ObjectiveToanalyzethepathogensofsuppurativeendophthalmitis,theirdistributionanddrug susceptibility.Provide valuable information for the clinical options in effective use of antimicrobial agents.Methods Collected 259 cases of endophthalmitis from the patients with aqueous humor and vitreous fluid for pathogens and drug sensitivitytestinordertocarryontheanalysisoftheresults.Results 113casesofbacterialculturepositivein259 cases of specimens,with a detection rate of 43.63%.6 cases of multiple bacteria infection,and 2 case of fungal infec-tion.The total separation of pathogens was 120 strains.89 strains were gram positive coccus,accounting for 74.17%, 11 were gram positive bacillus strains, accounting for 9.17%, 13 strains of gram-negative bacilli, accounted for 10.83%,gram-negative in 5strains(4.17%),fungus in 2strains(1.67%).Conclusion Main pathogenic bacteria suppurative endophthalmitis is gram-positive, the pathogen species distribution and antimicrobial susceptibility of change should be timely monitored and the rational using of antibiotics can reduce the generation of drug-resistant strains.

BACKGROUND: Lumbar spinal nerve root compression causes the damage of neural function and abnormal neural action of lumbar spinal nerve root, which is related not only to mechanical injury of the compressed but also to inflammatory reaction.OBJECTIVE: To establish the animal model of lumbar nerve root compression and investigate the effect of acupuncture on the ultramicrostructure of the injured lumbar nerve and inflammatory medium of rats.DESIGN: A randomized controlled trial.SETTING: Department of Acupuncture & Moxibustion, Shanghai Sixth People's Hospital Affiliated to Shanghai Jiaotong University.MATERIALS: The study was conducted in the Experimental Animal Center of Shanghai Sixth People's Hospital Affiliated to Shanghai Jiaotong University from January 2001 to January 2004. Totally 90 healthy male Wistar rats were randomly divided into model group and model + electroacupuncture group with 45 rats in each.METHODS: Clominorex was injected intraperitoneally for anesthesia.With interspace of spinous process of L5-6 as the center, the special silica gel slice was placed where the right side of L5 nerve root ran out of the dura mater sac, stabilized, sutured layer by layer, and covered with the aseptic gauze. Modeling group: No special disposal was made and the rats were fed regularly after modeling. Modeling + electro-acupuncture group:One day after modeling, a filiform needle of 28 gauge and 3.3 cm (1 inch)was used to puncture Huantiao and Yanglingquan points of the right hindlimb and Jiaji point of the right side of the oppressed nerve root. The oppressed injured lumbar nerve tissue was disposed after 15 days of modeling and 30 daysof modeling + electro-acupuncture, and observed with the optic microscope and electron microscope. The content of inflammatory medium such as prostaglandin E2 and5-hydroxy tryptamine was measured.MAIN OUTCOME MEASURES: [1] Results of damaged lumbar nerve root tissues under the electron microscope and light microscope; [2] content of inflammatory medium such as prostaglandin E2 and 5-hydroxy tryptamine.RESULTS: All the 90 rats entered the final analysis. [1] Hyperemia around the nervous tissues, edema and nerve root demyelination of electroacupuncture 15 days (model + electro-acupuncture group) were relieved.The structure of Schwann cells was normal. Massive inflammatory cells gathered around the nervous tissues in 30-day model group (model group).There was severe demyelination of nerve root tissues, and the gap between the nervous tissues widened and red blood cells leaked out. Demyelination of electro-acupuncture 30-day group (model + electro-acupuncture group)became mild and the edema in nerves was alleviated. [2] The content of prostaglandin E2 in the locally oppressed rat lumbar nerve tissues of 15-day and 30-day model + electro-acupuncture groups was obviously lower than that in 15-day and 30-day model groups [(61.17±9.77), (76.65±7.39) ng/L;(56.05±7.98), (71.04±6.99) ng/L, P ＜ 0.01], while the content of 5-hydroxy tryptamine in the locally oppressed rat lumbar nerve tissues of 15-day and 30-day model + electro-acupuncture groups was obviously lower than that in 15-day and 30-day model groups [(1 413.4±194.5), (2 587.7±214.7) ng/L;(780.4±133.0), (1 355.0±147.7) ng/L, P ＜ 0.01].CONCLUSION: Electro-acupuncture can alleviate inflammatory reaction around the injured lumbar nerves and improve the ultramicrostructure of lumbar nerve root. It has the effect of decreasing the content of prostaglandin E2 and 5-hydroxy tryptamine in oppressed lumbar nerve root tissues and partially inhibiting inflammatory reaction.

