1.8 Cases of Calyceal Diverticula.

We wish to report 8 cages of calyceal diverticula which were diagnosed tentatively among cases of I.V.P. during the period from January 1965 to December 1971 at St. Mary's Hospital. There were 5 male and 3 female patients. The right kidney was involved in all patients The upper calyx was involved in 6 patients and the middle calyx in 2 patients. Lumbar or abdominal pain was present in 5 patients. 1 of the 8 patients had undergone appendectomy with removal of a norma1 appendix prior to diagnosis.Pyuria were found in 3 patients and one patient had single stone within the diverticulum.

2.A Study on The Flexural Bond Strength of The Gold and The Co-Cr Alloy To The Denture Base Resins.

In general, the three major oral functions of edentulous patients-mastication, phonation, esthetics-can be rehabilitated by the complete dentures, and both the resin based complete denture and the metal based complete denture are commonly used by many clinicians today. For the sake of many advantages such as the excellent thermal conductivity, low volumetric change, high strength, low risk of fracture and the better patient's adaptation, the metal based complete dentures are indicated to the several cases. But, there are common failures of these type of dentures mainly by the fracture or the debonding between the resin structures and the metal frameworks which is caused by the discrepancies by the discrepancies of the flexural strength and the coefficient of thermal expansion. This is aggravated by the water contamination of the interface when exposed to the oral environment and results in the failure of complete denture treatment. So, the purpose of this study is to compare the bond strength and the fracture patterns of the gold alloy based and the Co-Cr alloy based complete dentures using the PMMA resins and the 4-META adhesive resins. The results of this study were as follows. 1. Both to th PMMA resin and the 4-META resin, the flexural bond strength of gold alloy is lower than that of Co-Cr alloy(p<0.05). 2. To the Co-Cr alloy, the bond strength of the 4-META resin is significantly higher tan that of PMMA resin(P<0.05). 3. The flexural strength of the group with the mechanical retention form is significantly higher than that of the group without retention form(P<0.05). 4. Comparing with the other groups, the fracture patterns of the group 3 are quite different from the group 1, 2, 5.

3.The Gamma Intramedullary Nailing for Peritrochanteric Fractures.

This study consisted of 66 peritrochanteric fractures which were treated by the modified Gamma nail (Asian Pacific nail) from August 1993 to October 1995. Sixty-one intertrochanteric fractures and five subtrochanteric fractures were treated in our institution. Average length of follow-up was 15.0 months. Four patients died during follow-up. The results were as follows; Lag screws were ideally introduced in 55 (83%) cases. The mean length in lag screw sliding was 4.3 mm in stable trochanteric fractures and 5.7 mm in unstable ones. Fifty-one out of 62 patients (82%) were returned to their previous ambulatory status. Intraoperative complications were as follows; distal crack (3 cases); rotation of femoral head (2 cases); entry point crack (I case); medialization of proximal fragment (7 cases); and missing of the interlocking screw (1 case). All of these complications did not affect the outcome. All medial cortical gaps were closed after sliding of lag screws. Three distal cracks did not propagate to the shaft. Postoperative complications were a case of superior cut-out, and two second fractures after falls. Callus formed more early in the stable trochanteric fractures (5+/-2.1 weeks) than unstable fractures (7.3+/-2.1 weeks). It was statistically significant. Union time between stable and unstable trochanteric fractures was not statistically significant. In conclusion, the Asian Pacific (AP) nail could appropriately fix the osteoporotic peritrochanteric fractures in elderly patient. It could offer early rehabilitation to these patients and decrease the mortality and morbidity.

4.Clinical Studies on the Supracondylar Fractures of the Humerus

Supracondylar fractures of the humerus is the most common elbow fractures in children. Many papers has been published about the treatment of the supracondylar fractures of the humerus in children and prevention of both Volkmann's ischemic contractures and nerve injuries associated with these fractures, however, unfortunately it is still one of the most difficult fractures to manage. After successful reduction, the late complications of loss of elbow motion, the change in carrying angle, myositis ossificans, and progressive ulnar nerve palsy still lie ahead. For the period of 5 years from January 1975 to December 1979, 110 patients who had been treated for supracondylar fractures of the humerus at Severance Hospital, Yonsei University Coilege of Medicine were studied and analyzed retrospectively and the results are summerized as follows: 1. The age of the patients varied from 1 to 26 years, the majority (80.0%) being between 4 to 11 years and the fractures were on the left side in 71.8% and males comprized 75.5%. 2. The extension type comprized 94.5%. 3. Most common complications were cubitus varus deformities. 4. Percutaneous pinning and open reduction and internal fixation elicited rare cubitus varus diformities. 5. Limitation of motion was most frequent camplication after open reduction and internal fixation. 6. All cases which showed cubitus varus deformities were initially medially displaced fractures. 7. Four median nerve injuries were associated with posterolateral displacement and one radial nerve injury was associated with posteromedial displacement. 8. Finally, with prompt and adequate treatment, there were no Volkmanns ischemic contractures seen in this study.

