3.C.T.Findings and Prognosis of 25 Cerebellar Infarction Cases.

Cerebellar infarction (CbI) is rare but because of its location it can give vital influence on the neighbouring structures. Therefore, it is important to make correct diagnosis and know general clinical pictures including prognosis. We studied 25 cases of CbI about their C.T. findings and clinical pictures, and the following results were obtained. 1. Location of the CbI was in inferior cerebellum 20 cases, superior 4 cases. Prognosis was good in 17 inferior and 2 superior cases. 2. Size of the CbI was variable, but fatal cases showed more than 5-6cm in diameter. 3. Deformity of the 4th. Ventricle and slight hydrocephalus were seen in 12 among 20 acute cases. 2 cases expired. 4. Among the total 25 cases 20 cases showed clinically complete cure or improvement, 2 cases worse, 1 cases undetermined, and 2-cases expired.

4.The Effect of pH on Sodium Lauryl Sulfate Irritancy Potential.

Five volunteers received patch tests with 5% sodium lauryl sulfate (SIS) in solutions of differing pH. The irritant effect was monitored by visual scoring as well as by a laser Doppler vlelocimeter, evaporimeter, cutometer, and colorimeter. The non-invasive methods used in this study with the exception of the cutometer were effective in the evaluation of skin irritation. No significant differences in the skin responses to SIS in different pH solutions were found either clinically or by the non-invasive methods used for quantification. It was concluded that the pH of SIS is not a major factor in the degree of skin irritation produced by SIS.

5.A Comparison Study of the Ketamine and the Thiopental Sodium as an Induction Agent in the Cesarian Section.

This study was undertaken to estimate the effects of the induction agents on the bioparameters such as changes in blood pressure, pulse rate, Apgar score, patients movement, fetal arterial and venous blood gas analysis, memory and emergence reactions. 116 parturients undergoing cesarian section were divided into two groups: ketamine group and thiopental group, and were given 1.2 mg/kg ketamine in ketamine group and 4 mg/kg thiopental sodium in thiopental group as an induction agent respectively. The results were as follows; I) Blood pressure increased in both groups, but ketamine group less increased than thiopental group statistically. Pulse rate did not increased in skin incision in ketamine group statistically. 2) The patient's movement were 5 case(9%) in ketamine group and 17 cases(29%) in thiopental group. 3) There was not significant difference in fetal arterial and venous blood gas analysis. 4) In Apgar score, ketamine group is better than thiopental group. 5) There was no psychologic side reactions in both groups. 6) Postoperative recalling of intraoperative awareness occured in seven patients(12%) only in the thiopental group.

6.One Case of Duplicated Ureter Associated with Ectopic Ureteral Orifice.

In a case, while normal self-voiding, urinary leakage has been complained since infant without other urinary symptoms and she was treated with right ureteronecystostomy in Paquin's method.

7.Concomitant Boost Radiotherapy for Stage 3 Non - Small Cell Lung Cancer.

PURPOSE: This study was undertaken to evaluate the treatment outcome and side effects of accelerated radiotherapy (RT) using concomitant boost for stage III non-small cell lung cancer (NSCLC). METHODS: Between April 1991 and December 1994, 102 patients with stage III NSCLC who had the favorable prognostic factors by CALGB criteria, were treated with concomitant boost radiotherapy. Patients were treated with standard large fields to 54 Gy in 6 weeks. The boost treatment was administered concomitantly during the last 2 weeks with a dose of 13 Gy in 10 fractions. The interfraction interval was at least 6 hours. The total tumor dose was 66-70 Gy, given over 6 weeks. RESULTS: With 30 months median follow-up period for survivors, median survival was 15 months with 2 and 3-year overall survival rates of 34% and 19%, respectively. Thirty patients (29%) who had achieved complete remission after RT showed significantly better 2-year survival rates than those without complete remission (58% vs 22%, p 0.001). Local failure and distant metastases as the first or only failure occurred in 40 (44%) and 13 (14%), respectively, and ultimate local and distant failure rates were 45% and 29%, respectively. Although Grade IV esophageal complication of T-E fistula was observed in one patient, most patients with pulmonary complication showed mild, transient radiation pneumonitis. CONCLUSION: This result suggests that the treatrnent of stage III NSCLC with concomitant boost RT may improve survival rates without enhanced radiation induced toxicity compared with conventional RT. Further investigation of dose escalation by conformal radiotherapy of combining chemotherapy and accelerated RT is warranted.

