1.Factors affecting Insomnia by Degree of Subjective Noise Perception in School Meal Facility Workers

Purpose:
This study aims to identify the level of subjective noise perception among School Meal Facility Workers and analyze the relationship between subjective noise perception and insomnia, as well as the factors affecting insomnia.
Methods:
School meal facility workers were surveyed from November 1 to December 31, 2021, and 717 people were selected as participants for the study. Using SPSS/WIN 25.0, t-test, x 2 test, and multiple logistic regression analysis were conducted. The Korean Insomnia Severity Index (ISI-K) was used to evaluate insomnia.
Results:
The variables that showed significant differences in insomnia in general characteristics, health-related characteristics, subjective health perception, and subjective noise perception of the study participants were mental disorders such as subjective health perception, work stress, depression, etc., gastrointestinal disorders, and subjective noise perception, among which subjective health perception, mental disorders such as depression, and subjective noise perception were analyzed as factors influencing insomnia.
Conclusion
To improve the quality of insomnia and sleep among school meal facility workers, there is a need to improve healthcare, emotional management, and noisy work environments.

6.A Case of Neuroendocrine Carcinoma Presenting as a Subepithelial Tumor Diagnosed by ESD.

A gastric neuroendocrine carcinoma is very rare, and the histological diagnosis is very difficult. These carcinomas result in a poor prognosis because they are preceded by severe lymphovascular invasion and early metastasis. In particular, it is difficult to distinguish between adenocarcinoma and neuroendocrine carcinoma by endoscopy when no specific symptoms are present (e.g., dyspepsia, nausea). According to published articles in Korea, most cases were diagnosed as adenocarcinoma initially; however, they were confirmed postoperatively as neuroendocrine carcinoma based on a histological examination using immunohistochemical staining. A case of a 55-year-old man, who had an incidental finding of a subepithelial tumor during his health check-up, was recommended for an endoscopic submucosal dissection (ESD). But the patient was lost to follow-up for 4 years. When he was examined again, the size of the tumor had increased from the previous exam. He underwent ESD and was diagnosed with a well-differentiated neuroendocrine carcinoma.

7.Circulating BMP-7 Level is Independent of Sarcopenia in Older Asian Adults

Background:
In vitro and animal studies have demonstrated that bone morphogenetic protein-7 (BMP-7), renowned for its osteogenic properties, also exerts beneficial effects on muscle metabolism by enhancing myogenesis and reversing muscle atrophy. Despite being proposed as a common regulatory factor for both muscle and bone, the impact of BMP-7 on human muscle health has not been thoroughly investigated.
Methods:
This cross-sectional study involved 182 community-dwelling older adults who underwent a comprehensive geriatric assessment in South Korea. Sarcopenia was diagnosed using Asian-specific cutoffs, and serum BMP-7 levels were quantified via enzyme immunoassay.
Results:
The mean age of the participants was 72.2±7.3 years, with 62.6% being female. After adjustments for confounders, serum BMP-7 levels were not significantly different between individuals with and without sarcopenia, nor were there differences based on skeletal muscle mass, strength, or physical performance levels (p=0.423 to 0.681). Likewise, no correlations were detected between circulating BMP-7 levels and any sarcopenia assessment metrics such as skeletal muscle index, grip strength, gait speed, or chair stand completion times (p=0.127 to 0.577). No significant associations were observed between increases in serum BMP-7 concentrations and the risk of sarcopenia or poor muscle phenotypes (p=0.431 to 0.712). Stratifying participants into quartiles based on serum BMP-7 levels also indicated no differences in sarcopenia-related parameters (p=0.663 to 0.996).
Conclusion
Despite experimental evidence supporting BMP-7’s role in muscle metabolism, this study found no significant association between serum BMP-7 levels and clinical indicators of muscle health in older adults. These findings challenge the utility of serum BMP-7 as a biomarker for sarcopenia in this demographic.

Objective:
This study aimed to investigate the effect of three-dimensional (3D) printed clear aligners (CA) with different designs on the extrusion of mandibular premolars using a force/moment measurement system and digital image correlation (DIC).
Methods:
The forces and moments applied to the mandibular canines, first and second premolars were measured using a multi-axis force/ moment transducer when an extrusion of 0.5 mm was planned, assuming the mandibular first premolars were intruded by 1 mm. In addition, displacement and strain changes in the CA were analyzed using the DIC method. CA designs were categorized based on the presence of first premolar attachment and subdivided into equigingival margins, 1-mm extended margins, equi-margins with 1-mm thickness and height, and equi-margins with 1-mm reduced buccolingual width.The CA was printed directly at a thickness of 0.5 mm, and the experiments were conducted at 37°C.
Results:
The results showed that attachment played an important role in the extrusion of first premolars in both the force/moment measurement system and the DIC method. Intrusion was observed without attachment, even though extrusion was planned. CA designs apply greater force to the cervical region by extending the margin or reducing the buccolingual width, thereby improving extrusion efficiency.
Conclusions
Force and moment changes in direct 3D printed CA are complex and difficult to predict; however, modifying aligner designs, such as extending the margin or reducing buccolingual width, and using appropriate attachments could minimize unwanted tooth movement, optimize planned treatment, and increase treatment predictability.
