2.Perpendicular double-locking plating system for the internal fixation of type C distal humerus fractures through two kinds of postcubital approaches

BACKGROUND: Distal humerus fracture is often comminuted, and is difficult to receive reduction and fixation due to its special anatomical structure. Choosing which surgical approach is still a controversy.OBJECTIVE: To compare and analyze the curative efficacy of the perpendicular double-locking plating system for the internal fixation of type C distal humerus fractures through two kinds of postcubital approaches.METHODS: Thirty-two cases of type C distal humerus fractures were enrolled and divided into groups A (osteotomy of olecranon approach) and B (tricep anconeus flap approach). There was no significant difference in the baseline data between two groups. The operation time, intraoperative blood loss, hospital stay, healing time,postoperative follow-up and complications were recorded and analyzed. The patients were followed up at 2 weeks, 1, 3,6, 12, and 18 months. The healing time and complications were recorded according to radiographs and physical examinations. The Mayo Elbow Performance Score was used to determine the elbow function at 1 year postoperatively.RESULTS AND CONCLUSION: (1) The group A had a longer operation time and more intraoperative blood loss than those in the group B (P < 0.05), but showed a better elbow flexion and extension degrees, and higher Mayo Elbow Performance Score at 1 year postoperatively (P < 0.05). (2) There was no significant difference in the healing time between two groups (P > 0.05). (3) These results suggest that the articular surface of the distal humerus fracture is displayed well through two approaches, and the perpendicular double-locking plating system for the internal fixation of type C distal humerus fractures is rational. Noticeably, the tricep anconeus flap approach dose little damage to the muscle, while the osteotomy of olecranon approach obtains better elbow function and less complications.
3.Effects of plateau hypoxia on the cyanide ion metabolism and correlative toxicant mechanism

Objective The aim of present study was to explore the effects of plateau hypoxia on the cyanide ion metabolism and correlative toxicant mechanism.Methods 12 adult rabbits and 72 male SD rats were randomly divided into two groups separately:plain control group with NaCN intoxication and plateau NaCN intoxicant group.2mg/kg NaCN was subcutaneously injected into the back of rabbits,the femoral vein blood was then collected at different designated time points for the measurement of cyanide,hemoglobin and ferrihemoglobin concentrations.Meanwhile 3.6 mg/kg NaCN was subcutaneously injected into the back of rats,the cardiac blood and the hepatic tissue were then collected at the different designated time points for the determination of cyanide ion,cytochrome oxidase activities and for the detection of pathologic changes of hepatic tissue.Results Under the condition of plain and plateau environments,the pharmacokinetics of rabbits induced by NaCN injection was characterized by one-compartment model.The blood cyanide concentration of rats in both plain and plateau groups reached peak value at 30min,and the activity of cytochrome oxidase decreased.Furthermore,the pathologic diagnosis of rats hepatic tissue suggested that liver injury induced by NaCN intoxication at high altitude was more serious than in plain intoxicant.Conclusion Hypoxia could markedly disturb the metabolic process of NaCN in vivo,aggravate the inhibition of cytochrome oxidase activity and lead to serious pathologic injury.
4.The perioperative observation under endoscopic sinus surgery with asthma.

OBJECTIVE:
Aimed to discuss the perioperative treatment of the patient with asthma under endoscopic sinus surgery, to reduce the complications, adverse events and improve cure rate.
METHOD:
One hundred and fifty-two patients with asthma under nasal endoscopic surgery were analyzed retrospectively.
RESULT:
After specification of perioperative treatment, all patients with good control of asthma were in good condition before surgery. One hundred and sixteen patients were safely backed to ward after surgery. There were 13 cases had asthma attack of different degrees. The other 35 patients were send to ICU for observation of 24 to 48 h. One case had an acute attack after returned from ICU. One patient had instantly attacks of asthma after extubation. All patients improved after treatment,and recovered well after 1 year's follow-up.
CONCLUSION
We should pay more attention to the close relationship of CRS and asthma, standardize the perioperative treatment, master the principles of management of acute attack of asthma. As long as to make sufficient perioperative preparation, the nasal endoscopic surgery with asthma is safe, and it can have good efficacy.

