The excimer laser diopter correction has proven to be efficient and safe. This paper presents the principle of excimer laser refractive surgery. Based on analyzing the mathematics model of the human eye cornea, the authors have proposed a new model which can be used to proceed the myopia, hyperopia, astigmatism diopter correction. Also studied were the excimer laser's ablation mechanism and the flying-spot scanning technology. The research results have been directly applied to Ophthalmic excimer laser system. The correction of diopter is well improved.

2.Study on differentiation of adipose-derived stem cells into neural cells induced by supernatant liquid from brain tissue in rats

Objective To observe the effect of supernatant liquid from brain tissue on the differentiation of adipose-derived stem cells (ADSCs) into neural cells in normal brain tissues, the homogenate of infarcted cerebral hemisphere and the opposite side in the rats. Methods The ADSCs were obtained from rat retroperitoneal adipose tissue. The normal brain tissues, the homogenate of the infarcted cerebral hemisphere and thc opposite side got from middle cerebral artery occlusion in rats were used to induce ADSCs. Immunocytochemistry or immunofluorescence were used to identify the cell types at the 3rd day. Positive expression rate was counted by fluorescence microscope. Results (1)The neuron-specific enolase (NSE) positive cells, microtubule-associated protein 2 (MAP-2) positive cells and glial fibrillary acidic protein (GFAP) positive cells were much more in the homogenate of the infracted cerebral hemisphere than in others (P<0.05). (2)The NSE positive cells, MAP-2 positive cells and GFAP positive cells were much more in the homogenate of the normal brain tissues and the opposite side than normal level ( P < 0. 05 ) . Conclusions The homogenate of the infracted cerebral hemisphere and the opposite side can induce adipose-derived stem cells into neural-like cells and express neural cells markers in rats.
3.Early predictive values of P-selectin, thrombus precursor protein and D-dimer for portal vein thrombosis after splenectomy in patients with portal hypertension

Objective To investigate the predictive values of P-selectin (P-sel),thrombus precursor protein (TpP) and D-dimer (D-D) in the early diagnosis of portal vein thrombosis (PVT) in patients submitted to splenectomy (or to devascularization).Methods The clinical data of 48 patients with portal hypertension (the PVT group,n=26; and the non-PVT group,n=22) who received operation in our hospital from 2009 to 2011 were retrospectively analyzed.Detecting the P-sel,TpP and D-D levels in the two groups of patients were done on preoperative day 1 and postoperative day 1,3,5,7,14.The SPSS software was used for statistic analysis.Results There were no significant differences on the preoperative day 1 levels of P-sel,TpP and D-D in the two groups (P>0.05).The postoperative day1 levels of the three indicators in the PVT group were significantly higher than the non-PVT group (P<0.05).Receiver operating characteristic (ROC) curves showed the area under curve (AUC) of P-sel was largest (0.893),followed by D-D,and TpP.The combined detection of the 3 indicators was highest,with the AUC up to 0.977.Conclusions Combined detection of P-sel,D-D and TpP levels were useful in the early diagnosis of PVT after splenectomy in patients with portal hypertension.
4.Studies on cognitive function in patients of first-episode schizophrenia with and without metabolic syndrome

Objective To discuss the difference of cognitive function in patients of first-episode schizophrenia with and without metabolic syndrome.Methods Event related potential detection and Wechsler Memory Scale (WMS) was determined in 57 patients of first-episode schizophrenia with metabolic syndrome (research group) and 57 patients of first-episode schizophrenia without metabolic syndrome (control group),and the psychiatric symptom in two groups was assessed by the application of the positive and negative symptoms scale (PANSS).Results The memory quotient score was (78.51 ± 12.54) scores in research group,and (92.01 ± 17.49) scores in control group,and there was significant difference between two groups (t =-2.130,P=0.047).The short-term memory,instantaneous memory impairment in research group were lower than those in control group(t =2.151-3.054,P=0.007-0.046).The latency of P300 was (325.10 ±22.46) ms in research group and (284.60 ±22.46) ms in control group,and there was significant difference (t =3.454,P =0.003).The amplitude was (6.56 ± 1.61) μ V in research group and (9.78 ±1.73) μV in control group,and there was significant difference (t =-4.307,P =0.000).Conclusion The patients of first-episode schizophrenia with metabolism syndrome has more severe cognitive impairment than those without metabolism syndrome.

