1.Research on slotting optimization of medical devices warehouse in class A tertiary hospital

Objecive To explore modern management and optimization of hospital warehouse slotting with some class A tertiary hospital taken as an example.Methods EIQ_ABC classification method,multi-objective assignment model and genetic algorithm were used to analyze and simulate the data of 126 kinds of medical devices for 20 orders.Results The medical devices were divided into nine categories of Ⅰ A,Ⅰ B,Ⅰ C,Ⅱ] A,Ⅱ B,Ⅱ C,Ⅲ A,Ⅲ B and Ⅲ C.It's suggested that emphasized management and precision slotting storage were carried out for A categories of devices,dedicated storage for B categories and random storage for C categories.At the same time,the layout of the warehouse was optimized according to the importance of the goods so that the important goods were placed near the passageway.Slotting analog simulation diagram was obtained based on multi-objective assignment model of Ⅲ A category medical device.Conclusion Classified management and allocation as well as slotting optimization of medical devices contribute to improving hospital modern management in efficiency,cost and error rate.
2.Determination of ginsenoside Rg_1 and Re in Shenqi Granula by HPLC

Objective To establish an HPLC method for the determination of ginsenosides Rg1 and Re in Shenqi Granula.Methods Chromasil C18 column(250 mm?4.6 mm)was used with acetonitrile-0.05% phosphoric acid solution(21∶79)as mobile phase.The flow rate was 1 mL/min and the detected wavelength was 203 nm.Results Ginsenosides Rg1 and Re could be baseline separated with in 30 min.The average recovery rates were 99.60% and 98.5%,corresponding RSD were 1.93% and 2.31% for ginsenoside Rg1 and Re,respectively(n=5).Conclusion This method is fast and accurate and can be used for quality control of Shenqi Granula.

4.Study on the association between vitamin D receptor gene polymorphism of ApaI and BsmI and vitamin D deficiency rickets

ObjectiveTo study the distribution characters and linkage disequilibrium of vitamin D receptor(VDR) gene ApaI and BsmI polymorphism,and explore the genetic susceptibility of VDR and vitamin D deficiency rickets.MethodsVDR gene ApaI and BsmI polymorphisms were determined by PCRRFLP technology in 56 cases of rickets and 76 cases of normal children.The frequencies of the VDR genotype and allele were compared between the two groups.The software SHEsis was used to make linkage disequilibrium analysis.Results No significant difference was found in either the frequency distribution of VDR gene ApaI and BsmI polymorphism or allele of them between two groups; Two polymorphisms didn't show linkage disequilibrium and D' and r2 were 0.23 and 0.01,respectively.ConclusionsThe ApaI and BsmI polymorphism of VDR gene might be not associated with vitamin D deficiency rickets.Two polymorphisms didn't show linkage disequilibrium.
5.The study on nucleic acid testing for bacterial detection and its preliminary application on concentrated platelet

Objective To establish a nucleic acid testing assay for bacterial detection with meeting automatic requirement,and use it preliminarily to detect bacterial contamination in concentrated platelet.Methods Optimizing the bacterial DNA extraction methods,evaluating the sensitivity and specificity of the optimized assay with different bacterial strains,and applying the assay on bacterial detection for 100 units of concentrated platelet.Results The optimized extraction methods met the criteria of automatic testing.The sensitivity of the assay is 10,150,65,15 CFU/mL for Escheriehia coli,Stphylococcus aureus,Bacillus subtilis,and Pseudomonas aeruginas respectively.There was no non-specific amplification detected.And the assay could detect 1 contaminated bacterial from 100 units of concentrated platelet as same as BactALT bacterial culture.Conclusions Nucleic acid testing for bacterial genome 16S DNA could detect common contaminated bacterial sensitively and fast from platelet product.This assay could have the same effect when compared to bacterial culture methods when it was applied in concentrated platelet products.
6.Molecular Characterization of Avian-like H1N1 Swine Influenza A Viruses Isolated in Eastern China, 2011

