1.Comparison of laparoscopic and open partial nephrectomy for duplication anomalies in children

Objective To compare the outcome of laparoscopic with open partial nephrectomy for duplication anomalies. Methods A total of 64 patients with mean age of 17 months were divided into 2 groups.Open group consisted of 36 patients,and laparoscopic group consisted of 28 patients.In open group,there were 19 boys and 17 girls,15 cases were of ectopic ureter,18 of ureterocele,2 of vesicoureteral reflux (VUR) and 1 of ureteropelvic junction obstruction.Mean age was 18 months.In laparoscopic group,there were 18 boys and 10 girls,9 were of ectopic ureter,16 of ureterocele and 3 of VUR.Mean age was 1 6 months.There were no significant differences in age and diseases.Results There were 2 conversion to open surgery in laparoscopic group and no other intra-operative and post-operative complications in 2 groups.Mean duration of operation was 2.5 h for open group,the difference of pre- and post-operation hemoglobin was 2.64 g,the volume of drainage was 91.4 ml,the time of drainage was 4.2 d and mean hospital stay was 19.3 d.In Iaparoscopic group,the figures were 2.9 h,1.45 g,55.4 ml,3.4 d and 14.3 d,respectively.The differences of the figwres were significant between the 2 groups (P<0.05). Conclusion Laparoscopic partial nephrectomy is effective and safe for the treatment of duplicated kidney with a dysfunctional moiety.
2.Effectiveness of dyadic intervention on discharge readiness of elderly stroke patients and family caregivers

Objective:To explore the application effect of dyadic intervention scheme based on dyadic disease management theory and Information, Knowledge, Attitude, and Practice model in the discharge preparation of elderly stroke patients and family caregivers.Methods:The 92 pairs of elderly stroke patients and their caregivers hospitalized in the Department of Neurology in People′s Hospital of Zhengzhou University were conveniently selected. The non synchronous control method quasi experimental research was adopted. Totally 46 pairs of subjects who met the criteria for admission and discharge from May to July 2022 were set as the control group, and routine nursing was carried out; from August to October 2022, 46 pairs of subjects who met the criteria for admission and emission were set as the observation group to implement the dyadic intervention program. The scores of discharge readiness, self-efficacy and unplanned readmission rate of patients between the two groups were compared, and the scores of caregiver readiness, self-efficacy and caregiver stress between the two groups were compared.Results:Finally, 85 pairs of subjects completed the study, with 42 pairs in the control group and 43 pairs in the observation group. On discharge day, the total scores of discharge readiness and caregiver readiness in the observation group were (95.19 ± 4.47), (23.02 ± 2.20) points, respectively, which were higher than those in the control group (85.71 ± 5.31), (19.57 ± 1.65) points, with statistically significant differences ( t=8.91,8.16, both P<0.01); the self-efficacy levels of patients in the observation group at discharge and one month after discharge, as well as those of caregivers at discharge and one month after discharge were (73.86 ± 4.87), (75.91 ± 4.51), (75.67 ± 4.99), (79.21 ± 4.90) points, respectively, higher than those in the control group (71.62 ± 5.19), (73.33 ± 4.91), (73.48 ± 4.24), (75.48 ± 4.24) points, with statistically significant differences ( t values were from 2.05 to 3.75, all P<0.05); the pressure levels of caregivers in the observation group at discharge and one month after discharge were (7.51 ± 2.48), (6.28 ± 1.99) points, respectively, lower than those in the control group (8.76 ± 2.55), (7.45 ± 2.36) points, with statistically significant differences ( t=-2.29, -2.48, both P<0.05); the unplanned readmission rate of patients in the observation group one month after discharge was 7.0% (3/43), lower than the control group′s 23.8% (10/42), with statistically significant difference ( χ2=4.65, P<0.05). Conclusions:The implementation of dyadic intervention on elderly stroke patients and caregivers can make their discharge preparation process more adequate, thus reducing the caregiver′s care pressure, reducing the unplanned readmission rate of patients, and improving their health outcomes.
3.Establishment and application of specific nursing quality indicators for infectious disease

