2.MR Imaging of Meniscal Tears' Correlation with History of Trauma.

PURPOSE: The medial meniscus is injured much more than the lateral meniscus. Because the medial meniscus is much larger in diameter, is thinner in its periphery and narrower in body than the lateral meniscus, and does not attach to either cruciate ligament. We evaluated correlations with sites of tear and history of trauma. METHODS AND MATERIALS: We reviewed retrospectively in 43 patients with meniscal tears on MRI(51 cases) and correlated them with history of trauma. RESULTS: The most common site of injury was the posterior horn of the medial meniscus(32/51), but high incidence of lateral meniscal tear compared with previous reports was seen. In the cases which had history of trauma, the posterior horn of medial meniscus was most commonly injured(26/34) and 5 meniscal tears were combined with meniscal tear in the other site. The tear in the anterior horn of the medial meniscus was seen only in a patient which had history of trauma and combined with meniscal tear in the other site. But in the meniscal tears without definite history of trauma, the incidence of meniscal tear was different from the meniscal tear with history of trauma. The incidence of lateral meniscal tear(11/17) was highter than medial meniscal tear and the posterior horn of lateral meniscus was commonly injured. CONCLUSION: We concluded that the medial meniscus was commonly injured, especially posterior horn, but in the cases which had no definite history of trauma, the lateral meniscus was commonly ipjured. An awareness of prevalent site of meniscal injuries may be helpful in the diagnostic interpretation of MR imaging of knee.

3.Analysis of Satisfaction with Removal Dental Prosthesis among Hansen`s Patients.

The purposes of this study were to investigate the satisfaction of use of removable denture elated to general, functional and esthetic factors among dental patients in national sorokdo hospital, and to establish the structure relationships of dental treatment satisfaction. Two hundred fifty hansen`s patients who consented with this survey were selected and examined their oral status. Satisfaction degree was estimated by means of self-developed questionarires, filled out by face-to-face interview. Collected data were analyzed by t-tast The obtained results were as follow : 1. The satisfaction degree of four subscales was highest in esthetic actor (3.17+/-0.36), denture retention factor(3.10+/-0.60), general treatment satisfaction factor(2.94+/-0.56), and masticatory function factor (2.90+/-0.68) in descending order. It was appeared that patients was generally satisfied with all four factor. 2. On the comparison between proportion of denture satisfaction and satisfactio 02n of subscales, as the satisfaction of general treatment, masticatory function and denture retention were increased, the proportion of denture satisfaction of esthetic function and proportion of denture satisfaction. 3. The masticatiory function factor, denture retention factor and genernal satisfaction of treatment factor were directly affected on the denture satisfaction, but esthetic factor was affected indirectly on the denture satisfaction as a result of path analysis

4.Immediate and Sustained Improvement in Behavior and Life Quality by Adenotonsillectomy in Children With Sleep-Disordered Breathing.

OBJECTIVES: To investigate longitudinal changes in behavior and quality-of-life (QoL) in children with sleep disordered breathing (SDB) after adenotonsillectomy (AT). METHODS: This study prospectively enrolled 148 children who underwent AT for SDB. Caregivers filled out Korean attention deficit hyperactivity disorder (ADHD) rating scale (K-ARS) and Korean version of the obstructive sleep apnea-18 (KOSA-18) at preoperative 1 day, postoperative 1 month, and postoperative 6 months. Longitudinal changes in K-ARS and KOSA-18 were evaluated and compared among subgroups. RESULTS: Both K-ARS and KOSA-18 scores improved immediately at postoperative 1 month and were maintained at postoperative 6 months. The mean preoperative K-ARS scores of the non-ADHD, ADHD-trait, and overt-ADHD groups showed uniformly immediate and sustained improvements. Also, the mean preoperative KOSA-18 scores of the small, moderate, and large impact groups were significantly improved both at postoperative 1 month and 6 months. The changes in behavior and QoL showed no significant differences with regard to the aforementioned factors. CONCLUSION: After AT, we may expect immediate and sustained improvements in behavior and QoL in children with SDB regardless of preoperative symptom severity. Moreover, by performing AT, improvements in behavior and QoL are expected regardless of sex, age, allergic trait, or concurrent coblation-assisted turbinoplasty. These findings may help our colleague physicians counselling pediatric SDB patients and their caregivers.

5.Factors associated with Receiving Rescreening in High Risk Group Diagnosed by Endoscopic Screening of Stomach Cancer.

