3.Detection of Glaucomatous Visual Field Defect by Screening Program of Humphrey Field Analyzer.

We have experienced automated and computerized perimetry using a Humphrey Field Analyzer in our Glaucoma Services. To evaluate its clinical efficacy, we report the results of the Armaly central field screening test in comparison with those of central 30-2 threshold test of the Humphrey Field Analyzer. One hundred fortythree eyes of 83 cases of normotensives, ocular hypertensives and glaucoma patients with open-angle or narrow-angle were enrolled in this study. Fiftyfour out of 61 patients (88.5%) and 73 out of 81 eyes (90.1 %) with visual field defect were detected by the screening test of the Humphrey Field Analyzer. Eight out of 81 eyes (9.9%) with visual field defect detected by the screening test were confirmed as false positive. All points of false positive were solitary, which tended to occur more frequently on superior vusual field. False negative of 8 eyes (12.9%) detected by the screening test were confirmed by the threshold test. The point of false negative was more frequent on superior field and showed to locate more peripherally from the fixation point. There was no significant difference in either central sensitivity or age of the false ring.

4.A Case Report of Primary Aldosteronism with Sustained Hypertension After Adrenalectomy.

We experienced a case of primary aldosteronism due to adrenal adenoma. The patient was 45-year old female, whose main complaints were weakness, headache, and fatigue. By use of abdominal CT scan, ultrasonogram, adrenal angiogram and by result of clinical laboratory findings, we conculuded the diagnosis of adrenal adenoma, and confirmed it by surgical and pathologucal findigs. The clinical sympyoms disappeared after left adrenalectomy, and laboratory findings of renin, aldosterone, electrolutes were normalized but hypertension persisted, so she has been managed by antihypertensives.

5.Exfoliation Syndrome: Clinical analysis of 6 cases.

The first comprehensive clinical description of exfoliation syndrome was published by Vogt. The appearance is described as a deposition of white granular fluffy material on the anterior lens surface, pupil margin, zonule, ciliary body and anterior chamber angle, and floating free in the aqueous. At past, this syndrome was thought to be rare outside of the Scandinavian countries, but the relatively high prevalence has been noted in many other countries outside of Scandinavia. The clinical significance of this syndrome is high frequency of glaucoma. Six cases in this report illustrate some of typical clinical features of exfoliation syndrome. The purpose of this study is to concentrate our attention upon the presence of this syndrome in Korea.

6.Clinical experience of exfoliation syndrome.

Six patients with exfoliation syndrome were seen in our glaucoma clinic. Four patients presented unilateral ocular involvement and the other 2 patients were bilaterally involved. Eight eyes had exfoliation material on the anterior surface of the lens and pupillary border, and 7 eyes exhibited it on the anterior chamber angle. Sampaolesi`s line was detected in 4 eyes. Five patients (6 eyes) of 6 patients (8 eyes) with exfoliation exhbited an elevated intraocular pressure (IOP) exceeding 21 mmHg. Among them, 3 patients (3 eyes) had an extremely high maximum IOP over 47 mmHg, and 2 patients (3 eyes) had a slightly elevated IOP less than 26 mmHg during the follow-up period. Argon laser trabeculoplasty (ALT) was performed twice on each unilateral eye of the 2 patients on whom the inital trabeculectomy failed. However, longterm IOP control after laser treatment was not obtained in either case, which might be due to the extremely high prelaser lOP level, in exoess of 40 mmHg.

7.A Clinical Survey on 65 Cases of the Tumors in the Eye and Adnexa.

1. Sixty five cases of the tumors in the eye and adnexa(one to 78 old age) were observed at the National Medical Center in Korea, between 1959 and 1961. Those are analyzed and studied from the clinical view point. 2. Fifty-two tumor cases were primary and thirteen cases were secondary. The most frequent malignant tumor was retinoblastoma(15 cases) in childhood group and cancers(11 cases) in older age group. The malignant melanoma case(1 case) was definitely few in number. 3. The most frequent origin was the eye ball(19 cases) and next the lid(12 cases). The secondary tumors camed mostly from paranasal sinuses(12 cases). 4. Among the clinical diagnostic methods, it is revealed that the X-ray tomography with air insufflation into the orbit was most reliable. 5. The visual disturbance, exophthalmos, displacement of the eye, and impaired ocular movement were the most frequently seen as the clinical symptoms and of value for diagnosis.