Currently,three predominant subtypes of influenza virus are prevalent in pig populations worldwide:H1N1,H3N2,and H1N2.European avian-like H1N1 viruses,which were initially detected in European pig populations in 1979,have been circulating in pigs in eastern China since 2007.In this study,six influenza A viruses were isolated from 60 swine lung samples collected from January to April 2011 in eastern China.Based on whole genome sequencing,molecular characteristics of two isolates were determined.Phylogenetic analysis showed the eight genes of the two isolates were closely related to those of the avian-like H1N1 viruses circulating in pig populations,especially similar to those found in China.Four potential glycosylation sites were observed at positions 13,26,198,277 in the HA1 proteins of the two isolates.Due to the presence of a stop codon at codon 12,the isolates contained truncated PB1-F2 proteins.In this study,the isolates contained 591Q,627E and 701N in the polymerase subunit PB2,which had been shown to be determinants of virulence and host adaptation.The isolates also had a D rather than E at position 92 of the NS1,a marker of mammalian adaptation.Both isolates contained the GPKV motif at the PDZ ligand domain of the 3' end of the NS1,a characteristic marker of the European avian-like swine viruses since about 1999,which is distinct from those of avian,human and classical swine viruses.The M2 proteins of the isolates have the mutation (S31N),a characteristic marker of the European avian-like swine viruses since about 1987,which may confer resistance to amantadine and rimantadine antivirals.Our findings further emphasize the importance of surveillance on the genetic diversity of influenza A viruses in pigs,and raise more concerns about the occurrence of cross-species transmission events.
7.Application value of low radiation dose coronary angiography using 320-slice volume CT

Objective To evaluate the image quality and radiation dose of prospective electrocardiography-triggered coronary 320-slice volume CT angiography with different kV, and the feasibility of coronary scan with < 1 mSv radiation dose.Methods Eighty consecutive patients were randomly divided into two groups equally.The tube voltage according to paradigm was 100 kV in group A and 120 kV in group B.All raw data in group A was reconstructed by the software AIDR in CT system to create a new group named as A1. Such parameters as the mean intraluminal attenuation (SI),noise (SD),signal-to-noise ratio (SNR),contrast-to-noise ratio (CNR),effective radiation dose(E) and image quality score measured in group A were compared with those in group B.The values such as SI,SD,SNR,CNR,image quality scores were compared between group A and group A1.The significance of group B and group A1 was compared in SI,SD,SNR,CNR,image quality scores as well.Results E in group A was significantly lower than that in group B[ E =(0.67 ± 0.18) mSv in group A vs.E =(3.08 ± 1.04) mSv in group B].The value of E in group A was decreased by 78% compared to group B(t =- 14.30,P<0.05 ).There was no significant difference in mean image quality scores between two groups(4.57 ± 0.57in groupA vs.4.59 ± 0.59 in group B,t=-1.17,P>0.05).The values of SI,SD,SNR,CNR in group A were (570.8 ±131.5)HU,25.1 ±6.9,24.5 ±9.1,19.8 ±6.1.And the values of SI,SD,SNR,CNR in group B were (460.6 ± 14.3) HU,15.1 ±3.6,31.7 ±7.7,29.3 ±6.8.The values of SI and SD in group A were significantly higher than those in group B(t =4.49,8.18,P <0.05). The values of SNR and CNR in group A were lower than those in group B (t =-4.24,-6.19,P<0.05).The valuesofS1,SD,SNR,CNR,image quality scores in group Al were (557.9 ±24.5) HU,21.1 ±6.0,27.7±10.0,23.4±7.8,4.60 ± 0.56.There was no difference in the SI and the image quality scores between group A and group A1 ( t =1.09,- 1.90,P > 0.05).Conclusions 320-slice volume CT with 100 kV tube voltage and prospective ECG-triggered technique can reduce the radiation dose to less than 1 mSv and obtain optimal images in diagnosis of coronary arterial diseases.
8.Diagnostic value of 18 F-FDG PET/CT imaging for primary pulmonary lymphoepithelioma-like carci-noma

