1.THE TREATMENT OF TRIGEMINAL NEURALGIA BY MICROSURGICAL NEUROVA- SCULAR DECOMPRESSION
Medical Journal of Chinese People's Liberation Army 1982;0(03):-
Since April, 1986, 26 cases of trigeminal neuralgia have been treated by microsurgical neurovascu-lar decompression, and the result was excellent in all. The microscopic findings during the operation suggested that the compression on the trigeminal nerve by neighboring vessels was the anatomic cause of trigeminal neuralgia. As the nerve and vessels were fixed by the thickened arachnoid adhesions, thus promoting the compressing effect, inflammation of the arachnoid membrane seemed to be the pathological foundation of trigeminal neuralgia.

Objective To study the microanatomy of the petrosal vein(PV) and discuss the surgical management of the vessel during microvascular decompression. Methods A total of 15 cadaveric(30 PVs) heads were obtained from dead adults. The position, morphology, branches, and variation of the PV,and anatomical relationship between the trigeminal, facial and acoustic nerves and the vessel were observed.Sixty patients with trigeminal neuralgia or hemifacial spasm were also enrolled in this study.Microvascular decompression was performed on the cases, and the PV and its branches were observed during the operation. Results The PV is formed by the union of 2 to 3 branches. It passes through the the subarachnoid space and terminates in the inner two third of the superior petrosal sinus.Classfied by number of the unilateral PVs,3 types of the vessel has been observed:single-stem(9 sides,30.0%),double-stem(17 sides,56.7%) and triple-stem types(4 sides,13.3%).Moreover,according to the anatomical relationship between the end of the PV and auditory meatus, the vein has 3 subgroups:interior-side(17/55,30.9%),intermediate(24/55,43.6%),and exterior-side(14/55,25.5%) subgroups. In our specimens,8 PVs(the main stem or a branch) touched the trigeminal nerve, and 19 PVs were less than 1 mm away from the nerve.None of the PVs connected to the faical and acoustic nerves. In the 60 patients who recieved microvascular decompression,107 PV branches(19 single-stem veins,35 double-stem vessels, and 6 triple-stem veins) were found during the operation, among which 33 belonged to interior-side subgroup,46 were classifed as intermediate type, and 28 were exterior-side veins. Conclusions Management of the PV is the key step for cerebellopontine angle surgery. The vein or its branches can be cut off when they compress the surrounding tissues or obstruct the surgical field.

Chinese Journal of Orthopaedic Trauma 2022;24(2):168-172
Objective:To investigate the effects of types of ulnar styloid process fracture on the treatment of distal radius fracture. Methods:The 80 patients were analyzed retrospectively who had been treated at The First Department of Hand Surgery, Honghui Hospital from January 2019 to January 2020 for fracture of distal radius complicated with fracture of ulnar styloid process. They were 25 males and 55 females, aged from 30 to 85 years (average, 58.6 years). According to the types of ulnar styloid process fracture, 40 patients were assigned into a Hauck type Ⅰ group and the other 40 into a Hauck type Ⅱ group. The 2 groups were compared in terms of operation, postoperative complications, hospital stay, bone union, visual analogue scale (VAS) on postoperative 1 to 3 days, and modified Mayo wrist function score, wrist range of motion and quality of life by WHOQOL-BREF at the last follow-up.Results:The 2 groups were comparable because there was no significant difference in age, gender, American Society of Anesthesiologists (ASA) rating, or time from injury to operation between them ( P>0.05). All the patients were followed up for 12 to 24 months (average, 17 months). There was no significant difference between Hauck type Ⅰ group and Hauck type Ⅱ group in operation time, intraoperative blood loss, hospital stay, rate of postoperative complications, fracture union, modified Mayo wrist function score or VAS on postoperative 1 to 3 days ( P>0.05). At the last follow-up, the palm tilt and ulnar inclination angles were 13.8°±1.9° and 21.6°±2.8° in Hauck type Ⅰ group, significantly larger than those in Hauck type Ⅱ group (11.9°±1.6° and 18.8°±2.3°) ( P<0.05). At the last follow-up, Hauck Ⅰ group scored 85.3±6.4,85.6±6.5, 84.7±6.3 and 85.0±6.7 respectively in the domains of physical health, psychology, environment and social relationships, significantly higher than those Hauck type Ⅱ group did (78.5±6.5, 78.9±6.5, 77.8±6.1 and 77.9±6.3) ( P<0.05). Conclusions:In open reduction and internal fixation for distal radius fracture, Hauck Type Ⅰ fracture of ulnar styloid process has no significant effect on the functional recovery of the wrist but Hauck Type Ⅱ fracture of ulnar styloid process may. Therefore, surgical fixation needs to be strengthened if Hauck Type Ⅱ fracture of ulnar styloid process is complicated.

This study briefly introduces the tongue diagnostic equipment of traditional Chinese medicine. It analyzes and discusses the key points of technical evaluation of tongue diagnostic equipment from the aspects of product name, performance parameters, image processing functions, product use methods, clinical evaluation, etc. It analyzes the safety risks and effectiveness indicators of tongue diagnostic equipment, hoping to bring some help to the gradual standardization of tongue diagnostic equipment and the registration of enterprises.
