1.Clinical outcomes of radical surgery for pancreatic body and tail tumor accompanied with sinistral portal hypertension

Objective To investigate the clinical outcomes of radical surgery for pancreatic body and tail tumors accompanied with sinistral portal hypertension (SPH).Method The clinicopathological data of 35 patients with pancreatic body and tail tumors accompanied with SPH operated from January 2004 to December 2014 were retrospectively analyzed.Results Of 35 patients,22 patients had body and tail pancreatic carcinomas,10 patients had malignant solid pesudopaillary tumors and 1 patients had a neuroendocrine tumor.All these patients developed splenomegaly and varices in the gastric fundus with normal hepatic function.The splenic vein pressure was (27.3 ±3.8)cmH2O (1 cmH2O =0.098 kPa),its average diameter was (1.3 ± 0.3) cm,and the speed of splenic vein blood flow was (8.9 ± 0.8) cm/s.Of the 35 patients with pancreatic body and tail tumors who underwent radical resectional operations,22 patients in addition underwent devascularization.There were 13 of these 22 patients who underwent pericardical devascularization and the remaining 9 underwent total or proximal gastrectomy.The main complications were pancreatic fistula (n =5,14.2%),intra-abdominal infection (n =4,11.5%),delayed gastric emptying (n =1,2.8%),lymphatic fistula (n =1,2.8%) and gastric perforation (n =1,2.8%).The post-complication morbidity rate was 34%.All these patients were followed-up for 6 to 60 months after operations.There was no upper gastrointestinal bleeding which occurred within 6 months of operation.Conclusion Multi-disciplinary treatment,complete preoperative evaluation,correct perioperative and individualized management enhanced efficacy in the surgical treatment of patients with pancreatic body and tail tumors with SPH.
2.Clinical efficacy analysis on radical operation of gastric cancer with liver cirrhosis

Objective To investigate the clinical efficacy of radical operation for gastric cancer with liver cirrhosis.Methods The clinicopathological data of 56 gastric caner patients with liver cirrhosis performed on between Jan 2000 and Dec 2013 were analyzed retrospectively.Results The morbidity rate was 71%.The main complications of the group with liver cirrhosis were postoperative ascites (7 cases),delayed gastric emptying(6 cases),pulmonary infection(8 cases),intra-abdominal infection (5 cases),anastomotic leakage (3 cases).wound infection (2 cases)and,astrointestinal bleeding(1 cases).Postoperative complications of gastric cancer with liver cirrhosis were associated with age (> 60 yrs),Child-pugh grade(B),lymph node dissection(D2).There were no mortality.The 1-,3-and 5-year survival rates of the 56 cases were 73.2%,41% and 26.7%.Conclusion Multi-disciplinary treatment,complete preoperative evaluation,correct perioperative management,individualized principle were the best mode on effective treatment of the patients with liver cirrhosis.

OBJECTIVE:To determine the dissolubility of paracetamol in Ganmaoqing tablets produced by different manufactories and to evaluate the internal qualities METHODS:The content of paracetamol in Ganmaoqing tablets was determined by HPLC with Nova-Pak C18 or Sphereclone ODS(2) column as fixed phase and methanol-water(1∶3) as mobile phase Paracetamol was detected at 249 nm with UV detetor The flow rate was 0 5ml/min Rotating basket method was used in dissolution test RESULTS:The method could determine the content of paracetamol in Ganmaoqing tablets satisfactorily The dissolution rates of eight samples were quite different CONCLUSION:The method is simple,rapid and reliable It is very necessary to establish the dissolubility standard for Ganmaoqing tablets
4.Stability of Ceftazidime and Ormidazole Chloride Sodium Injection with Sequential Intravenous Dripping

Objective: To investigate the compatible stability of ceftazidime and ormidazole chloride sodium injection with sequential intravenous dripping.Methods: The contents and the absorption curves of ceftazidime and ormidazole sodium chloride injection in the mixture were determined by UV.The appearance of the solution was observed and the pH value was determined.Results: With the quality ratio of ceftazidime to ormidazole at 1∶25, 1∶50 and 1∶100, the mixture color changed from light yellow to light pink in 40, 45 and 48 min, respectively, and gradually deepened with the extension of time.With the quality ratio of ormidazole to ceftazidime mixture at 1∶400, 1∶800 and 1∶1 600, the color was stable in 3 h.There were no evident changes in the appearance, pH, content and absorption curves.Conclusion: The solution containing ormidazole and ceftazidime might have changes in color.Clinical pharmacist suggests that ormidazole chloride sodium injection be given intravenously, and then sequentially followed by ceftazidime.
5.ASPS induces G_2/M arrest of H446 cells by activation of ERK signal pathway