6.Measurement and Comparison of Hydration and Lipid Levels between Patients with Acne vulgaris and Patients with Atopic Dermatitis.

BACKGROUND: Skin surtice lipids increase and decrease in tints with acne vulgaris and in patients with atopic dermatis, respectively. Notably, hydratior. in decreased in patients with atopic dermatitis. Clinically it is common for the two diseases to occur together. OBJECTIVE: We would like to find out the rate, clinical cteristics, and hydration and lipid levels when acne vulgaris and atopic dermatitis concur. METHODS: We classific the clinical levels and measured the hydration and lipid levels of the face and forearm area of patients with acne vulgaris, patients phatopic dermatitis, and patients with both diseases, who visited the Youngdong Severance Hospital. RESULTS: 1) 13.8% (14/102) of patients with acne vulgaris had at, epidermatitis. 31.1% (14/45) of patients with atopic dermatitis had acne vulgaris. 2) Patients with acne vulgaris had increased lipid levels and normal hydration levels. 3) Patients with atopie dermatitis had decreased lipid and iylration levels. 4) Patients withoth cnevulgaris and atopic dermatitis a no severe grade of acne vulgaris, and showed decreased hydration but normal lipid levels. CONCLUSION: In patients with both arne vulgaris and atoic dermatitis, clinically acne vulgaris was not severe and hydation levels were lower than normal controls.

7.Association of Allergic Disease with the Sinusitis in Children.

Sinusitis manifested as symptomatic imflammation of the paranasal sinuses is a common pediatric diagnosis. Many studies have focused more directly on the question of relationship between allergy and sinusitis but the relevance of the relationship between allergy and sinusitis still needs further examination, The purpose of this study is to assess of relationship of allergy to extent or severity of sinusitis. A clinical evaluation was performed on 199 children, age 2 to 15 years, in whom the diagnosis of sinusitis was confirmed by paranasal sinus radiographic assessment at our hospital Pediatric OPD and in the ward from July 1991 to June 1993. The results were as follows: 1) Male was more prominent than female (M:F ratio 1.76:1). The age distribution, 100 cases(55.2%) of the total case were 4-7 year old. 2) The symptoms were coughing, most common (77,9%), rhinorrhea, nasal discharge and nasal stuffiness, in decreasing order. The clinical signs were postnasal drip (62.8%), most common, throat injection, wheezing sound, and allergic shiner in decreasing order. Eighty eight patients were evaluated acute and compared with 111 patients with chronic sinusitis. 3) Comparable study between two groups were summarized as follwos: (1) There was no significant difference in age and sex distribution between acute and chronic group. (2) The lowest seasonal incidence of acute group was seen in summer and autumn but there was no significant variation between both group (p>0.05). (3) Allergc disease was more prevalent in chronc group than acute group (p<0.05). Asthma incidence was higher in chronic group than acute group (p<0.05). (4) The frequency of total IgE level checked by PRIST higher than 200 IU/ml in acute group and chronic group was 23.2% and 42.3%, respectively (p<0.05). The incidence of positive skin prick test in acute group and chronic group was 25.0% and 46.5% respectively (p<0.05). (5) Otitis media, nasal polyp and adenoid hypertrophy incidence were higher in chronic group than acute group (p<0.05). (6) There was no significant difference if ESR between both groups. The eosinophilia in peripheral blood higher than 5% in the acute group and chronic group was 15.9% and 42.3% respectively (p<0.05). (7) The patients with chronic group had more severe involvement of the paranasal sinuses than acute group (p<0.05). (8) There were no significant difference in chest radiographic findings and follow up paranasal sinus radiographic findings after treatment (P>0.05).

8.A Case Report of Primary Aldosteronism with Sustained Hypertension After Adrenalectomy.

We experienced a case of primary aldosteronism due to adrenal adenoma. The patient was 45-year old female, whose main complaints were weakness, headache, and fatigue. By use of abdominal CT scan, ultrasonogram, adrenal angiogram and by result of clinical laboratory findings, we conculuded the diagnosis of adrenal adenoma, and confirmed it by surgical and pathologucal findigs. The clinical sympyoms disappeared after left adrenalectomy, and laboratory findings of renin, aldosterone, electrolutes were normalized but hypertension persisted, so she has been managed by antihypertensives.

9.A study on the adhesion of a soft liner containing 4-META to the base metal alloy and its viscoelastic property.

