ObjectiveTo investigate the correlation between any two of Capsule Endoscopy ScroringIndex (Lewis score),simplified Crohn Disease Activity Index (CDAI) and C-reactive protein (CRP) in small bowel Crohn disease (CD).MethodsA total of 58 consecutive patients with known small bowel CD were enrolled. We evaluated disease activity with Lewis score and simplified CDAI. Correlations among CRP,simplified CDAI and Lewis score were calculated with Spearman's rank order correlation coefficient. The optimal CRP cut-off value was calculated using the ROC curve.ResultsThe Lewis score showed inactive, mild and moderate-severe patients were 13,21 and 24,respectively.CRP of moderate-severe group was significantly higher than that in mild and inactive groups ( P < 0.05 ).The optimal CRP cut-off value that differentiated patients with moderate to severe disease from the others was 13.50 mg/L with sensitivity of 87.5% and specificity of 82.4%.The area under the ROC curve to analyze the cut-off was 0.849.Lewis score was moderately correlated with CRP (r =0.58,P < 0.01 ),and weakly correlated with the simplified CDAI (r =0.40,P < 0.01 ).ConclusionSerum CRP and the simplified CDAI cannot replace Lewis score for capsule endoscopy in the assessment of disease activity in small bowel CD.However,CRP may be considered as an inflammatory marker for evaluating the moderate to severe capsule endoscopic activity.

Objective:To explore the effects of two kinds of indwelling needles with different materials on catheterization related complications and comfort of patients undergoing cerebral angiography. Methods:A total of 120 patients who were hospitalized in Neurology Department of Qinhuangdao First Hospital and were given venous indwelling needles for intravenous infusion from July 2018 to May 2019 were randomly enrolled. According to admission order, they were numbered and divided into observation group and control group by random number table method, 60 cases in each group. The control group was given common intravenous indwelling needle, while observation group was given indwelling needle of positively pressurized needle-free link-reservation. The differences in catheterization related complications, catheterization time and comfort were compared between the two groups. Results:At 24 h after catheterization, the incidence rates of tube blockage, fluid exudation and phlebitis in observation group were 0, 1.67% (1/60), 0, while 3.33% (2/60), 3.33% (2/60), 3.33% (2/60) in control group, there were no significant differences between the two groups ( χ2 values were 2.037, 0.348, 2.039, P> 0.05). At 48 h after catheterization, the incidence rates of tube blockage, fluid exudation and phlebitis in observation group were 1.67% (1/60), 1.67% (1/60), 1.67% (1/60), while 10.00% (6/60), 8.33% (5/60), 6.67% (4/60) in control group, there were no significant differences between the two groups ( χ2 values were 3.793, 2.814, 1.884, P>0.05). At 72 h after catheterization, the incidence rates of tube blockage, fluid exudation and phlebitis in observation group were all 3.33% (2/60), which were lower than 16.67% (10/60), 15.00% (9/60), 13.33% (8/60) in control group, there were significant differences between the two groups ( χ2 values were 5.934, 4.905, 3.937, P<0.05). At 24, 48 and 72 h after catheterization, liquid flow rates per unit of time before and after tube flashing in observation group were (506.15 ± 66.84), (483.55 ± 36.87), (410.62 ± 41.66) ml/h, which were higher than (474.77 ± 45.71), (417.80 ± 22.14), (310.87 ± 20.96) ml/h in control group ( t values were 3.002, 11.842, 16.568, P<0.05). The comfort score was (8.53 ± 1.41) points in observation group, higher than (7.97 ± 1.23) points in control group, and indwelling time of indwelling needles was (84.45 ± 8.95) h in observation group, longer than (76.97 ± 6.77) h in control group ( t values were 2.318, 5.163, P<0.05). Conclusions:Indwelling needle of positively pressurized needle-free link-reservation can effectively reduce incidence of complications in patients undergoing cerebral angiography, and improve their comfort during catheterization.

OBJECTIVE To study the protective effects of Dachengqi decoction (DCQD) on intestinal septic mice, and to explore the possible mechanisms from the Toll-like receptor 4(TLR4)/myeloid differentiation factor 88(MyD88) signaling pathway. METHODS The SPF male C57BL/6J mice were randomly divided into Sham group, Sham+DCQD-H group, model (CLP) group, DCQD-L group, DCQD-H group and Positive group. The model of intestinal sepsis was established by cecal ligation and puncture in CLP group, DCQD-L group, DCQD-H group and Positive group. Three days before the operation and seven days after the operation, DCQD-L group and DCQD-H group were given DCQD intragastrically at 4, 8 g/kg (calculated by crude drug), respectively. Positive group was given ulinastatin intraperitoneally 2 h before operation and 7 d after the operation (at 50 000 U/kg). In Sham group and Sham+DCQD-H group, only cecum of mice was exposed without ligation and puncture. Sham+DCQD- H group was given DCQD intragastrically (8 g/kg,calculated by crude drug) 3 days before the operation and 7 days after the operation. Both the Sham group and CLP group were given normal saline 0.2 mL intragstrically and intraperitoneally each day, for 10 consecutive days. After the operation, the severity of sepsis was assessed, and the 7 d survival rate of mice was assessed. One hour after the last medication, the levels of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) in serum and ileum of mice were determined; the pathological and morphological changes of mice’s liver, lung, kidney and ileum were observed; mRNA expressions of the TLR4 and MyD88 in ileum were tested. RESULTS Compared with CLP group, sepsis score, the levels of TNF-α and IL-6 in serum and ileum (except for IL-6 in ileum of DCQD-L group), damage score of the liver, lung, kidney and ileum, mRNA expressions of TLR4 and MyD88 in ileum were all decreased significantly in DCQD-L group and DCQD-H group (P<0.05 or P<0.01), while 7 d survival rate (except for DCQD-L group) was increased significantly (P<0.05). The damage to liver tissue in mice was significantly improved, and inflammation infiltration and apoptosis were reduced; lung tissue damage had been alleviated, with varying degrees of improvement in alveolar atrophy, bleeding and edema; the renal tissue damage was improved and weakened dilation of renal tubular lumen was weakened; the damage and edema of ileal tissue were significantly improved. CONCLUSIONS DCQD may exert a protective role on intestinal septic model mice. The mechanism may be related to the inhibition of systemic inflammation, the reduction of multiple organ damage, and down-regulation of TLR4/MyD88 signaling pathway.