Objective To study the efficacy and feasibility of the one-stage and two classification method to treat concomitant gallbladder stones and common bile duct stones.Methods 56 consecutive patients with concomitant gallbladder stones and common bile duct stones in Central Hospital of Huzhou were treated with the one-stage and two classification method from January 2013 to January 2017.According to the size and quantity,common bile duct stones were divided into difficult stones and non difficult stones.In plan A (n =35),endoscopic stone clearance and laparoscopic cholecystectomy were undertaken in a single operation to treat non difficult stones,In plan B (n =21),laparoscopic cholecystectomy and CBD exploration without T-tube insertion were undertaken in a single operation to treat difficult stones.The operation success rate,postoperative complications,hospitalization stay and follow-up outcomes were analyzed.Results Fifty-six patients were enrolled in the study.Fifty-one patients were treated by the one-stage and two classification method.Among them,thirty-two patients followed treatment plan A.Three patients were convened to plan B because of failure of bile duet intubation.Nineteen patients were treated by plan B.One patient was converted to laparotomy,and another underwent T-tube drainage.The incidence of complication was 7.1%.Post-ERCP pancreatitis happened in one patient.Post ERCP bleeding happened in another patient (1.6%).Bile leakage occurred in one patient,and incision infection occurred in one patient.There were no deaths.During a follow-up of 6-24 months,no stone recurrence and bile duct stenosis were observed.Conclusion The one-stage and two classification method was an effective and feasible alternative to manage concomitant gallbladder stones and common bile duct stones.
6.The mechanism and application of the correction for spherical myopia using small beam excimer laser.

In the laser ablation refractive surgeries, the corneal ablation model and its realization by laser are difficult to design. They greatly affect the results of those surgeries. This paper firstly presents a mathematical model for corneal ablation to correct the refractive error of spherical myopia, and then gives a technique of planning laser focus positions on cornea to realize the ablation model. Based on the principle of the correction for spherical myopia using small beam laser, our excimer laser corneal surgery system has been able to perform such refractive surgery. Now the corneal surgery system has been applied to clinical practice. Thirty-nine eyes with spherical myopia underwent LASIK using this kind of system. Their refractive states have been greatly improved. Preoperatively, the mean sphere was -5.57+/-2.95 D and the mean uncorrected visual acuity 0.12+/-0.07. One day after surgery, the mean sphere was -0.03+/-0.57 D and the mean uncorrected visual acuity 0.90+/-0.29. One month after surgery, the mean sphere was -0.68+/-0.98 D, and the mean uncorrected visual acuity 1.0+/-0.26.

7.The effect of MK gene overexpression in hepatic carcinoma cells on multidrug resistance

Objective To study the effect of enhanced MK gene expression in hepatic carcinoma cells.Methods The recombinant plasmid pIRES2-EGFP-MK was transfected into SMMC 7721 cells.The mRNA and protein expression levels of MK gene in these cells were determined by real-time PCR,Western blotting and flow cytometry.The intracellular DNR accumulation of these cells was measured by flow cytometry.To investigate the effect of MK gene mediated multidrug resistance,MTT assay was employed to determine the cellular sensitivity of different chemotherapeutic drugs in MK-overexpressed SMMC 7721 cells.Results The mRNA and protein expression levels of MK gene significantly increased after the recombinant plasmid pIRES2-EGFP-MK transfected into SMMC 7721 cells,suggesting that the recombinant plasmid pIRES2-EGFP-MK can enhance the transcription of MK effectively.The DNR accumulation of MK transfected cells decreased significantly (4.06 ± 0.88,P < 0.05),and IC50 of MK transfected cells to ADM/5-FU increased significantly (15 ± 3,27 ± 4,P < 0.05).Conclusions After the recombinant plasmid pIRES2-EGFP-MK transfected into hepatic carcinoma cells,expression of midkine increased,enhancing the resistance of hepatic carcinoma cells to chemotherapeutic drugs.
8.The effects of observing good swallowing on the swallowing ability of stroke survivors

Objective:To observe the effect of observing good swallowing on the swallowing action of stroke survivors with dysphagia.Methods:Eighteen stroke survivors with dysphagia were randomly divided into a treatment group ( n=9) and a control group ( n=9). In addition to routine swallowing rehabilitation therapy, the treatment group was asked to simulate swallowing after watching a video of normal people′s swallowing action. They did so 5 times a week for 10 minutes, while the control group just watched landscape videos at the same time. The treatment lasted 8 weeks. Before and after the treatment, both groups were assessed using the eating assessment tool (EAT-10), the functional oral intake scale (FOIS) and the penetration and aspiration scale (PAS). Functional magnetic resonance imaging (fMRI) was also used to observe their swallowing action. Results:There was no significant difference between the two groups in any of the measurements before the treatment. After the 8 weeks of treatment the average EAT-10, FOIS and PAS scores of the treatment group were all significantly better than before the treatment and better than the control group′s averages at the time. fMRI showed significantly more areas activated in the precuneus, parietal lobe, posterior central gyrus, BA7, BA5, frontal lobe and paracentral lobule in the treatment group compared with before the intervention and also more than in the control group.Conclusions:Observing proper swallowing action can improve dysphagia and activation of the swallowing-related brain areas of stroke survivors.
9.Risk factors of deep vein thrombosis among patients in Rehabilitation Department

