1.Characteristics of Magnetic Resonance Arthrography Findings in Traumatic Posterosuperior Rotator Cuff Tears.

BACKGROUND: Few studies have investigated magnetic resonance (MR) characteristics of traumatic posterosuperior rotator cuff tears involving the supraspinatus and infraspinatus. We hypothesized that traumatic rotator cuff tears may have MR characteristics distinguishable from those of non-traumatic tears. METHODS: Preoperative MR arthrography and intraoperative tear size measurements were compared in 302 patients who underwent MR arthrography and subsequent arthroscopic rotator cuff repairs for traumatic (group T, 61 patients) or non-traumatic (group NT, 241 patients) tears. The inclusion criteria for both groups were posterosuperior full-thickness rotator cuff tear and age between 40 and 60 years. For group T, traumas were limited to accidental falls or slips, or sports injuries, motor vehicle accidents; injuries were associated with acute onset of pain followed by functional shoulder impairment; and time between injury and magnetic resonance imaging (MRI) was 6 weeks or less. RESULTS: In group T, 72.1% of shoulders (44 patients) had tendon tears with blunt edges while 27.9% of shoulders (17 patients) had tears with tapering edges. In contrast, 21.2% of patients in group NT (51 patients) had blunt-edge tears, while 78.8% (190 patients) of tears had tapering edges. These results were statistically significant (p<0.001) and estimated odds ratio was 9.6. The size of tear did not vary significantly between groups. CONCLUSIONS: We found no exclusive MR characteristic to define traumatic tears. However, oblique coronal MRI of traumatic tears showed a significant tendency for abrupt and rough torn tendon edges and relatively consistent tendon thicknesses (without lateral tapering) compared to non-traumatic cuff tears.

2.A Clinical Observation of the Pelvic Bone Fracture

62 cases of the fracture of the pelvis admitted to the orthpedic department of Wonju Christian Hospital during past 5 years from 1975 to 1979 were reviewed and clinical results were obtained as follows: 1. Males were frequent than females and almost 3rd to 4th decades of life. 2. The causes of injury were traffic accident, coal minor accident, and falling. 3. Stable type is the most common in this series and among the unstable types, lateral compression type is the most common and then straddle type and vertical shear type. 4. The most common complication is uro-genital injury. 5. The residual complications at the end of treatment remained in some cases, Arthritis of sacroiliac joint, Impotence, Urethral stricture, Low back pain, etc. 6. All of the cases were treated conservatively and their results were somewhat good except 4 cases. 7. Surgical treatment would be considered, if necessory, to decrease the complications and reduce hospital days.

3.Two Cases of Primary Biliary Cirrhosis.

Primary biliary cirrhosis(PBC) is a chronic cholestatic liver disease of unknown origin. The small and medium sized intrahepatic bile ducts are destroyed by an inflammatory process, which, it has been suggested, is of the autoimmune type. It is strongly associated with the presence of antimitochondrial antibodies, predominantly IgM and IgG. The liver changes are classified into four stages, of which stage IV represents the development of cirrhosis, which required orthotropic liver transplantation in the longrun. The prevalence rates was reported 128 per millon in Sweden , but the disease is relatively rare in Oriental area. In medical treatment, long-term administration of ursodeoxycholic acid improves both clinical and biochemical signs, slows the progression of the disease and reduces the complication requiring liver transplantation. We report two cases of PBC, one with histologically proven cirrhosis, and the other with bile duct destruction consistent with stage III and hypothyroidism.

5.Multiple Bile-duct Hamartoma: Two Case Report.

Authors report the radiologic findings of two cases of multiple bile-duct hamartoma in the liver. Ultrasonography and computed tomography of the liver were performed in two female patients. In one patient, ERCP was performed. Ultrasonography and computed tomography showed scattered multiple hypoechoic and low attenuated lesions, measuring up to 2cm in both lobe and ERCP showed communication of these lesions with the biliary system. Multiple bile-duct hamartoma should be included in the differential diagnosis of multiple small cystic lesions in the liver.

Hyperosmolar nonketotic coma complicated in diabetes mellitus has been a rare cause of rhabdomyolysis, although increasingly reported recently. Acute renal failure can be complicated in 15 percent of rhabdomyolysis patients, but is rare in the case of rhabdomyolysis caused by diabetic hyperosomolar nonketotic coma. We report a 14 years-old boy with acute renal failure complicated by rhabdomyolysis caused by diabetic hyperosmolar coma.