BACKGROUND: Stomach cancer is the most common cancer in Korea. Lifelong health management program recommends that males over 40 years and women over 50 years should undergo stomach cancer screening by endoscopy or upper gastrointestinal series every two years. The importance of re-screening of stomach cancer in a high risk group is emphasized. METHODS: A telephone questionnaire was done one year after to 123 patients over 40 years old considered as a high risk group with either chronic atropic gastritis, intestinal metaplasia, gastric ulcer, gastric adenoma among 804 subjects who had undergone anendoscopic examination from February 2002 to June 2003 at a university hospital health promotion center. RESULTS: The number of patients who responded were 109. The average recognition rate of high risk group was 53.2%. The rate of recognition of high risk group was lower in good subjective health estimation group and in less educated group, in old age group, and in chronic atrophic gastritis and intestinal metaplasia group. The rate of not receiving re-screening after 1 year was 48 (44.0%). Re-screening rate in patients with intestinal metaplasia (35.3%) and chronic atropic gastritis (39.1%) compared to gastric ulcer (77.8%) and gastric polyp (90.0%), in those not recognizing themselves as high risk group, in low education group, in old age, was lower. CONCLUSION: In patients with chronic atropic gastritis and intestinal metaplasia, in those not recognizing themselves as high risk group, in old age and in good subjective health state. We need to educate the importance for regular screening of stomach cancer more intensively.

6.A case of isolated congenitally corrected transposition of the great arteries with complete atrioventricular block.

Patients with isolated congenitally corrected transposition of the great arteries (CCTGA) comprise 1% of all CCTGA patients. They are usually asymptomatic until functional abnormalities such as atrioventricular block or systemic atrioventricular valve regurgitation become complicated. We report here a case of a 33 year-old man with isolated CCTGA and complete AV block that underwent implantation of a permanent pacemaker using endocardial leads. During a 12 month follow-up period, no symptoms developed and lead-related parameters have been stable.

7.Antioxidative Effect of Ginseng Saponin on Cardiac Endothelial Cells in Culture.

The cytotoxic effects of oxygen free radicals and the antioxidative effect of ginseng saponin (SPN) on cardiac endothelial cell cultures derived from 3-day old rats were studied. Reactive oxygen species were generated by hypoxanthine (HX) and xanthine oxidase (XO) mixture to the culture medium. Exposure of cardiac endothelial cells to this oxygen-radical-generating system resulted in significant time-dependent decrease of MTT activity and increase of lactate dehydrogenase (LDH) release. These results correlated well with the morphological examination of randomly selected cultured cardiac endothelial cells, which showed large cytoplasmic vacuoles, disordered organelles, pronounced increase of endoplasmic reticular swelling, and decreased maintenance of membrane integrity. The decrease in cell viability and increase of LDH release induced by the oxygen free radicals in cardiac endothelial cell cultures were blocked during the first two hours by antioxidants such as ginseng saponin (SPN), deferoxamine (DFX), and ginseng saponin/deferoxamine mixture (SPN/DFX). These antioxidative effects were significantly greater in the SPN-treated group than in the other antioxidant-treated groups. Especially, the cells of the SPN-treated group showed well developed cytoskeletons, which enabled them to firmly attach to the culture vessel. In conclusion, these results indicate that ginseng saponin has a significant antioxidative effect on cardiac endothelial cells in culture and plays an important role in stimulating the formation of cytoskeleton and maintaining the integrity of cell membrane.

8.Metabolic Syndrome and Risk of In-Stent Restenosis: Clinical Outcomes in Patients Undergoing Percutaneous Coronary Intervention.