Objective:To explore the risk factors of deep vein thrombosis (DVT) in hospitalized rehabilitation patients.Method:Clinical data of 1 130 patients, 737 males and 393 females with the mean age of 63.0 (54.0, 73.0) years, admitted in the Department of Rehabilitation Medicine of the Second Hospital of Jiaxing from August 2021 to September 2022 were retrospectively analyzed. Patient underwent vascular color ultrasound examination, DVT was detected in 194 cases (DVT group) and not detected in 936 cases (control group). The general clinical data, venous thromboembolism (VTE) risk stratification and traditional VTE risk factors of patients were documented. The risk factors of DVT formation were analyzed with multivariate logistic regression.Results:Compared with the control group, the DVT group had a higher proportion of patients with older age, longer length of hospital stay, and positive D-dimer (all P<0.001). The proportion of patients with high risk of VTE in the DVT group was 49.5% (96/194), which was higher than that in the control group (27.0% (253/936), P<0.01). The proportion of patients with age≥70 years, previous venous thromboembolism, and history of recent (≤1 month) trauma or surgery in the DVT group was significantly higher than that in the control group (all P<0.001). The proportion of patients with primary diagnoses of quadriplegia, hemiplegia or other motor disorders in the DVT group was significantly higher than that in the control group ( P<0.01). Multivariate logistic regression analysis showed that old age ( OR=1.014, 95% CI: 1.012-1.016), length of hospital stay ( OR=1.001, 95% CI: 1.001-1.002), positive D-dimer ( OR=2.508, 95% CI: 2.368-2.655), high risk of VTE ( OR=1.178, 95% CI: 1.10-1.250), quadriplegia ( OR=2.776, 95% CI: 2.552-3.021), hemiplegia ( OR=3.232, 95% CI: 2.996-3.488), motor disorders ( OR=2.308, 95% CI: 2.110-2.525), paraplegia ( OR=1.878, 95% CI: 1.622-2.175), previous venous thromboembolism ( OR=1.385, 95% CI: 1.314-1.460), history of recent (≤1 month) trauma or surgery ( OR=1.987, 95% CI: 1.886-2.093) (all P<0.001) were independent risk factors for DVT in rehabilitation inpatients. Conclusion:Age, length of hospital stay, primary diagnosis of quadriplegia, hemiplegia, paraplegia or other motor disorders, positive D-dimer, high risk of VTE, previous venous thromboembolism, and history of recent trauma or surgery are independent risk factors for DVT in rehabilitation specialty inpatients.
10.The effects of transcranial magnetic stimulation on post-stroke dysphagia

Objective:To observe the effect of applying repetitive transcranial magnetic stimulation (rTMS) on swallowing function and auditory evoked potentials in the brain-stems of stroke survivors with dysphagia.Methods:Sixty stroke survivors with dysphagia were randomly assigned to an observation group and a control group, each of 30. Both groups were given 30 minutes of routine swallowing training 6 times a week for 5 weeks, while the observation group was additionally provided with two-second bursts of rTMS on the left and right brain hemispheres alternately for 20 minutes daily. The frequency was 3.0Hz and the strength was 80% of the resting motor threshold (RMT), with an interval of 10 seconds between bursts. Before and after the treatment, both groups′ swallowing was assessed using the functional communication measure (FCM) and the modified Mann swallowing capacity evaluation scale (MMASA). Brainstem auditory evoked potentials (BAEP) were recorded to detect any changes in the peak latency (PL) and inter-peak latency (IPL). The clinical efficacy was also observed.Results:After the treatment, the average FCM results of both groups had improved significantly, but the observation group had improved significantly more. The average MMASA total scores and the average scores on all the sub-items had improved significantly more in the observation group except for the expressive speech disorder and anarthria scores. After the treatment, improvement was observed in respiration, anarthria, range of motion of the tongue muscles, strength of the tongue muscles, gag reflex and soft palate movement compared with before the intervention. Peak and inter-peak latencies had also improved significantly more in the observation than in the control group, on average.Conclusions:rTMS combined with conventional swallowing training can significantly improve the swallowing of dysphagic stroke survivors, and shorten the PL of the BAEP so as to better regulate deglutition.