Objective To investigate ASPS induced G_2/M arrest in lung cancer cell line H446 and its effect on ERK MAP kinase signal transduction pathways. Methods Cell cycle phases were inspected by flow cytometery (FCM) ; Western blot analysis was used to inspect the proteins of ERK, p-ERK. Results Compared with control group, G_2/M phase cells increased with concentration significantly, G_0/G_1 phase cells were not different, G_2/M phase cells and G_0/G_1 phase cells were not different when pre-incubated with PD98059 prior to exposure to ASPS of different concentrations, protein of p-ERK was significantly increased, expression of ERK was no different. Conclusion ASPS may induce G_2/M arrest of H446 cells possibly by activation ERK MAP kinase pathways.
6.Antifungal Susceptibility of Candida spp.to Lisea Cubeba Oil:a Broth Microdil ution Tests and Electron Microscopy

Objective To determine the susceptibility of yeasts and to explore the antifungal m echanismof Lisea cubeba oil,so as to provide evidence for the development of antifungal medicinal herbs.Methods The minimuminhibitory concentrations(MIC)of Lisea cubeba oil and fluconazole were determined by broth microdilution method according to the National Com mittee on Clinical Laboratory Standards(NCCLS)reference method for broth dilution antifungal susceptibility testing of yeasts.Ultra-structur al changes of C.krusei were observed by electron microscopy before and after Lisea cubeba oil treatment.Results Five previously identified quality control(QC)strains(Candida albicans,Candida tropical is,Candida glabrata,Candida parapsilosis,Candida krusei)were susceptible to Lisea cubeba oil,their MICs were 14.14?3.64?g /mL,23.22?2.85?g /mL,31.24?2.88?g /mL,76.19?4.40?g /mL,28.30?2.54?g /mL,respectively.After treatmen t with Lisea cubeba oil the mor-phology and ultrastructure of Candida krusei showed obvious changes:their cell wall and cytoplasmic membrane ruptured;intracellar components dissolved;organellae swollen and di ssolved.There were no changes observed in the morphology and ultrastructure of C.krusei treated with fluconazole.Conclusions The data indicate that Lisea cubeba oil has antifungal effect not only on C.albican and other medically important Candida spp.,but also on fluconazole resistant isolates such as C.krusei and thus has great clinical importan ce.The antifungal mechanismof Lisea cubeba oil may be that the structure of cell w all and cell membrane of C.krusei being inhibited.
7.Comparison of laparoscopic and open partial nephrectomy for duplication anomalies in children

Objective To compare the outcome of laparoscopic with open partial nephrectomy for duplication anomalies. Methods A total of 64 patients with mean age of 17 months were divided into 2 groups.Open group consisted of 36 patients,and laparoscopic group consisted of 28 patients.In open group,there were 19 boys and 17 girls,15 cases were of ectopic ureter,18 of ureterocele,2 of vesicoureteral reflux (VUR) and 1 of ureteropelvic junction obstruction.Mean age was 18 months.In laparoscopic group,there were 18 boys and 10 girls,9 were of ectopic ureter,16 of ureterocele and 3 of VUR.Mean age was 1 6 months.There were no significant differences in age and diseases.Results There were 2 conversion to open surgery in laparoscopic group and no other intra-operative and post-operative complications in 2 groups.Mean duration of operation was 2.5 h for open group,the difference of pre- and post-operation hemoglobin was 2.64 g,the volume of drainage was 91.4 ml,the time of drainage was 4.2 d and mean hospital stay was 19.3 d.In Iaparoscopic group,the figures were 2.9 h,1.45 g,55.4 ml,3.4 d and 14.3 d,respectively.The differences of the figwres were significant between the 2 groups (P<0.05). Conclusion Laparoscopic partial nephrectomy is effective and safe for the treatment of duplicated kidney with a dysfunctional moiety.
8.Clinical and pathological analysis of aggressive angiomyxoma

