1.Stydy on the changes of intraocular pressure in obstructive sleep apnea syndrome

Objective To compare the changes of intraocular pressure (IOP) in 24 hours between obstructive sleep apnea syndrome(OSAS) and the non-obstructive sleep apnea syndrome(non-OSAS).Methods Sixty patients with OSAS were divided into two groups:OSAS group(n =30) and non-OSAS group (n =30).The following indicators were detected:(1) Awake oxygen saturation ((HSaO2) %),(2) The lowest oxygen saturation (LSaO2) %) ; (3) Mean oxygen saturation ((MSaO2 %)) ; (4) Oxygen desaturation index ((DI4),time/h:number of times that the hourly oxygen desaturation ≥ 4%) ; (5) The percentage of the time that oxygen saturation ≤ 90% accounts the total time ((SIT90)%) and 24-hour IOP.IOP was measured from early morning 5:00 and measured once every four hours.The measurement results were compared between two groups.Results There was no significant difference on age ((62.60 ± 12.44) years old vs (65.20 ± 10.66)years old,t =1.48),Course of disease ((22.40 ± 6.88) month vs (25.49 ± 7.22) month,t =1.97),gender (The ratio of male to female is(20/10)vs (17/13),x2 =0.007) between the OSAS and the non-OSAS(P >0.05).Value of AHI(h-1) ((27.9 ±6.0) vs (2.5 ±1.1),t =8.78),LSaO2 ((74.7 ±11.7)% vs (91.8 ±5.9)%,t=3.44),SIT90((13.2±12.4)% vs(0.2±1.1)%,t=9.92) and ODI4(h-1) ((28.9 ±13.9)vs (6.1 ±4.1),t =8.09) of OSAS was significantly higher than that of non-OSAS(P <0.05 or P <0.01).Value of IOP of 21:00 o'clock((20.61±4.15)mm Hg vs(19.60 ± 4.03)mm Hg,t =2.18),1:00 o'clock((23.12 ±3.11)mm Hg vs (20.60 ± 3.29) mm Hg,t =4.64) and 5:00 o' clock ((22.82 ± 2.99)mm Hg vs (17.21 ±3.55) mm Hg,t =4.23) of OSAS was significantly higher than that of non-OSAS (P < 0.05 or P < 0.01).The wave ((10.40 ± 2.85)mm Hg vs (8.40 ± 2.55) mm Hg,t =4.15) and maximal ((23.60 ± 3.29) mm Hg vs (21.23 ±3.43)mm-Hg,t =2.60) value of IOP of OSAS was significantly higher than that of non-OSAS(P <0.05 or P <0.01).There was no significant difference on minimum of IOP between the OSAS and the non-OSAS ((13.20 ± 4.08)mm Hg vs (12.70 ± 4.22) mm Hg,t =0.54,P > 0.05).Conclusion There are higher wave and maximal value of IOP in the patients of OSAS during the night.It is important to pay attention to IOP in order to protect those patients' visual function.
2.Clinical value of electrocardiogram changes on the diagnosis of hyperkalemia

Objective To research clinical significance of electrocardiogram(ECG) measuring in diagnosis of hyperkalemia. Methods ECG changes and serum potassium concentration from 72 cases have been analyzed and contrasted. Results Forty-one cases with hyperkalemia are detected by ECG changes. Coincidence rate is up to 56.9% with serum potassium test. When the serum potassium concentration exceeds 5.5mmol/L, the accuracy of ECG in diagnosis of hyperkalemia was 22.2%. The main features arc peaked T wave with tabernacle shape ;when the serum potassium concentration exceeds 6.7mmol/L, the accuracy of ECG in diagnosis of hyperkalemia was 74.3%. The main features are ST debased,P wave with low amplitude and time limit increased. Some patients appeared sinus node-ventride conduction. One or two appeared malignant ventricular arrbythmia; when the serum potassium concentration exceeds 10.0mmol/L,the accuracy of ECG was 100% ,most patients appeared malignant ventricular. Conclusion The electrocardiogram measurement has the better consistency with serum potassium test, and abnormal electrocardio-gram will be eccurred,when mixed other electrolyte unbalance. Pseudo-hyperkalemia can be distinguished accurately and conveniently by using this method.
3.The DTI in the evolution of vascular cognitive impairment mechanism based leukoaraiosis