5.Chiropractic and acupuncture therapy in the treatment of spleen deficiency anorexia in children: clinical observation of 50 cases

Objective To explore the Chinese massage therapy for the treatment of infantile anorexia of spleen and stomach type clinical curative effect.Methods 2 to 6 years old anorexia children treated from September 2010 to September 2012 for general treatment and health care in our hospital were randomly recruited into a Qi deficiency of both spleen and stomach group and a Yin deficiency of both spleen and stomach group with 25 cases in each group.Both groups were treated with chiropractic therapy.On this basis,Qi deficiency of both spleen and stomach group was applied point-massage on Weishu,Pishu,and Zusanli; Yin deficiency of both spleen and stomach group was applied point-massage on Weishu,Ganshu,and Shenshu.Results ① The comparison of disease and syndromes:the total effective rate was 92% (23/25)and 88% (22/25) in Qi deficiency of both spleen and stomach group and Yin deficiency of both spleen and stomach group separately,showing no significant difference (x2=0.886,P>0.05); the total symptoms improving rate was 92%(23/25) and 76%(19/25)in Qi deficiency of both spleen and stomach group and Yin deficiency of both spleen and stomach group separately,the difference showing statistically significant (x2=0.625,P<0.05).② Symptom score comparison before and after treatment:The main symptoms score and accompanied symptoms score before and after the treatment were [(18.32±2.42) %,(4.12±0.17) points] and [(16.80±3.77) %,(5.30 ±0.41) points] in the Qi deficiency group and [(18.18±2.10),(4.45±0.30) points] and [(19.66 ± 3.62)%,(6.01 ± 1.02) points] in the Yin deficiency group.Both groups showed obvious improvements in the main and accompanied symptom scores after the treatment (P<0.01),but no significant difference between the two groups (P>0.05).Conclusion The treatment of anorexia due to deficiency of both spleen and stomch with chiropractic and point-massage was effective.

7.Effects of breathing booster training and inhalation of terbutaline and ambroxol aerosol on pulmonary function in postoperative lung cancer patients

Objective To study the effects of.breathing booster training and aerosol inhalation with terbutaline and ambroxol on pulmonary function in postoperative lung cancer patients. Methods A total of 84 patients requiring resection operations for lung cancer were randomly divided into treatment and control groups.In the peri-operative period,breathing booster training and terbutaline and ambroxol aerosol inhalation were given to the treatment group,while only aerosol inhalation was given to the control group.Therapeutic effects were evaluated according to patients pulmonary function and postoperative complications 2 weeks and 1 day before the operation,and again 2 weeks after the operation.Postoperative quality of life (QOL) was evaluated with St.George's respiratory questionnaire (SGRQ) 1 month after the operation. Results There was no statistically significant difference in average pulmonary function between the two groups 2 weeks before the operation.Two weeks after the operation,pulmonary function had decreased in both groups,but it was significantly better in the treatment group than in the control group.The treatment group also had significantly fewer pulmonary complications.The QOL of patients in the treatment group had improved significantly 1 month after the operation. Conclusion Breathing booster training and inhalation of terbutaline and ambroxol aerosol during the peri-operative period can significantly improve pulmonary function,reduce respiratory complications and improve the QOL of patients requiring lung cancer resection operations.This is most important for promoting their early recovery.
8.Effect of isoflurane preconditioning on autophagy during focal cerebral ischemia-reperfusion injury in rats

