BACKGROUND:Chitosan (CS) holding a good biocompatibility, biodegradability, antibacterial property and hemostasis effect cannot only promote wound healing and tissue repair, but also exhibit sustained-control release when carrying berberine hydrochloride (Bh). Therefore it functions as an excellent antibacterial and hemostatic material. OBJECTIVE:To prepare the porous CS/Bh composite xerogel and observe its antibacterial and hemostatic properties. METHODS:The porous CS/Bh composite xerogel was prepared using freeze-drying method, and its microstructure was characterized by scanning electron microscope and Fourier transform infrared spectrometer. The biocompatibility was detected through MTT assay and the in vitro antibacterial and hemostatic properties were observed through antibacterial test and blood clotting index. RESULTS AND COCNLUSION:The porosity of the CS/Bh composite xerogel was 83.1%-90.5%and almost degraded after 72 hours of immersion in PBS. The in vitro antibacterial and hemostatic properties of the composite xerogel were significantly better than those of CS, which were enhanced with the Bh content increasing. Additionally, the CS/Bh (10%) composite xerogel showed the best hemostatic and antibacterial abilities in vitro. To conclude, the CS/Bh composite xerogel possesses a good biocompatibility and strong hemostatic and antibacterial abilities, which is a promising postoperative antibacterial and hemostatic material.

Objective To explore the nutrition condition improvement function of Jianpihuazhuoshengxue mixture to the long-term maintenance hemodialysis patient. Methods Before the treatment, to determine the nutrition condition of 43 cases dialysis patient that are divided to treatment group and control group. The two groups are all dialyze, the treatment group are added of Jianpihuazhuoshengxue mixture. To measure nutrition condition of the two groups again after one treatment course. Results and Conclusion Jianpihuazhuoshengxue mixture can remarkably raise the dialysis patient's nutritional level, especially the plasma albumin level. Possibly the mixture improvement the nutrition condition of dialysis patient mainly through raising the plasma albumin level, thus improve quality of life of the dialysis patient.

BACKGROUND: Human leukocyte antigen-E (HLA-E) is one of non-classical HLA class I genes. Up to now, the polymorphism analysis is mainly aimed at the variation in exon 3 of HLA-E, which determines HLA-E*01:01 or HLA-E*01:03. However, the identification of the full-length HLA-E and its novel alleles is rare reported.OBJECTIVE: To establish the method of identification of HLA-E genomic full-length sequence, and to identify its novel alleles in healthy blood donors in Shenzhen, China.METHODS: Peripheral blood DNA samples were extracted from the subjects, and the amplified primers and sequencing primers in conserved regions were designed according to the sequences of HLA-E published in the IMGT/HLA database.A high-fidelity reaction system was used to amplify the genomic full-length of HLA-E, followed by sequencing,assembling, confirming and typing.RESULTS AND CONCLUSION: Herein, we successfully established the method for amplifying genomic full-length sequence and sequence-based typing. Two novel HLA-E alleles were nominated by WHO HLA Nomenclature committee as HLA-E*01:01:01:06 and HLA-E*01:01:01:07. Compared with the most related allele HLA-E*01:01:01:01,HLA-E*01:01:01:06 had one nucleotide change at nt-26(G->T) in 5'-promoter, and HLA-E*01:01:01:07 had one nucleotide change at nt3345(T->C) in 3'-UTR. The polymorphism data of genomic full-length HLA-E in Chinese individuals need to be filled, and the method we developed here supplies the key technique for the further studies.

Objective To explore the effect of continuous nursing based on WeChat public platform on the patients with high risk of pressure ulcers. Methods Selecting 165 pairs of high-risk patients and their caregivers from the emergency department of pressure ulcers in the General Hospital of Chinese People′s Armed Police Forces, and they were divided into control group and experimental group by paired random method. There were 83 pairs in control group, and 82 pairs in experimental group. The control group gave oral education according to the traditional model, and adopted the way of telephone follow-up for continuous nursing. While the experimental group let patients and their family members follow the Wechat public account that specially developed on Wechat platform and get continuous nursing in new models to come true networking health education and communicate with patients and their family members by multi-dimensional nurse. After 3 months intervening, compared patients′quality of life, the care behavior of caregivers and acquisition of knowledge of pressure scores, the incidence of pressure ulcers, degree of satisfaction of high-risk patients and caregivers between the two groups. Results The psychological function score, social function score and quality of life score was respectively ( 67.61 ± 1.67), (58.60 ± 1.52), (50.41 ± 2.70) points in experimental group, and (63.21 ± 1.30), (53.23 ± 1.92), (45.83 ± 2.59) points in control group, the comparative difference of the two groups was statistically significant( t=-4.638,-4.930,-2.749, P<0.05). There was no significant difference in the material life and body function score between two groups(P>0.05). The care behavior of caregivers score and pressure sores knowledge score was respectively (50.84 ± 1.92), (27.83 ± 1.48) points in experimental group, and (46.43 ± 2.40), (19.28 ± 1.31) points in control group, the comparative difference of the two groups was statistically significant (t=-3.192,-9.738, P<0.05 or 0.01). The incidence of pressure ulcers was 15.9%(13/82) in experimental group, significantly lower than that in control group (50.6%, 42/83), two groups was statistically significant (χ2=22.413, P<0.01). The satisfying degree and scores of patients or caregivers for follow-up form, nursing service attitude, follow-up content and the total score was respectively (24.83 ± 3.11), (24.10 ± 2.55), (25.03 ± 2.12), (76.23 ± 2.17) points in experimental group, and (18.34 ± 1.41), (17.41 ± 1.34), (19.85 ± 1.92), (63.25 ± 3.03) points in control group, the comparative difference between two groups had statistical significance (t=-7.797--4.061, all P<0.01). Conclusions Utilizing Wechat public platform continuity of care for patients at high risk of pressure ulcers can improve the patients quality of life, reduce the incidence of pressure ulcers, improve the efficient communication of nurses and patients, and enhance the degree of satisfaction of the patients. This method is convenient, shortcut, economic, practical and worthy for clinical use and promotion .

