Objective To explore the feasibility of gallbladder mucosal ablation by hysteroscopic rollerball electrocoagulation after cholecystostomy.Methods Under epidural or general anesthesia combined with local anesthesia,20 patients were examined and treated with hysteroscopy after cholecystostomy.After inserting a Storz endoscope,the epithelium of the proximal cystic duct and the mucosa of the gallbladder were ablated with the roller-ball and roller-barrel electrodes respectively.The electrocoagulation power was set at 60-70 W with the roller-barrel excursion rate between 10-15 mm/s,which makes the inner wall of the cyst homogenously brown-grey.Results The mean endoscopic operation time was 35 minutes(range: 25-55).Among the patients,the gallbladder lumen was completely obliterated within one month in 12 patients,and within 3 months in 5;3 patients developed retention cysts.Conclusion Endoscopic electrocoagulation of the cholecystic mucosa is feasible for obliteration of the gallbladder.
3.Low-dose 256-slice spiral CT Scanning for Superficial Varicosis of Lower Limbs

Purpose The patients with varicose veins of lower limbs need to undergo extensive CT or X-ray examinations with high doses of radiation before and after treatment.The aim of this study is to investigate the value of low-dose CT contrast-enhanced scan for the superficial varicosis of lower limbs.Materials and Methods Forty patients diagnosed as superficial varicosis of lower limbs were selected prospectively.According to the scanning parameters,40 patients were randomly divided into four groups:group Ⅰ (standard group) scanned by 120 kV and 250 mAs,group Ⅱ scanned by 120 kV and 100 mAs,group Ⅲ scanned by 100 kV and 200 mAs,group Ⅳ scanned by 80 kV and 300 mAs.The effective dose (ED),signal noise ratio (SNR),contrast noise ratio (CNR) and image quality were evaluated.Results (① There were no significant differences in age,body mass index and height between the four groups (P>0.05).② The ED values of the four groups were statistically significant (P<0.05).③ The differences of SNR and CNR had statistical significance between the four groups (F=3.7 and 3.6,P<0.05).All images met the diagnostic requirements.Conclusion The lower tube voltage and the higher tube current (80 kV,300 mAs) can reduce the radiation dose and have less influence on the image quality,which can meet the clinical diagnostic requirements and can be used as routine scanning parameters for patients with superficial varicosis of lower limbs.
4.Quality of life of discharged senile patients with chronic diseases and influencial factors

Objective To investigate the quality of life of patients with chronic diseases as well as the influencing factors. Method Two hundred and fifty five elderly patients with chronic disease from 3 hospitals in Guangzhou involved in the survey by adopting WHO quality of life-old(WHO OQOL-OLD)to investigate the quality of life and influencial factors.Results The quality of life of the elderly patients was in middle level.In the dimension of Past,Present and Future Activities,the quality of life was statistically different between the patients with different chronic diseases(P<0.05);In the dimension of Autonomy and Death&Dying, the quality of life was statistically different between those with and without cerebrovascular diseaseor(all P<0.05);In the dimensions of Autonomy,Social Participation,Death and Dying and Intimacy,the quality of life was statistically different between those needing care and no care after discharge(all P<0.05).Conclusions The quality of life of the elderly patients with chronic diseases is in middle level.The influencing factors for the quality of life include contracting multiple diseases,needing care after discharge and cerebrovascular diseases.At discharge,nurses should regulate instructions for them for the purpose of improving their autonomy,social participation and adaptability after discharge. Thus,their quality of life can be improved.
5.AppIication of enzyme-Iinked bridging assay method to siRNA pharmacokinetic evaIuation

OBJECTIVE To investigate the feasibility and application of enzyme-linked bridging assay(ELBA)method to the pharmacokinetic evaluation of antisense strand siRNA drug. METHODS Antisense strand RNAs were diluted in LNCap cell lysates from 5 to 50 000 pmol·L-1 to construct the quantification curves. We transfected the intact double-strand siRNA at a final concentration 100 nmol·L-1 targeting Polo-like kinase into the LNCap cells and investigated the specificity of ELBA quantitating the siRNA antisense strand in cell supernatant,cell lysates and RNA-induced silencing complex( RlSC). Quantification curves were constructed and validated in biological matrices such as plasma (5-25 000 pmol·L-1 )and multiple tissues(liver,heart,spleen,and kidneys)(3-6250 pmol·L-1 ). The prostate specific membrane antigen aptamer siRNA delivery system with the intact siRNA concentration of 15 nmol·kg-1 was prepared. The siRNAs were delivered into the LNCap xenogrant tumor model in C57 mice by tail vein injection. The concentration of siRNA antisense strand was determined in plasma and tissues 30 min post administration by ELBA. RESULTS The quantitative range of antisense strand siRNA in cell lysates was 5-50 000 pmol·L-1 ,and ELBA method could quantify the siRNA antisense strand concentration from cell lysates and RlSC in LNCap cells transfected with double-strand siRNA. ln addition,ELBA could specifically reflect the single antisense strand concentration instead of intact siRNA double strands in plasma. The quantification range of siRNA antisense strand using ELBA in plasma was 5-25 000 pmol·L-1 and 3-3125 pmol·L-1 in tissues. About 30 min post administration of PSMA aptamer-siRNA,the antisense strand of siRNA was distributed mainly to the tumor,liver,kidneys,blood and spleen in sequence. The distribution profile might be attributed to the target delivery and siRNA pharma-codynamics. CONCLUSION The ELBA method is successfully applied to the siRNA antisense strand pharmacokinetic evaluation,which provides an alternative for pharmacokinetic studies of siRNA-based drugs.
6.Study of the risk factors for syncope in children with orthostatic intolerance

