1.Relationship between GFAP and malignant degree of astrocytoma

Objective To study the relationship between GFAP and the malignant degree of astrocytoma. Methods After successful transfer of retrovirus with GFAP (PLBsKG) into C6 cells mediated with liposome, the morphology, growth curve and the proliferative cycle of the target cells were observed. The expression of the protein and mRNA of GFAP was determined with RT-PCR and immunocytochemical staining. In addition, the relationship between the staining GFAP-IR and the malignant degree of astrocytoma was analyzed according to Kermohan′s grading standard in 20 samples of the cells of astrocytoma with imaging analytic system. Results PLBskG resulted in the decrease of GFAP in the target C6 cells, and changed the morphology and increased the proliferation of C6 cells. The number of the cells in the G 2+M phages was increased. The intensity of GFAP-IR staining was negatively correlated to the malignant potency of astrocytoma. The more intensive staining of the cells, the lower of the malignant degree of astrocytoma and vice versa. Conclusion There is a close relationship between GFAP and malignant degree of astrocytoma. The level of GFAP expression serves the index of the malignant degree and prognosis of astrocytoma in certain degree.
2.Prognostic risk factors for surgery in patients with cirrhotic portal hypertension

Objective To investigate the prognostic risk factors for surgery in patients with cirrhotic portal hypertension.Methods One hundred and sixty one patients with cirrhotic portal hypertension who received surgery in our hospital in the past 10 years were studied.The data were entered into a pre-designed form.24 predictors including patients′ age,sex,degree of liver atrophy,ChildPugh classification,coagulation profile,splenic size,renal function,blood pH,base excess (BE),operative time,volume of ascites,and intraoperative and postoperative hemorrhage were recorded and analyzed.For each of the predictors,2-3 subgroups were compared.Results Seven predictors were clearly related to surgical prognosis:postoperative bleeding within 30h (B0.356,P<0.001) and a bleeding volume >2 L were awarded 3 points; liver volume (B-0.160,P<0.001) and severe liver atrophy (antero-posterior diameter of the left liver lobe ≤55 mm,oblique diameter of the right lobe ≤110 mm) were awarded three points; blood pH (B0.141,P<0.001),pH<7.35 was awarded 2 points; BE (B-0.123,P<0.001),BE<-3 (mmol/L) was awarded 2 points; decrease in PLT (B0.065,P =0.015),PLT< 3 (T/L) was awarded 2 points; intraoperative bleeding (B0.062,P=0.014),bleeding volume >2 L was awarded 2 points; decrease in RBC (B0.053,P=0.024),<3(G/L) was awarded 1 point.Of the 147 patients who recovered from surgery,all had ≤3 points,except one who had 4 points.Of the 14 patients who died,all had ≥5 points except one who had 4 points.Conclusions Postoperative bleeding (>2 L),severe liver atrophy (antero-posterior diameter of the left live lobe ≤55 mm,oblique diameter of the right lobe ≤110 mm),blood PH<7.35,BE <-3 (mmol/L),PLT<30 000(T/L),intraoperative major bleeding (>2 L) and RBC<3 (G/L) were significant prognostic risk factors for surgery.For patients who had a score of 5-6 points; death was likely following surgery.A score ≥8 points should be considered as a contraindication to surgery.To reduce operative mortality,active treatment should be given before surgery to keep the score to be 4 points or less.
3.Effects and mechanism of single enzyme and compound enzyme digestive juice on pancreatic islet cell isolation of adult pigs