8.Effects of Ischemic Preconditioning on Hypoxia-Reoxygenation Injury of Cardiac Myocyte in Culture.

Short period of ischemia and reperfusion protect heart against subsequent prolonged ischemia-reperfusion injury. This phenomenon was first described by Murry et al in 1986, who demonstrated that four 5-minute coronary artery occlusions followed by equal period of reflow at each time before a subsequent prolonged occlusion resulted in a reduction of infarct size in dog. Although the precise mechanism of preconditioning remains unknown, this phenome-non is present among different species of mammals, including dogs, rats, pigs, rabbits, and human. The objects of present study was to investigate effect of ischemic preconditioning on cell viability, structural changes and apoptosis during 60 min hypoxia and 60 min reoxygenation of the cell. In present study we investigated through cell culture system using myocyte of three days old neonatal rat cultured for three days. During hypoxia and reoxygenation, differences between preconditioned and nonpreconditioned of beating counts, morphological and structural changes are investigated through inverted phase contrast microscope and transmis-sion electron microscope. To detection of apoptotic cell, TUNEL (TdT-mediated dUTP-biotin nick end labeling) stain was accomplished, and through which we invesigate the effects of preconditioning on apoptosis. Viabiliy of each cell and it's mitochondria were measured quantitatively by MTT assay. After 60 min of hypoxia and 60 min of reoxygenation, beating rate decreased remarkably. But at the time of 60 min of reoxygenation, there was marked increase in beating count in pre-conditioned cell. Swollen mitochondria with amorphous granules in inner membrane, destroyed mitochondrial cristae, indented nuclear envelope, chromatin condensation, contracture of myofibril, fragmentation of myofilaments, cytoplasmic shrinkage were observed in both preconditioned cell and nonpreconditioned cell. But it is much less in pre-conditioned cell than in nonpreconditioned cell. MTT activity decreased in both experimental groups in compared with normal group, but in preconditioned group, MTT activity increased markedly in compared with nonpreconditioned group. And apoptosis is decreased by precontitioning in TUNEL staining. These results suggest that cardioprotective effects of ischemic preconditioning is mediated by attenuating structural destroy, increasing cell viability, decreasing apoptosis.

We recently treated an infant with abnormal calcification in the soft tissues of the wrist and hand caused by an intravenous injection of calcium gluconate for treatment of neonatal hypocalcemia. The cause of the soft tissue calcifications was quite puzzling, if the incident of calcium gluconate extravasation had been overlooked, because the lesions mimicked for cellulitis or abscesses and treated as such. All radiographic evidence of calcification gradually resorbed by conservative management.

10.Reevaluation of the “falx sign”

Visualization of falx cerebri on non-enhanced CT of children with severe head injuries (the falx sign) has been regarded as an evidence of subarachnoid hemorrhage. On the contrary, other authors have reported reverse results. To evaluate clinical significance of the falx sign, authors studied frequency of visualization and CT number of falx cerebri and dural sinuses in 65 children with head injury and 65 children without head injury examined by cranial CT at the Dep. of Radiology, St. Paul's Hospital, Catholic Medical College from March to Sept. 1981. All patients with head trauma were studied within 1 week of the traumatic event. Visualization of falx cerebri was observed at slice in the region of lateral ventricle and slice near to vertex respectively. On the slice in the region of lateral ventricle, falx cerebri was identified in 82% of all of the children examined, of which 92%showed partial visualization of falx cerebri and remaining 8% totally. On the slice near to vertex, falx cerebri was identified in 92% of all the children examined, of which 38% showed partial visualization of falx cerebri and remaining 62% totally. In head trauma group, frequency of visualization of falx cerebri was 78% on the slice in the region of lateral ventricle and 89% on the slice near to vertex; in non-traumatic group, frequency of visualization of falx cerebri was 86% and 94% respectively. The highest numerical value of the falx densities averaged 47 Hounflieds (range, 32-63) in non-traumataic group, averaged 49 Housfields (range, 32-69) in head trauma group. All or a portion of the superior sagittal sinus was visualized in 59% of all of the cases studied, 50% in head trauma group, and 69% in nontraumatic group. The straight sinus was identified in 45% of all of the cases studied, 39% in head trauma group, and 51% in non-traumatic group. In conclusion, there was no distinction between head trauma and non-traumatic group in visualization of falx cerebri and dural sinuses, and we could frequently identify the falx density in normal. Also we could find that frequency for visualization of falx cerebri and drual sinuses increased as the age increased.
