1.Generalized Pustular Psoriasis: Report of a Case.

Generalized pustular psorissis which is originary described by von Zumbusch in 1910, is the most intense form of the acute exanthematic variant among psoriasis and it is to be occured as a rare disease. We presented a case of generalized pustular psoriasis of a 16 year-old girl, who suddenly developed wide spread pustules during the acute exacerbating course of the psoriasis which had started from the primary plaque on her left anterior chest one month ago. She was free from subjective symptom. As the objective sign, the leukocytosis was only one of the significant laboratory findings. All skin lesions were almost eleared by the oral administration of methotrexate in 3 months.

3.A Case Report of Primary Aldosteronism with Sustained Hypertension After Adrenalectomy.

We experienced a case of primary aldosteronism due to adrenal adenoma. The patient was 45-year old female, whose main complaints were weakness, headache, and fatigue. By use of abdominal CT scan, ultrasonogram, adrenal angiogram and by result of clinical laboratory findings, we conculuded the diagnosis of adrenal adenoma, and confirmed it by surgical and pathologucal findigs. The clinical sympyoms disappeared after left adrenalectomy, and laboratory findings of renin, aldosterone, electrolutes were normalized but hypertension persisted, so she has been managed by antihypertensives.

4.ERCP findings of extrahepatic bile duct carcinoma

In the diagnosis of bile duct carcinoma, oral or intravenous cholangiography is of no air in the majority ofpatients with bile duct carcinoma who are jaundiced. Recently ultrasonography and CT are widely used for evalutionof biliary disease, but direct visualizing methods of the biliary tract by ERCP and PTC gives more detailed information and exact localization of the lesion. ERCP is less invasive and dangerous and has some more advantages than PTC. We analyzed 33 cases of confirmed extrahepatic bile duct caracinoma who were performed ERCP. The resultswere as follows; 1. The 7th decade was the predilection age, and the radio of male to female was 3.:1. 2. Thelocations of extrahepatic bile duct carcinomas were common bile duct in 45.5%, common hepatic duct in 27.3%,junction of cystic duct and widely extended in 12.1% respectively and junction of hepatic duct in 3.05 in order offrequency. 3. ERCP finding of extrahepatic bile duct carcinomas revealed complete obstruction of bile duct in mostcases, and irregular margined protuberant type was more common than smooth margined constricted type atobstruction site. 4. ERCP finding according to the location of lesion showed that protuberant type was relativelyfrequent in common bile duct and constircted type in common hepatic duct respectively.

5.Min's Glasses are New Effective Method as an Alternative and also an Initial Treatment in Amblyopic Children.

To search for the more effective amblyopia treatment, we prospectively compared the results of initial group and alternative group with use of Min's glasses. Initial and alternative groups consisted of 18 and 64 patients. 15 patients (83.3%) of initial group were compliant with the treatment while 50 patients(78.1%) of alternative group were. No significant differences existed between them based on age, sex, causes of amblyopia and initial visual acuity (P>0.05). Average visual acuity improvements were 2.13 +/- 1.81 lines in initial group, compared to 2.51 +/- 1.50 lines in alternative group and no significant differences existed between them according to age, causes of amblyopia and initial visual acuity (P>0.05), However, in cases of female, there was significant visual acuity improvement in alternative group in comparision with initial group (P=0.034). In conclusion, Min's glasses were effective not only in initial treatment group but also in alternative treatment group, and in female patients who previously failed with conventional occlusion therapy the effect was more pronounced than male patients.

6.Effects of Na-Ca Exchange Mechanism on the Action Potential and Membrane Currents in the Single Cells of the Guinea-Pig and the Rabbit Heart.

In single atrial and ventricular cells isolated from the guinea-pig and the rabbit heart, action potentials and membrane currents were recorded by using the whole cell voltage clamp technique. In rabbit atrial cells the repolarization showed two distinctive phases, referred as the early and late phases(early and late plateau phase), but in guinea-pig atrial cells there was a maintained plateau and less distinctive two phases of repolartization. Increasing intracellular sodium or reducing external sodium by replacement with lithium suppressed the late phase of the action potential in rabbit atrial cells and shortened the plateau of action potential in rabbit ventricle and guinea-pig atrial cells. Reducing external sodium decreased Ca-current and late inward current in voltage clamp. Ouabain in the concentration of 10(-5)M shortened the duration of action potential and shifted the holding current level to outward direction, decreased Ca-current and moved late inward current to outward direction. Ryanodine 10(-6)M which is known to be an inhibitor of Ca-release in the intracellular store, suppressed the late phase of action potential in rabbit atrial cells and shortened the plateau of action potential in rabbit ventricular cells. Ryanodine also decreased Ca-current and shifted late inward current to outward direction. It is concluded that an inward current activated by intracellular calcium contributes to the late Phase of the action potential in rabbit atrial cells and to the late plateau in rabbit ventricular cells and in guinea-pig atrial cells. It may be carried by the Na-Ca exchange precess and/or by calcium-activated non-specific channels but preferably Na-Ca exchange machanism.

9.Retroperitoneal cystic lymphangioma in an aged man: report of a case and review of the literature.

Retroperitoneal cystic lymphangioma is a rare benign tumor which had previously been difficult to diagnose preoperatively by conventional radiographic technique(1). Recent reports describe the computed tomographic and ultrasonographic findings in cystic lymphangioma(1,2). We report a case of retroperitoneal cystic lymphangioma, accurately diagnosed by lymphography, with computed tomographic findings and percutaneous drainage findings. To the best of out knowledge, this is the most aged case of retroperitoneal cystic lymphangioma of preoperatively diagnosed.
