2.A Clinical Study of Anatomic Position Change on the Intra

Despite a variable degree of comminution, the intra-articular fracture of the distal radius comprises four basic fragments : 1.Radial shaft; 2.Radial styloid; 3.Dorsal medial fragment; 4.Palmar medial fragment. For the period of 3 years from May 1984 to May 1987, 26 patients who had been treated for intra-articular fracture of the distal radius by various methods at the Department of Orthopedic Surgery, Kang Nam Sacred Heart Hospital, Hallym College are presented. According to the objective criteria of Scheck(18) used for calculating the degree of anatomic position change, the result obtained were as follows : 1. The most common fracture type was Type II by Melon(6) classification. 2. The average age of patients was 34 years old. 3. We agree that the accurate anatomic reduction with internal fixation assures better results in the distal radius intra-articular fracture. 4. CT scan was necessary for the accurate diagnosis of articular surface destruction. 5. Accurate relationship of the anatomic and functional result could be analyzed by Scheck(18) and Melon(6) concepts.

3.Pseudospondylolisthesis (Review of 22 Cases).

Study of 22 cases with the pseudospondylolisthesis established that it occurred more frequently in the older women, and far more frequently at the L4 level, as same results as reported by others. The slipping occurred as a result of degenerative disease of the articular process and facet joint, but it never exceeded 25% in this series. This study was performed to recognize the stability of the lumbosacral joint by the measurement of the anterior and posterior vertical heights(deformity) of the body of the L5 vertebra and lumbosacral angles in our cases. We also attempted to observe whether the facet angle changes at the involved level was meaningful or not. Result : lumbosacral angles in the our patients with pseudospondylolisthesis were increased greater than normal, so lumbar lordosis was less than normal. The body of the L5 vertebra was not as wedged anteroposteriorly as it normally was. The contour of the body of the L5 vertebra resembled rectangle shape. The pedicle-facet angles were increased at involved level, but this measurement was not correctly demonstrated on the plain roentgenograms. Conclusion : The stability of the lumbosacral joint in the patients with pseudospondylolisthesis was increased due to rectangle shape of the body of the L5 vertebra and less lumbar lordosis. The pedicle-facet angles at the involved level were increased, but not correct diagnostic values.

6.Surgical Experience on directly treated 18 Intracranial Aneurysm.

Eighteen intracranial aneurysm were operated upon with direct approach under the surgical microscope. The aneurysms arising in the anterior communicating artery(8) were most common, followed by internal carotid artery(6), middle cerebral artery(2), posterior communicating artery(1), anterior cerebral artery(1) in order. We preferred pterional approach. And the timing of operation was scheduled as soon as possible if other conditions permitted. Mannitol administration, hypotensive anesthesia, Sugita clip, biobond or aneurysmostat were utilizes during the operation. The aneurysmal neck was clipped with Sugita clip mostly, but wrapping was also carried out in certain condition. The elasticity of the Sugita clip is seemed to be better than others. In even Sugita clip, however, overopening at the time of clipping made it weak and resulted in incomplete occlusion of the aneurysm neck. To obtain successful aneurysmal surgical result, following surgical care should be taken. 1) lesser brain retraction, 2) complete removal of subarachnoid clots, 3) Meticulous hemostasis of micro-oozing from bridging vein and internal dural surface. Uneventful two surgical cases were found to be fatal due to hidden post-operative oozing clots from the internal surface of the dura.

7.Effect of Postoperative Parathyroid Hormone Administration on Osteoporotic Intertrochanteric Fractures of Females

Purpose:
This study examined the effects of the postoperative administration of parathyroid hormone (PTH) on fracture healing in intertrochanteric fractures accompanied by osteoporosis in elderly females.
Materials and Methods:
Female patients aged 65 years and more who underwent surgery after a diagnosis of intertrochanteric fractures and osteoporosis during the period from July 2013 to December 2017 were included as subjects. The subjects were divided into two groups: PTH-treated group and non-PTH-treated group. The formation time of the first callus, timing of the bridging callus, and time of bony union for both groups were evaluated.
Results:
In the PTH-treated group, the mean time of the first callus formation, average time of bridging callus, and the average time of bony union were 32, 58, 83 days, respectively, which were significantly shorter than that of the untreated group.
Conclusion
PTH, a treatment for osteoporosis, promotes callus formation and the healing process. Therefore it will be helpful in intertrochanteric fractures accompanied by osteoporosis in elderly females.
8.Gene expression profile changes in epithelial ovarian carcinomas using tumor-specific cDNA microarray.

To identify new bio-markers as well as potential targets for new drugs for epithelial ovarian cancer (EOC), we compared the gene expression profiles of cancer tissues from 25 EOCs with human ovarian surface epithelium (HOSE) using in-house cDNA microarray specified to EOC. Based on a comprehensive method and information from the National Cancer Institute (NCI) Cancer Genome Anatomy Project (CGAP), the cDNA library was constructed. After excluding the overlapping clones, 768 spots were included in the array. We identified the genes and expressed sequence tags (ESTs) (30 up-regulated and 34 down-regulated) that are differentially expressed in EOC tissues. To confirm the expression data, we performed real time RT-PCR experiments. Using microdissected EOC tissues and cell lines, we investigated the expression status of the NET-1 gene, clusterin gene, and actin-binding LIM protein 1. The information provided here will be useful for identifying genes whose products might serve as molecular signatures for the biomakers of EOCs.

9.Thoracic Epidural Cavernous Hemangioma.

A 31-year-old male presented with severe back pain and paraparesis. Imaging studies demonstrated an extraosseous, extradural mass without bone invasion at the T11-T12 vertebral level, located dorsal to the thecal sac. The spinal cord was compressed ventrally. The lesion was completely excised after a T11-T12 laminectomy. Histopathological examination revealed a cavernous hemangioma. The patient's symptoms improved after excision of the lesion.

10.Pregnancy in Woman with Spinal Cord Stimulator for Complex Regional Pain Syndrome: A Case Report and Review of the Literature.

Spinal cord stimulation (SCS) is used to manage chronic pain syndromes and it is accepted a cost-effective therapy. Child-bearing women who had SCS become or choose to become pregnant despite these policies pregnancy is a relative contraindication. A 32-year-old woman had SCS as a treatment for the CRPS I of the left lower extremity. During various check up tests, we happen to find out that her serum beta-hCG was positive and confirmed pregnancy. SCS is not recommended in pregnancy because the effects of SCS on pregnancy and nursing mothers had not been confirmed. However, many female patients suffering from chronic pain may expect future pregnancy and we think that they must be informed about the possibility of pregnancy and the effects of SCS device implantation in the course of pregnancy. First of all, a good outcome requires a multidisciplinary team approach, including obstetrics, neonatology, pain medicine and anesthesia, as was used from an early pregnancy. Unfortunately, she had a misabortrion after 6 weeks.
