rats with stress-induced gastric injury;and the mechanism may be related with enhancing the expressions of EGF and EGFR and promoting ulcer healing.