Objective To evaluate the effect of isoflurane preconditioning on autophagy during focal cerebral ischemia-reperfusion (I/R) injury in rats.Methods Thirty-six healthy male Sprague-Dawley rats,weighing 250-280 g,aged 7-8 weeks,were divided into 3 groups (n =12 each) using a random number table:sham operation group (group S),cerebral I/R group (group I/R) and isoflurane preconditioning group (group IP).Focal cerebral I/R was induced by 2 h middle cerebral artery occlusion followed by 24 reperfusion.Group IP inhaled 1.5% isoflurane for 1 h per day for 5 consecutive days,the other two groups only inhaled 30% oxygen,and focal cerebral I/R was induced at 24 h after the last inhalation.At 24 h of reperfusion,neurologic deficit was assessed and scored,the rats were then sacrificed,and brains were removed for determination of cerebral infarct size (using triphenyl tetrazolium chloride staining) and expression of microtubule-associated protein 1 light chain 3-Ⅱ (LC3-Ⅱ) and Beclin-1 (by Western blot).Results Compared with group S,the neurologic deficit scores and cerebral infarct size were significantly increased,and the expression of hippocampal LC3-Ⅱ and Beclin-1 was significantly up-regulated in I/R and IP groups (P<0.05).Compared with group I/R,the neurologic deficit scores and cerebral infarct size were significantly decreased,and the expression of hippocampal LC3-Ⅱ and Beclin-1 was significantly upregulated in group IP (P < 0.05).Conclusion The mechanism by which isoflurane preconditioning ameliorates focal cerebral I/R injury is related to enhancement of autophagy in the rats.

Objective To observe the changes of the concentration of Ca2+, contents of cAMP, CaM and activity of Ca2+/CaM-PK II in pheochromocytoma PC12 cells after combined soman and hypoxia injury. Methods The changes of [Ca2+], and activity of CaM, cAMP and Ca2+/CaM-PK II in PC12 cells were studied after combined soman and hypoxia injury with radioimmunoassay. Results The changes of [Ca2+], the contents of CaM, cAMP were significantly higher in hypoxic and soman intoxicated group than in soman intoxicated group and control group under hypoxia; but the activity of Ca2+/CaM-PK Ⅱ were significantly decreased. Conclusion [Ca2+], CaM, cAMP and Ca2+/CaM-PK Ⅱ exert important role in the damage of PC12 after combined soman and hypoxia injury.
10.Role of mitochondrial ATP-sensitive potassium channel in mitigation of cerebral ischemia-reperfusion injury by isoflurane preconditioning in rats: the relationship with JNK signaling pathway

Objective To evaluate the role of mitochondrial ATP-sensitive potassium (mitoKATP) channel in mitigation of cerebral ischemia-reperfusion (I/R) injury by isoflurane preconditioning in rats and the relationship with c-Jun N-terminal kinase (JNK) signaling pathway.Methods Thirty-two male Sprague-Dawley rats,weighing 280-320 g,were randomly divided into 4 groups (n =8 each) using a random number table:sham operation group (group S),group I/R,isoflurane preconditioning group (group Ⅰ-pre),and 5-hydroxydecanoate (5-HD,a selective mitoKATP channel antagonist) group.Cerebral I/R was produced by modified 4-vessel technique described by Pulsinelli in anesthetized rats.In group Ⅰ-pre,the rats were exposed to 1.5% isoflurane for 1 h everyday for 5 consecutive days before ischemia.In group 5-HD,5-HD 15 mg/kg was injected intraperitoneally at 30 min before ischemia and the other procedures were similar to those previously described in group Ⅰ-pre.Neurological behavior was evaluated at 24 h of reperfusion.The rats in each group were sacrificed at 72 h of reperfusion,and the brains were removed for determination of neuronal apoptosis (by TUNEL) and expression of caspase-3 and phosphor-JNK (p-JNK) protein (using Western blot) in hippocampal tissues.Apoptotic rate was calculated.Results Compared with group S,the number of grid cross was significantly decreased,hanging time was shortened,apoptotic rate was increased,and caspase-3 expression was up-regulated in I/R,Ⅰ-pre and 5-HD groups,the expression of p-JNK protein was up-regulated in IR and 5-HD groups,and no significant change was found in the expression of p-JNK protein in group Ⅰ-pre.Compare with group I/R,the number of grid cross was significantly increased,hanging time was prolonged,apoptotic rate was decreased,and the expression of caspase-3 and p-JNK protein was downregulated in group Ⅰ-pre,and no significant change was found in the parameters mentioned above in group 5-HD.Compared with group Ⅰ-pre,the number of grid cross was significantly decreased,hanging time was shortened,apoptotic rate was increased,and the expression of caspase-3 and p-JNK protein was up-regulated in group 5-HD.Conclusion The mitoKATP channel is involved in mitigation of cerebral I/R injury by isoflurane preconditioning through blocking the JNK signaling pathway in rats.