Objective To assess the diagnostic value of 18 F?FDG PET/CT imaging for primary pul?monary lymphoepithelioma?like carcinoma (LELC). Methods A total of 19 patients (10 males, 9 fe?males;average age 56.4 years) with suspected primary pulmonary LELC undergone whole?body 18F?FDG PET/CT and chest contrast CT scan were enrolled in this retrospective study. The SUVmax and uptake pat?terns of FDG, the histological types and the clinical stages were recorded. Image characteristics of contrast CT and FDG PET/CT were observed, and the diagnostic efficiency was calculated. Two?sample t test and one?way analysis of variance were used to analyze the results. Results The diagnostic sensitivity and spe?cificity of LELC were 12/13 and 4/6 for FDG PET/CT. The SUVmax of LELC lesions was significantly differ?ent from that of other lung malignant tumors or lung benign lesions (F=3.67, P<0?05). The lesion had longer diameter, lower CT density, higher SUVmax in stage Ⅳ patients, compared with that in stageⅠ-Ⅲpatients ( F=7.01, P<0.05) . The significant difference of SUVmax was found between lesions with ring up?take and lesions with other uptake types (3.94±0.67 vs 2.86±0.35; t=4.07, P<0?05). The SUVmax was higher in Schmincke tumors when compared with that in tumors of Regaud subtype ( 3. 61 ± 0. 71 vs 2. 76 ± 0?29; t=2.99, P<0.05) . Conclusion 18 F?FDG PET/CT could be an accurate method in differential diag?noses of primary pulmonary LELC.
9.Establishment of three-dimensional finite element digital foot model

BACKGROUND:To overcome the disadvantages of traditional mechanical analysis of specimens, and establish the finite element model of realistic foot, are the important basements for the finite element mechanical analysis on foot.
OBJECTIVE:To establish three-dimensional finite element model of foot and lay the foundation for the finite element analysis of normal foot and foot injury.
METHODS:A healthy female volunteer was involved in this study and was detected with spiral CT scanning on the feet. The resulting image was used to reconstruct the three-dimensional model by using Mimics software. Then entity model was generated in Geomagic software. Final y three-dimensional finite element model was established based on the digital main structure in Ansys. RESULTS AND CONCLUSION:The established finite element digital model of human foot included al bone, cartilage and ligament, skin and soft tissue. The three-dimensional finite element model of human foot was established based on CT data and using Mimics, Geomagic, Ansys softwares. The established model had similar size and shape with skeletal mode, and can rotate freely in any angle for a variety of measurement, the foot bones can be arbitrarily split or merge, which is suitable for biomechanical analysis.
10.MPS in diagnosing and treating gastrointestinal submucosal protrusive lesions

Objective To evaluate the value of miniprobe sonography (MPS) in diagnosing gastrointestinal submucosal protrusive lesions and selecting the indicated manual for treatment. Methods According to the sizes, properties, and depth of SMTs in the gastrointestinal tract detected by the MPS, different methods of resection were performed. Results Of 24 cases, 11 SMTs lying in submucosa under 2cm in diameter (2 benign gastrointestinal stromal tumors, 3 lipomas, 5 cysts, 1 granular cell tumor) were attempted with EMR or argon plasma coagulation (APC) ; there were no complications of hemorrhage or perforation. Thirteen SMTs lying in muscularis propria or with size of SMTs above 2 cm in diameter (4 malignant GISTs, 6 benign GISTs, 1 lipoma, 2 aberrant pancreas) were performed by surgical resection. Preoperative diagnoses of SMTs by MPS were consentient with their histological diagnoses. Conclusion MPS may detect the size, property, and depth of SMTs in the gastrointestinal tract and is helpful in selecting indicated cases for endoscopic resection. Endoscopic therapy of SMTs lying in mucosa or submucosa under 2cm in diameter is a safe and effective procedure.
