1.Analysis of foreign body in the children's airway and follow-up study.

The author analysed foreign body in the airway, divided into two groups vegetable foreign body group(VFBG) and non-vegetable foreign body group(N-VFBG), according to the nature, in 88 patients aged from 5 months to 15 years who were diagnosed and treated under ventilating bronchos-copy at Department of Pediatrics and Otolaryngology, Pusan National University Hospital from 1980 to 1990. Of the 88 patients analysed, 13 were followed for review, which comprised clinical assessment, chest radiography, and perfusion lung scan. The follwing results were obtained. 1) The highest incidence was seen in the age group of 1 to 3 years old especially in VFBG, bur no difference in incidence among each age group was observed in N-VFBG, The ratio of incidence between male to female was 2:1 ein total and, in N-VFBG, the incidence of male 8 times higher than female. 2) A wide variety of foreign bodies was recovered, with peanut being by far the most common (50%) and the ratio of incidence between VFBG and N-VFBG was 6 to 4. 3) The time interval of less than 24 hours between aspiration and admission was most frequently seen in 38% and of more than 3 weeks in 11%. The time interval in VFBG and the group with left bronchial lodging was longer than of N-VFBG and the group with right bronchial lodging, each respectively but no statistical difference was observed. The time interval in the group with bronchial lodging was significantly longer than of the group with laryngeal and tracheal lodging. 4) Frequent symptoms and signs were coughing, dyspnea, fever, decreased air entry, coarse breathing sound and rales. In total, clinical manifestations were more common in VFBG, especially in the incidence of fever, decreased air entry, coarse breathing sound and rhonchi. 5) The roentgenographic findings on admission were emphysema, pneumonia and atelectasis in the order of frequency. The incidence of emphysema and complex roentgenographic findings were more common in VFBG whereas in N-VFBG radioopaque foreign body normal roentgenographic findings were more common. 6) The prevalence of bronchial lodging was over 70% regardless of age group. No difference of predilection was observed between right and left bronchial lodging before 7 years old, but after 7 years old right bronchial lodging was significantly seen. 7) The longer the time lag before admission after aspiration, the longer the duration of hospitalization was in VFBG but with no statistical significance. However, no correlation between time lag and the duration o hospitalization was observed in N-VFBG. The group with bronchial lodging had significantly longer duration of hospitalization compare to that of the group with laryngeal and tracheal lodging, The group with left bronchial lodging had longer duration of hospitalization compare to that of the group with right bronchial lodging but with no statical significance. 8) Of the 13 cases reviewed, one had abnormal lung perfusion scan in spite of normal clinical assessment and chest X-ray, who was treated on 20 days days after aspiration of peanut in the right bronchus.

3.Production of Monoclonal Antibody to Polychlorinated Biphenyl Induced Cytochrome P-450 LMII in Rat Liver.

Cytochrome P-450 (CP-450) is one of the three components of the liver microsomal enzyme system which hydroxylates fatty acids, hydrocarbons and a variety of drugs and other foreign compounds. Female Balb/c mice were immunized with purified polychlorinated biphenyl (PCB) – induced CP-450 LMII. The spleen cells derived from immunized mice were fused with SP2 myeloma cells using polyethylene glycol (PEG 3500). The hybrid cells were selected by hypoxanthine-aminopterin-thymidine (HAT) medium and the culture fluid were screened by enzyme-linked immunosorbent assay to CP450 LMII. The hybrid cells(×107) were inoculated into intraperitoneal cavity of Balb/c mice for the purpose of production of ascetic fluids. Monoclonal antibody (Mab) was purified from ascitic fluid by DEAE cellulose ion exchange chromatography and I¹²⁵ labeled Mab was also confirmed by autoradiography and SDS-polyacrylamide gel electrophoresis (MW:55,000 and 110,000)

4.Purification of Band 3 from the Human Erythrocyte Membrane and its Incorporation into Liposome.

Band 3, the predominant 95,000 dalton anion transport protein, is the major intrinsic glycoprotein of the human erythrocyte membrane. This anion carrier exists as a dimer and binds the cytoskeletons such as spectrin, ankyrin and actin. And the liposomes are vesicular structures which form spontaneously upon hydration of phospholipids. These artificial lipid vesicles have been investigated as model of the biological membranes and as a mean of improving the delivery of nucleic acids, drugs, proteins and biological substances to specific target tissues and cells. In this study, we were purified Band 3 from the human erythrocyte membrane (ghost) was prepared by hemolysis of intact human erythrocyte with weak alkali-hypotonic solution. Band 6 was removed from ghost by extracting with solution of an ionic strength of 0.15. Band 3 and Band 4 were solubilized selectively by extracting Band 6-depleted ghosts with Triton X-100 under nondenaturing conditions. Band 3 was then purified from Triton X-100 extract treated with p-chloromercuribenzoate by sucrose density gradient ultracentrifugation. This purified Band 3 was incorporated into liposomes prepared by reverse-phase evaporation. Phosphatidyl L-serine and cholesterol (1:1 molar ratio) were dissolved in chloroform and the chloroform was removed by rotator evaporation under reduced pressure. Band 3 solution without Triton X-100 was introduced into a mixture of lipids and diethylether. Diethylether was subsequently removed by evaporation. This purified Band 3 and its incorporation into liposomes were confirmed by sodium dodecyl sulfate-polyacrylamide gel electrophoresis.