Objective To summarize the clinical manifestations,pathological character,diagnosis and treatment of aggressive angiomyxoma (AAM).Methods A computer-based online search of PubMed database and CHKD database was undertaken for literature about AAM published from all the relevant documents with the key words of aggressive angiomyxon.According to the condition 210 articles were analyzed.All the articles were analyzed about natural history,clinical manifestation,diagnosis,pathological character,treatment and prognosis of AAM.Results A total of 282 cases in well-documented articles had been reported,among which 64 were male and 218 were female,with male to female ratio of 1:3.4.The age of the patients from 1 to 83 years(mean 40.38 years).The most common sites were the perineum,genital tract and soft tissue in pelvic cavity in females and the scrotum,spermatic cord and groin in males.None of the cases could be accurately diagnosed as AAM preoperatively.The minimum diameter of the tumors was 1 cm,and the maximum was 60 cm.All the specimens showed typical pathological features of AAM as reported previously.Immunohistochemistry indicated that AAM tended to be strongly positive for vimentin,CD34,Desmin,estrogen receptor,progesterone receptor but mostly negative for S-100 and Ki-67 and Actin.The medical history was from 1 month to 20 years.The recurrence of the postoperative follow-up was 2 months to 20 years.The diagnosis depended on pathological examination.Conclusions AAM is a sort of unusual soft connective tissue tumor.It is a kind of unknown cause,slow progression,locally invasive,easy to recur after tumor resection.Long-term follow-up is quite necessary because of the high rate of local recurrence.
9.Determination of Landiolol Concentration in Human Blood by LC-MS/MS

Objective To establish an LC-MS/MS method for the detection of landiolol concentration in human blood.Methods After pretreatment with neostigmine and a deproteinization procedure, landiolol and the internal standard venlafaxine were eluted isocratically using a mobile phase consisting of acetonitrile and 10 mmoL·L-1 ammonium acetate with 0. 1% formic acid in a ratio of 3664 ( V/V ) . Separation of the respective compounds was achieved on a Waters XTerra? RP18 column (150 mmí4. 6 mm,5 μm). Quantitative analysis of landiolol was conducted by a triple-quadrupole mass spectrometer with positive-electrospray ionization source,monitored under a multiple reaction monitoring ( MRM) mode. The extracted ions monitored following MRM transitions were m/z 510. 5→423. 1 for landiolol and m/z 278. 2→215. 1 for the internal standard venlafaxine. ResultsThe calibration curve of landiolol in human blood showed good linear relationship in the range of 1. 010-2 020 μg·L-1 . The lower limit of quantitation was 1. 010 μg · L-1 . The RSD of within-day and between-day precision was less than 6. 5% and 4. 8%, respectively. The recovery rate was 92. 6%-100. 9%. Conclusion The method is proven to be simple,rapid and reliable,and can be applied to study the pharmacokinetics of landiolol hydrochloride in healthy Chinese volunteers.
10.The effect of reoperation on the prognosis of gallbladder carcinoma patients found by first cholecystectomy

Objective To investigate the effect of the second resection for unsuspected gallbladder carcinoma discovered after cholecystectomy.Methods A retrospective clincopathological analysis was conducted for 45 cases of unsuspected gallbladder carcinoma receiving second operation at our hospital from January 2000 to December 2010.Result Of the 45 cases with unsuspected gallbladder carcinoma (33 pT2,12 pT3 ),40 cases received second radical reeection of the liver bed with lymph dissection; the remaining 5 cases received palliative operation.Amongst 45 patients lymph metastasis (4 pT2,6 pT3 ) was found in 10 cases,liver metastasis ( 2 pT2,1 pT3 ) in 3,parietal seeding in 1 ( pT3 ) and distant metastasis ( pT2 ) in one.The 5 patients receiving palliative operation died in 3 ~ 8 months and 40 patients receiving the radical operation achieved long-term survival ( 40.4 ± 2.7 months) after the operation.The effect of second operation which was done within 4 weeks after the first cholecystectomy was better than that of the operation done beyond 4 weeks ( survival time 37.1 ± 2.2 vs 22.4 ± 5.8months).Conclusions Radical resection for unsuspected gallbladder carcinoma discovered after the initial cholecystectomy helps improve prognosis and prolong patients survival time.