Dyslipoproteinemia is correlated with the occurrence and development of atherosclerosis and cardiocerebral vascular diseases. The metabolism of serum lipoprotein, however, is a complicated and ordered metabolic process. On one hand, every organ, tissue and even cell of the body participates in and affects the state of serum lipoprotein dynamic balance (SLDB) in different degrees. On the other hand, SLDB is a direct factor for maintaining the normality of whole vascular system, especially the vascular endothelium system. The serum lipoprotein dynamic imbalance, as an inducement or even etiological factor, can bring about various clinical diseases. Therefore, integrated analysis is needed to appropriately assess the state of SLDB. It is suggested that serum lipoprotein dynamic imbalance syndrome should be the key risk factor for those related diseases. Serum lipoprotein electrophoretogram can ideally express the state of SLDB and be taken as a new index for cognizing the serum lipid metabolism and the function of speen-qi to distribute Jing (Essonse) in the integrated traditional Chinese and western medicine. Practice has showed that concrete integration and bilateral development should be one of the main tasks in current researches of integrated traditional Chinese and western medicine.

Objective To observe the effects of propofol or thiopental on the changes of blood brain barrier (BBB) permeability in focal cerebral ischemia Methods Focal cerebral ischemia model was induced with occluding the middle cerebral artery (MCA) Sixty adult male SD rats were divided randomly into 5 groups: control group(group MC): without any anesthetics; propofol pretreated group (group MP1) or thiopental pretreated group (group MT1): intravenous bolus of propofol 20mg/kg or thiopental 40mg/kg, followed by intravenous infusion of propofol or thiopental at rate of 50 60 mg?kg 1 ?h 1 or 100 120 mg?kg 1 ?h 1 ,respectively ,to achieve and maintain the EEG burst suppression 30 min before MCA occlusion until the beginning of reperfusion; propofol therapy group (group MP2) or thiopental therapy group (group MT2): from the beginning to the end of the reperfusion , propofol or thiopental was administered at the same dose as group MP1 or group MT1; Blood pressure, P ET CO 2, ECG and EEG were monitored throughout the experiment Rectal temperature was maintained between 37 5℃ and 38 5℃ At the end of the experiment, after intravenous bolus of 2% evans blue dye(EB) 3ml/kg, the brain was taken out to check out the extends and areas of EB staining on the surface and standardized serial coronal sections of the brain ,under the optical microscope and electromicroscope Results 1 Both propofol groups did not alleviate BBB permeability changes and cerebral damage 2 Cerebral injury was most slightly in group MT1 3 BBB permeability and brain injury in group MT2 were slighter than those in group MC and MP1, MP2 Conclusions Thiopental at the dose maintaining EEG burst suppressioncan can reduce the brain injury induced with ischemia and reperfusion, with the better protective effects following the prophylactic administration Propofol at that dose can not improve the cerebral ischemic reperfusion injury

To observe the influence of different concentrations of sevoflurane on blood brain barrier permeability in rats, thirty adult male SD rats were randomly divided into three groups: control group (without general anesthetics), 1 MAC sevoflurane and 1.5 MAC sevoflurane groups (maintaining end expiratory concentration to 2.2% and 3.3% respectively for 1 hour). Before the end of the experiment, 2% Evans′ blue dye (EB, 3ml/kg) was intravenously injected. The range and areas of EB staining on the surface and standardized serial coronal section of the brain were assessed. The brain was also observed under the light microscope and electron microscope. The results indicated that sevoflurane at both concentrations did not increase the permeability of BBB to EB and lanthanum, and there were no significant changes in morphological features of central nervous system after inhalation of sevoflurane.

Six years' English teaching in department of obstetrics and gynecology in Wenzhou Medical College were reviewed. Language and cultural differences are the main reasons hindering teaching quality. Rational use of a variety of teachers,preparing for lessons adequately,adoption of English image data,supplying and revising English teaching materials,using network auxiliary teaching and forming ef-fective education mode are conducive to improving English teaching quality in department of obstetrics and gynecology for foreign students.

Objective The aim of this study was to investigate the clinical value of human serum Kallikrein 6 for the diagnosis and monitor of pithelial ovarian cancer. Methods Serum levels of KLK6 were analyzed with ELISA in 30 cases of epithelial ovarian carcinoma, 20 cases of benign ovarian tumor and 30 cases of healthy women. In the meantime, serum CAi25 was determined with chemiluminescence. Furthermore, serum levels of KLK6 and CA125 were also detected in 12 case of epithelial ovarian carcinoma with the same methods one week and the 3rd month postoperation of follow-up. Results Serum levels of KLK6 in epithelial ovarian carcinoma was higher than that in benign ovarian tumor and healthy women (P < 0.05). KLK6 also showed positive correlation with clinical stage, cytological grade, pelvic lymph node metastasis, recurrent or dead disease (P < 0. 05). On the contrary, KLK6 showed no significant correlation with pathological types (P >0. 05). After surgery of follow-up, KLK6 and CA125 were significantly decreased in 12 case of epithelial ovarian carcinoma (P < 0. 05). Furthermore, the total sensitivity and specificity of KLK6 in the diagnosis of epithelial ovarian carcinoma was 73.3% and 85.0% respectively, followed by the sensitivity to be 50. 0% and 88. 9% for the diagnosis of stage Ⅰ-Ⅱand Ⅲ-Ⅳ disease. Conclusion Our resuits showed KLK6 may be one of the reliable indexes for the diagnosis and monitor of ovarian cancer.