Objective To investigate the measurement standards and clinical application of observing anatomical morphologies and contractile function of puborectalis muscle on pelvic floor parasagittal plane by endoluminal two-dimensional ultrasonography.Methods The thickness and thickening rate of puborectalis muscle of 78 young nulliparous women were measured on pelvic floor parasagittal plane at three levelsurethral level (anterior),vaginal level (central) and rectal level (posterior),both at rest and during contraction.And the thickening rates of these parts during contraction were compared with each other.Interclass correlation coefficients were calculated to evaluate the consistency of the datas.Results At rest,thickness of anterior part of the left was (9.23 ± 0.20)mm,and that of the right was (8.99 ± 0.20)mm.During contraction,thickness of anterior parts of bilateral puborectalis muscle were significantly thicker than that of central or posterior parts (P ＜0.05).The interclass coefficients were more than 0.93 and 0.83.Conclusions The endoluminal two-dimensional ultrasonography can be used to observe morphologies and contractile function dynamically of puborectalis on pelvic floor parasagittal plane.It is simple,reproducible and worthy of clinical promotion.

With the increasing of doctor-patient conflicts,the communication between them gradually becomes a critical element in medical service activities.How to improve the doctor-patient communication is an important content in resident communication skill training.Oncology is a developing discipline with fast development and high risk and residents in oncology department need more communications with patients in the era which individualized treatment is emphasized.Systematization and institutionalization of the training system of doctor-patient communication is beneficial to popularizing doctor-patient communication experiences,protecting the rights and interests of them and ensuring the smooth process of medical treatment.

Objective To evaluate the value of fetal dual Doppler ultrasonography in the diagnosis of fetal arrhythmias.Methods 77 53 1 fetuses were examined by echocardiography.If fetal arrhythmias were found,information of pulsed wave Doppler signals in hepatic vein(HV) and descending aorta(DAo) using dual Doppler were collected simultaneously.Results On dual Doppler examination,13 1 of the fetuses showed signals of cardiac dysrhythmias,including 60 fetuses with irregular fetal heart rhythm but having normal heart rate,45 with tachyarrhythmias and 26 with bradyarrhythmias.Sixteen and 2 with tachy- or bradyarrhythmias fetuses complicated fetal malformation or severely maternal systemic diseases.Eighteen and 3 with tachy- or bradyarrhythmias fetuses revealed intrauterine death or intermediate pregnancy induction.The three main kinds of fetal arrhythmias had statistical differences in complicating with fetal malformation,maternal diseases,intrauterine fetal demise or intermediate pregnancy induction(all P <0.05).There was a significant difference in the accuracy of diagnosing rhythm abnormalities in the fetus between conventional ultrasonography and dual Doppler ultrasonography(k = 0.562,P < 0.05).Sixty cases of irregular fetal heart rhythm,42 cases of fetal tachyarrhythmias,and 8 cases of fetal bradyarrhythmias were changed to normal rhythm at the third-pregnancy or prenatal period through appropriate treatment.All surviving fetuses were examined by echocardiography within 3 days,without any combination of congenital heart disease,and 1 months after the delivery,the ECG were shown as sinus rhythm.Conclusions Dual Doppler ultrasonography is a precise and rapid method of assessing various types of fetal arrhythmias.Different types of fetal arrhythmias have different fetal prognosis.

OBJECTIVE:To prepare ofloxacin hollow suppository(OHS) and to observe its in vitro dissolubility.METHODS:OHS was prepared with PEG 6000,PEG 400 and Carbopol-940 as wase material and wax as retardant.The content of ofloxacin in OHS was determined with the first order derivative UV-spectrophotometry.RESULTS:The wax,as a retardant,could retard the release of drug.The in vitro dissolution of OHS revealed the first order dissolution pattern:K0～1=27.81/h,indicating a speedy effect,K2～6=5.94/h,indicating sustained-release effect.CONCLUSION:This preparation is feasible in technology and controllabe in quality.The preparation of OHS extends the kinds of dose-form of ofloxacin

OBJECTIVE:To investigate the preparation technic of pantoprazole sodium freeze-dried injection.METHO_ DS:The kinds of auxiliary were screened based on the color and luster,external appearance,pH value,clarity and the compatible stability of pantoprazole sodium lyphilization injection with other infusion solutions;the content and other associated material of freeze-dried injection were determined by HPLC method.RESULTS:With Mannitol being freeze-dried powder supporting agent and disodium edetate being metal ion chelation agent and pH value at9.5～11.5adjusted by NaOH,the calibration curves of the prepared freeze-dried injection was linear within12.0～60.0?g/ml in concentration(r=0.9998),the average recovery rate of which at100.26%,RSD=1.14%(n=9),the labeled amount at98.6%.CONCLUSION:The preparation technics is appropriate and the quality control method is simple and feasible.And the prepared freeze-dried injection is able to meet both the pharmaceutical and clinical requirements.
