1.Significance of MRI Cord Signal Patterns in Acute Spinal Trauma.

In the diagnosis of acute spinal trauma, meticulous physical examination and history taking is the most important tool and auxilliary support support can be given by plain X-ray, CT or myelography, etc. But these cannot show the state of cord injury directly. On the contrary, MRI shows the cord directly as well as the soft and bony tissues. We analysed retrospectively 39 patients who suffered from acute spinal trauma at the level of cervical and thoracic spine and had their spine MRI taken. We verified the meaningful correlation between the signal change in the spinal cord on MRI and the degree of neurologic deficit and prognosis, using Frankel classification and trauma motor index at the time of admission and the final follow-up.

5.Leiomyoma of the Lung: A case report.

We present a 37-year-old male who was found to had mas within the bronchus. This patients was admitted for the evaluation of cough. Chest CT scan showed endobronchial mass in the bifurcation of LUL and LLL bronchus. The left lower lobe was atelectatic. Lobectomy of the left lower lobe was done. On opening of the bronchus, there was a 2x1x1 cm sized endobronchial mass. Histologically, the mass was smooth muscle tumor composed of densely packed eosinophilic spindle cells in interlacing bundles with pale elongated nuclei covered by bronchial epithelium. On immunohistochemical staining, the tumor cells were positive for desmin. Ultrastructurally, the tumor cells exhibited numberous cytoplasmic microfilaments with focal densities, pinocytotic vesicles, and a thick basal lamina.

6.Metastatic Adenocarcinoma of Colon in Meningioma: A case report.

Cases of metastases from extracranial tumor to intracranial tumor are very rare. The world wide review of the literatures until 1992 revealed 44 cases of primary intracranial tumors containing metastatic tumors which are unrelated extracranial primary malignant tumors; the intracranial recipient tumor is a meningioma in 35 cases among them. Carcinomas of the lung and the breast are the most common extracranial donor tumors. Metastases from colon cancer to meningioma are extremely rare. A 74 year-old-female presented with headache for 2 weeks. CT revealed a round mass with high signal intensity, measuring 4 cm in diameter, which is located in the left parietal lobe. The patient had colon cancer 2 years ago and lymphoma I year ago. On operation, the tumor is relatively well delineated and attached to the meninx. Microscopically, the tumor is composed of fascicles of long slender, fibroblast-like spindle cells with indistinct cytoplasmic border, variable amount of collagen deposit and many psammoma bodies. A few scattered glands are present in periphery of the meningioma. The tumor glands are composed of columnar cells with basally located hyperchromatic nuclei and similiar to the glands of the adenocarcinoma of the colon.

7.Experience of the Tarsometatarsal Fracture-Dislocation

Injury of the tarsometatarsal joints is known to be very uncommon. Injury was most frequently seen as the result of high energy trauma and appear to be occuring more frequently. Early recognition was imperative, since significant long-term disability can result from inadequate treatment. We have reviewed and clinically anafysed 14 cases of tarsometatarsal Fracture-Dislocation treated at our clinic during the period frrqn March 1980 to March 1985. The following result were obtained. 1. Majority of cause of injury was traffic accidents in 10 cases(71%). 2. According to the Wilsons classification, First stage of supination was most common. 3. Associated injuried was 92%. 4. There was no significant difference between operative treatment and conservative treatment.

9.The Pathologic Anatomy of Degenerative Spondylolisthesis.

Degenerative spondylolisthesis, which occurs frequently in middle-aged women, is a disease that causes canal stenosis of the lumbar spine and low back pain. Although the clinical features have been described characteristically, its pathogenic mechanism and therapeutic principles are still to be clarified. Some investigators have claimed that the disease originates from morphologic abnormalities of laminae and facet joints or that it is primarily due to degeneration of the intervertebral disks and degenerative changes of the facet joints. We selected two groups of patients retrospectively. The one was a group of patient who had normal findings on their magnetic resonance imaging(Group I ) and the other was a group of patients who showed degenerative spondylolisthesis at L4-5 and had been managed surgically(Group II ). We measured the total lordosis from Ll to S 1 and segmental lordosis of L3-4, L4-5 and LS-S 1 in both groups by Cobbs method. In group 3, we measured the degree of slippage of the L4 vertebral body and investigated the associated congenital anomalies of the lumbosacral vertebra on simple anteroposterior and lateral radiographs in both groups. And we measured the facet joint angles of the L3-4, L4-5 and L5-Sl at both side on MRI. Then we analysed the results statistically and got the following conclusions. 1. In Group I, total lordosis decreased significantly. The segmental lordosis of L4-5 and LS-S1 decreased, too, but the segmental lordosis of L3-4 increased. 2. The facet joint angles of L4-5 in Group I increased significantly in both sides. 3. The intercrestal line was lower in Group I than in Group II. 4. The degree of slip was not related with the facet joint angles of L4-5.

10.Clinical Effect of Spectinomycin on Acute Gonorrheal Urethritis.

Forty five gonococcal patients were treated with spectinomycin hydrochloride at the Department of Urology, St. Mary's Hospital from August 1. 1972 to December 32. 1972. All patients were male and confirmed as acute gonorrheal urethritis by gram stained smear or culture. The cure ability in this series was very satisfactory The results were as follows; 1. Gram negative ;intracellular diplococci was controlled in 29 patients (96.7%) among 3O patients with pure gonococcal infection with a single injection of spectinomycin hydrochloride. 2. Gram negative intracellular diplococci was controlled in 14 patients (93.3%) among 15 patient with mixed infection with a single injection of spectinomycin hydrochloride. 3. Only two in all patients were controlled in gram negative intracellular diplococci with repeat injection. 4. 4 patients (13.3%) of the pure gonoccoccal patients with a single injection were converted to non-specific urethritis, which were treated with combination of other antibiotics. 5. 5 patients (33.3%) of the mixed infected patients with a single injection were converted to nonspecific urethritis, which were treated with combination of other antibiotics. 6. No side effect was noted in all patients.