Objective To explore the integrity of white matter changes in patients with vascular mild cognitive impairment (VaMCI)based Leukoaraiosis.Methods The patients which appeared white matter hyperintensities in subcortical were performed psychological testing,then 48 VaMCI patients and 18 without cognitive impairment volunteers(NC)were choosed and performed DTI scanning,to determine the white matter’s values of FA and ADC in the following bilaterally symmetrical regions of interest(ROI):entorhinal cortex lobe,hippocampal vault,anterior of corpus callosum,posterior of corpus callosum,inferior frontal lobe,superior temporal lobe and angular gyrus of occipital lobe. Then the differences of the data of DTI between two groups and the relevance between neuropsychological score and DTI’s data were analysed.Results ADC values were significantly higher of VaMCI group in the left inferior frontal lobe,left anterior of corpus callo-sum,left hippocampus vault and right angular gyrus compared with NC group(P <0.05),and in the left hippocampus ADC values and MoCA scores was negatively correlated(r=-0.34,P =0.02).There was no significant difference of FA values between groups. Conclusion The ADC values in many cognitive related areas was reduced in VaMCI patients.
4.Combination use of TACE, PVE and HIFU for the treatment of portal vein cancerous thrombus:a clinical study

Objective To evaluate the combination use of transcatheter arterial chemoembolization (TACE), portal vein embolization (PVE) and high intensity focused ultrasound (HIFU) in treating portal vein tumor thrombus(PVTT). Methods A total of 85 patients with primary hepatocellular carcinoma complicated by PVTT, who were encountered during the period from Jan. 2011 to Feb. 2012 at authors’ hospital, were enrolled in this study. The patients were divided into the study group (n=47) and the control group (n=38). TACE, PVE and HIFU were performed in the patients of the study group, while only TACE and PVE were carried out in the patients of the control group. The therapeutic process was as follows: PVE was carried out 2 weeks after TACE was performed, and for the patients of the study group additional HIFU was conducted about 10 days after PVE procedure. Results The short-term effective rate in the study group and the control group was 89.4% (42/47) and 39.5% (15/38) respectively, and the difference between the two groups was statistically significant (P< 0.05). The 6-month, one-year, and two-year survival rate in the study group were 87.2%(41/47), 66.0%(31/47) and 27.7%(13/47) respectively; the median survival time was 15.4 months. In the control group, the 6-month, one-year, and two-year survival rate were 55.3% (21/38), 39.5% (15/38) and 10.5%(4/38) respectively;the median survival time was 10.3 months. The differences between the two groups were statistically significant (P< 0.05). Conclusion For the treatment of primary hepatocellular carcinoma associated with portal vein tumor thrombus, transcatheter arterial chemoembolization, portal vein embolization together with high intensity focused ultrasound is an safe and effective therapy as it can significantly improve the therapeutic effect and prolong the survival time.
5.Clinical application of DTI and MRS in the diagnosis of AIDS-related cerebral tuberculosis

Objective To investigate the clinical diagnostic values of quantitative DTI and MRS in AIDS-related cerebral tuberculosis. Methods 17 cases confirmed with AIDS and brain tuberculosis,16 volunteers were recruited to perform routine MRI,DTI and MRS sequences. Morphological characteristics of lesions were observed.ADC,FA,rADC,rFA,NAA/Cho,NAA/Cr and Cho/Cr of the lesions solid areas,edematous areas,normal areas and contralateral corresponding normal areas were measured and their variances in different areas were analyzed. Results Significant differences of the values were observed among the three regions of AIDS-related brain tuberculosis,the results of multiple comparisons between the three areas had statistical significances (P <0.05)excepted NAA/Cr between edematous and contralateral areas.ADC,FA,rADC,rFA,NAA/Cho,NAA/Cr on solid areas were lower than that on edematous areas,Cho/Cr on solid areas was higher than that on other two areas.The diagnostic efficiency of rFA value to distinguish solid and edematous areas was the highest by ROC analysis(P <0.05).The normal areas of the two groups had statistical significances(P <0.05)excepted Cho/Cr.Conclusion DTI is valuable to display the lesions micro-structure changes and MRS can reflect the early pathology metabolites changes of AIDS-related tuberculosis.
6.Development of Traditional Chinese Medicine in Iraq