Objective To study the role of total flavonoids of hippophae rhamnoides in improving contactile function of stretched cardiac myocyte.Method Flavonoids were given to stretched myocytes which were proved their contractile function decline and then myocyte contractile mechanics characteristics and calcium transfer were measured.Result Flavonoids increased myocyte contractility,as indicated by myocyte shortening,velocity of shortening,peak+dL/dt and peak-dL/dt during shortening,in a concentration-dependent manner (r>0.9,P<0.001),and with no relation to the intracellular calcium transfer in the myocytes.Conclusion Flavonoids of the traditional Chinese drug hippophae rhamnoids is effective in improving the contractile function of stretched cardiac myocyte in low dosage.

Objective To explore the clinical effect of the radial artery perforator flap on repairing soft tissue defect after palm postoperative scar contracture. Methods Eighteen patients with palm scar contracture were selected. The palm soft tissue defect was repaired by radial artery perforator flap after the operation of scar removal and soft tissue release. The areas of soft tissue ranged from 5 cm × 3 cm to 8 cm × 5 cm. The area of donor flap exceeds 20%of the wounds area, and wounds were repaired by free skin grafting. Six months after operation, the hand function was compared with that before operation. Results All the patients were followed up for 6-15 months (mean 10.8 months), all flaps survived with good shape, and flap donor site wounds were healed by skin grafting. The patients were evaluated 6 months after operation according to the Chinese Medical Hand Surgery Society of upper part of functional assessment criteria: excellent in 11 cases, good in 5 cases and general in 2 cases, but preoperative functional evaluation was excellent 0 case, good in 3 cases, general in 7 cases and poor in 8 cases. Compared with that before operation, the postoperative function was significantly improved. Conclusions It is a commendable approach of repairing soft tissue defect after postoperative palm scar contracture by radial artery perforator flap, because it can provide reliable blood supply, and significantly improve hand function with exactly clinical effect.

Objective:To investigate the role of epidermal growth factor receptor (EGFR)in the regulation of B[a]P-induced silent information regulator 1 (SIRT1 ) activation in the human bronchial epithelial cells (BEAS-2B),and to clarify the relationship between EGFR/SIRT1 signal transduction pathway and the occurrence and development lung cancer.Methods:The prediction analysis of transcriptional factor binding sites of SIRT1 was performed by MOTIF SearchTM software.The primary cultivated BEAS-2B cells were divided into control group (without B[a]P exposure)and B[a]P groups (the cells were exposed to B[a]P for 6,12,24,48 h).RT-PCR and Western blotting method were carried out to detect the EGFR mRNA and protein expression levels.The BEAS-2B cells transfected with SIRT1 promotor luciferase reporter gene plasmid were treated with human epidermal growth factor (hEGF)and Genistein (tyrosine protein kinase inhibitor)for 24 h,the morphology of BEAS-2B cells was observed by inverted microscope, and luciferase reporter assay was used to test the SIRT1 transcriptional activity.The human lung tissue biopsies were acquired and the immunohistochemical analysis was used to determine the EGFR protein expression level.Results:The transcriptional factor binding sites of SIRT1 contained EGF, 2Fe-2S and vWF.Compared with control group,the expression levels of EGFR mRNA in B [a]P groups were increased in a time-dependent manner,and reached the peak at 12 h (P < 0.05);the EGFR protein expression levels were also increased (P <0.05).The SIRT1 luciferase activity in hEGF group was increased compared with control group (P <0.05);when hEGF and B[a]P worked together,the SIRT1 luciferase activity was increased even further (P <0.001). The cells showed arrangement and morphologic changes gradually when the B[a]P concentration was above 30 μmol · L-1. Genistein (30 μmol · L-1 )inhibited the increase of SIRT1 luciferase activity induced by B [a]P ,and there was significant difference compared with control group (P <0.05).The immunohistochemistry results showed that EGFR expression level in lung cancer tissue was higher than that in normal lung tissue (P < 0.001).Conclusion:EGFR can regulate the B [a]P-induced SIRT1 expression in BEAS-2B cells,and to cause lung chronic inflammation;EGFR/SIRT1 signal transduction pathway may play a role in the occurrence and development of lung cancer.
