Microsportcm gypseum is a geophilic fungi abundant in soil throughout the world. The infections may occur either from animal to man(zoophilic) or from soil to man (geophilic). Patierit of 13-year-old Korean male sought medical attention with skin lesion on scalp which showed scaly patch with broken hairs and a round pilar cyst like nodule. Identification of Microsporum gypseum was made by culture on Sabouraud agar media and microscopic exarnination.

3.Experimental studies of 1, 4-Bis-trichlormethyl-benzol on the infection of Clonorchis sinensis(I).

The efficacy and toxicity of 1, 4-Bis-trichlor-methyl-benzol were examined in the animal infections(rat, rabbit and mouse) of Clonorchis sinensis. The effective dosages to kill the worm in bile ducts were 1 x 150 mg/kg in rat and 5 x 150 mg/kg in rabbit. During 48 to 72 hours after the administration of the drug, the inactive posteriorly swollen worms and variously degenerated worms were observed from liver ducts, gallbladder and intestine of the experimental animals. In gall bladder of rabbits, a plenty of fragments of the worms and clewed mass of eggs were confirmed under microscope. The E.P.G. was not decreased to zero in 3 weeks observation after the administraiton of the drug except rabbit No. 9 which was treated with the dose of 5 x 250 mg/kg. Minimal congestion and mild cloudy swelling of liver were observed in the groups of mice which were given 5 x 150 kg and 3 x 500mg/kg of the drug respectively. The body weight of experimental animals showed no evidence of changes.

5.MR findings of failed back surgery syndrome.

Recurrent disc herniation and postoperative fibrosis are the main disease entities causing failed back surgery syndrome (FBSS) and magnetic resonance (MR) imaging has become a major diagnostic modality in differentiating the two. To observe the variable entities of FBSS and their MR findings, we retrospectively analyzed 15 MR images in 12 patients. The causes of FBSS were as follows; normal (no organic cause), fibrosis, new or recurrent disc herniation, discitis, osteomyelitis, inflammation at operation site, epidural abscess, arachnoiditis, and hematoma. Except a case of hematoma, gadolinium enhancement scan was necessary and informative in the diagnosis of FBSS and MR imaging only was not enough in the diagnosis of arachnoiditis.

7.A Case of Tick bite Caused by Ixodes monospinosus.

The occurrence of tick bites and related dermatoses have been rarely described in Korea. Ticks may cause various infectious diseases associated with dermatoses by transmission of the mediating pathogens such as virus, bacteria and rickettsia. We describe herein, to our knowledge, the first tick bite with Ixodes monospinosus in Korea, The patient, a 64-year old male, was bitten by a tick while climbing a mountain which is located in the Kanghwado. He had an asymptomatic, yellowish pea-sized tick attached on the right antecubital area for 7 days' duration, which was identified as an adult female of I. monospinosus by the scanning electron microscopic examination.

8.The Prognostic Significance of Immunophenotype in Children with Acute Lymphoblastic Leukemia.

PURPOSE: The prognostic factors in acute lymphoblastic leukemia was patient's age at diagnosis, sex, hepatosplenomegaly, initial WBC & platelet count. Recently immunophenotype has being studied as guidance for several treatment protocol. We performed a retrospective study to evaluate the significance of immunophenotype on clinical course & long-term survival in children with acute lymphoblastic leukemia. METHODS: One hundred two children with acute lymphoblastic leukemia were admitted to the Department of Pediatrics, Yonsei University College of Medicine from Jan.1991 to Dec. 1995, and bone marrow aspiration biopsies and immunophenotyping were performed. The prognostic significance of immunophenotype was evaluated base on the median event free survival and probability of survival at the end of follow-up. RESULTS: 1) In 102 children comprised 14 cases under 2 years old, 71 cases from 2 to 10 years of age and 17 cases above 10 year old. The ratio of male to female was 6 : 4. 2) By CCG risk grouping, standard risk in 21 cases, intermediate risk in 35 cases, high risk in 46 cases. 3) By cytologic classification of FAB, Ll in 65 cases, L2 in 31 cases, L3 in 2 cases 4) Remission induction rate in early pre-B ALL was 100% (39/39), among those recurrence rate 20% (8/39). 5) Remission induction rate in pre-B ALL was 90% (26/29), among those recurrence rate 54% (14/26). 6) Remission induction rate in B-cell ALL was 33%(113), and all cases was expired during follow-up. 7) Remission induction rate in T-cell ALL was 100% (12/12)), among those recurrence rate 41% (5/12). 8) Remission induction rate in T-B mixed ALL was 71%(517), among those recurrence rate 0% (0/5), but non-remission cases was expired during follow-up. 9) Remission induction rate in Myeloid Ag(+) ALL was 83%(516), among those recurrence rate 80% (415). CONCLUSIONS: These results suggest that better result on long-term survival was observed in early pre-B ALL than in T-cell, 7-B mixed, or Myeloid antigen positive ALL and immunophenotype have prognostic impact in acute Iymphoblastic leukemia.

10.Risk Factors of Gestational Diabetes Mellitus.

OBJECTIVE:The purpose of this study was to identify the prevalence of risk factors related to Gestational Diabetes Mellitus (GDM). METHODS:We retrospectively analyzed medical records of 1,091 pregnant women who had a 50 g oral glucose tolerance test (OGTT) at performed at the department of Obstetrics and Gynecology, Chungbuk National University Hospital from June, 1999 to March, 2006. The 1-hour plasma glucose level higher than 140 mg/dL was considered as a positive screening result. Patients with positive results underwent a 100 g OGTT as the diagnostic test for GDM, and more than 2 positive results were diagnosed as GDM, according to the American National Diabetes Data Group (NDDG) guideline. We investigated age, prevalence of obesity, weight gain, history of fetal anomaly, fetal death in uterus (FDIU), macrosomia, preeclampsia, GDM and familial history of diabetes mellitus, as the risk factors of GDM. RESULTS:Elderly gravida, history of FDIU, perinatal death, preeclampsia, and GDM, familial history of diabetes mellitus were the risk factors of GDM and macrosomia to a statistically significant degree. We found out that BMI and 50 g OGTT results were increased in GDM group. However, there was a pregnant woman with no risk factors who had been diagnosed with GDM. CONCLUSION : Although the pregnant women with risk factors should be managed carefully to detect GDM, we also suggest all pregnant women to take screening test even if they have no risk factors, to improve pregnancy outcomes by detecting and treating unexpected GDM early in pregnancy.
