3.Experimental study of the effect of ropivacaine combined with sufentanil analgesic on neuropathic pain in rats

Objective To explore the effect of the ropivacaine combined with sufentanil on neuropathic pain in rats.Methods The constriction injury method was performed to establish rat neuropathic pain model and 40 neu-ropathic rats were produced in accordance with the above modeling methods,which were divided into group A(ropiva-caine and sufentanil combined administration,n =10),group B(ropivacaine administration,n =10),group C(sufen-tanil administration,n =10)and group D(no administration,n =10).A total of 10 healthy rats without any treatment, the normal feeding,named as group E.At 1,3,7,11,14,21d after operation von -Frey filaments determination of mechanical hyperalgesia(MWT),and the Hargreaves method for the determination of thermal hyperalgesia(TWL) were measured and the ELISA Kit (IL -6 and TNF -α) were used to detect serum IL -6 and TNF -αconcentrations.Results Before and at 1 day after the operation mechanical allodynia and thermal hyperalgesia and allergy treatment had no significant difference(all P >0.05).Mechanical allodynia and thermal hyperalgesia of group A,B and C were significantly lower than those before the treatment and at 1 day after operation(all P <0.05),and mechanical hyperalgesia and thermal hyperalgesia of group A was significantly lower than those of group B and C (all P <0.05).The serum IL -6 and TNF -αconcentrations of 7,14 and 21 days in group A were significantly lower than those of group B and C(t =7.10,5.84,9.11,all P <0.05).Conclusion The effects of ropivacaine combined with sufentanil analgesic on neuropathic pain of rats was significantly higher than that of ropivacaine or sufentanil alone in the treatment.
4.Non-negative matrix factorization with sparseness constraints for neural activity in rat prefrontal cortex during working memory task

Objective To analyze neural activity of in rat prefrontal cortex with the use of nonnegative matrix factorization with sparseness constrains (NMFs) as a methodology and to study how to express neural ensemble with higher precision during working memory task.Methods Experiment data were obtained from neural population activity in the period 5 s before and after the working memory event.From the zero point,the neuronal firing times were binned in windows of 200 ms with 50 ms overlapping.The normalized neuronal bin-count matrix is decomposed by NMFs into mixing matrix and source component matrix with sparseness constraints.Meaningful components were extracted to reconstruct the input by an inverse of NMFs transform.Results By analyzing the ten groups of data from 2 rats,with the numbers of the sparse sources of 10 and 15 respectively,explicit neural ensembles with the feature components were obtained in the sparse reconstructed activity.Comparing to rate coding,the spatiotemporal location of neural ensemble was more precisely detected.Conclusion The working memory information is encoded with neural ensemble activity.NMFs could find the sparse firing pattern robustly in neuron population activity.NMFs removes much redundancy and demonstrate the possibility to express neural ensemble with higher precision compared with rate coding,which would be helpful to infer correlations between cortical firing pattern and working memory event.
5.Study on level and correlation of biochemical joint indicators among stroke high risk populations in Yichun City

Objective To understand the level and correlation of biochemical joint indicators in stroke high risk population in Yi-chun City ,and the differences in biochemical joint indicators between community and rural stroke high risk populations .Methods Three hundreds and fifty-four high risk people during stroke screening in 2015 and 2016 included 115 community high risk cases , 239 rural cases ,226 males and 128 females ,aged 40 to 86 years old .Thirty-one cases of non-stroke high risk included 15 cases of male and 16 cases of female ,aged 28 to 85 years old .Seven biochemical joint indicators were detected and statistically analyzed ,in-cluding glucose (GLU) ,glycated hemoglobin(HbA1c) ,total cholesterol(TC) ,triacylglycerin (TG) ,high-density lipoprotein choles-terol(HDL-C) ,low-density lipoprotein cholesterol (LDL-C) and homocysteine (Hcy) .Results In the comparison between stroke high risk population and non-high risk populations ,CH ,TG ,LDL-C and Hcy had statistically significant difference (P< 0 .05) . HDL-C ,GLU ,HbA1c had no statistically significant difference (P>0 .05) .In the comparison between community and rural stroke high risk populations ,CH ,TG ,LDL-C and GLU had statistically significant difference (P<0 .05);HDL-C ,Hcy and HbA1c had no statistically significant difference (P>0 .05) .Conclusion The increase of CH ,TG ,LDL-C and Hcy is the risk factors of stroke , while CH ,TG ,LDL-C and Hcy are the interventional risk factors and play an important role in its early diagnosis ,treatment and prognosis .The key to prevent stroke is the healthy diet ,good living habits ,moderate exercises ,and regular monitoring of CH ,TG , LDL-C ,Hcy and GLU ,forewarning ,prevention and treatment ,which can avoid the occurrence or less occurrence of stroke ,thus re-duces the its incidence rate ,greatly decrease the morbidity ,recurrence rate ,and reduces the familial and social burden of manpower , material resources and financial resources .
6.Clinical efficacy of voice training in the perioperative period of vocal cord polyp

OBJECTIVE To study the clinical efficacy of voice training on voice recovery in patients with vocal cord polyps during perioperative period.METHODS A total of 72 patients with vocal cord polyp were chosen in this study and they were randomly divided into study group and control group. After operation, the control group received routine treatment, and the study group received voice training on the basis of routine treatment. The results of voice acoustic analysis in the two groups were compared at 8 weeks after operation.RESULTS After 8 weeks, the maximum phonation time(MPT), dysphonia severity index (DSI) and the highest frequency(F0-High) of the study group were significantly higher than that of control group (P<0.05). The fundamental frequency perturbation(Jitter) of the study group was markedly lower than that of the control group(P<0.05). There was no statistical difference in the fundamental frequency(F0), the lowest intensity(I-Low) and amplitude perturbation(Shimmer) between the two group (P>0.05).CONCLUSION Voice training in the early postoperative period of vocal cord polyps was helpful to the recovery of the voice.