Iraq is a multi-ethnic, multi-religious federal country located in southwest Asia and northeast of the Arabian Peninsula. Medical insurance is based on the primary health care model, supplemented by private medical care. Traditional medicine in Iraq is dominated by Islamic medicine. With the support of the Chinese government, the first Traditional Chinese Medicine (TCM) center was established in 2000. However, due to safety issues, the management of the center had problems such as insufficient staff, limited medical experience, and non-persistentsupport. At present, TCM has not been included in the Iraqi medical insurance system, which has affected the people’s medical choice and hindered the development of TCM. It is recommended to strengthen the construction of existing TCM centers, improve the accessibility of TCM education, and strengthen the cooperation and exchanges of traditional medicine between the two countries to promote the spread and development of TCM in Iraq.
7.Development of Traditional Chinese Medicine in Serbia

Serbia is a parliamentary republic country in Southeast Europe. It implements a health security system with social insurance as the mainstay and social assistance as a supplement. Ischemic heart disease, stroke and Alzheimer’s disease are the major causes of death, and the mortality rates of cardiomyopathy and chronic kidney disease are increasing. Serbia has relevant legislation on traditional medicine. Acupuncture, as the main form of Traditional Chinese Medicine (TCM), has been introduced to Serbia in an early stage. Acupuncture was recognized as a legal medical method in 2005, and then the corresponding educational institutions of TCM were established soon. However, the limitation of acupuncture treatment application, difficulty in access to Chinese medicine, and undeveloped of TCM education system, all of them restrict the development of TCM in Serbia. It is recommended to improve the development and dissemination of Chinese medicine in Serbia, like to increase the promotion of acupuncture, pay attention to the Chinese medicine for preventive health care, and strengthen TCM education.
8.Application value of dual-source CT dual-energy imaging in diagnosis of metastatic lymph nodes of gastric cancer

Objective To investigate the value of standardized iodine concentration and energy spectrum curve of dual-source CT in diagnosis of metastatic lymph nodes of gastric cancer.Methods The clinical data of 60 patients diagnosed as gastric cancer were collected in our hospital.All patients underwent dual-source CT routine abdominal scanning and dual-phase energy spectrum enhancement.According to the pathologic results,the lymph nodes were divided into metastatic and non-metastatic groups.Then the enhanced degree,dual-phase normalized iodine concentration (NIC)were measured and compared between metastatic lymph nodes and non-metastatic lymph nodes.The slope of the energy spectrum curve of metastatic lymph nodes and non-metastatic lymph nodes were calculated by dual-energy software.Results When the enhanced degree was higher than 80 HU,the sensitivity (82.4%)and specificity (78.6%)were higher. The artery-phase NIC values of metastatic lymph node and non-metastatic lymph node were(0.546±0.086)and(0.144±0.077),respectively. The difference was statistically significant (P<0.05).The venous-phase NIC values of metastatic lymph node and non-metastatic lymph nodes were (0.479±0.013)and (0.307±0.082),respectively.The difference was statistically significant (P<0.05).The dual-phase energy spectrum curves of metastatic and non-metastatic lymph nodes were all descending patterns.The difference was obvious at 40-80 keV and minimal at 80-140 keV.The sensitivity of diagnosing metastatic lymph nodes was 83.5%,and the specificity was 85.6%when the arterial curve slope was 6.87.The sensitivity of diagnosing metastatic lymph nodes was 63.6% and the specificity was 96.7%when the venous curve slope was 6.67.Conclusion The multiple parameters provided by dual-source CT dual-energy imaging are helpful to the qualitative and quantitative diagnosis of metastatic lymph nodes in gastric cancer.
9.Loneliness and associated factors among adolescents with mixed anxiety and depressive disorder