BACKGROUND AND OBJECTIVES: Patients with metabolic syndrome (MS) have an increased risk of cardiovascular events. However, only limited studies are available on the effect of MS on restenosis and on the clinical outcome of patients undergoing percutaneous coronary intervention (PCI). The aim of this study is to assess the role of MS in the development of restenosis, and risk of a 6-month major adverse cardiac event (MACE) and a 12-month MACE, as well as the difference of outcome between the use of bare metal stents (BMSs) and the use of drug eluting stents (DESs). SUBJECTS AND METHODS: This is a one center, retrospective study. The study population comprised 151 patients undergoing percutaneous coronary intervention (PCI) with BMSs and 200 patients undergoing PCI with DESs. The study population was classified into two groups of patients with MS and patients without MS. RESULTS: The baseline clinical characteristics were similar in the two groups (with or without MS) except for hypertension, diabetes, body mass index, triglyceride level and high-density lipoprotein level. The frequency of in-stent restenosis of the patients that were implanted with BMSs or DESs was not different between two groups, based on a 6 month follow-up quantitative coronary angiographic analysis (BMSs: 30% vs 22.2%, p= 0.352; DES: 3.3% vs 2.2%, p=0.76; for patients with and without MS, respectively). The percent of patients with a 6-month MACE for patients implanted with BMSs was not statistically different for patients with or without MS (30% vs 22.2%, p=0.352) but the percent of patients with a 12-month MACE showed a statistically significant higher level for the MS group (38.6% vs 23.5%, p=0.044). The percent of patients with a 6-month MACE for patients implanted with DESs was also not statistically different between two groups (5.8% vs 1.7%, p= 0.123). CONCLUSION: Patients with MS undergoing BMSs or DESs implantation do not show higher levels of in-stent restenosis and levels of a 6-month MACE. The number of 12-month MACEs of patients implanted with BMSs is statistically higher, but the number of 6-month MACEs of patients implanted with DESs is not different for the MS group. We conclude that MS is not risk factor of in-stent restenosis in PCI but MS may influence the long-term clinical outcome in patients undergoing PCI.

In clinical therapy, the current goal of dental implants is to enhance quantity and quality of osseointegration. Surface roughness and oxide structure are considered to influence the behavior of adherent cells. The purpose of this study is to evaluate the effect of different surface treatment on cellular response. The attachment and proliferation of osteoblast-like cell on sandblasted, sandblasted and etched, thermal oxidated surfaces have been compared. Sandblasting was done with Al2O3 particles(grain size of 50micrometer), etching was processed with NH4OH : H2O2 : H2O(1:1:5) at 90degrees C for 1 minute. Thermal oxidation was followed sandblasting and etching at 400degrees C, 600degrees C, 800degrees C for 2 hours. Measurement of surface roughness after the different treatment did not show any differences of Ra value between terated surfaces. Cell attachment and proliferation were increased during experiment period, but no difference was observed. SEM evaluation revealed a similar pattern of osteoblastlike cells, well attached with dendritic extension and producing numerous matrix vesicles on cell surface. The results of this study showed that oxide layer alteration by thermal oxidation did not affect the attachment and proliferation of osteoblast-like cells. This suggests the possibility that the cellular responses are further influenced by surface roughness than titaniun oxide structure. 10.The Safety of Tirofiban Therapy in Patients with Acute Coronary Syndrome.

BACKGROUND AND OBJECTIVES: Recent randomized studies have shown a significant reduction in the rate of recurrent cardiac events with adjunctive pharmacotherapy, using Glycoprotein IIb/IIIa inhibitors (GPI), in patients undergoing percutaneous coronary intervention (PCI) procedures. However, in Korea, there is a paucity of data concerning complications of tirofiban therapy in patients with acute coronary syndrome (ACS). Therefore, this study was performed to evaluate the safety of tirofiban therapy. SUBJECTS AND METHODS: ACS patients who underwent tirofiban administration between May 2002 and October 2003 were reviewed. The rates of bleeding, transfusion, and thrombocytopenia were analyzed, and the rates of complications by ages, gender and PCI compared with medical treatment, renal function and vascular access route. RESULTS: A total of 261 ACS patients (male/female=150/111) underwent tirofiban therapy. The mean ages of the subjects was 64.5 years, the rates of minor bleeding, major bleeding, transfusion and thrombocytopenia were 8.1% (n=21), 2.3% (n=6), 4.6% (n=12) and 1.2% (n=3), respectively. Minor bleeding occurred at similar rates in both sexes (8 vs. 8.1%) and to a greater extent in old age (> or =65) (12.5 vs. 2.6%, p=0.093), but major bleeding occurred to a greater extent in females and old age (5.4 vs. 0% and 4.2 vs. 0%, p=0.25 and 0.093, respectively). The rates of thrombocytopenia and transfusion were greater in old age (2.1 vs. 0% and 8.3 vs. 0%, p=0.052 and 0.087, respectively). In-hospital days were greater in old age (14.7+/-9.5 vs. 11.9+/-4.4 days, p=0.065). The rates of complications were similar in both groups when compared by PCI or medical treatment and vascular access route. The serum creatinine was 3.3 mg/dL in those with major bleeding, which was higher than in the other groups (p=0.000). CONCLUSION: The tirofiban therapy in patients with ACS did not induce an increase in the bleeding rates, in-hospital days and it was safe and well tolerated in old age.