ObjectiveTo investigate the risk factors for syncope in children with orthostatic intolerance (OI).Methods The clinical data from 136 children with OI were retrospectively analyzed. The children were divided into syncope group and non-syncope group based on the existence of syncope. Results Among the 136 children, 77 children (56.62%) had syncope and 59 children (43.38%) did not have syncope. The differences in BMI, prevalence of inducement of prolonged standing, prevalence of accompanied symptoms of blurred vision/ blacking out, cold sweating and pallor, prevalence of the history of motion sickness and blood flow velocity of MCA were statistically significant between two groups (P<0.05). Conclusions Children with OI who has low BMI, inducement of prolonged standing, accompanied symptoms of blurred vision/ blacking out, cold sweating and pallor, the history of motion sickness, and fast blood flow velocity of MCA, are prone to syncope.
7.Contrast ultrasound in the assessment of renal cortical perfusion in rabbits

Objective To explore the value of the ultrasound contrast agent in conjunction with advanced contrast imaging techniques in the evaluation of the renal cortical perfusion. Methods The animal model was established with ligating renal artery to cause the renal cortical perfusion decrease of rabbits. Real-time harmonic gray scale imaging was performed to visualize the contrast enhancement of the renal cortex. After bolus injection of contrast agent SonoVue, dynamic image was observed and recorded during the first three minutes. According to the time-intensity curve (TIC), accelerating time (AT),Peak intensity (A), time to half of peak intensity (T), difference between peak and base (PBD), slope rate of the curve (β) and the product of A and β (A·β) were obtained before and after the renal artery operation. The t test and correlation analysis was used to examine the above parameters. Results Before the renal artery operation, renal cortex enhanced rapidly and obviously, the way of the enhancement was renal artery-cortex-pyramids. After ligation of renal artery, AT prolonged evidently (P<0.001), A decreased obviously (P<0.001), T obviously shortened (P<0.001), PBD decreased markedly (P<0.001), while the decline of βinduced (P<0.002) and A·βobvious dropped(P<0.001). AT, BPD and A·βcorrelated with stenosis level of renal artery. Compared with 31%-69% group, AT prolonged evidently (P<0.001). A decreased obviously (P<0.029), PBD decreased obviously (P<0.001), A·β was dropped markedly (P<0.02) in 70%-93% stenosis group. Conclusions Contrast agent combining with developed contrast specific imaging is able to detect changes of renal cortical microcirculation sensitively with the help of analysis of TIC.
8.Diagnostic and therapeutic status of the elderly outpatients with benign prostatic hyperplasia in Guangzhou

Objective To evaluate the diagnostic and therapeutic status of lower urinary tract symptoms (LUTS) in the elderly outpatients with benign prostatic hyperplasia (BPH) in Guangzhou.Methods The investigation was conducted in outpatient departments for elderly patients in 3 hospitals of Guangzhou. The eligible male patients with LUTS/BPH were chosen from patients who came to the clinic in the process of investigation. Results The 29.7% (1824/6140) of them had history of LUTS/BPH previously or presently. In 134 patients received medical questionnaire, the proportion of LUTS/BPH patients with mild, middle and severe degree based on international prostate symptom score (IPSS) was 24.5%, 72.5% and 3.0% respectively. The abnormal percentage of PSA was 37. 3 %. The most common examinations that patients received were directeral rectun examination (96.8%), PSA (88. 7%) and transabdominal ultrasonography (84.8%). The 44.7% of cases were given the treatment of 5α-reductase inhibitor, and 24.7% of cases received a combination therapy of 5α-reductase inhibitor plus α receptor blocker. The 16.7% of cases were given a combination treatment of 5α-reductase inhibitor plus herbs. The proportion of therapy with α receptor blocker and herbs was similar (6.8% and 7.1% respectively). Conclusions LUTS/BPH is one of the most common diseases in elderly male patients. The examinations conducted by doctors are unreasonable.The doctors are easy to ignore inquirement of patient's history and IPSS. The drugs conducted by doctors are reasonable basically.
9.Relationships between Islet-Specific Autoantibody Titers and the Clinical Characteristics of Patients with Diabetes Mellitus

Dysimmunity plays a key role in diabetes, especially type 1 diabetes mellitus. Islet-specific autoantibodies (ISAs) have been used as diagnostic markers for different phenotypic classifications of diabetes. This study was aimed to explore the relationships between ISA titers and the clinical characteristics of diabetic patients. A total of 509 diabetic patients admitted to Department of Endocrinology and Metabolism at the Affiliated Hospital of Nantong University were recruited. Anthropometric parameters, serum biochemical index, glycosylated hemoglobin, urinary microalbumin/creatinine ratio, ISAs, fat mass, and islet β-cell function were measured. Multiple linear regression analysis was performed to identify relationships between ISA titers and clinical characteristics. Compared with autoantibody negative group, blood pressure, weight, total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), visceral fat mass, fasting C-peptide (FCP), 120 minutes C-peptide (120minCP) and area under C-peptide curve (AUCCP) of patients in either autoantibody positive or glutamate decarboxylase antibody (GADA) positive group were lower. Body mass index (BMI), waist circumference, triglycerides (TGs), body fat mass of patients in either autoantibody positive group were lower than autoantibody negative group. GADA titer negatively correlated with TC, LDL-C, FCP, 120minCP, and AUCCP. The islet cell antibody and insulin autoantibody titers both negatively correlated with body weight, BMI, TC, TG, and LDL-C. After adjusting confounders, multiple linear regression analysis showed that LDL-C and FCP negatively correlated with GADA titer. Diabetic patients with a high ISA titer, especially GADA titer, have worse islet β-cell function, but less abdominal obesity and fewer features of the metabolic syndrome.