8.The efficacy of microbubbles augmentation thrombolysis using low-frequency ultrasound: an in vitro study

Objective To promote in vitro tissue-plasminogen activator(t-PA)thrombolysis by using microbubbles augmented low frequency ultrasound(US) insonication.Two kinds of microbubbles,albumin-coated vs.lipid-coated were compared.Methods Human blood clot weighted 200~300 mg was made from of 0.8 ml fresh blood.Twenty kHz ultrasound,lipid and albumin-coated microbubbles were applied to separate clots groups with or without t-PA administration.The clots before and after processing were weighed and then the clot dissolution ratios were calculated.Results The simple ultrasound group and the basic t-PA group dissolved((24.72)?(4.83))% and((35.66)?(3.34))% of the clots,respectively.The clot dissolution ratio rised to((42.06)?(4.20))% when coordinated US and t-PA(P(0.05)).Conclusions The in vitro US augmentation of t-PA thrombolysis can be significantly promoted by introduction of microbubbles.But there is no significant thrombolysis difference between albumin and lipid-coated microbubbles.
9.Expression of CD40 and CD40L on Peripheral Blood Mononuclear Cells in Patients with Recurrent Genital Herpes

Objective To investigate the expression of CD40/CD40L on peripheral blood mononuclear cells (PBMC) in patients with recurrent genital herpes (RGH). Methods Flow cytometric analysis was employed to identify the expression of CD40 and CD40L on PBMC and the T lymphocyte subgroups in the peripheral blood of 30 patients with RGH and 20 healthy controls. Results Percentage of the cells with either CD40 or CD40L expression in the patients with RGH was significantly lower than that in the controls (P = 0.0061 and 0.041, respectively). The RGH group had a significantly lower percentages of CD3+ T cells (P=0.025) and CD4+ T cells (P = 0.032) as compared to the controls. Conclusion Reduced costimulatory interaction of CD40 and CD40L in RGH patients may be one of the important factors for the recurrence.
10.Analysis of clinical features of pregnant women with hypertrophic cardiomyopathy

Objective To discuss the clinical features of pregnant women with hypertrophic cardiomyopathy (HCM).Methods There were 28 patients with HCM who delivered in Renji hospital of Shanghai Jiaotong University from January 2000 to August 2012.Clinical data were analyzed,including diagnosis,cardiac functional grading,gestational weeks of delivery,delivery mode,birth weight,Apgar scores,etc.Results (1) Of all the 28 patients,14 (50%) were diaguosed before pregnancy and others (50%) were diagnosed during pregnancy.(2) Four cases were obstructive HCM (14%),3 with cardiac function grade Ⅰ and 1 with grade Ⅱ.Twenty four cases were non-obstructive HCM (86%),14 with cardiac function grade Ⅰ,9 with grade Ⅱ and 1 with grade Ⅳ.(3) Of all the 28 patients,4 had family history,18 (64%) had clinical symptoms or signs which occurred in 8-32 gestational weeks.Twenty-three cases had abnormal ECG (82%).Among them 21 had non-obstructive HCM (88%),with average interventricular septal thickness of(22 ± 3) mm.The other 2 patients had obstructive HCM,with average interventricular septal thickness of (23 ± 4) mm.7 patients (7/28,25 %) had mild-to-moderate pulmonary hypertension [6 with non-obstructive HCM (6/24,25%)],and 10 patients had abnormal myocardial enzyme spectrum or troponin levels [9 with non-obstructive HCM (9/24,38%)].(4) Among all the patients,only one had vaginal delivery and others received cesarean section.Twenty-two patients had term pregnancies and 6 had preterm birth.The average gestational weeks of delivery in non-obstructive HCM and obstructive HCM were (36.5 ± 2.5) and (38.5 ± 0.4) weeks,respectively.The average birth weight of neonates were (2684 ± 563) and (3164 ± 321) g,and Apgar scores were 9.9 and 10 (10 minutes) respectively.Patients transferred to NICU after delivery were 8 and 0.There was 1 maternal death(with nonobstructive HCM whose ejection fraction was only 26%) and no perinatal death.Conclusions More attention should be paid to the clinical signs and abnormal ECG.HCM could be definitely diagnosed by timely echocardiography.Patients with hypertrophic cardiomyopathy were mainly non-obstructive HCM,with cardiac function grade Ⅰ and Ⅱ.Monitoring the change of ejection fraction during pregnancy would help.Perinatal outcomes were fine.
