Microsportcm gypseum is a geophilic fungi abundant in soil throughout the world. The infections may occur either from animal to man(zoophilic) or from soil to man (geophilic). Patierit of 13-year-old Korean male sought medical attention with skin lesion on scalp which showed scaly patch with broken hairs and a round pilar cyst like nodule. Identification of Microsporum gypseum was made by culture on Sabouraud agar media and microscopic exarnination.

4.Crown angulations of posterior teeth of normal occlusion measured from marginal ridge plane.

In the previous studies about prescription of preadjusted appliance, occlusal plane was used as a reference plane for crwon angulation (tip) measurement But this reference plane is not parallel to the he connecting the facial axis points at which the centers of brackets are positioned (Andrews' plane), due to the curve of Spee. Therefore, we developed a new reference plane unaffected by the curve of Spee and more parallel to the Andrews' plane. It is an imaginary line connecting mesial and distal marginal ridges of each posterior tooth, and we named it 'marginal ridge plane'. In this study, crown angulations of posterior teeth of 29 normal occlusion samples were measured and measurements from both reference planes were compared. Crown angulation measurements measured from occlusal plane were different from crown angulation measurements from marginal ridge plane in the upper and lower 2nd molars (p<0.01), and 1st premolars (p<0.05). These results were analyzed as the crown angulation measurements from occlusal plane were affected by the curve of Spee. Crown angulations should be vaned according to the amount of curve of Spee to maintain- the continuity of marginal ridges. To solve this problem, determining bracket angulation as the bracket slot is parallel to the marginal ridge plane of each posterior teeth is recommended.

5.A Case of Recurrent External Ophthalmomyiasis Caused by Lucilia sericata Meigen in a Healthy Patient.

PURPOSE: To report a case of recurrent external ophthalmomyiasis caused by Lucilia sericata Meigen in a healthy patient. CASE SUMMARY: A 72-year-old male, who was diagnosed 7 months earlier with ophthalmomyiasis and obtained permanent relief by surgical treatment, presented with severe tearing, redness, foreign body sensation, pain in both eyes and aural discomfort in the left ear. On presentation, visual acuity in both eyes was hand movement. On slit lamp examination, total corneal epithelial defect with stromal infiltrations was observed. However, there were no motile organisms in his eyes due to previous removal of maggots at a local clinic. On physical examination, eschar change was observed at the inferomedial area of the left lower eyelid. On otorhinolaryngological examination, several motile organisms with tympanic membrane perforation were observed in the left external acoustic meatus. The patient was treated with topical antibiotic eye drops hourly with systemic antibiotics, and 6 larvae were removed from the external acoustic meatus using otoscopy. Microscopic findings of the removed larvae revealed L. sericata in their third stage of development. After 1 month, corneal epithelial defect and stromal infiltration improved. However, visual acuity in both eyes was 0.1 due to remaining stromal opacities. CONCLUSIONS: In a healthy patient without any predisposing factors, recurrent ophthalmomyiasis accompanied with aural myiasis can develop under poor hygiene conditions.

6.Clinical Usefulness of Systolic Time Intervals and QT-QS2 Value as Complication Predictors in Myocardial Infarction.

Over the physiologic range of heart rate, the duration of electrical systole(QT) is shorter than that of electromechanical systole(QS2), which reflects direct physiologic link between electrical repolarization and mechanical events of the cardiac cycle. The present study designed to test usefulness of systolic time intervals and QT-QS2(QT minus QS2) value as complication predictors, which may predict prognostic status of myocardial infarction. The studied subjects were consisted of 33 healthy adults as control(22 men, 11 women) and 35 patients with myocardial infarction. And studied patients were divided into 3 groups; group I of 14 patients with or without complication in acute stage, group II of 11 uncomplicated patients under recovery of chronic status, group III of 10 complicated patients under recovery or chronic status. The observation periods were 3 to 13 days in group I, 5 to 96 weeks in group II and III after their onset. The results were follows; 1) The normal ranges were defined as mean+/-2 standard deviations of the measured values of control, that of QTc were shorter than 431ms of man and 458ms of woman, that of QS2 I were longer than 503ms of man and 514ms of woman, that of PEP/LVET ratio was less than 0.40, that of QT-QS2 value was less than 0 ms respectively. 2) QTc of all patient groups were significantly prolonged compared with that of control, and that of group III was significantly longer than that of group II(481+/-25 vs 432+/-32, p<0.005). 3) The changes of QS2 I were not significant in all patient groups compared with QS2 I of control. 4) QT-QS2 value showed QT>QS2 in group I and III. and QT

BACKGROUND: In recent times, it has been gradually emphasized that an aspect of diseases impacting on the quality of an individuals life goes beyond physical impairments with the development of economic and medical technology. Skin diseases especially may be associated with a variety of negative psychologic effects, such as low self-esteem, feelings of inferiority because skin lesions are exposed to others and cause cosmetic problems. Many researchers have been already established various dermatology-specific questionnaires and disease-specific questionnaires in developed countries, but these are rare in Korea. OBJECTIVE: We tried to survey the social, psychological and physical aspects of patients with vitiligo and psoriasis to evaluate the impact of skin diseases on the quality of life in Korea. METHODS: We inquired after 64 patients by using a dermatology-specific questionnaires Skindex recently developed by Chren et al. It has 6I-items and eight scales including cognitive, social, depression, fear, embarrassment, anger, physieal discomfort, and physical limitatians.

9.Comparison between the Results of Minimally Invasive Total Knee Arthroplasty Performed with Mini-Midvastus Technique and Quadriceps-Sparing Technique.

PURPOSE: To compare the short-term clinical results of mini-midvatus minimally invasive total knee arthroplasty(TKA) with quadriceps-sparing minimally invasive TKA. MATERIALS AND METHODS: Between August 2005 and February 2006, 23 bilateral (46 knees) minimally invasive total knee arthroplasties were performed simultaneously using quadriceps-sparing surgical technique at one side and mini- midvastus minimally invasive technique at the contralateral side. The same surgeon performed all the operations with the same type of prosthesis(Nexgen LPS-flex Total Knee System) using MIS quadriceps-sparing instrument. The pos- toperative clinical and radiological results were analyzed in each group. RESULTS: There were no significant differences between the two groups in the operating time, total blood loss, length of skin incision, radiological results and postoperative HSS scores at 8 weeks and 1 year follow-up(p>0.05). Also, there were no differences in the postoperative range of motion and pain score assessed by visual analog scale at 1 day, 3 days, 7 days, 14 days, 8 weeks and 1 year follow-up(p>0.05). Conclusions: Compared to quadriceps-sparing TKA, mini-midvastus minimally invasive TKA revealed no significant differences in the short-term clinical and radiological results. Therefore, it can be considered as an effective, alternative minimally invasive technique.