Objective To explore the establishment and application of specific nursing quality indicators for infectious disease. Methods On the basis of nursing system, nursing criterion, nursing routine and nursing process, the specific nursing quality indicators were established and implemented in our hospital from October 2015 to June 2016. The specific nursing quality indicators were established according to different characteristics of patients, including psychological nursing, isolation protection, high fever nursing and so on. Every indicator was standardized by structure, process and result modules. The 3 modules included 7 items, which is organization management, nursing assessment, measures implementation, shift execution, nursing records, health guidance and related knowledge, and every item was composed of several standard rules. The correct rate of nursing assessment, the rate of measures implementation, the awareness rate of patients′ disease knowledge, the accuracy of nursing record writing, the qualified rate of shift duty, patients′ satisfaction and qualified rate of specific nursing knowledge were compared before and after implementing the specific nursing quality indicators.Results After the implementation of the specific nursing quality indicators, the correct rate of nursing assessment increased from 74. 89% to 89. 33%, the rate of measures implementation increased from 75. 79% to 88.44%, the awareness rate of patients′disease knowledge increased from 65.47% to 79.56%, the accuracy of nursing record writing increased from 65.02% to 88.00%, the qualified rate of shift duty increased from 60.08% to 81.33%, patients′satisfaction increased from 92.06% to 97.08%, the qualified rate of specific nursing knowledge increased from 69. 59% to 86. 49%. Conclusions The establishment and application of specific nursing quality indicators for infectious disease can normalize and standardize nursing work, improve the process quality and final quality of nursing care, help continuous improvement of nursing quality, and ensure the effective implementation of nursing care.
4.Correlation analysis of the mental state and social support among young patients with tuberculosis

Objective To investigate the status and influence factors on mental health among young stuberculosis patients, and explore the correlation between mental health and social support, so as to provide scientific basis for the development of interventions.Methods A total of 200 young tuberculosis patients, aged ≤40 year, were selected in Xuzhou Infectious Hospital from May 2015 to August 2016,and investigated by self-designed general information questionnaire, Symptom Checklist (SCL-90) and Social Support Rating Scale (SSRS). The correlations were analyzed between mental health and social support.Results Compared with 1986 and 2006 national norm, the sum score of SCL-90 was (147.34±46.39) for involved youth (t=5.298, 5.280;P<0.05). In multiple regression analysis, the main influence factors included educational level, annual income for score of SCL-90. The score of SCL-90 was negatively correlated with scores of social support, subjective support and degree of support utilization (r=-0.232, -0.221, -0.407;P<0.05).Conclusions The mental health status of young patients with tuberculosis is poor, and social support is closely related to mental health. Medical staff should timely carry out psychological intervention measures to improve the mental health and promote the rehabilitation.
5.Application of specialized nursing quality management in the hepatitis B liver cirrhosis patient in decompensated period

Objective To study the specialized nursing quality management of infectious diseases in the role of hepatitis B liver cirrhosis patient care in liver disease internal medicine during decompensated period.Methods For 180 patients with decompensated period of hepatitis B cirrhosis, quality of care was carried out in of the liver specialized subject index management to ensure decubitus standardized rate, gastrointestinal bleeding predictive rate, hepatic encephalopathy predictive nursing accuracy for the corresponding nursing quality monitoring indicators. The comparisons were conducted in the nurse's core competence evaluation results, patient's satisfaction during hospitalization for nursing and common complications for specialized quality indicators before and after implementations.Results After the introduction of specialized nursing quality indicators, nurse's disease clinical core competency assessment qualified rate increased significantly in nurse's ability of disease observation and assessment increasing from 64.71% to 88.24%, professional knowledge master inclining from 61.76% to 94.12%, strain capacity increasing from 55.88% to 85.29%, and patient care satisfaction improving from 85.56% to 97.78% (χ2=5.2308,10.3497,7.0833,8.8000;P< 0.05).Conclusions Specialized nurse quality index is applied to manage patients with hepatitis B cirrhosis in decompensated period for nursing quality control management, which can improve the core competence of nurses and patient satisfaction on nursing service, and decrease complications, as well as enrich the connotation of the nurse quality.
6.Limited value of recovery phase-limited ST segment depression of treadmill exercise test.

BACKGROUNDClinical meaning of recovery phase limited ST segment depression of a treadmill exercise test is controversial. The aim of this study was to re-assess the diagnostic and prognostic value of ST segment depression during the recovery phase with the active phase of a treadmill exercise test in suspected coronary artery disease patients.
METHODSClinical, exercise and angiographic data were retrospectively collected from 602 patients in the study. Five hundred and seventy-six patients developed ST segment depression during the active phase of the treadmill exercise test (group 1) and 26 patients developed ST segment depression only during the recovery phase (group 2).
RESULTSWith similar major clinical features, the prevalence of significant coronary artery stenosis and average Gensini scores were lower in the recovery phase-limited depression patients (group 2 vs. group 1, 50.0% vs. 66.9%, P = 0.031 and group 2 vs. group 1, 1.5 vs. 8.5, P = 0.04). At a median follow up of 50.9 months for 22 group 2 and 34.8 months for 438 group 1 patients, the prevalence of total cardiac events was higher in group 1 than in group 2 patients (RR 1.60, 95% CI 1.00-2.54, P = 0.049).
CONCLUSIONThe present study provides preliminary evidence that the diagnostic and prognostic value of recovery phaselimited ST segment depression of treadmill exercise test is limited.