BACKGROUND: Recently,the technique of isolation procedure of viable islets cell from large animals has been mature and used in clinical islet cell transplantation. Some study results indicate that low yields of pig islet cell isolations is probably not due to the presence of higher or more aggressive enzyme activities during the porcine isolation procedure. Therefore, the causative factors resulting in the inconsistent results should be sought for the intrinsic properties of the pancreatic islet and pancreas. The purpose of the study is to get more pancreatic islet cells for diabetic patients.OBJECTIVE: To investigate the effect of single enzymatic solution (SES)and multi-enzymatic solution(MES) on adult porcine pancreatic islet cell isolation,and to provide the theory base for diabetic patients rehabilitation.DESIGN: A completely randomized and controlled experiment based on animals.SETTING: General surgery and gerontology department in a university hospital.MATERIALS: The experiment was conducted in the Second Affiliated Hospital of Harbin Medical University from June 2003 to May 2004. Twenty consecutive random-bred adult pigs, 12 - 24 months old, weighing average 100 kg,were selected for experiment from Hada Slaughterhouse of Harbin.INTERVENTIONS: After slaughtering,the spleentic lobe of the pancreas was excised using sterile surgical gloves and surgical instruments. Each was immediately transported in 500 mL of sterile RPMI1640 solution at 4 ℃ to the laboratory for processing. After removal of fat peritoneum and superficial blood vessel,each pancreas was first immersed in 1:5 000 Liquor chlorhexidine for 3 minutes,then washed three times with cold RPMI1640, next mechanically minced into 1 mm×1 mm×1 mm fragments,weighed,and equally divided into two groups. Adult porcine islets were isolated with two different collagenase solutions(SES and MES).MAIN OUTCOME MEASURES: Islet count was performed with DTZ-staining. The viability was assessed by trypan-blue staining. Porcine islet insulin-secretory function was assessed by insulin content of cultured porcine islets and by insulin release. Islet morphological integrity was finally established by electron microscope examination.RESULTS: Overall islet cell content were statistically different between the two methods[(1 782 ±427) IE/g vs (1 293 ±451) IE/g,P<0.05]. No significant difference was found in viability,function or morphology of islet between MES and SES(P>0. 05).CONCLUSION: An average of isletisolated with MES method represents a uniquely massive yield in comparison with that by SES method. A good viability was also confirmed by static incubation and culture. The transplantation of those pancreatic islet cells can help to control the syndromes of diabetes.
4.A phantom study of the influence of tube voltage on image quality and radiation dose in DR chest radiography

Objective To investigate the influence of tube voltage on image quality and radiation dosage to subjects in chest digital radiography (DR).Methods An adult chest phantom was exposed in the range of 80-130 kV with the interval of 10 kV and the automatic exposure control (AEC) setting was selected from-4 to 4.The entrance skin dose (ESD) of the phantom was measured,and the relative noise,contrast-to-noise ratio (CNR) and the effective dose of each exposure was calculated.Results The ESD was (0.062 9 ±0.027 4) mGy,and the effective dose was (0.012 7 ±0.004 5) mSv.The effective dose increased linearly with the ESD,and there was a positive correlation (r =0.912,P < 0.01).When the tube voltage was held constant,the relative noise decreased as the effective dose increased (r =-0.967,-0.969,-0.968,-0.969,-0.968,-0.970,P < 0.01).The CNR increased with the effective dose increasing under the same tube voltage(r =0.987,0.987,0.986,0.987,0.988,0.989,P <0.01).The ESD and effective dose decreased as the kV values increased under the same AEC,and the degree of decrease could be up to 50% and 20%.The relative noise decreased as the kV values increased under the same AEC,and the degree of decrease could be up to 23%.The CNR values increased up to 8%.Conclusions The use of higher kVp in chest digital radiography can reduce the radiation dose to some extent
5.Expression of hNa_v1.8 channel protein in affected nerve ultrastructure of patients with trigeminal neuralgia

Objective:To observe whether there is abnormal expression of tetrodotoxin-resistant (TTX-R)hNa v1.8 channel protein in demyelinated fibers of the affected nerves in patients with trigeminal neuralgia(TN), and to explore the relationship between ectopic discharges and TN.Methods: Six affected inferior alveolar nerves obtained from patients with idiopathic TN were studied. One great auricular nerve and one normal inferior alveolar nerve were used,which were obtained from patients undergoing combined radical neck dissection with glossectomy (negative control) and mandibulectomy (normal controls) . One rat spinal nerve was used as positive control. Immunohistochemical method and electron microscope were used to observe the expression of TTX-R hNa v1.8 channel protein in all groups. Results: Strong expression of hNa v1.8 channel protein was found in the demyelinated fibers of the affected nerves in patients with TN, weak expression in the neuraxon of rat spinal nerve,and none in the normal inferior alveolar nerve and the great auricular nerve. Conclusion: Abnormal expression of hNa v1.8 channel protein in the demyelinated fibers in patients with TN may play an important role in the pathogenesis of TN.