Objective:
To investigate loneliness and associated factors among adolescents with mixed anxiety and depressive disorder(MADD), and to provide reference for drawing up effective and targeted interventions.
Methods:
Using convenience sampling method, 265 adolescents with MADD in Daqing Third Hospital were selected from September 2020 to June 2021. General information questionnaire, Pittsburgh sleep quality index(PSQI), Resilience Scale for Chinese Adolescents(RSCA), Child and Adolescent Peer Relationship Scale and UCLA Loneliness Scale were used to investigate the status and influencing factors of loneliness among adolescents with MADD.
Results:
The average score of UCLA among 265 adolescents with MADD was (56.49±10.83). The results of univariate analysis showed that age, gender, burden of medical expenses, drinking behavior and parents migrant work had statistically significant differences in the UCLA scores. The results of Pearson correlation analysis showed that loneliness in MADD adolescents was negatively correlated with psychological resilience( F/t=57.65, -60.62, 21.92, 8.52, 16.22, P <0.01), and positively correlated with sleep quality and interpersonal distress( F/t=-0.69, 0.76, 0.50, P <0.01). The results of multiple stepwise regression analysis showed that phase of study, sex, burden of medical expenses, interpersonal distress, sleep quality and resilience were the influencing factors of loneliness among adolescents with MADD( B=0.11, 0.13, 0.09, 0.46, 0.10,-0.24, P <0.01).
Conclusion
Adolescents with MADD have moderate loneliness, which needs to be improved. Nursing staff should pay attention to the negative emotional experience such as loneliness among adolescents with MADD, and reduce their loneliness by improving sleep quality and resilience, reducing interpersonal distress.
10.Influencing factors and effectiveness of community follow-up in patients with cardiac implantable electronic device

Objective:To explore the influencing factors and effectiveness of community follow-up in patients with cardiac implantable electronic device (CIED) implantation.Method:A total of 132 patients who received CIED implantation in the Department of Cardiology of Tongren Hospital, Shanghai Jiao Tong University School of Medicine from February 2021 to February 2022 were enrolled in this prospective cohort study. Among them 33 patients were followed up in community health service centers associated with Tongren Hospital (community follow-up group) and 99 matched patients were followed up in the CIED outpatient clinic of the hospital (outpatient follow-up group) with a ratio of 1∶3. The clinical data of the selected patients were collected through a questionnaire survey; the follow-up data were extracted through the CarelinkExpress electronic follow-up platform and the CIED outpatient information system of Tongren Hospital. Adjustment of the treatment protocol or CIED parameters at follow-up, and the referral from the community health service centers were defined as visit with-an-action (VWA). The endpoint of follow-up was the occurrence of major adverse events. The multivariate logistic regression model was used to analyze the factors influencing patient selection for community follow-up.Results:The univariate analysis showed that the frequency of visits to community health service centers and the service contracting rate in community follow-up group were higher than those of outpatient follow-up group ( P<0.05). The multivariate logistic regression analysis showed that the contracted community physician service was an independent influencing factor of patient choosing community follow-up ( OR=2.143, 95% CI: 1.103-4.166, P=0.025). A total of 469 visits of followed up occurred in 132 patients, including 45 community visits and 424 outpatient visits. VWA accounted for 22.2% (10/45) in the community follow-up group, and 17.2% (73/424) in the outpatient follow-up group ( P>0.05). There was no significant difference in the safety and effectiveness indicators (VWA, major adverse events, and unplanned follow-up) between the two groups ( P>0.05). More patients in the community follow-up group walked to the hospital than the outpatient follow-up group ( P<0.05);and the main transportation for the later was by bus or taxi(42(42.4%)or 41(41.4%)). The average waiting time in the community follow-up group was significantly shorter than that in outpatient follow-up group ( P<0.05). The total time required for a single follow-up in the community follow-up group was 50.0 (45.0, 59.5) minutes, which was significantly shorter than that in the routine outpatient follow-up group (107.0 (90.0, 135.0) minutes, P<0.05). Conclusions:The contracting with community physicians is an independent influencing factor for CIED implanted patients to choose community follow-up. The safety and effectiveness of community follow-up are comparable to routine outpatient follow-up, and community follow-up is more convenient.
