2.Total Micrografting Hair Transplantation Using a New Hair Implanter.

BACKGROUND: Hair transplantation is a continuously evolving era towards a more natural style and micrografting in greater quantities is becoming an unmistakable trend. OBJECTIVE: A rapid, simple method to make a micrograft implanter with low cost and a case of total micrografting using this new hair implanter are presented. METHODS: We devised a simple method to make a micrograft hair implanter using a spinal tap-ping needle and did total micrografting with this device 3 times on 1 patient. RESULTS: Cosmetically, hair transplantation using our device produces hair that is slightly less dense in a normal individual. However, our technique gives a good balance in distribution of hair with no scarring which contribute to a natural style appearance. CONCLUSIONS: Total Micrografting with our new implanter produces a natural appearance without the artificial look and scars typical of the minigrafts or punch grafts in Koreans. So we consider this device as a another alternative device in hair transplantation.

4.Distribution patterns of cytoskelectal proteins in cardiac endothelial cells : Investigation using monoclonal antibodies.

To investigate the changing patterns of microfilament and microtubule arrangement and influence of myocardial cells and colchicines to microfilament and microtubule formation in cardiac endothelial cells the authors carried out indirect immunofluorescence stain for actin and tubulin with supernatant monoclonal antibodies. Secondary antibodies were IgG FITC conjugate. The results were summarized as follows. Fiberform reactions were stronger in the cells with many processes and spread cytoplasm and they became weaker after the endothelial cells formed monolayer. In the endothelial cells cocultured with myocardial cells the fiberform of the microtubule became less visible compared to control group but fiberform of the microtubule maintained strong intensity as endothelial cells formed monolayer. In the group treated with colchicines, there were no visible differences in microfilaments compared to control group but fiberform of microtubule revealed weaker intensity after colchicines treatment. The intensity of microtubule fiberform returned to control level after 2 days.

5.Primary Malignant Lymphoma of the Gallbladder: A case of report.

Lymphomatous involvement of the gallbladder is rarely identified and manifests mostly as a local involvement of disseminated disease. There have been reported only about 20 primary malignant lymphomas of the gallbladder without involvement of other sites. A 63-year-old woman presented with jaundice, epigastric pain, and weight olss of two months duration. There was no systemic lymphadenopathy elsewhere. Abdominal computed tomograph revealed a focal thickening of the gallbladder wall. The removed gallbladder contained a well circumscribed, 5x3 cm-sized, slightly elevated firm area with accentuated mucosal papillary excrescences near the neck protion. The lesion involved whole thickness of the wall, and was made up of diffuse atypical lymphoid cell infiltrates of variable size accompanying plasmacytoid differentiation in areas. Some large tumor cells were pleomorphic and contained prominent nucleoli. To our knowledge, this is the first case of primary malignant lymphoma of the gallbladder in Korean literature.

7.Comparison between Pathologically and Clinically Diagnosed Group of Acute Postinfectious Glomerulonephritis.

PURPOSE: Postinfectious acute glomerulonephritis usually needs no renal biopsy. But atypical clinical course and laboratory results indicate a need for renal biopsy. Therefore, to investigate clinicopathological characteristics of postinfectious acute glomerulonephritis, we compared clinical manifestations of biopsy group with those of non-biopsy group. METHODS: We reviewed the records of clinical and pathological data of 20 cases diagnosed by renal biopsies and compared them with 23 cases only diagnosed clinically. RESULT: Male : female ratio was 4.8 : 1 in biopsy group and 1.2 : 1 in non-biopsy group, so the male is more predominent in number in the biopsy group. Hypertension was documented in 52.2% of cases of non-biopsy group, which is significantly high compared to incidence of hypertension (20%) in the biopsy group. Laboratory data showed that serum creatinine, cholesterol, and 24- hour urine protein losses are significantly higher in the biopsy group. In the biopsy group, tentative diagnosis on admission were acute glomerulonephritis(45.0%), nephrotic syndrome(15.0%), membranoproliferative glomerulonephritis(15.0%), pyelonephritis(10.0%) and so on. Mean time from onset of symptoms to renal biopsy was 29.3+/-24.1(7-110) days. Fifty percent of the cases showed exudative phase, 25.0% exudative-proliferative phase. In three cases over 7 weeks, two showed proliferative phases and one sclerotic phase. CONCLUSION: Our cases of postinfectious acute glomerulonephritis diagnosed by renal biopsy had a male predominence, and lower incidence of hypertension They also tended to have decrease renal function and more urinary protein loss compared to clinically diagnoses ones without renal biopsy.

10.Multiple Fractures of Forearm & Humerus by Belt Injuries

In the workshops where the rorating belts are used belt, injuries frequently occur for lack of a safety device. Those injuries have several characteristics compared with other long bone fractures. Those are mostly multiple open fractures with considerable soft tissue damages, forearm both bones and humerus fractures by bending force and rotational distraction as mechanism of the injury, and sometimes particularly segmental fracture of the radius. From Aug. 1972 to Jan. 1985, we have traced 47patients of multiple fractures of forearm and humerus by belt injuries except 17 patients of solitary fractures of forearm and humerus. 1. This injury is more prevalent in males(M:F=4:1) with the peak incidence between the third and fourth deades(59%). 2. Multiple fractures of forearm and humerus by belt injuries were classified by fracture numbers and mechanism of injury as follows: Type I Fractures of Forearm Both Bones(53.2%). Type II Fractures of Forearm Both Bones and Humerus(27.7%). Type IU Segmental Fracture of Radius and Ulna(10.6%). Type IV Segmental Fracture of Radius, Ulna and Humerus(8.5%). 3. Most were open fractures with soft tissue damages(74%). 4. Nerve paralysis was the most frequent associated injury(27%). 5. All but 7 cases were treated by open reduction. Implants used for internal fixation were compression plates in 20 cases as elective surgery and Rush pins in 15 cases of forearm open fractures as emergency surgery. 6. By Anderson's critcria, satisfactory result(excellent or good) was obtained in 55% of cases.