Objective To emphasize the correlation and concordance of electronic medical record through analysis of frequently asked problems on handwritten medical record.Methods The problems of electronic medical record were classified and found the typical cases to analyze.Results In order to improve the quality of medical record,the quality control of electronic medical record must be strengthened and the phenomenon that some doctors pay attention to hand-written med-ical record more than electronic medical record must be overcame.Conclusion The normative management of electronics medical is strengthened that is fundamental guarantee for paper-based medical records.
7. Identification and evaluation of occupational hazards during construction of a large construction project

OBJECTIVE: To analyze the occupational hazard factors and their critical control points in the process of construction of a large-scale construction project.METHODS: The engineering analysis, worksite survey occupational health and occupational hazard factor testing were conducted in a large-scale construction project, and the results were analyzed. RESULTS: During the process of construction of this large-scale construction project, there were many occupational hazard factors such as silicon dust, welding smoke, wood and other dusts, inorganic compounds of manganese, carbon monoxide, carbon dioxide, nitric oxide, ozone, noise, high temperature, hand-transmitted vibration and ultraviolet irradiation, among them, silicon dust and noise were the most common ones. The over standard rates of exposure concentration of short term of total dust and respirable dust in the workplace were 68.2%(15/22) and 40.9%(9/22), and the over standard rates of exposure concentration of time weighted average were 54.5%(12/22) and 13.6%(3/22), respectively. The over standard rates of the noise intensity of area sampling and personal sampling in workplace were 69.2%(45/65) and 61.0%(25/41) respectively. The four hours energy equivalent frequency-weighted acceleration to vibration of three hand-transmitted vibration positions has been detected, and the result has surpassed the occupational exposure limit.The results of other occupational hazard factors such as high temperature, ultraviolet radiation, wood dust, welding smoke, other dust, manganese inorganic compounds, carbon monoxide, carbon dioxide, nitric oxide, nitrogen dioxide, hydrogen sulfide, hydrogen cyanide and ozone all met the occupational exposure limits. CONCLUSION: There are various occupational hazard factors in the process of construction of this large-scale construction project, among them, noise, dust and hand-transmitted vibration are the most prominent hazards.These hazards are the critical control points of this type of construction projects.
8.Research of using image noise characteristic to evaluation image quality in digital mammography

ObjectiveTo find the proper way of assessing the image noise characteristic through the analysis of noise characteristic in the digital mammographic images.MethodsThe CDMAM phantom was firstly exposed with different filter materials and tube voltages in the range of 23 to 35 kVp at a fixed tube current 90 mAs,then exposed with different filter materials and tube current in the range of 20 to 200 mAs at a fixed tube voltage 29 kVp.The parameters such as background standard deviation,joint standard deviation,subtracted standard deviation and relative standard deviation were calculated and measured from both 60 pixels of signal disc and background beside of the disk for all the images.The IQFinvs were calculated for the achieved images.Non-parametric rank sum tests were used to compare the difference of the background standard deviation,joint standard deviation and subtracted standard deviation,t test was used to compare the IQFinvs between Mo and Rh filers in each group.ResultsWith the same exposure parameters in the test above,the maximum value was the background standard deviation,while the subtracted standard deviation was the minimum,and the value of joint standard deviation was in the middle. With other parameters unchanged,the values of three kind of standard deviation increased with the increasing of tube voltage or tube current,while the differences between them getting more significant.Under the condition of 90 mAs and Mo filter material,background standard deviation,joint standard deviation,subtracted standard deviation were 10.91,10.34 and 9.87,the results were 11.24,10.57 and 9.96 while Rh filter material was used,significant difference was found ( x2 =26.0,P < 0.01 ). Under the condition of 29 kVp and Mo filter material,background standard deviation,joint standard deviation,subtracted standard deviation were 12.30,11.61 and 11.05,the results were 12.29,11.58 and 10.87 while Rh filter material was used,significant difference was found( x2 =20.0,P < 0.01 ).The IQFinvs were 149.14 ± 23.87 and 139.16 ± 23.14 under the condition of 90 mAs and Mo filter material ( t =9.129,P < 0.01 ).The IQFinvs were 149.75 ±34.85 and 145.2 ±34.64 while the parameters were 29 kVp and Rh filter material(t =1.304,P >0.05 ).The value of relative standard deviation decreased with the increasing of tube voltage or tube current,and the image quality got better with the decreasing of relative standard deviation.ConclusionRelative standard deviation is suitable not only for the expression of the digital mammographic images,but also for the evaluation of the image quality.
9.The influence of bismuth shielding on image quality and radiation dose to eye lens in sinus multi-detector CT