6.The level of residual monomer in injection molded denture base materials.

STATEMENT OF PROBLEM: The residual monomer of denture base materials causes hypersensitivity on oral mucosa and intereferes with the mechanical properties of the cured resin. The amount of residual monomer is influenced by materials, curing cycle, processing method, and etc. PURPOSE: The aim of this study was to investigate the residual methyl methacrylate(MMA) content of injection molded denture base polymer, and to compare this with the self-cured resin and the conventional compression molded heat-cured resin. MATERIALS AND METHODS: Disc shaped test specimens (50mm in diameter and 3mm thick) were prepared in a conventional flasking technique with gypsum molding. One autopolymerized denture base resins (Vertex SC. Dentimex. Netherlands) and two heat-cured denture base resins (Vertex RS. Dentimex. Netherlands, Ivocap. Ivoclar Vivadent, USA) were used. The three types of specimens were processed according to the manufacturer's instruction. After polymerization, all specimens were stored in the dark at room emperature for 7 days. There were 10 specimens in each of the test groups. 3-mm twist drills were used to obtain the resin samples and 650mg of the drilled sample were collected for each estimation. Gas chromatography (Agillent 6890 Plus Gas Chromatograph, Agillent Co, USA) was used to determine the residual MMA content of 10 test specimens of each three types of polymer. RESULTS: The residual monomer content of injection molded denture base resins was 1.057+/- 0.141%. The residual monomer content of injection molded denture base resins was higher than that of compression molded heat cured resin (0.867+/-0.169%). However, there was no statistical significant difference between two groups (p > 0.01). The level of residual monomer in self cured resin(3.675+/-0.791) was higher than those of injection molded and compression molded heat cured resins (p<0.01). CONCLUSION: With respect to ISO specification pass/fail test (2.2% mass fraction) of residual monomer, injection molding technique(1.057+/-0.141%) is a clinicaly useful and safe technique in terms of residual monomer.

8.A Case of Disfiguring Xanthelasma Palpebrarum.

We report a case of xanthelasma palpebrarum (XP) with bilateral extensive and disfiguring involvement of all four eyelids. The patient was a 28-year-old woman and noticed this condition eight years ago. Physical examination showed yellowish flat topped bilateral extensive plaques around the eyes. Laboratory result was remarkable for a decrease of a serum high density lipoprotein cholesterol (HDL-C). Other laboratory findings were within normal limits. Histopathologic findings showed diffuse infiltrations of foamy cells, histiocytes and foreign body giant cells in the dermis. Electronmicroscopic examination showed that the dermal infiltrate was composed of lipid laden histoncytes showing multiple villi which indicated an activated state of histioncyte. Bilateral and extensive XP is very rare. Only two cases have been reported in the literatures as far as we know.

9.The Result of the Posterolateral Fusion with Knodt Rod and without Knodt Rod in Spondylolysis and Spondylolisthesis of the Lumbar Spine

Since Hadra first used metalic internal fixation in the human spine with wire loop in 1895, various implants have been used. Knodt rod was designed on the principle that the distraction produced a localized flexion attitude in the area to be fused, thus increasing the size of the intervertebral foramina and decreasing the risk of intraforaminal encroachment on the nerve root. We have performed the posterolateral fusion with or without Knodt rod as a primary procedure in spondylolysis and spondylolisthesis of lumbar spine, which required arthrodesis of more than one level. For period of 8 1/2 years' from Sep. 1977 to Apr. 1986, 12 cases of diseased spine were treated with posterolateral fusion without Knodt rod, and 13 cases were treated with posterolateral fusion using Knodt rod. We compared with two groups, and the following results were obtained. 1. Solid bony fusion was 100% in posterolateral fusion with Knodt rod, and 92% without Knodt rod. 2. An average period of post-operative immobilization was 2 months in cases with Knodt rod, and 2.5 months without Knodt rod. 3. In posterolateral fusion with Knodt rod, 4 cases with the root symptoms were all improved. In posterolateral fusion without Knodt rod, 6 cases with root symptoms, were all improved. But in 1 case without the root symptoms, developed the root symptoms. 4. No significant changes in radiologic evaluation were found between posterolateral fusion with Knodt rod and without Knodt rod. 5. Post-operative complications developed in 4 cases (31%) with Knodt rod, and 6 cases (50%) without Knodt rod. 6. Clinically, the satisfactory results were 100% with Knodt rod, and 83% without Knodt rod.