STATEMENT OF PROBLEM: Soft lining materials, also referred to as tissue conditioning materials, tissue treating materials, relining materials, soft liners or tissue conditioners, were first introduced to dentistry by a plastic manufacturer in 1959. Since the introduction of the materials to the dental field, their material properties have been continually improved through the effort of many researchers. Soft lining materials have become widely accepted, particularly by prosthodontists, because of their numerous clinical advantages and ease of manipulation. Unfortunately, few reports have been issued upon the topic of increasing the bond strength between the base metal alloy used in cast denture bases and PMMA soft liner modified with 4-META, nor upon the pattern of debonding and material change in wet environment like a intra oral situation. PURPOSE: The purposes of this study were comparing the bond strength between base metal alloy used for the cast denture bases and PMMA soft liner modified with 4-META, and describing the pattern of debonding and material property change in wet environment like the intraoral situation. MATERIAL AND METHODS: This study consisted of four experiments: 1. The in vitro measurement of shear bond strength of the adhesive soft liner. 2. The in vitro measurement of shear bond strength of the adhesive soft liner after 2 weeks of aging. 3. A comparison of debonding patterns. 4. An evaluation the gelation time of modified soft liner. The soft liner used in this study was commercially available as Coe-soft (GC America.IL., USA), which is provided in forms of powder and liquid. This is a PMMA soft liner commonly used in dental clinics. The metal primer used in this study was 4-META containing primer packed in Meta fast denture base resin (Sun Medical Co., Osaka, Japan). The specimens were formed in a single lap joint design, which is useful for evaluating the apparent shear bond strength of adhesively bonded metal plate by tensile loading. Using the 20x20mm transparent grid, percent area of adhesive soft liner remaining on the shear area was calculated to classify the debonding patterns. To evaluate the change of the initial flow of the modified adhesive soft liner, the gelation time was measured with an oscillating rheometer (Haake RS150W/TC50, Haake Co., Germany). It was a stress control and parallel plate type with the diameter of 35mm. CONCLUSION. Within the conditions and limitations of this study, the following conclusions were drawn as follows. 1. There was significant increase of bond strength in the 5% 4-META, 10% 4-META containing groups and in the primer coated groups versus the control group(P<0.05). 2. After 2 weeks of aging, no significant increase in bond strength was found except for the group containing 10% 4-META (P<0.05). 3. The gelation times of the modified soft liner were 9.3 minutes for the 5% 4-META containing liner and 11.5 minutes for the 10% 4-META liner. 4. The debonding patterns of the 4-META containing group after 2 weeks of aging were similar to those of immediaely after preparation, but the debonding pattern of the primer group showed more adhesive failure after 2 weeks of aging.

10.Effects of Ischemic Preconditioning on Hypoxia-Reoxygenation Injury of Cardiac Myocyte in Culture.

Short period of ischemia and reperfusion protect heart against subsequent prolonged ischemia-reperfusion injury. This phenomenon was first described by Murry et al in 1986, who demonstrated that four 5-minute coronary artery occlusions followed by equal period of reflow at each time before a subsequent prolonged occlusion resulted in a reduction of infarct size in dog. Although the precise mechanism of preconditioning remains unknown, this phenome-non is present among different species of mammals, including dogs, rats, pigs, rabbits, and human. The objects of present study was to investigate effect of ischemic preconditioning on cell viability, structural changes and apoptosis during 60 min hypoxia and 60 min reoxygenation of the cell. In present study we investigated through cell culture system using myocyte of three days old neonatal rat cultured for three days. During hypoxia and reoxygenation, differences between preconditioned and nonpreconditioned of beating counts, morphological and structural changes are investigated through inverted phase contrast microscope and transmis-sion electron microscope. To detection of apoptotic cell, TUNEL (TdT-mediated dUTP-biotin nick end labeling) stain was accomplished, and through which we invesigate the effects of preconditioning on apoptosis. Viabiliy of each cell and it's mitochondria were measured quantitatively by MTT assay. After 60 min of hypoxia and 60 min of reoxygenation, beating rate decreased remarkably. But at the time of 60 min of reoxygenation, there was marked increase in beating count in pre-conditioned cell. Swollen mitochondria with amorphous granules in inner membrane, destroyed mitochondrial cristae, indented nuclear envelope, chromatin condensation, contracture of myofibril, fragmentation of myofilaments, cytoplasmic shrinkage were observed in both preconditioned cell and nonpreconditioned cell. But it is much less in pre-conditioned cell than in nonpreconditioned cell. MTT activity decreased in both experimental groups in compared with normal group, but in preconditioned group, MTT activity increased markedly in compared with nonpreconditioned group. And apoptosis is decreased by precontitioning in TUNEL staining. These results suggest that cardioprotective effects of ischemic preconditioning is mediated by attenuating structural destroy, increasing cell viability, decreasing apoptosis.