Objective To explore the influence on image quality and the reduction of radiation dose to eye lens when using bismuth shielding in sinus MDCT.Methods The standard water phantom was scanned using clinical scanning protocols of sinus in 16-MDCT,and the images were acquired with none,1 layer,2 layers and 3 layers of bismuth shielding severally.Using the above protocol,the cadaveric head was scanned with no shield,1,2 and 3 layers of bismuth shielding material covered on both eyes,and the organ dose to eye lens in each scanning was measured with thermoluminescence dosimeter (TLD).The sponge with the thick of 0.5,1.0,1.5,2.0 cm was placed between the shielding material and the surface of subjects separately and the radiation doses to eye lens was measured with sinus scanning conditions in the same way.The CT values of phantom with the distances of 2.0,4.0,6.0 and 8.0 cm to shield material were measured.The influence of bismuth artifacts on anatomic structures was evaluated as well.Results The organ doses to the eye lens in the sinus clinical CT were 20.0 mGy.Doses decreased significantly to 13.7,10.9 and 9.4 mGy separately when using 3 types of bismuth shielding thickness.With different thicknesses of shielding material,the greater the shield gap,the smaller the degree of reduction of organ dose,but the increasment of CT value was significantly reduced.Using 1-layer shield no gap and 2-layer shield with 1.5 cm gap in sinus MDCT,the radiation doses were reduced to 13.7 and 13.1 mGy with the reduction rates of 31.5% and 34.5% respectively.Conclusion The reasonable bismuth shielding can effectively reduce the radiation dose to eye lens in sinus CT on the premise of ensuring image-diagnostic quality.
10.Radiation dose reduction to the lens of eye with bismuth shielding in head and neck MSCT

Objective To explore the effects on image quality and dose reduction to the lens when using bismuth shielding in head and neck MSCT.Methods The standard phantom and the cadaveric head with none,1,2 and 3 layers of bismuth shielding were scanned with protocols of brain,temporal bone and paranasal sinuses using a 16-MDCT scanner.The organ dose to the lens in each scanning was measured with thermoluminescence dosimeters(TLD).The above scanning with sinus protocol was repeated with a sponge with thickness of 5,10,15 and 20 mm placed between the shielding and phantom/head.The CT attenuation of phantom with the distance of 2,4,6 and 8 cm to shielding were measured.The image quality was subjectively evaluated by 2 physicians.Results The organ doses of the lens with protocols of brain,temporal bone and sinuses were 24.31,27.60 and 20.01 mGy,respectively.The doses were decreased significantly when using bismuth shielding.With the increase of the shield gap,the degree of dose reduction was reduced,but the increasing degree of CT attenuation significantly reduced.Using 2-layer and 3-layer bismuth shield in brain and temporal bone CT scan,the radiation doses were reduced by 47.1% and 59.1%,respectively while maintaining the diagnostic image quality.Using 1-layer shield without gap and 2-layer shield with 1.5 cm gap in sinus CT,the radiation doses were reduced by 31.5% and 34.5%,respectively.Conclusions Reasonable usage of bismuth shielding can effectively reduce the radiation dose to the lens of eye in head and neck MSCT.