8.Angiographic Evaluation of Occlusive Coronary Arterial Disease

Angiographic findings were analysed in 22 cases of coronary occlusive disease which were selected from 50 cases of coronary arteriographies done in Department of Radiology, Seoul National Un iversity Hospital from September, 1981 to August, 1984. There were 16 cases of single vessel disease, in which 12 were LAD involvement and 4 were RCA involvement, 2 cases of two vessel and 4 cases of three vessel disease. Site, degree and extent of stenotic involvement in those occlusive coronary diseases were anal ysed with review of literature for selective coronaryarteriography. Since coronary arteriography is the final and accurate examination for the evaluation of occlusive coronary arterial disease, it is expected to be widely used for the diagnosis of occlusive coronary arterial disease which is rapidly increasing recently in this country.

9.The Clinical Observation of Miliary Tuberculosis in Children.

We made a clinical observation of 63 cases with miliary tuberculosis at the pediatric department of Hanyang University Hospital in Seoul, Korea, during the period from January, 1973 to September, 1979. The result were as folowing: 1. The incidence of miliary tubeculosis in children was 0.57%(63 cases/11,040) of total admission cases. The incidence was increased for the last 2 years, that is 0.54% in 1978 and 0.96% in 1979 respectively. 2. The highest incidence by age was noticed as 79.4% in the group under the age of 3 years with the 1 case of 2 months old. 3. The seasonal prevalence were winter and spring showing incidence of 63% 4. The source of infection was found in 49.2% of all cases, and majority (58.1%) of the infection was caused by their parents. 5. Only 9.5% of cases had received BCG innoculation. 6. The chief compalints on admission were revealed in the order of fever (71.4%), coughing (66.7%), vomiting (49.2%), irritability (25.4%), etc. 7. The principal clinical features on admission were marked emaciation (46.0%), abnormal neurologic sign (46.0%), hepatosplenobegaly (34.9%) in order. 8. The tuberculin skin test with 5TU PPD was noticed positive reaction in 54%. 9. The causative organism was isolated in 42.3% of all 26 observed cases. 10. Hematologic findings revealed mild degree of anemia (63.9%), leukocytosis (75.5%), increased ESR (63.4%). 11. Miliary density on X-ray were disappeared within 4 months following treatment in 73.9% of all 23 observed cases. 12. The complications or combined diseases were tuberculous meningitis (41 cases), cervical lymphadenopathy (10 cases) and tuberculous peritonitis (4 cases), etc. 13. The mortality rate was 14.3% (9 cases/63).

10.Two Cases of Primary Biliary Cirrhosis.

Primary biliary cirrhosis(PBC) is a chronic cholestatic liver disease of unknown origin. The small and medium sized intrahepatic bile ducts are destroyed by an inflammatory process, which, it has been suggested, is of the autoimmune type. It is strongly associated with the presence of antimitochondrial antibodies, predominantly IgM and IgG. The liver changes are classified into four stages, of which stage IV represents the development of cirrhosis, which required orthotropic liver transplantation in the longrun. The prevalence rates was reported 128 per millon in Sweden , but the disease is relatively rare in Oriental area. In medical treatment, long-term administration of ursodeoxycholic acid improves both clinical and biochemical signs, slows the progression of the disease and reduces the complication requiring liver transplantation. We report two cases of PBC, one with histologically proven cirrhosis, and the other with bile duct destruction consistent with stage III and hypothyroidism.