7.HIV-1 p24 Antigen Detection by Immune Complex Dissociation and Neutralization in HIV-1 Infected Persons in Korea.

BACKGROUND: HIV-1 p24 antigen assay is useful for the detection of circulating viruses, and infection prior to seroconversion. However, circulating HIV-1 p24 antigen may be complexed with HIV antibody and prevent it from being detected by antigen capture assay. To detect HIV-1 p24 antigen in the specimen, it is necessary to dissociate immune complexes and confirm the presence of HIV-1 p24 antigen after the neutralization with the specific antibody. METHODS: We tested 32 sera from HIV-1 infected persons who were diagnosed from 1987 tO1996 in Korea for HIV-1 p24 antigen by enzyme linked immunosorbent assay (ELISA.) with or without the dissociation of immune complexes. And we confirmed the antigen assay results by the neutralization with HIV-1 specific antibody as a confirmatory test. We also calculated the concentration of HIV-1 p24 antigen in each specimen. RESULTS: Eleven of 32 sera tested were initially positive for HIV-1 p24 antigen. After the dissociation of immune complexes for 29 sera except two of which signal/cutoff ratios were higher than 7.0 and except one which was not enough for the assay,13 were shown to be positive for HIV-1 p24 antigen and their signal/cutoff ratios were increased by 10 times. Five of antigen negative cases were turned to be positive after the immune complex dissociation. After neutralization with HIV-1specific p24 antibody for sera of 13 which were positive for HIV-1 p24 antigen with or without the immune complex dissociation, all were shown to be neutralized. We have observed more than 90% neutralization reduction for 12 sera and more than 50% less than 90% for one. The average concentration of HIV-1 p24 antigen was8.76pg/ml by antigen assay and was increased to 76.81~g/m~ after immune complex dissociation. CONCLUSION: We concluded that the sensitivity and the specificity of HIV-1 p24 antigen assay could be increased by dissociation of the immune complexes and neutralization with the specific antibody.

8.Clinical Evaluation of 68 Cases of Spinal Cord Tumors.

The author analyzed 68 cases of the spinal cord tumors who had been operated at the Department of Neurosurgery of the Hanyang University Hospital from 1985 to 1989. The results were summerized as followings : 1) The spinal cord tumor was most common in the 5th decade of age(18 cases, 26.5%) and the ratio of male to female was 1.5 : 1, male predominant. The most common pathologic type was neurogenic tumor(29 cases, 42.6%). 2) The tumors were located most frequently in the thoracic area(35 cases, 51.5%) and in the intradural extramedullary space(30 cases, 44.1%). The most common initial presentation was pain and the paraparesis was most frequent symptom on admission. 3) The ratio of positive findings in plain radiologic study was 55.9% and all cases were shown the complete or partial blockings in myelography. 4) The computed tomographic scanning with water-soluble contrast media was useful in planning of operation to provide the exact relationship between the tumor, spinal cord and location of dura. 5) The total or gross total removal was possible in 60.3%(41 cases) and the result within postoperative one month was recovery or improvement in 73.5%(50 cases), the follow-up result was 83%(49 cases among 59 cases). 6) The most common postoperative complication was the spinal deformity in children(37.5%), and the osteoplastic laminotomy was required in prevention of this. 7) The incidence of metastatic tumor was 16.2%(11 cases), relatively high rate and the lung was most common primary focus. In plain X-ray examination, the bone destruction was most commonly found(54.5%) and the follow-up result was poor than the other tumors.

9.The Discolytic Effects of Disken(R)(Chymopapain, made in Korea): Experimental Study.

The purpose of this study is to evaluate the effect of Disken(R)(chymopapain) on the intervertebral discs of Mongolian Dogs. Three healthy Mongolian dogs at same age(9 months) were underwent laparotomy, and the intradiscal injections of Disken(R) were made from the anterior. After sequential roentgenographic examination, the dogs were sacrificed at 1 month, and the gross and histological examination were performed. The results were as follows: 1) The discolytic effects of Disken(R) were evidently observed through the roentgenographic and histological examination. 2) The maximal changes of intervertebral disc space narrowing developed within 1 week after injection. 3) The reduction percentage of disc spaces was ranged between 45% to 50%, compared to the pre-injections stage. 4) The nucleus pulposus was evidently affected, but the annulus fibrosus and epiphyseal cartilage were not affected. 5) The adverse reactions were not observed in this study.
